입원 말기암 환자가 지각한 가족지지와 삶의 질의 관계 (Relationship between Perceived Family Support and Quality of Life in Hospitalized Patients with Terminal Cancer)

  • Purpose: This study was to identify the relationship between perceived family support and quality of life in hospitalized patient with terminal cancer. Method: Study subjects were 104 patients with terminal cancer who were hospitalized and treated at K university hospital, in Busan. Perceived family support and quality of life were measured using the Kang's Revised Family Support Scale and Youn's Quality of Life scale for terminal patients. Results: 1) The mean score of perceived family support was $4.23{\pm}0.61$. The mean score of quality of life was $5.83{\pm}1.37$. 2) The perceived family support was significantly different with primary care giver, food type, medical period after diagnosis. 3) The levels of quality of life was significantly different by number of children, effect of religion on the one' life, perceived state of disease and pain. 4) There was moderate positive correlation between perceived family support and quality of life. Conclusions: Increase in perceived family support was associated with increase in quality of life in hospitalized patients with terminal cancer. It is necessary that the development of nursing education program for family which help to support the patient with terminal cancer for increasing the quality of life of patient with terminal cancer.

뇌졸중 환자의 인두기능 회복을 위한 전기자극기 설계 (Electrical Stimulation System Design for Pharyngeal Dysfunction of Stroke Patients)

  • The purpose of this study is to design electrical stimulation system fur stroke patients with pharyngeal dysfunction(dysphagia). Dysphagia is that the food has trouble in passing to the mouth from the stomach. Although we have many treatment methods for dysphagia, electrical stimulation system will be useful for stroke patients having dysphagia. Electrical stimulation system can be divided into body and electrodes. The body stimulation is composed to frequency counter, time control and current measurement part. These parts are to control the frequency, stimulating time and current intensity And they can be variable according to the patient's clinical assessment. The electrode plays a role to deliver the current from the system to the muscle. Also the position of the electrode can be variable according to the treatment method. We performed the clinical experiment with stroke patients who had swallowing disorder. The videofluoroscopy was used fur the observation. From the result of clinical experiment based on electrical stimulation, we expected that the dysfunction(in pharynx) level of the patient could be improved. However we could not have enough effectiveness of the treatment because of the number of patients, patient's adaptation and treatment period. We will design the optimized electrical stimulation system based on enough clinical experiment in the future.

측두근의 외상성 골화성 근염 (Myositis Ossificans Traumatica in the Temporalis Muscle)

  • Myositis ossificans is a condition characterized by ossification within a muscle. It is a rare and unusual pathologic entity that has defied medical efforts to establish a definite etiology, pathogenesis, and satisfactory treatment of the disease. The condition predominantly affects the flexor muscles of the upper limbs and thighs, but rarely the head and neck area. A 53-year-old male patient visited our medical institution complaining of trismus, defined as limited mouth opening. The patient had a history of trauma to the facial bones and the computed tomography scans revealed calcification in the left temporalis muscle. The patient underwent surgical removal of the calcified mass with bilateral coronoidectomy under general anesthesia. Mouth opening at the end of post-operative 2 months was 28 mm. His oral intake of food was satisfactory. Myositis ossificans of the temporalis muscle is a very rare case. Satisfactory outcome was obtained by combining surgical excision of the affected muscle, coronoidectomy, and detachment of the insertion site of the ossified muscle.

End-of-Life Assessments and Communication for Dying Patients and Their Families

  • End-of-life assessments aim to help dying patients and their families plan clinical interventions in advance and prepare them for a peaceful end of life, in which the patient accepts life and death, and the family accepts the patient's departure. It is important to assess whether death is imminent within a few days, because critical hospice care is provided intensively during that period. The following five changes constitute objective evidence of the end of life: diminished daily living performance, decreased food intake, changes in consciousness and increased sleep quantity, worsening of respiratory distress, and end-stage delirium. As subjective evidence, it is suggested that sensitive perceptions of experienced nurses and the feelings of family members caring for patients should also be considered. When notifying a patient or family members that the end of life is approaching, the members of the multidisciplinary hospice team must communicate with each other, share accurate information, and provide consistent explanations. They must also listen to non-verbal communication in an empathic and supportive manner.

환자의 신약 접근성 강화 정책 제안 (Policy Suggestions to Improve Patient Access to New Drugs in Korea)

  • Objective: This study aimed to overview and assess the effectiveness of the policies and regulations that have governed new drug access in Korea, and to propose policies to enhance patient access to drugs, particularly for new innovative medicines. Methods: We approached drug access issues in two perspectives: approval lag (or availability) and reimbursement lag (or affordability). The issues were identified and evaluated through the review of literature, public documents, reports published by the government agencies and private organizations, and news articles. Results: To shorten approval lag, it is recommended to hire and train more reviewers at the Ministry of Food and Drug Safety. Increasing user fees to a realistic level can facilitate this process. To reduce reimbursement lag, flexible incremental cost-effectiveness ratio threshold, alternative cost-effectiveness evaluation, and establishment of funding source other than the national health insurance are identified as the areas to be improved. Conclusion: The current policies and regulations had to be supplemented by new systems to drastically promote patient accessibility to new drugs, consequently in order to promote national public health.

연령에 따른 환자의 병원음식 만족도 (Degree of Satisfaction on Hospital Foods by Patients according to Age)

  • The purpose of this study was to analyze the patient's satisfaction on hospital foods according to their age. The subjects were selected among the patients treated in the Eulgi University hospital during May of 2003. Two hundred twenty one patients (110 men, 101 women) were surveyed through questionnaires in terms of satisfaction on hospital foods and the food preference. The data were statistically analysed using $\chi$$^2$-test and their correlation. Satisfaction on general taste(p<0.05) and variety of hospital foods (p<0.05) were significantly different according to age. However, saltiness and serving temperature of hospital foods were significantly different according to sex(p<0.05). The preference of cooking method for meat, fist vegetable and the preferred kinds of Kimchi(p<0.05) and milk & milk products(p<0.05) were significantly different according to age.

후두개절제환자에서 시술한 인두위문합술 -1예 보고- (Pharyngogastrostomy in an Epiglottectomized Patient -A Case Report-)

  • The patient was 21-year old male who had gastrostomy and tracheostomy after swallowing lye-stuff in July 1971. He could restore his normal voice and breathing after removal of his destructed epiglottis obstructing his upper airway two years later. Pharyngogastrostomy was performed in Nov 1973. The esophagus which was totally obliterated in its full length was removed and the stomach was brought high up to the level of pharynx where it was anastomosed to the posterior wall of pharynx. His postoperative course was temporarily complicated by aspiration of small food into trachea which could be completely relieved with training, and he is doing his normal life quite well on the follow-up.

Mandibular Reconstruction using Simulation Surgery with 3D RP Model in Osteoradionecrosis Patient: A Case Report

  • One of the most serious complications after head and neck radiation is osteoradionecrosis (ORN) of the jaw. The etiology of ORN is extraction, minor dental procedure or dental implant surgery. When ORN of the jaw progressed to stage III, free fibular flap is the most useful methods for reconstruction. In this case report, a 67-year-old ORN patient who underwent fibular free flap reconstruction using simulation surgery with 3-dimensional rapid prototype (3D RP) model was reviewed. After partial mandibulectomy, a osteocutaneous fibula flap was used for reconstruction. Oro-cutaneous fistula was resolved after operation. Patients reported improved food intake after operation without pus discharge. Functional and esthetic results showed successful reconstruction.

Efficacy of Korean Red Ginseng in the Treatment of Alopecia Areata

  • Alopecia areata (AA) is an autoimmune disease that can affect any hair-bearing area. AA is known to be caused by immunological disorder but still, the pathogenesis is not fully understood. Many therapeutic modalities have been used to treat alopecia areata, with variable efficacy and safety profiles. Unfortunately, none of these agents is definitely curative or preventive alone. We studied hair growth efficacy and safety of Korean red ginseng (KRG) in AA comparing corticosteroid intra-lesional injection (ILI) alone patient group with ILI with KRG taking patient group using Folliscope 2.5 for 12 wk. Herein, we would like to report the efficacy of KRG in the treatment of AA and recommend KRG as a useful complimentary food for gaining efficacy of treatment for AA.

급체 환자의 심박변이도 특성에 대한 임상적 연구 (Clinical Study for Characteristics of Heart Rate Variability in Acute Dyspepsia Patients)

  • Objectives: This research was designed to study the characteristics of heart rate variability in acute dyspepsia patients compared with normal group. Methods: The testing of HRV was carried out at Oriental Medical Center of ${\bigcirc}{\bigcirc}$University with the participation of 94 acute dyspepsia patients and 85 control group people. We checked HRV of the 2 groups that were checked on the first visit day and compared HRV index between groups. Results: In the frequency domain analysis, HF, LF, VLF, LF/HF ratio, TP were significantly lower than control group on acute dyspepsia patient group. HF, LF, VLF, LF/HF ratio, TP were not significantly associated with age. Conclusions: According to this study, autonomic nerve system was more decreased on the acute dyspepsia patient group compared with the control group. Though further studies will be needed, the present finding provide us the possibility that stress is a major cause of the acute dyspepsia along with food factor.
