A Comparative Study on The Effect of Cupping and Moxibustion Therapy on Treating Constipation of Stroke Patients (부항-뜸 병행치료와 뜸 단독치료가 뇌졸중 환자의 변비 완화에 미치는 효과 비교)

  • Objectives : This comparative study aimed to identify the effect of the Buddeumi(cupping-and-moxibustion device) therapy and moxibustion-only therapy on treating constipation of stroke patients who were admitted in the hospital of oriental medicine. The research design was a nonequivalent control group pretest-posttest design. Methods : The subjects of the study consisted of 23 stroke patients, and they were divided into two groups : one was the experimental group of 11 patients and the other was the control group of 12 patients. The Buddeumi therapy and the moxibustion-only therapy were given respectively. Each Buddeumi and moxibustion-only therapies on the 天樞(ST25, right & left) in abdomen were administered to the each group for 40 minutes a total at 10 times. The posttest included the frequency of defecation with the Constipation Assessment Scale every day for one week after the experiment. The data were analyzed by the $x^2$ test, t-test, Fisher exact test, and repeated measures ANCOVA using SPSS/WIN 12.0 program. Results : The results were as follows : First, Hypothesis 1 : 'The patients who are given the Buddeumi therapy in the experimental group have a lower Constipation Assessment Scale than the control group.' was supported(F=4.367, P=0.049). Second, Hypothesis 2 : 'The patients who are given meridian acupressure in the experimental group have a higher frequency of defecation per week than the control group.' was supported(F=0.947, P=0.034). Accordingly, 'The patients who are given the Buddeumi therapy in experimental group have a higher effect on constipation than the control group.' was supported. Conclusion : The Buddeumi therapy reduced the constipation assessment scale of stroke patients and increased the frequency of their defacation per week. Therefore, the Buddeumi therapy could be an effective treatment for constipation of stroke patients.

The Effect of Warming Patients Before or During the Surgical Operations on the Patients' Body Temperature and Shivering (수술전 가온과 수술중 가온이 수술환자의 체온과 전율에 미치는 영향)

  • Purpose: The purpose of this study is to examine the changes of surgical patients' body temperature in applying warming to patients. The study of an effective nursing intervention, which aims to prevent hypothermia during surgical operations, use of anesthesia, and to remove dermal discomforts. The nonequivalent control group pre-test/post-test design was used for this quasi-experimental study. Method: The study subjects were adult patients who would take a surgical operation under general anesthesia in C Hospital; the surgical operations done were, total abdominal hysterectomy or Myomectomy; 20 patients were included in experimental group I, 20 patients were included in experimental group II, and 20 patients were in the comparative group. The total number of study subjects was 60. The data was collected from September the 1st, 2001 to October the 20th, 2001. The data was analyzed by SPSS program, F-test and Repeated measures of ANOVA. Multi-comparison method of DUNCAN was used for the sections that show the significant differences at the level of p<.05, which was a posterior examination. Result: 1) "The body temperatures of the three groups of patients will be respectively different at the end of the operations; experimental group I to which warming was applied before the operations, experimental group II to which warming was applied during the operations, and the comparative group with no warming being given," showed (F=12.609, p=.000). 2) "Degrees of shivering symptoms for the three groups will be respectively different at the end of the operations; experimental group I which applied warming before operations, experimental group II which applied warming during operations and the comparative group with no warming." Showed assumed (F=6.626, p=.000). Conclusion: Summing up the above study, the warming assumed during operations was a more effective nursing intervention for preventing patients' hypothermia than the warming assumed before operations.

Factor Analysis of Low Back Pain Patients (일부 요통환자의 인자분석)

  • Objectives: The objective of this study was to investigate factor analysis of low back pain patients. Methods: The data were collected from hospital located in Daegu. Observation was made on 40 low back pain patients and factor analysis study was also performed in terms of their abnormal physical findings. Results: The low back pain patients occupied 20% of all clinical patients in male group and 20% in female group. The occurrence of acute low back pain in the first factor tended to be higher among in male group than among in female group. While it was significantly higher muscle weakness of the back in female group. The most prevailing abnormal finding among low back pain patients in the first factor were tender point on the back, While it was significantly lower Decreased tendon reflex in both sexes Conclusions: The factor pattern of abnormal physical findings in low back pain patients was much different between male and female patients, suggesting the different pattern of etiology. Although low back pain is one of the most common symptoms causing limitation activity, as yet there is no known specific effective treatment.

The comparison about lumbar extensor strength between pre op group and post op(after 6 weeks) group in HIVD patient (만성요통환자의 레이져 수술전과 수술6주후의 요부신전근력에 관한 비교)

  • The purpose of this study were to compare the lumbar extensor strength between pre op patients group and after 6 weeks post op patients group. To evaluate lumbar extensor strength of total 273 patients with HIVD. Lumbar extensor strength was measured in 151 male patients and 122 female patients(Lumbar extensor strength was measured in 91 PELD patients group and 182 OLM group patients) by Medx lumbar extension machine. Maximum voluntary lumbar extension strength was appear $149.36{\pm}61.92ft$-lbs in pre op of PELD group, $158.47{\pm}54.67ft$-lbs in post op of PELD group and $135.54{\pm}54.24ft$-lbs in pre op of OLM group. $147.19{\pm}52.42ft$-lbs in post op of OLM group in male. Maximum voluntary lumbar extension strength was appear $83.85{\pm}30.22ft$-lbs in pre op of PELD group. $92.99{\pm}28.66ft$-lbs in post op of PELD group and $75.16{\pm}24.98ft$-lbs in pre op of OLM group, $79.88{\pm}25.25ft$-lbs in post op of OLM group in female. Male and female lumbar extension strength was statistically significant difference(P<.05). Lumbar flexion/extension ratio of the two group was 2.14:1 pre op and 2.05:1 post op in lumbar flexion 72 and 0 degree. The ratio of post op group was lesser than pre op group.

  • This study was carried out tn invstigate and compare biomechanical characteristics during free speed gait in hemiplegic patients after stroke who took therapeutic exercise by analyzing kinematic and kinetic data in the sagital plane and electromyographic data. Six patients($41\~69$ years old) and age-matched six volunteers in good health(51-61 years old) wire studied. The patients were sorted into two groups, depending on their self-speed of walking : fast speed group(3 patients) and slow speed group(3 patients). The results were as fellows. : 1. In spatio-temparal parameters, affected and unaffected side of fast group showed symetry but blew group showed asymetry of single limb support, opposite foot contact and stance phase (p<0.05). Compared with normal group, patient group showed slower velocity, shoter stride length and longer double limb support (p<0.05). 2. In the pelvic anterior tilt, patient group showed lower valued than normal group. It. In the ground reaction force-vertical force, fast group showed similar double peak gragh compared with normal group, butvslow group showed lower values without double peak (p<0.05).

Clinical Characteristics and Prognosis of Heat Stroke (열사병의 임상적 특징 및 예후에 관한 연구)

  • Purpose: The aim of this study was to evaluate the clinical characteristics of classic heat stroke in Korea and to identify factors of prognosis for heat stroke by comparing a survival group with a non-survival group. Methods: We retrospectively analyzed 27 patients with heat stroke who visited the Emergency Department of Kyungpook National University Hospital from March 2001 to February 2005. First, we divided the patients into two groups, the classic heat stroke group and the exertional heat stroke group, and compared them. Second, we compared the survival group with the non-survival group. Age, sex, cause, place where patients were found, underlying diseases, cooling time, performance of endotracheal intubation, initial Glasgow Coma Scale, initial vital sign, and laboratory findings were reviewed. Results: Five of 27 patients in heat stroke died. The classic heat stroke group had 20 patients. They were old and had more patients in the bathroom than the exertional heat stroke group had. The non-survival group showed lower blood pressure, lower initial GCS score, and higher respiratory rate than the survival group. In laboratory findings, the non-survival group also showed lower$HCO_3-$ level, lower albumin level, lower glucose level, more prolonged PT, and higher CK-MB level than the survival group. Delay in recognition of heat stroke and cooling were poor prognostic factors in heat stroke. Conclusion: The classic heat stroke group had patients who were old and found in the bathroom. Early recognition and treatment of heat stroke is important to reduce mortality. Cooling time, initial GCS score, mean arterial pressure, resipratory rate, $HCO_3-$, PT, CK-MB, and albumin seem to be meaningful when forming a prognosis for heat stroke patients.

A Clinical Study for Recurrence Rate of Chronic Paranasal Sinusitis in Adults (성인 부비동염의 채발율에 대한 임상적 고찰)

  • Objective: To know the recurrence rate of paranasal sinusitis in adults using follow-up investigations. Method: Data was collected from 37 patients who were treated in the Department of Ophthalmology & Otolaryngology & Dermatology, Kangnam Oriental Medical Hospital from 2001-10-01 to 2005-05-31 for chronic paranasal sinusitis in adults over 21 years of age. Results & conclusions: 1. The males made up 51.4% and females made up 48.6%. The recurrence rate was 78.4%. 2. By residence, 78.4% lived in apartments; 21.6% lived in houses. 3. Of the main symptoms, nasal obstruction was the most common. 4. The duration of the disease was within 1 month to 40 years; the group with difficulties within the last 5 years made up the largest group. 5. The patients who had chronic nasal family histories made up 40.5%; Patients who didn't have chronic nasal family histories made up 59.5%. 6. In regards to the period of Tx distribution. the group treated within 4 weeks made up the largest group at 54.1%. 7. As far as methods of Tx distribution, the group of patients treated with herbal medicine and acupuncture were larger (62.2%) than the group treated with only herbal medicine (37.8%). 8. The most common nasal condition symptom in patient histories was chronic rhinitis. 9. The most frequent areas affected by paranasal sinusitis on PNS images were the lateral maxillary sinus, the unilateral maxillary sinus and the ethmoidal sinus. 10. The number of patients who drank little was larger (75.7%) than the number of drinking patients(24.3%); There were more non-smoking patients (78.4%) than smoking patients(16.2%). 11. The patients who slept for 6-7 hours made up the larger group (40.5%). 12. The number of patients who preferred warm water was larger making up 54.1 %; the number of patients that preferred cool water made up 21.6%. 13. The patients who had a common cold 3-5 times a year was the largest group at 54.1%.

Characteristics of Gastric Carcinomas in Two Extreme Age Groups (약년층과 노령층 위암 환자의 비교)

  • Purpose: The aim of this study was to compare the clinicopathological characteristics and postoperative prognosis of two age groups with gastric cancer in order to establish the proper treatment strategy in these groups of patients. Materials and Methods: Among patients with gastric cancer who underwent a gastrectomy between 1990 and 1996 at the Department of Surgery, Kyungpook National University, the clinicopathological characteristics, including the postoperative survival, of patients younger than 40-years old were compared with those of patients over 69-years old. Results: During that period 1,125 gastric cancer patients underwent a gastrectomy. Among them, 111 patients ($9.9\%$) were less than 40-years old, and 96 patients ($8.5\%$) were over 69-years old. In the young group, 79 patients ($71.2\%$) had a poorly differentiated adenocarcinoma, and 59 patients from the elderly group ($61.5\%$) had a well- or moderately differentiated adenocarcinoma (P<0.05). Curative surgery was performed for 88 ($79.3\%$) patients in the young group and 78 ($81.3\%$) patients in the elderly group. This difference was not statistically significant. There was no statistically significant difference in the 5-year survival rates between the young and the elderly ($66.3\%\;vs.\;47.2\%$) groups, but the 5-year survival rate of the young group was significantly higher than that of the elderly group after a curative resection ($83.2\%\;vs.\;53.3\%$; P<0.05). Also the 5-year survival rate of the elderly group was significantly higher than that of the young group after a non-curative resection ($20.4\%\;vs.\;4.6\%$; P<0.05). Conclusions: For young patients, more efforts are needed to detect gastric cancer in its early stages so that a curative surgery can be performed. However, for the elderly, surgical treatment is recommended at any stage because of the relatively high survival rate, even after non-curative surgery.

A STUDY ON THE VERTICAL DYSPLASIA IN THE SKELETAL CLASS III MALOCCLUSION (골격형(骨格型) III급(級) 부정교합자(不正咬合者)의 수직부조화(垂直不調和)에 관(關)한 연구(硏究))

  • This study was designed to analyse vertical dysplasia such as open bite or deep bite in persons with skeletal Class III malocclusion. The subjects consisted of 60 control patients, 40 Class III open bite patients and 40 Class III deep bite patients. The mean age was 19.8 years in the control group, 17.8 years in the Class III open bite group and 16.5 years in the Class III deep bite group. The results were as follows: 1. In Class III malocclusion patients, the characteristics of the vertical dysplasia are under the palatal plane. 2. In Class III malocclusion patients, the items showing the characteristics of the vertical dysplasia are mandibular plane angle, lower gonial angle, lower facial height, dental height & inclination of the upper first molar, interincisal angle, maxillary & mandibular occlusal plane angle. 3. In Class III malocclusion patients, LPFH/LAFH ratio shows the highest significance among the facial height ratios. 4. In Class III malocclusion patients, open bite group has a upward cant of maxillary occlusal plane & downward cant of mandibular occlusal plane. And deep bite group has a downward cant of maxillary occlusal plane & upward cant of mandibular occlusal plane. 5. In Class III malocclusion patients, the molar teeth of the open bite group are measially inclined and those of the deep bite group are upright.

Effects on Blood Flow Velocity, Wound Healing and Pain in Hand Microsurgery Patients Following Heating on Non-affected Side (건측 온열요법이 수부 미세수술 환자의 혈류속도, 상처치유 및 통증에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of heating on the non-affected hand on blood flow velocity, wound healing, and pain for hand microsurgery patients. Methods: This study was designed using the nonequivalent control group pretest-posttest design. Thirty-nine patients were assigned either to the experimental group (20 patients) or control group (19 patients). Data were analyzed with $x^2$-test, Fisher's exact test, t-test, and repeated measure ANOVA using SPSS/WIN 17.0 program. Results: After treatment in this program, blood flow velocity (F=5.13, p=.008) and wound healing (F=4.11, p=.020) improved significantly in the experimental group compared to the control group. But there was no significant improvement in pain in the experimental group compared to the control group (F=2.40, p=.097). Conclusion: Based upon these results, the non-affected side hand heating was recommended as an independent nursing intervention for the patients who need improvement in blood flow velocity and wound healing such as patients who have microsurgery. As the heating was effective even when applied on the non-affected side, it is the applicable to patients who cannot tolerate any therapy on affected side.
