A Study on orthodontic treatment knowledge and attitude among people in general (일반인의 교정치료에 대한 지식수준, 태도에 대한 연구)

  • Objectives : The purpose of this study was to examine the knowledge of people in general about orthodontic treatment and their attitude to that by conducting a survey in a bid to provide information on the improvement of their incorrect awareness of orthodontics and on the development of clinical orthodontic treatment. Methods and Results : The subjects in this study were 389 people in general who resided in Seoul and Gyeonggi Province. A self-administered survey was conducted, and the findings of the study were as follows: 1. As for awareness of orthodontic treatment among the people in general by gender, the women knew better about orthodontic treatment than the men, and the gender gap was significant(p<0.05). 2. Concerning satisfaction with the shape and arrangement of the teeth by age, those who were in their 20s were most satisfied, and the teens were satisfied the least. There were significant gaps according to age (p<0.05). 3. As a result of checking the degree of their inconvenience about the use of chewing teeth by age, the people who were in their 30s felt the most inconvenience, and the gap between them and the others was significant(p<0.01). 4. As a result of asking them whether they had an intention to receive orthodontic treatment, the women had a more intention to do that than the men(p<0.001). By age, the younger respondents had a more intention, and the gaps between them and the others were significant(p<0.001). 5. As a result of analyzing their perception of the positives of orthodontics by gender, the women took a more positive view of it than the men, and their gap was significant(p<0.01). Conclusions : When orthodontic treatment or orthodontic treatment counseling is provided to orthodontic treatment patients or people in general in the field of clinical dentistry, it should be noted that the way patients and people in general look at orthodontic treatment is different according to their age and gender, and they should be informed of the positives of orthodontic treatment to step up the development of orthodontic treatment.

The Effect of the Combined Analgesia of Epidural and Cryoanalgesia after Thoracotomy Surgery (개흉술 후 냉각요법과 경막외 Fentanyl 병용요법의 비교)

  • Background: Thoracotomy is the operation that produces the most postoperative pain, necessitating the highest requirements for postoperative analgesics. The common methods of treating postthoracotomy pain are the use of thoracic epidural analgesia, intemittent or continuous intercostal nerve blocks, intravenous narcotics and cryoanalgesia. We designed to assess the analgesic effect of epidural analgesia, cryoanalgesia and the combined analgesia in thoracic surgery. Methods: A prospective study was carried out in 59 patients undergoing elective thoracotomy for parenchymal disease. Patients were randomized into three groups: C (cryoanalgesia), CE (cryoanalgesia and thoracic epidural analgesia), E (epidural analgesia). All patients had standard anesthesia with endotracheal intubation using a double lumen endotracheal tube, and one-lung ventilation. Subjective pain relief was assessed on a visual analog scale. Analgesic requirements, complications and the degree of satisfaction were evaluated during the 7 days following surgery. Results: Subjective pain relief was significantly better in Group CE and Group E in comparison with Group C (P < 0.05). Cryoanalgesia provided a better pain score on the 6th and 7th POD than the early postoperative periods. Analgesic requirements were higher in Group C than in the Group CE and Group E during the first POD. The incidence of side effects was similar in Group CE and Group E. Conclusions: If we can reduce the concentration of fentanyl and local anesthetics in combined analgesia of epidural and cryoanalgesia, the disadvantages of each method would be overcome and would be a better method of postthoracotomy pain control.

A Research on Anti-diabetic Functional Food intake of the Subjects with type 2 Diabetes mellitus in Daejeon (대전지역 제 2형 당뇨병 환자의 항당뇨기능성 식품 섭취 실태조사)

  • The present study was investigated on type 2 diabetics' actual status of anti-diabetic functional food intake by patients who came to oriental medicine clinics in Daejeon. The male was 37.3% and female was 62.7% of the subjects. The onset of the disease was most common in the age of 50's. Also 40% of the subjects have been suffering for more than 5 years. 35.7% of men, 53.2% for women had family history. Also 61.5% of the patients chose to carry out both diabetic therapy and exercise at the same time. 69.3% of the subjects have had experiences in anti-diabetic functional foods. Anti-diabetic functional foods used to surveyed people were bean, ginseng, and ginseng steamed red of which efficacies were already well known. Besides these foods, other foods such as loach, crucian carp were also used as anti-diabetic foods, but their benefits as anti-diabetic functional foods are not yet fully investigated. Subjects first knew about the therapy because friends or relatives recommended to them and most of them have used for 6 months to 1 year. However, 78.9% of subjects answered 'not so effective' in a question about satisfaction of anti-diabetic functional food. Although many of the patients have tried taking functional foods, they only use them for short period of time and they don't show prominent effect. Therefore these results suggest that in order to use scientifically studied functional foods, education about anti-diabetic functional foods should taken.

The Effects of Paracetamol, Ketorolac, and Paracetamol Plus Morphine on Pain Control after Thyroidectomy

  • Background: The aim of this study was to compare the efficacy of ketorolac, paracetamol, and paracetamol plus morphine on pain relief after thyroidectomy. Methods: Eighty patients were randomly allocated to one of the 4 groups: normal saline (group C), ketorolac 30 mg (group K), paracetamol 1 g (group P), and paracetamol 700 mg plus morphine 3 mg (group PM). Each regimen was administered intravenously (IV) 30 min. before the end of surgery. If pain was not relieved, patients received an IV bolus of pethidine hydrochloride 25 mg. Pain intensity using a visual analogue scale (VAS) was recorded at 0.5, 1, 2, 4, and 6 hr after the end of surgery. Results: VAS at 0.5 and 1 hr after the end of surgery were significantly lower in group K, group P, and group PM than in group C (P < 0.05). The number of patients receiving pethidine hydrochloride at 0.5 and 1 hr after the end of surgery was significantly lower in group K, group P, and group PM than in group C (P < 0.05). There was no significant difference among the groups in the incidences of adverse events associated with study medications and patient satisfaction (P > 0.05). Conclusions: Paracetamol 1 g IV possesses a similar analgesic efficacy to ketorolac 30 mg IV after thyroidectomy. Paracetamol may represent an alternative to ketorolac for pain prevention after mildly to moderately painful surgery in situations where the use of NSAIDs is unsuitable.

Factors Influencing Insufficient Physical Activity in Older Cancer Patients: Using 2014 Survey of Living Condition of Elderly Study (노인 암 환자의 신체활동 부족 영향 요인: 2014 노인실태조사 자료 활용)

  • The purpose of this study was to identify the factors influencing insufficient physical activity in older cancer patients. Data were analyzed from the 2014 Survey of Living Condition of Elderly study. Metabolic Equivalent Tasks(MET) hours were calculated using self-reported weekly frequency, duration and types of physical activities to measure the degree of physical activity. Factors influencing insufficient physical activity in older adults included limitations in ADL and IADL, depression, impaired cognitive function, current smoker, lower levels of social activity participation and life satisfaction with social or leisure activities. When adjusted for demographic factors, patients with ADL limitation had 2.8 times (OR=2.762, CI=1.110, 7.952) higher risks of insufficient physical activities than those without. Current smokers had 2.4 times (OR=2.426, CI=1.113, 5.288) higher risks than non-smokers.

Telehealth for Rural Health Problems: Experiences in The U.S.A and Korea (농촌의 보건의료문제 해결을 위한 원격보건 : 미국과 한국의 경험)

  • Telehealth is widely tested in the U.S.A and other developed countries. This system is expected to solve rural health problems reducing professional isolation. Some demonstration projects showed that the system can provide quality care in reasonable prices to rural residents. However, few study has been done on whether telehealth system can attract physicians by reducing professional isolation. The system is not available to most of the rural hospitals because the price for the equipment and telephone charges are not low enough. It is promising that the system cost and telephone charges are decreasing gradually. As time passes, rural hospitals will be more viable for the system. Satisfaction of the physicians and patients is a key factor for the implementation of the system. The physicians need to understand more about telecommunication and computer systems. After physicians are well-versed about the system, we can expect wide use of telehealth in rural areas. Effort for the confidentiality and standardization should be devoted to assure patient's privacy and compatibility of patient records and exam results. In Korea, two projects are being operated in Uljin and Kurye. A study evaluated the economic efficiency of the projects suggesting that increase of the number of patients up to three times of current number or decrease in hardware costs and telecommunication charges into two thirds of the current costs. The hardware and telecommunication charges are decreasing. Another area telehealth system can be applied is psychiatric accommodation facilities. Establishment of telehealth in the psychiatric facilities will increase the access of psychiatric care for patients and is expected to be economically efficient.

Surgical Outcome of Reconstruction of Neglected Chronic Achilles Tendon Ruptures (진구성 만성 아킬레스 건 파열의 수술적 치료의 결과)

  • Purpose: The purpose of this study was to report the surgical outcome of reconstruction of neglected chronic Achilles tendon ruptures with various methods including Achilles tendon allograft. Materials and Methods: Between October 2003 and November 2008, 8 consecutive neglected chronic Achilles tendon ruptures with the defect gap of more than 4 cm underwent surgical reconstruction including V-Y advancement, gastrocnemius fascial turn-down flap, flexor hallucis longus transfer and Achilles tendon allograft. There were 7 males and 1 female who were evaluated at more than 18 months after surgery. At the time of followup, all patients were assessed with regard to postoperative complications, their self-reported level of satisfaction, the American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot scale, 10 repetitive single heel rise, single leg hopping test, and ankle range of motion. Results: The AOFAS score increased from average 71.4 (50-87) to 96.4 (86-100). All patients were able to perform 10-repetitive single heel raise and single leg hopping at the latest follow up. No patient experienced wound complications and deep infection. Six patients were rated as 'excellent' and the other two as 'good'. Conclusion: Neglected chronic Achilles tendon ruptures could be successfully treated with careful selection of the reconstruction method according to the amount of defect gap. With an extensive defect, Achilles tendon allograft can be a good option when the reconstruction is not feasible otherwise.

Application of 3D printing technology for rehabilitating maxillary defects (악안면 결손 환자에서 3D printing을 이용한 보철 수복 증례)

  • The use of an obturator prosthesis for patients with maxillary defects is a common treatment method to improve their oral function and achieve esthetic satisfaction. However, due to various difficulties and complexities, conventional methods for fabricating dental obturators continue to pose a challenge for dentists and patients, as well as laboratory technicians. CAD-CAM technologies may make it simple to fabricate maxillofacial prostheses including hollow obturators, which could improve comfort for clinicians by reducing burdensome manipulations. In addition, patients without a specialist in their vicinity will be able to be treated via cooperation between a nearby general practitioner and a distant prosthodontist. The aim of this clinical report is to investigate the possibility of using digitally fabricated maxillofacial prostheses that can be designed in one location, and manufactured in another in clinical situations.

A Randomized Clinical Trial of Local Acupoints Compared with Distal Acupoints in Chronic Neck Pain Patients (만성 경항통 환자에 대한 근위취혈과 원위취혈 침치료 효과 비교연구)

  • Objectives : The purpose of this study is to compare the efficacy after acupuncture on local acupoints group and distal acupoints group for chronic neck pain. Design : A randomized, crossover clinical trial. Methods : From 15st, September 2010 to October 30th, 2010. 20 patients with chronic neck pain were randomly assigned to either group A or group B. Group A received acupuncture at local acupoints then after 1 week washout period acupuncture at distal acupoints. Group B received the treatment in reverse order. To evaluating efficiency and satisfaction, visual analog scale(VAS), neck disability index(NDI), cranio-cervical flextion test(C-CFT) and five-point likert scale were measured before and after each treatment. Results : Patients in local acupoints group experienced greater improvement than distal acupoints group in VAS. Both local acupoints group and distal acupoints group showed significant improvement in NDI but not in C-CFT and the NDI score change comparison between the two groups had no significance. Local acupoints group showed more effective than distal acupoints group on five-point likert scale. Conclusions : Local acupoints is more effective than distal acupoints in controlling pain in chronic neck pain.

Predictors of Sick Role Behavior in Patients Receiving Radiotherapy for Cancer (방사선치료를 받는 암환자의 환자역할 행위 예측인자에 관한 연구)

  • Cancer is still a threat to human beings. The incidence and mortality rate of cancer have been gradually increasing as the life span has been lengthened. Radiotherapy is one of the most commonly used treatments for cancer. This study explored the influence of social support and stress on sick role behavior of patients receiving radiotherapy for cancer. The subjects for this study were 60 patients undergoing radiotherapy for cancer, selected from the radiotherapy treatment unit of the out patient departments of two major medical centers in Jeonju. Data were collected from February 1 to 28, 1990 by a Likert Scale Questionnaire and an interview schedule designed by the inverstigator. Data analysis included percentages, mean and standard deviation, t or F-test, Pearson Correlation Coefficient and stepwise multiple regression. Results included the following : 1. Support came primarily from sons and daughters (90.1%) ; the type of support was primarily emotional support from friends(60.0%) ; informational support came from health personnel(81.7%) ; and material support was sons and daughters(40.0%) ; satisfaction with support was highest for the spouse(4.02$\pm$.52). 2. Among the patient's demographic status was occupation the was the only socioeconomic characteristic influencing sick role behavior (F=2.91 , p=.029). 3. Directly perveived support was positively correlated with sick role behavior (r=.2374, p=.034). 4. Stepwise multiple regression was used to determine the predictors of sick role behavior. Directly perceived support was the most significant predictor accounting for the hightest contribution to sick role behavior(5.6%). Directly perceived support. socioeconomic status, perceived stress and indirectly perceived support variables together, accounted for only 6.8% of sick role behavior.
