중소병원 간호사의 환자안전인식이 안전간호활동에 미치는 영향 (The Influences of the Awareness of Patient Safety Culture on Safety Care Activities among Nurse in small-medium Sized General Hospitals)

  • 본 연구는 중소병원의 간호사의 환자안전인식 및 안전간호활동을 조사하고 안전간호활동에 영향을 미치는 요인들을 분석함으로써 보다 안전한 의료서비스 제공의 기초자료로 삼고자 시행 되었다. 대상자는 2012년부터 4월 1개월 동안 3개의 중소병원 간호사 344명을 대상으로 실시하였다. 미국의 AHRQ에서 개발한 Hospital survey of patient safety culture라는 설문지와 의료기관인증문항 중 안전보장활동을 사용하였다. 자료분석은 SPSS 18.0 통계 패키지를 사용하여 기술통계 t-test 및 ANOVA와 Scheffe 사후 분석, Pearson's correlation coefficents, stepwise multiple regression으로 분석하였다. 환자안전인식 수준은 간호사의 경력, 근무시간, 근무부서에 따라 차이가 있었고, 안전간호활동은 간호사의 나이, 직위, 결혼, 간호사 경력, 근무시간에 따라 차이가 있었으며, 간호사의 안전인식수준이 높을수록 안전간호활동 수준이 높았다. 병원에서 환자안전인식 및 안전간호활동 수준을 개선하기 위해서 환자안전정책 및 근무환경개선제도가 요구된다. 근무조건 개선을 위해서 근무시간, 환자대 간호사의 비율을 정규화하는 정책 등이 요구된다.

환자안전에 관한 문헌 연구: 국내연구를 중심으로 (A literature review of patient safety in Korea: focused on domestic studies)

  • Objectives: The purpose of this study is to provide basic data on the continuous management and institutional measures in the future by understanding the research trends of patient safety in healthcare field. Methods:The data were extracted from 2011-2016 KoreaMed, KMBase, KISS, NDSL and KISTI. Data were analysis by frequency analysis using the SPSS 14.0 program. Results: 87.0% of the studies were quantitative studies. As for the method of sampling, 'No use' was the highest at 56.5%. Most of the participants in the study were 'nurses' (50.7%). 19 hospitals (35.8%) were the most common. The subjects of the study consisted of 35 (51.5%) patients' safety culture (awareness) and 20 (29.4%) 'safety nursing activities'. Conclusions: Patient safety and patient safety should be maintained. Further, a mature patient safety culture should be settled through cooperation management among medical staff.

병원 간호사의 환자안전관리 중요성 인식, 전문직 자율성 및 안전분위기가 환자안전간호활동에 미치는 영향 (The Effect of Perception the Importance of Patient safety Management, Professional Autonomy and Safety Climate on Patient Safety Nursing Activity on Hospital Nurses)

  • 본 연구는 병원 간호사의 환자안전관리 중요성 인식, 전문직 자율성 및 안전분위기가 환자안전간호활동에 미치는 영향을 파악하기 위한 서술적 조사연구이다. 종합병원 간호사 211명을 대상으로, 2021년 1월 11일부터 1월 25일까지 자료수집이 이루어졌다. 자료분석은 SPSS 21.0 program을 이용하여 독립표본 t-검정, 일원분산분석, 피어슨상관계수, 단계적 회귀분석으로 하였다. 병원간호사의 환자안전간호활동 영향요인은 안전분위기(β=.297, p<.001), 환자안전관리 중요성 인식(β=.287, p<.001), 근무부서(β=.169 p=.004)이었다. 연구결과를 종합해 볼 때, 환자안전간호활동의 증진을 위해서는 간호사의 근무부서의 업무고려 및 다양한 특성을 고려한 교육프로그램 개발 및 적용, 환자안전관련 중요성 인식의 향상, 긍정적인 안전분위기의 형성이 이루어져야 한다. 본 연구는 병원 간호사의 환자안전간호활동 증진의 방안마련에 토대가 되는 자료를 제공한다는 점에서 그 의의가 있다.

조직안전문화가 방사선사의 환자 안전에 대한 의식 및 안전 행동에 미치는 영향분석 (Analysis of the Impact of Organizational Safety Culture on Radiology Technologists Awareness of Patient Safety and Safety Behavior)

  • 본 연구는 조직 안전 문화가 방사선사의 환자 안전에 대한 인식도와 안전 행동에 미치는 영향을 살펴보고자 진행된 연구이다. 연구는 설문조사 결과를 바탕으로 진행하였으며 설문조사는 일반적 특성, 환자 안전을 위한 노력, 환자 안전 문화의 3개 요인으로 나누어 진행하였다. 연구 결과 환자 안전에 대한 인식의 차이를 나타내는 요인은 일반적 특성에서 연령, 업무경력, 업무에 대한 만족도가 영향 요인으로 나타났고 성별이나 학력, 근무 형태는 환자 안전에 대한 인식도의 차이에 영향을 주는 요소로 포함되지 않았다. 환자 안전을 위한 노력으로는 병원의 지원 및 교육, 환자 안전 활동 및 오류에 대한 대응 방식 모두 영향 요인으로 나타났으며 특히 오류에 대한 대응 방식이 다른 항목에 비하여 더 높은 연관성을 나타내었다. 환자 안전 문화는 팀워크와 개방적인 의사소통 두 가지 항목에 대해 분석하였으며 연구 결과 두 항목 모두 방사선사의 환자 안전에 대한 인식도와 높은 관련성을 나타내었다.

한국 노인요양시설의 환자안전문화 조사 연구 (Measuring Patient Safety Culture in Korean Nursing Homes)

  • Purpose: The purpose of this study was to investigate the patient safety culture in Korean nursing homes using the Nursing Home Survey Patient Safety Culture (NHS-PC), a valid tool, provided by the Agency for Healthcare Research and Quality (AHRQ) and to compare the results with AHRQ data. Methods: Administrators and staff (N=151) of six nursing homes in Seoul, Busan, Kyeonggi Province and Gyeongsangnam Province completed the survey in July, 2010. The data were analyzed using descriptive statistics, positive response rate, t-test, ANOVA, DUNCAN, Cronbach's ${\alpha}$. Results: The total mean (SD) positive response rate for patient safety culture was not significantly different from the AHRQ data. For composite levels, the results of 'handoffs' were significantly higher, and the results of 'feedback and communication about incidents' and 'nonpunitive responses to mistakes' were significantly lower than the AHRQ data. Conclusion: More effective strategies related to nonpunitive responses to mistakes and management activities for patient safety are needed to improve patient safety culture in nursing homes.

중소병원 간호사의 직무 스트레스가 환자안전간호활동에 미치는 영향 (The Impact of Job Stress on the Patient Safety Nursing Activity among Nurses in Small-Medium Sized General Hospitals)

  • Purpose: The purpose of the study is to examine the impact of job stress on the patient safety nursing activities among nurses. Methods: The subjects of the study are 258 nurses working at 15 small-medium sized hospitals in D city. Data analysis was done using frequency, percentage, average and standard deviation, t-test, one-way ANOVA, $Scheff{\acute{e}}test$, Pearson's correlation coefficient, stepwise multiple regression. Results: The job stress scored average 3.67 and patient safety nursing activity scored average 4.35. Job stress was positively associated with patient safety nursing activity. It explains 9.49% of the variance. Conclusion: Universal and inevitable job stress among nurses should not exceed optimum level to assure patient safety. So, personal and organizational efforts to manage job stress are needed.

환자안전사건 분석을 위한 한글 근본원인분석 소프트웨어 개발 (Development of Korean Root Cause Analysis Software for Analyzing Patient Safety Incidents)

  • Purpose: The purpose of this study is to develop the Korean root cause analysis (RCA) software that can be used to systematically investigate underlying causes for preventing or reducing recurrence of patient safety incidents. Methods: We reviewed the existing guidelines and literatures on the RCA in order to figure out the RCA process. Also we examined the existing RCA softwares for investigating patient safety incidents to design the contents and interface of the RCA software. Based on the results of reviewing literatures and softwares, we developed a draft version of the Korean RCA software that can be easily used in Korean hospital settings by RCA teams. Results: The Korean RCA software consisted of several modules, which are modules for identifying patient safety incidents, organizing RCA team, collecting and analysing data, determining contributory factors and root causes, developing the action plans, and guiding evaluation. Conclusion: The Korean RCA software included optimized RCA process and structured logic for cause analysis. Thus even beginners in RCA are expected to easily use this software for investigating patient safety incidents. As software has been developed with the public financial support, it will be distributed free of charge. We hope that it will contribute to facilitating patient safety improvement activities in Korea.

수술실 간호사가 인지하는 조직건강, 안전분위기, 간호근무환경이 환자안전관리활동에 미치는 영향 (The Effects of Operating Room Nurses' Perceptions of Organizational Health, Safety Climate, and the Nursing Working Environment on Engagement in Patient Safety Management Activities)

  • Purpose: This study aimed to examine the relationship between organizational health (OH), safety climate (SC), the nursing working environment (NWE), and engagement in patient safety management activities (PSMA) among operating room nurses and identify the factors that predict engagement in PSMA. Methods: From August 10th to 25th, 2018, 176 operating room nurses who were working in tertiary and general hospitals responded to a structured questionnaire. Using SPSS/WIN 25.0, the collected data were subjected to independent-samples t-test, one-way analysis of variance, Scheffe?test, and Pearson's correlational and multiple stepwise regression analyses. Results: OH and SC were significantly correlated with engagement in PSMA. The factors that predicted engagement in PSMA were OH, NWE, participation in accreditation, years of work experience, and hospital size; together, they explained 17% of the variance in engagement in PSMA. Conclusion: This study revealed that OH has a significant influence on engagement in PSMA among operating room nurses. Therefore, hospitals should aim to create healthy working environments to promote engagement in PSMA among operating room nurses, actively delegate responsibilities to increase their level of participation in accreditation, and implement strategies that maintain high levels of nurse retention.

임상간호사의 셀프리더십, 자기효능감, 조직몰입이 안전관리활동에 미치는 영향 (The Effects of self-leadership, self-efficacy, and organizational commitment on safety Management activities in clinical nurses')

  • 본 연구는 종합병원 간호사를 대상으로 셀프리더십, 자기효능감, 조직몰입이 안전관리활동에 미치는 영향을 파악하기 위한 서술적 조사연구이다. 연구대상은 G시에 위치한 3개의 종합병원 간호사로 코로나19 펜데믹 상황임을 고려하여 온라인 설문을 실시하였다. 자료수집은 2021년 8월 20일부터 8월 30일이었다. 연구결과, 셀프리더십(r=.618, p<.001), 자기효능감(r=306, p<.001), 조직몰입(r=0518, p<.001)은 안전관리활동과 양의 상관관계를 보였다. 또한 셀프리더십, 자기효능감, 조직몰입이 안전관리활동을 38.2%로 설명하였다 (F=20.478, p<.001). 그러므로, 종합병원간호사의 환자관리활동을 높이기 위해 정기적인 리더십 교육, 자기효능감 교육 등을 실시한다면 도움이 될 것이다. 또한 조직몰입 향상을 위해 긍정적인 업무환경 조성과 간호사 업무에 대한 법적 기준 마련, 적절한 보상 체계의 마련이 된다면 환자안전관리활동에 긍정적 효과가 있을 것이다.

QI전담자의 주요 업무 및 역할 규명 (The Task and Role of the Quality Improvement Facilitator)

  • Objectives: To outline overall duties of quality improvement (QI) performers within a health care organization, thus describing their key tasks, including task element-related frequency, importance and difficulty in enough detail. Methods: A DACUM (Developing A CurriculUM) workshop took place to outline overall job activities of QI performers. To examine the scope of their duty and task, we performed a questionnaire survey of 338 QI performers from 111 hospitals. Results: The results of our survey showed that for the task assigned to each QI performer, there were 10 duties, 31 tasks and 119 task elements. Respondents cited a project planning as the most frequent/important duty, and a research was the highest level of difficulty in their duty. They also said that the most frequent task was index management, the most important task was a business plan, and the highest level of difficulty was a practical application of QI research. QI performers added that the most frequent task element was receipt of patient safety reporting in patient safety system, the most important task element was an analysis for patient safety and its improvement, and the highest level of difficulty was a regional influence analysis related to the patient safety and its improvement. Conclusion: To ensure that QI performers play a pivotal role as a manager to better improve patient safety and the quality of health care services, proper training program for them should be developed by reflecting the results of our study.
