종합병원 간호사의 환자안전문화인식과 조직몰입이 환자안전관리활동에 미치는 영향 (The Effects of Patient Safety Culture Perception and Organizational Commitment on Patient Safety Management Activities in General Hospital Nurses)

  • 본 연구는 종합병원 간호사의 환자안전관리활동에 환자안전문화인식과 조직몰입이 미치는 영향을 검증하고자 시도되었다. 자료는 2017년 8월 2일부터 8월 16일까지 종합병원 간호사 191명을 대상으로 구조화된 설문지를 이용하여 자료수집하였고, SPSS WIN Version 22.0을 이용하여 분석을 실시하였다. 연구결과, 종합병원 간호사의 환자안전관리활동은 환자안 전문화인식과 조직몰입과는 양의 상관관계가 있는 것으로 나타났고, 환자안전관리활동의 영향요인은 환자안전문화인식, 조직몰입 및 환자안전교육 경험으로 나타났으며, 이들 변수의 설명력은 33.3%였다. 따라서, 종합병원 간호사의 환자안전관리 활동을 증진시키기 위해서는 환자안전문화인식, 조직몰입 및 환자안전교육을 고려하여 중재프로그램을 개발하고 적용하는 것이 필요하다고 사료된다.

간호대학생의 환자안전관리활동에 관한 연구 (A Study on Patient Safety Management Activity for Nursing Students)

  • 본 연구는 간호대학생의 환자안전관리활동에 영향을 미치는 요인을 파악함으로써 간호교육 내 환자안전에 대한 기초 자료를 제시하고 환자안전관리활동 수준 향상 교육 프로그램을 개발에 필요한 근거를 마련하고자 수행되었다. 본 연구는 간호대학 3-4학년 358명을 대상으로 하였으며 자가보고식 설문지를 이용하여 자료수집 하였다. 자료분석은, 기술통계, t-test, ANOVA, 상관분석과 단계별 다중회귀분석을 시행하였다. 연구결과 환자안전역량, 수행자신감, 대처 능력은 간호대학생의 환자안전관리활동과 유의한 긍정적인 상관관계를 나타내었다. 회귀분석 결과, 간호대학생의 환자안전관리활동에 대한 유의한 영향요인은 환자안전역량, 수행자신감으로 나타났다. 본 연구에서 대처는 간호대학생의 환자안전관리활동에 영향을 주지 않는 것으로 나타났으나, 환자안전역량, 수행자신감은 간호대학생의 환자안전관리활동에 영향을 미치는 유의한 예측변수로 나타났다. 따라서 간호대학생의 환자안전관리활동 증진을 위해 환자안전역량, 수행자신감을 고려한 교육 중재 프로그램을 개발할 것을 제안한다.

한국 노인요양시설 실무종사자들이 인식하는 환자안전문화와 환자안전도 (Perception of Workers on Patient Safety Culture and Degree of Patient Safety in Nursing Homes in Korea)

  • Purpose: The purpose of this study was to investigate provide basic data for improving patient safety in nursing homes in Korea by measuring the patient safety culture of nursing homes and understanding its influencing factors. Methods: This study is a secondary analysis of a descriptive research study using data from development and validation of the Korean patient safety culture scale for nursing homes. A total of 982 cases were analyzed using the SPSS Statistics 20 program. Results: For the safety culture of the patient, there was a significant difference based on the size and location of the facility. For the degree of patient safety, age, education, occupation, marital status, and the size of the nursing home were significant factors. Patient safety culture and the degree of patient safety had a positive correlation. The regression model of the degree of patient safety was significant (F=20.73, p<.001) and the explanatory power of the model was 27.4%. Conclusion: The study results indicate that patient safety culture is a factor influencing safety of elders in nursing homes. To improve patient safety for nursing homes in Korea, continuous evaluation and improvement projects need to be done at a national level.

병.의원의 의료서비스와 환자복에 대한 소비자 만족 (Consumer Satisfaction with Medical Services and Hospital Patient Gowns)

  • This study identifies consumer satisfaction with medical services and the patient gowns of hospitals. Also analyzed are the elements that influence hospital satisfaction and the general satisfaction with patient gowns. A survey was conducted among Korean male and female ex-patients regardless of their age. A total of 513 responses were analyzed using descriptive statistics, paired t-test, correlations, regression, and factor analysis derived from data collected in April and May, 2009. The results are as follows. The general satisfaction with the hospital recently visited was higher than the general satisfaction with all of the hospitals visited. The satisfaction with medical services were high in good services, trustful medical examinations, easy access to utilities, and clean utilities. Hospital satisfaction was determined by good services, trustful medical examinations, easy access to utilities, patient gowns, and fresh indoor air. The satisfaction factors of general patient gowns were determined as functionality, fabric/design, sewing, and the management system factors. The most important factor explaining patient gown satisfaction was fabric/design, and this was the most unsatisfied factor at the same time.

군병원 응급실 간호서비스에 대한 환자만족도 (Patient's Satisfaction with Nursing Care Services in Military Hospital Emergency Department)

  • Purpose: The purpose of this study is to provide basic data for the improvement of the quality in the emergency nursing services by investigating the expectation level and actual received level with nursing care and identifying the factors that influence patient satisfaction with the emergency nursing care in the military hospitals. Method: Using a self report questionnaire, data were collected from 150 patients who had been admitted in military hospitals emergency department. The data collection was done between April 17 and April 30, 2006. At least, 145 questionnaires were analyzed and used in this study. Results: The average sum score was 61.93 out of 76 with standard deviation 9.590. The mean of the patient's expectation levels with the overall emergency nursing services was $(3.25{\pm}.504)$ out of the highest score 4. the average sum score was 63.27 out of 76 with standard deviation 9.902. The mean score of the patient's actual levels with the overall emergency nursing services was $(3.33{\pm}.521)$ out of the highest score 4. Conclusion: nursing management strategies must be established to remove patient's dissatisfaction factors and to improve the emergency nursing care quality through continuous measurement and assessment of the patient's satisfaction with emergency nursing services in military hospitals.

간호대학생의 환자권리에 대한 인식과 윤리적 가치관이 생명의료윤리의식에 미치는 영향 (Influence of Perception of Patient rights and Ethical Values on Biomedical Ethics Awareness in Nursing Students)

  • Purpose: The purpose of this study was to identify nursing students' perception of patient rights, their ethical values and biomedical ethics awareness, and to examine the factors affecting the nursing students' biomedical ethics awareness. Methods: The participants of this study were 273 nursing students in B and K metropolitan city. Data collection was conducted through the structured questionnaires from March 2 to March 25, 2016. Data were analyzed using t-test, ANOVA, Scheffe's test, Pearson's correlation coefficient, and multiple regression analysis with SPSS WIN v 21.0. Results: The mean scores of nursing students' perception of patient rights, ethical values and biomedical ethics awareness were $4.56{\pm}0.38$, $3.26{\pm}0.31$, $2.91{\pm}0.20$, respectively. Biomedical ethics awareness was positively correlated with the nursing students' perceptions of patient rights (r=.38, p<.001) and ethical values (r=.25, p<.001). Factors affecting the nursing students' biomedical ethics awareness were the perception of patient rights (${\beta}=.36$, p<.001) and ethical values (${\beta}=.13$, p=.023). Conclusion: The results suggest that nursing educational program should include perception of patient rights and ethical values to foster biomedical ethics awareness for nursing students.

신규간호사의 환자안전문화에 대한 인식과 안전간호활동 (Perception of Patient Safety Culture and Safety Care Activity of Entry-level Nurses)

  • Purpose: We investigated the relationship between patient safety culture and safety care activity, and identified factors for safety care activity of entry-level nurses. Methods: The subjects of the study were 204 entry-level nurses working at five general and advanced hospitals with over 500 beds located in C and D City. The data were analyzed using descriptive analysis, t-test, ANOVA, Pearson's correlation and hierarchical multiple regression with SPSS/WIN 18.0 program. Results: Perception of patient safety culture had significant correlations with safety care activity. Multiple regression analysis showed that demographic variables of quality of nursing on patient safety and health state of entry-level nurses affected safety care activity. In addition, patient safety culture in ward and communication affected safety care activity. These factors explained 38% of variance. Conclusion: Based on the results of the study, patient safety culture promotion strategies to facilitate supportive work environment and effective communication are needed.

의료 인력의 확보가 환자 입원일수에 미치는 영향 (The Effects of Medical Staffing Level on Length of Stay)

  • Purpose: The objective of this study was to analyze the effects of medical staffing level as bed-to-medical staff ratio on patient outcomes as length of stay (LOS) among hospitals in Korea. Methods: Two hundred and fifty one hospitals participated in the study between January and March 2008. Data for the study was requested by an electronic data interchange from the Health Insurance Review Agency in 2008. In data analysis, SPSS WIN 15.0 program was utilized for descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple regression. Results: The mean score for length of stay was 13.6 days. The mean of operating bed-to-nurse ratio was 7.93:1. The predicting factors for LOS were bed-to-nurse's aide ratio, bed-to doctor's ratio, severely ill patient rate, and hospital type. These factors explained 28.9% of the variance in patient outcomes. Conclusion: This study results indicate that the relationship between medical staffing level and patient outcomes is important in the improvement of the quality of patient care. Thus, improvements in the quality of the nurse practice environment could improve patient outcomes for hospitalized patients.

병원 간호사의 환자중심 간호 경험 (Hospital Nurses' Experience of Patient-Centered Nursing)

  • Purpose: This study aimed to explore frontline nurses' experience of patient-centered care and understand the factors affecting its implementation in hospitals. Methods: Four focus group interviews were conducted with 30 nurses in two university hospitals. The following theoretical framework of patient-centered care was used: 1) Respect for patients' values, preferences, and expressed needs, 2) Care coordination and integration, 3) Information, communication, and education, 4) Physical comfort, 5) Emotional support and alleviation of fear and anxiety, 6) Involvement of family and friends, 7) Care transition and continuity, and 8) System issues. We performed a directed content analysis. Results: The most frequent patient-centered nursing practices of the hospital nurses were "promoting physical comfort" in inpatient settings and "providing information and communicating" in outpatient settings. The factors influencing patient-centered nursing included the health professionals' mindfulness, work overload and staff shortage, and unreasonable social demands and regulations. Conclusion: A more comprehensive patient-centered nursing practice should be implemented by improving "care transition and continuity," "family/caregiver involvement," and "system building." Health professionals' mindfulness is significant, and organizational supports addressing work overload and staff shortage are needed alongside change in social awareness.

병원 간호사의 환자안전관리활동 영향요인 연구 (Factors Affecting Patient Safety Management Activities at Nursing Divisions of Two University Hospitals)

  • This study was conducted to find out factors affecting patient safety management activities at nursing devisions of two university hospitals. The indicators of patient safety activities used in the study were selected from Hospital Survey on Patient Safety Culture which was developed by The Agency for Healthcare Research and Quality in the United States. Data were collected from 372 nurses working at inpatient wards at two university hospitals located in Seoul and Won-Ju cities through the self-administered questionnaires, and analyzed using frequency analysis, t-test, correlation, and regression analysis. The result of this study were as follows. First, The mean of patient safety management activities was 3.41 which was a little bit higher than the average level. The mean of communication within the ward was the highest, whereas that of patient safety management culture was the lowest. Second, there were significant differences in the perception of nurses on patient safety management activities in terms of the educational background, length of employment, and work hours per week. Third, three variables of communication within the unit, attitude of top management, and attitude of nurses have significant positive effects on patient safety management activities. Fourth, 37.4% of the variation in the patient safety management activities was explained by the study variables. In conclusion, hospital administration and nursing managers should make an effort to learn the knowledge of patient safety program, show their deep interest on the patient safety to the employees and motivate them to communicate effectively each other within the work unit to develop a good patient safety culture and system.
