The Effects of Inspiratory Pause on Airway Pressure and Gas Exchange under Same I:E ratio in Volume-controlled Ventilation (Volume-Controlled Mode의 기계환기시 동일환 I:E Ratio하에서 Inspiratory Pause가 기도압 몇 가스교환에 미치는 영향)

  • Background : In volume-controlled ventilation, the use of inspiratory pause increases the inspiratory time and thus increases mean airway pressure and improves ventilation. But under the same I : E ratio, the effects of inspiratory pause on mean airway pressure and gas exchange are not certain. Moreover, the effects may be different according to the resistance of respiratory system. So we studied the effects of inspiratory pause on airway pressure and gas exchange under the same I : E ratio in volume-controlled ventilation. Methods: Airway pressure and arterial blood gases were evaluated in 12 patients under volume-controlled mechanical ventilation with and without inspiratory pause time 5%. The I : E ratio of 1 : 3, $FiO_2$, tidal volume, respiratory rate, and PEEP were kept constant. Results: $PaCO_2$ with inspiratory pause was lower than without inspiratory pause ($38.6{\pm}7.4$ mmHg vs. $41.0{\pm}7.7$ mmHg. p<0.01). P(A-a)$O_2$ was not different between ventilation with and without inspiratory pause $185.3{\pm}86.5$ mmHg vs. $184.9{\pm}84.9$ mmHg, p=0.766). Mean airway pressure with inspiratory pause was higher than without inspiratory pause ($9.7{\pm}4.0\;cmH_2O$ vs. $8.8{\pm}4.0\;cmH_2O$, p<0.01). The resistance of respiratory system inversely correlated with the pressure difference between plateau pressure with pause and peak inspiratory pressure without pause (r=-0.777, p<0.l), but positively correlated with the pressure difference between peak inspiratory pressure with pause and peak inspiratory pressure without pause (r=0.811, p<0.01). Thus the amount of increase in mean airway pressure with pause positively correlated with the resistance of respiratory system (r=0.681, p<0.05). However, the change of mean airway pressure did not correlated with the change of $PaCO_2$. Conclusion: In volume-controlled ventilation under the same I : E ratio of 1 : 3, inspiratory pause time of 5% increases mean airway pressure and improves ventilation. Although the higher resistance of respiratory system, the more increased mean airway pressure, the increase in mean airway pressure did not correlated with the change in $PaCO_2$.

Evaluation of the usefulness of the method according to changes in patient breathing during chest 4D CT imaging (흉부 4D CT에서 호흡 변화에 대한 일시 중지 및 재개 방법의 유용성 평가)

  • Purpose : In order to evaluate the usefulness of clinical application of the Pause & Resume methods by comparing and analyzing the data stability and dose reduction effect when repeat scan assuming irregular breathing and using the Pause & Resume method during chest 4D CT using QuasarTM Phantom. Materials and Methods : Using the QuasarTM Phantom, set the breathing rate per minute to 15 BPM and 7.5 BPM, and set the S15 point as an irregular breathing section, and then placed OSLD to this point and use the Pause & Resume method to measure the dose of S15. CTDIvol, DLP, and ALARA-CT were used for comparative analysis of radiation dose between Pause & Resume method and Repeat-scan. In order to evaluate the stability and usability of the data applying the Pause & Resume method, the captured images were sorted by Advanced Workstation Volume Share7 and then sent to EclipseTM, the diameter and volume were analyzed by forming a contour on the iron ball in the QuasarTM Phantom Results : When using Pause & Resume, the dose of OSLD measurement increased by 1.97 times in the section of S15. As a result of image evaluation, the average value of all volumes measured with and without the Pause & Resume method at 15 BPM and 7.5 BPM was 15.2 cm3±0.5%.Allthemeasuredvaluesfor the radius of iron ball were 3.1 cm regardless of whether Pause & Resume method was used or not. In the case of using Pause & Resume, 33% decreased from the lowest DLP value and 38% decreased from the highest DLP value of repeat scan, and the effective dose also decreased 32.1% from the minimum value and 37.6% from the maximum value. Conclusion: Irradiation dose was increased by Pause & Resume method because of the repeat scan on the S15 site where assuming irregular breathing occurred, However Pause & Resume method led to a significant reduction in dose on overall scan range. It also proved the usefulness of clinical application of the Pause & Resume method as a result of similar diameters and volumes of iron ball measurement.

Speech Rate and Pause Characteristics in Patients with Parkinson's Disease (파킨슨병 환자의 말 속도와 쉼 특성)

  • The purpose of this study is to investigate the speech rate characteristics (whole speech rate, articulation speech rate, and articulation percentage) and the pause characteristics (pause duration, pause frequency, and pause percentage) of Korean-speaking patients with idiopathic Parkinson's disease (referred to as IPD hereafter). The study aims first to examine the differences between the patient group with IPD and the other group without IPD concerning those measurements, and secondly to investigate the relevant measurements of the two groups following the sentence length changes. There were two groups of subjects in this study. The first group consisted of 7 subjects between the ages of 50 and 60 who were diagnosed as IPD with mild severity, and the second group consisted of 13 subjects without IPD who matched the age and gender of those in the first group. Those two groups were asked to read 8 different sentences in length at habitual speed. Speech rate and pause characteristics of the two groups were measured and compared each other. The followings results were observed. First, in a study of speech rate characteristics, the whole speech rate and the articulation speech rate of the patient group scored within the normal range, which is same as the group without IPD. On the other hand, with regard to the pause characteristics, differences between two groups were shown; the patient group had shorter pause duration, lower pause frequency, lower pause percentage, and higher articulation percentage. Secondly, in a study of relevant measurements following the sentence length, both groups showed a tendency for whole speech rate and articulation rate to increase as the length of the sentence increased, but the result of pause characteristics showed a difference between two groups. While the group without IPD showed a longer pause duration, higher pause frequency, and higher pause percentage as the length of sentences increases, no differences were shown among the patient group concerning the length of sentences. This study suggests a result that the patients with IPD of mild severity retained a normal speech rate and examined pause characteristics of the patient group which showed a different result from the group without IPD in terms of quality. Future studies on the speech rate and pause characteristics of Korean-speaking patients with IPD in various severities.

The effects of pause in English speaking evaluation

  • The main objective of this study is to investigate the influence of utterance internal pause in English speaking evaluation. To avoid possible confusion with other errors caused by segmental and prosodic inaccuracy, stem utterances with two different length obtained from a native speaker were manipulated to make a set of stimuli tokens through insertion of pauses whose length and position vary. After a total of 90 participants classified into three proficiency groups rated the stimuli, the scored data set was statistically analyzed in terms of the mixed effects model. It was confirmed that predictors such as pause length, pause position and utterance length significantly influence raters' evaluation scores. Especially, a dominating effect was found in such a way that raters gradually deducted scores in accordance with the increase of pause duration. In another experiment, a tree-based statistical learning technique was utilized to check which of the significant predictors played a more influential role than others. The findings in this paper are expected to be practically informative for both the test takers who are preparing for an English speaking test and the raters who desire to develop more objective rubric of speaking evaluation.

The Acoustic Realization of Phrasal Verb vs. Verb-preposition (구절 동사와 전치사 수반동사의 의미에 따른 음성적 실현)

  • Verb phrase could have two different meanings according to which is followed after verb; adverb or preposition. The meaning of 'verb+adverb' is deduced from a figurative meaning which is idiomatic expression, and 'verb+preposition' is interpreted as the literal meaning. The purpose of this study is to observe how English native speakers and Korean leaners of English distinguish two sentences of the same word strings with acoustic cues like pause and duration. According to the result, as pause was used for meaning distinction, it was likely that the pause length preceding prepositions was longer than that of following adverbs. To distinguish two sentences of the same word strings, all participants seemed to use pause, verb lengthening and adverb/preposition lengthening. Among them, there is a hierarchical significance; in sequence, pause, verb lengthening, adverb/preposition lengthening.

A Study on Perceptual Sensitivity to Prosodic Cues in Disambiguation (중의성 해소에 기여하는 억양단서의 인지적 민감도 연구)

  • This experimental study has a goal to explore the perceptual sensitivity to phonetic evidence such as duration, phrase accent, or pause in disambiguation. We argue that the realization of the intonational phrasal boundary at the meaningful grammatical boundary in structurally ambiguous sentences facilitates English native listeners to distinguish the meanings of the ambiguous sentences. Moreover, the duration of the phrase-final syllable, pitch range reset, or phrasal tones also provides listeners with important phonetic evidence in disambiguation. In our perception experiment, however, Korean English learners largely depend on the realization of pause. In the results from the perception experiment, all of the groups showed an increase in the response time from the perception of no pause to pause realization. This means that pause at the phonological phrasal boundary plays a role of facilitator to English native speakers with other prosodic cues such as duration, pitch accent, or phrasal tones, while an absolutely important cue to Korean English learners.

Study on the realization of pause groups and breath groups (휴지 단위와 호흡 단위의 실현 양상 연구)

  • The purpose of this study is to observe the realization of pause and breath groups from adult speakers and to examine how gender, generation, and tasks can affect this realization. For this purpose, we analyzed forty-eight male or female speakers. Their generation was divided into two groups: young, old. Task and gender affected both the realization of pause and breath groups. The length of the pause groups was longer in the read speech than in the spontaneous speech and female speech. On the other hand, the length of the breath group was longer in the spontaneous speech and the male speech. In the spontaneous speech, which requires planning, the speaker produced shorter length of pause group. The short sentence length of the reading material influenced the reason for which the length of the breath group was shorter in the reading speech. Gender difference resulted from difference in pause patterns between genders. In the case of the breath groups, the male speaker produced longer duration of pause than the female speaker did, which may be due to difference in lung capacity between genders. On the other hand, generation did not affect either the pause groups or the breath groups. The generation factor only influenced the number of syllables and the eojeols, which can be interpreted as the result of the difference in speech rate between generations.

Speech Rate and Pause Characteristics in Speaker with Flaccid Dysarthria (이완형 마비말장애 화자의 말속도와 쉼 특성)

  • The purposes of this study were to investigate the characteristics of speech rate and pause in patients with flaccid dysarthria. For this purpose, 15 patients with flaccid dysarthria and 15 normal speakers matched in gender and age participated as subjects. The overall speech rate, the articulation rate, the inter-sentence and the intra-sentence pause duration and pause frequency were measured during reading the standardized passage "Autumn"(Kim, 1996). As a result, the overall speech rate and articulation rate of patients with flaccid dysarthria were significantly slower than normal speakers and intra-sentence pause duration and frequency of patients with flaccid dysarthria were significantly higher than normal speakers, but those of inter-sentence weren't. The results from this study provides the speech rate index of flaccid dysarthria and indicates that to control the speech rate of flaccid dysarthria have regard to not only the overall speech rate and the articulation rate but also the intra-sentence pause duration and the frequency.

The effects of speakers' age on temporal features of speech among healthy young, middle-aged, and older adults (연령세대에 따른 말 산출의 시간적 특성: 말속도와 쉼을 중심으로)

  • The purpose of the this study is to observe the effects of healthy adults' age on temporal features of speech and identify which could differentiate older and young adults. We examined speech rates(i.e., overall speaking rate, articulation rate), occurrence of pause, and duration of pause per utterance by utilizing the National Institute of Korean Language's open corpus. We selected a total of 30 healthy adults (10 young, 10 middle-aged, and 10 older adults) in this study. There were significant differences among the groups in the overall speaking rate, articulation rate, total occurrence of pause, the occurrence of pause between syntactic words, total duration of pause, and duration of pause between syntactic words. The older and middle-aged adults showed slower speech rates and longer and more frequent pause than young adults. But there were no significant differences among the three groups in terms of pause within syntactic word. The overall speaking rate significantly differentiated older adults from young adults. These findings suggested that the effect of speakers' age was reflected in gradual changes in the temporal features of their speech.

Influences of Inter-syllable Pause Duration on Speech Discrimination Score in Children with Cochlear Implantation (음절 간 쉼 간격이 인공와우 아동의 어음이해도에 미치는 영향)

  • The aims of this study was to investigate influences of speech discrimination score(SDS) depending on inter-syllable pause duration in participant with child of cochlear implantation(CI). 12 child of CI-user participated. The word for SDS was used self-made meaningless three-syllable. The pause duration of inter-syllable was adjusted to 250, 500, 1,000 millisecond(ms). Discrimination score of closed-set speech was obtained at most comfortable loudness(MCL). SDS were improved in CI group for 62.08, 63.75, 69.58 %, but there were no significant changes in child of CI group(p = .4635). SDS was improved depending on inter-syllable pause duration in child of CI.
