A comparison of different gingival depigmentation techniques: ablation by erbium:yttrium-aluminum-garnet laser and abrasion by rotary instruments

  • Purpose: The aim of this study is to compare two different gingival depigmentation techniques using an erbium:yttrium-aluminum-garnet (Er:YAG) laser and rotary instruments. Methods: Two patients with melanin pigmentation of gingiva were treated with different gingival depigmentation techniques. Ablation of the gingiva by Er:YAG laser was performed on the right side, and abrasion with a rotary round bur on the opposite side. Results: The patients were satisfied with the esthetically significant improvement with each method. However, some pigment still remained on the marginal gingival and papilla. The visual analog scale did not yield much difference between the two methods, with slightly more pain on the Er:YAG laser treated site. Conclusions: The results of these cases suggest that ablation of the gingiva by an Er:YAG laser and abrasion with a rotary round bur is good enough to achieve esthetic satisfaction and fair wound healing without infection or severe pain. Prudent care about the gingival condition, such as the gingival thickness and degree of pigmentation along with appropriate assessment is needed in ablation by the Er:YAG laser procedure.

A Study on the Design of Hospital Gowns for Child Patients in Korea

  • This study investigated the usage of child hospital gowns to suggest a gown design suitable for child inpatients in Korea. The satisfaction on size suitability and design of child hospital gowns was surveyed among 219 child inpatients in 20 general hospitals. The most hospitals provided 2 to 4 different size pajamas for child inpatients. However, 53% of the subjects chose not to wear the uniform pants that the hospital provided and 28% did not wear the shirts. The reasons most often given for rejecting to wear the uniforms were unsuitable size (42.1%) and difficulty in taking on and off (26.2%). The dissatisfactory parts of hospital gowns were pants length (27.9%), sleeve length (19.6%), pants waist closure (16.9%), and neckline opening (11.9%). The preference in gown design for child inpatients varied by age (p〈0.5) and the styles that they had worn (p〈0.5). Significant differences for style preference were found in the opening of shirts or pants, pants waist closure type, and pants length. These results suggest that the hospital gowns for child inpatients need to reflect the wide diversity in their body size and preference in style.

Scarf(Z) osteotomy of the First metatarsal shaft in the Treatment of Hallux Valgus (무지 외반증 환자에서 스카프 절골술을 이용한 치료)

  • Purpose: The purpose of this study was to the evaluate the clinical and radiographic results and complication of the Scarf osteotomy which is one of the surgical treatment of Hallux valgus. Material and Method: Fifty feet (30 patients) of moderate to severe hallux valgus deformity were operated by Scarf osteotomy procedure from January 2002 to July 2002. Patients were asked about their satisfaction in terms of pain, appearance and motion. The AOFAS clinical ratio scales and radiographic results for hallux valgus were evaluated. Result: The mean Hallux valgus angle improved from an average of 30.6 degrees to 9.0 degrees and the first intermetatarsal angle improved from an average 13.3 degrees to 5.2 degrees respectively. The preoperative mean AOFAS score was 58 points and on last follow up AOFAS score improved to 88 points. Complication which including troughing, nonunion, malunion was not developed. Conclusion: We found that Scarf osteotomy is a reliable operation for mild to moderate Hallux valgus deformity as regards firm fixation and early postoperative ambulation and large contact area for union.

Itching among Burn Patients in the Rehabilitation Phase (재활단계 화상 환자의 가려움증)

  • Purpose: The purpose of this study was to investigate post-burn itching relevant characteristics in the rehabilitation phase. Methods: Ninety four burn patients who were hospitalized in the rehabilitation phase participated in the study. Self-reported Leuven itch scale-Korean was used to evaluate itching related characteristics. Data were analyzed with descriptive analysis, $x^2$ test, and Kruskal-Wallis test. Results: All subjects except two had the itching. Itching frequency revealed that 68.1% of subjects experienced itching one or more episodes a day or always, and 18.7% had episode durations lasting more than 2 hours per incidence. Mean severity and distress of itching was $5.3{\pm}2.4$ and $5.2{\pm}2.5$, respectively. Itching sensation dimension of tickling was 56.4%. More than 25% of subjects "often" or "always" affected reduced quality of life, loss of concentration, bad mood, and difficulty in falling asleep. Satisfaction with itching treatment was intermediate level. Conclusion: Post-burn itching in the rehabilitation phase was a significant problem despite pruritus treatment. Based on the finding of this study, development of comprehensive and tailored nursing intervention programs for itching could be suggested.

The Clinical Study on the Effect of Hominis Placenta Pharmacopuncture on Asthma (자하거 약침의 천식 환자에 대한 연속증례보고)

  • Objectives The purpose of this study is to report the effect of Hominis Placenta Pharmacopuncture(HPP) for patients with asthma. Methods We treated three patients who have asthma with HPP. To check the effect of HPP we used CAMSOM-V. To check the satisfaction of HPP we used Five-point Likert scale. Results After treatment symptoms were improved. CAMSOM-V decreased in all cases. The Likert scale points were checked 5 and 4 and 4. Conclusions In this study, we have found the meaning of the research that HPP, in the future will be efficacious against asthma.

  • The purpose of this study is to the improvement of general hospital outpatients waiting time using Theory Of Constraints(TOC) methodology and to the development of a Reception Desk in general hospital. This study is to provide decision-making guidelines for hospital managers and to provide feedback for the efficiency of job process. The target people of the study are outpatients and Cashiers on Chungnam national university hospital in Daejeon. The methods of study are summarized as follows. First, The team managers from a Reception Desk group were appointed. This team managers have the adjustment authority to the Outpatients schedule of doctor and Cashier members. Second, The consolidation of the general Reception desk and special inspection the Reception Desk. A movement line and waiting time of patients were simple and fast to accept. As a result of study, it shows that the TOC is the method for a job process and waiting time improvement, patients' satisfaction increase and we need an objective measurement indexes in the medical treatment industry.

  • Purpose: The aim of this study was to explore the relationship between nursing work environment and nursing outcome of clinical nurses. Also, the purpose of this study was to identify the nurses' workload. Method: The participants in this study were 246 nurses working at one hospital in a province. Data was collected using a structured questionnaire from October 2011 to November 2011. Open questions were subject to content analysis; closed questions were subject to descriptive statistics and Pearson correlation coefficient with SPSS 18.0 program were used. Result: The number of patients per nurse in day-duty and overtime work had a positive correlation. Nursing performance and the overtime work in night duty nurses had a negative correlation. The results of this study, with a much higher number of patients per nurse, showed an increase in overtime hours. In the nursing delivery system experience of nurses, 13 themes were identified and they were divided into five categories. Conclusion: Therefore, in order to improve nursing performance and increase job satisfaction, efforts must be made to reduce overtime, and nursing personnel placement to secure a substantial nursing staff.

Usefulness of Balloon-type Hemostatic Device After Transarterial Chemoembolization (간동맥 화학색전술 후 풍선형 지혈기구의 유용성)

  • Transarterial chemoembolization is one of the most representative procedures for puncture of the femoral artery. In addition, the same procedure is often repeatedly performed many times, and Hepatocellular carcinoma patients due to cancer is significantly lowered blood tests, regardless of platelet counts are not good enough to stop bleeding. More importantly, hepatocellular carcinoma has a high degree of complication and disease severity, which makes it less likely that the condition of the body will be relatively inferior to other patients. In order to prevent delayed hemorrhage of the femoral artery puncture site after the procedure, it is advised to absolutely stabilize the limb so that it does not bend the limb for 3 hours after climbing in the ward. Therefore, I have been complaining about inconvenience. In addition, in order to prevent delayed hemorrhage after hemostasis, balloon type hemostatic device was used instead of sand bag which was placed on hemostatic site. The results of this study were compared with the results of actual application. The use of a balloon-type hemostatic device to increase the effectiveness of continuous hemostasis and to minimize the inconvenience during the time of patient's absolute bed rest, rather than raising the sandbag to prevent primary delayed hemorrhage by various methods in transarterial chemoembolization. It can be used as a substitute for existing sand bags because it can alleviate pain, increase satisfaction, and can be used as a disposable one.

A Concept Analysis on Patient-Centered Care in Hospitalized Older Adults with Multimorbidity (복합질환을 가진 입원노인 대상 환자중심간호 개념분석)

  • Purpose : The aim of this study was to explore the attributes, antecedents, and consequences of patient-centered care (PCC) for older adults with multimorbidity in acute care hospitals. Methods : The concept analysis performed by Walker and Avant was used to analyze PCC. Fifteen studies from the literature related to PCC appear in systematic literature reviews in the fields of theology, medicine, psychology, and nursing. Results : PCC in acute care hospitals was defined according to the five attributes of 'maintaining patient autonomy', 'empowering self-care', 'individualized and relationship-based care', 'shared decision-making', and 'creating a homelike environment'. Antecedents of PCC were found to be a respect for patients' preferences, qualifications of the nursing staff, care coordination and integration, and organizational support. Consequences of effective PCC were a functional status; health-related quality of life; satisfaction with care, mortality, and medical costs from the perspective of the patient and family; and quality of care and therapeutic relationships from nurses' viewpoints. Conclusion : PCC as defined by the results of this study will contribute to the foundation of institutionalization and the creation of a safe and healthy acute care hospital culture focused on patients' preferences and values.

Effect of Fascial Distortion Model on the Cobb's Angle in Adolescents with Idiopathic Scoliosis

  • Purpose: This study examined the effects of the fascial distortion model (FDM) method on Cobb's angle. Methods: Fourteen subjects participated in this study. The idiopathic scoliosis group performed FDM treatments eight times in four weeks. A Shapiro-Wilks test was used to verify the normality of a group of idiopathic scoliosis patients. A paired t-test was performed to determine the satisfaction with the normality. Results: The difference in Cobb's angle was reduced significantly (p<0.05) by an average of $5.72^{\circ}{\pm}2.24$ from $15.51^{\circ}{\pm}1.81$ before the experiment. Conclusion: This study found that the use of the FDM method was effective in improving the Cobb's angle. The application of a FDM treatment appears to stabilize the fascia with decreased ability to adapt physiologically. The application of the CD and TB method of FDM treatment improved the asymmetry spine by dispersing moisture in fascia and improving the contraction and relaxation ability. The results suggest that the Cobb's angle is reduced when FDM is applied, thereby preventing the progression of curvature and avoiding psychological and physical problems that can arise from improving patients with scoliosis.
