간호학생의 대상자 교육후 교육경험 만족도, 실습만족도 및 자기효능감 정도 (The Relationships between Patient Education Experience, Satisfaction with Clinical Nursing Practice and Self-Efficacy in Nursing Students)

  • 본 연구는 일개 대학의 간호학생을 대상으로 모성간호학 실습시 대상자에게 제공한 교육경험의 만족도, 실습만족도 및 자기효능감과의 관계를 분석하여, 실습의 질 향상을 위한 개선방안을 제시하고자 하였다. 연구결과, 간호학생들은 대상자 교육시 문제점으로는 대상자와의 접근, 교육시간 확보의 어려움 및 자신감 부족 등 이었다. 교육실시후 교육경험만족도는 3.84점, 임상실습만족도 4.00점, 자아효능감 2.68점으로 나왔다. 일반적 특성에 따른 대상자 교육경험의 만족도는 교육방법과 실습시기, 이론교육의 만족도에 따라, 임상실습만족도는 실습자의 태도, 실습만족도, 실습장소에 따라, 자아효능감은 성적, 이론교육의 만족도, 교육장소에 따라 차이가 있는 것으로 나왔다. 따라서 모성간호학 실습의 어려움과 한계를 극복하기 위해 병원관계자와 긴밀한 협력관계 유지하고, 간호학생의 요구를 반영한 교육방법과 실습지도 방안을 다각적으로 모색할 필요가 있다.

교육전담간호사 제도 시범사업 적용에 따른 일 의료기관에서의 성과 (Application and Evaluation of the Pilot Program for the Education Nurse System in a Medical Institution)

  • Purpose: This study was conducted to evaluate the performance following the application of the pilot program for the education nurse system of the Ministry of Health and Welfare in a medical institution. Methods: This study was employed a non-homogeneous comparative group design by comparing new nurses who entered the medical institution after the pilot project from December 2019 to April 2020 with new nurses who entered before the pilot project during the same period. Satisfaction, academic achievement, job adaptation, personal turnover within one year, and patient safety incident rate were investigated as performance indicators. Results: After the pilot project, the overall satisfaction among new nurses, preceptors, and fellow nurses increased, but there were no significant changes in academic achievement and job adaptation in new nurses. The personal turnover rate decreased from 15.6% to 9.1%, and the patient safety incident rate also decreased from 26.3% to 15.7%. Also, the preceptor overtime also decreased from 3.67 to 0.66 hours. Conclusion: The performance of the pilot project for the education nurse system was related to improvements in satisfaction, turnover rate of new nurses, patient safety incident rate, and preceptor overtime. Above all long-term monitoring of each performance indicator is necessary through the continuation of the education nurse system of the Ministry of Health and Welfare.

수술실 간호사의 환자안전관리활동 영향 요인 (Factors Influencing on Patient Safety Management Activities in Operating Room Nurses)

  • 본 연구는 수술실 간호사의 직무 스트레스와 조직몰입, 환자안전관리활동 정도를 파악하고, 환자안전관리활동 정도에 영향을 미치는 요인을 확인하기 위한 서술적 조사연구이다. 대상자는 G광역시 소재 1개의 대학병원과 9개 종합병원 수술실에 근무하는 간호사로 대상자 선정기준에 적합한 총 136명이었으며 자료수집기간은 2014년 7월 25일부터 8월 7일까지이었다. 수집된 자료는 SPSS/WIN 21.0 program을 이용하여 기술통계, Independent t-test, one- way ANOVA, Pearson's correlation, multiple regression으로 분석하였다. 연구결과는 다음과 같다. 대상자의 직무 스트레스 정도는 5점 만점에 3.76점, 조직몰입 정도는 5점 만점에 3.09점, 환자안전관리활동 정도는 5점 만점에 4.15점이었다. 수술실 간호사의 환자안전관리활동 정도는 대상자의 근무경력, 적성, 근무만족도와 환자안전교육 횟수에 따라서 통계적으로 유의한 차이가 있었다. 환자안전관리활동은 조직몰입과 유의한 양의 상관관계가 있었고, 수술실 간호사의 환자안전관리활동에 영향을 미치는 요인은 조직몰입과 환자안전교육 횟수로, 이들 변수의 설명력은 23%이었다. 이상의 연구결과를 통해 조직몰입과 환자안전교육이 수술실 간호사의 환자안전관리활동을 증진시키는 주요 영향요인으로 확인되었다. 따라서, 수술실의 환자안전관리를 위해서 수술실 간호사의 조직몰입을 향상시키는 다양한 중재프로그램 개발과 환자안전관리활동을 위한 지속적이고 체계적인 환자안전교육의 강화가 필요하다고 사료된다.

3 차 의료기관에서의 영양업무 분석 : 업무수행 시간 및 적정인원 산출 (A Study for Dietetic Practitioner's Job Analysis : labor Time Spent and Staffing Need Indices)

  • This study was designed to analyze the dietetic practitioner's job in the over 600-ed hospitals in korea and to assess their labor time spent and staffing need indices. The actual time spent and expected labor time spent on dietitians' activities were investigated and the proper dietic staffing needs in the hospitals was also calculated. A job analysis questionnaires were developed and mailed to 20 hospitals. Completed questionnaires were received form 12 hospitals for a response rate of 60%. The followings are summary of the results. 1. The jobs dietitians at the hospitals were classified into the following 7 areas, direct patient care, indirect patient care, therapeutic patient care, education & counseling, meeting & research, delay & movement, and administration & food services. 2. The actual time spent on dietetic practice was 48.6 hours and expected labor time spent was 99.2 hours, Therefore, the proper time required to conduct classified jobs was 2.1 times higher than the time spent. Especially, the time required for performing clinical nutrition services including direct patient care, indirect patient care, therapeutic patient care was significantly higher than the time needed. 3. The average times required for the direct patient care was 1334.6min, for the indirect patient care was 796.3min, for the therapeutic patient care was 1634.5min, for the education & counseling was 265.2min, for the meeting & research was 366.7min, for the delay & movement was 327.3min, and for the administration & food services was 1170min. The staffing need indices was 12.3. As a conclusion, the standardized job descriptions for the dietitian to carry out their job at the hospital should be established. And the clinical dietitians as nutrition professionals have to be recruited to provide systematically hospitalized patients with medical nutrition therapy at each hospital.

Factors associated with Patient Activation for Self-management among Community Residents with Osteoarthritis in Korea

  • Purpose: The purpose of this study was to survey patient activation for self-management and to identify factors associated with patient activation for self-management among community residents with osteoarthritis in Korea. Methods: Cross-sectional study design was used. Survey data were collected from 270 community residents with osteoarthritis through direct interviews. Studied factors included patient activation, joint pain, physical function, depression, and general characteristics. Data were analyzed using chi-squared test, t-test and multivariate logistic regression analysis. Results: The participants' mean score of patient activation was $56.0{\pm}16.61$. The mean score of each factor was $10.6{\pm}5.89$ for joint pain, $5.5{\pm}3.56$ for physical function, and $19.3{\pm}10.01$ for depression. The patient activation level was significantly associated with depression and general characteristics such as education, religion, comorbid hypertension, and use of medical clinics (p<.05). Conclusion: The findings suggest that depression, education, religion, comorbid hypertension, and use of medical clinics may be important factors to be considered when developing programs of patient activation for self-management. This is the first study that measured patient activation, and further studies are suggested to find factors associated with patient activation for self-management among community residents with other chronic diseases.

임상실습 경험이 있는 간호대학생의 환자안전 관리 활동 구조모형 (Model Patient Safety Management Activities for Nursing Students with Clinical Experience)

  • 본 연구는 임상실습 경험이 있는 간호대학생을 대상으로 환자안전사고관리 활동을 설명하고 Ajzen의 계획된 행위이론 및 안전문화 풍토-안전행동모형을 개념적 기틀로 하고, 문헌고찰을 토대로 간호대학생의 환자안전사고관리활동의 가설적 모형을 제시하고, 수집된 자료를 통해 모형의 적합도와 가설을 검증하는 구조방정식 모형 구축연구이다. 연구 대상은 임상실습 경험이 있는 간호대학생 251명을 대상으로 구조화된 설문지를 이용하여 자료수집을 시행하였다. 본 연구결과 모형은 적합한 것으로 확인되었으며 간호대학생의 환자안전관리 활동에 영향을 미치는 예측변수로 환자안전관리 태도, 환자안전문화 및 안전동기임이 확인되었다. 따라서 환자안전관리활동을 향상을 위해서는 임상 실습 현장에서부터 졸업 후 임상에 나가기 전까지 간호대학생이 올바른 환자안전 관리행위를 올바르게 수행할 수 있도록 간호대학생 대상 효과적인 환자안전 사고관리 교육프로그램이 필요하며 이러한 교육 프로그램이 실무현장까지 연속성있게 이어지는 방안을 모색할 필요가 있겠다.

일 종합병원 간호사의 간호행위에 대한 환자 만족도 조사연구 (A Study on the patient satisfaction for nursing care in a general hospital.)

  • This study was done to evaluate patient satisfaction with nursing care. The study subjucts were 164 patients admitted in a general hospital in Incheon province. The instrument used in this study was developed by Risser (1975) and Lim Hae Kyoung (1980) based on classification of nursing care area and modified by Chow Kwang Nam (1995). The relaiability alpha of this tool was 0.93695. The survey method was applied to collect data and the period of data collection was total 30 days from April Ist to April 30th in 1997. The data was analyzed by statistical method of %, mean, t-test and ANOVA through SAS program. 1. The highest scores of satisfaction were shown in the area of reliance, area of treatment, area of environment and area of education in order. 2. The crossed analysis of patient general characteristics and satisfaction of nursing care showed a signficance in the department of patient's admission and the highest satisfaction score was found in the general surgery ward (p<0.05). 3. General characteristics of patients which influence on the satisfaction showed a significant response according to the department of patient's admission in the treatment area, education area and environment area of nursing.(p < 0.05). Recommendation The followings are deriven from the above study results. 1. Area of nursing education which showed the lowest satisfaction should be empathized by making improvement of nursing care approach because the area in the basis to achieve nursing goal. 2. The development of systematic and objective instrument is necessary for evaluation of nursing quality and the evaluation should be performed in the area of patient satisfaction, hospital facility and hospital environment as well.

  • 본 연구는 응급구조학과 학생들의 전공 교육과정에 대한 수행도, 중요도 및 요구도를 분석하여 응급구조학 전공자의 학습 역량을 강화하는데 기초 자료를 제공하고자 하였다. 연구대상은 충남지역의 3개 대학 응급구조학과 학생 217명으로 자료 수집기간은 2021년 12월 13일부터 12월 24일까지이며 1급 응급구조사의 직무 기반 핵심 역량을 바탕으로 구조화된 설문지에 응답한 자료를 분석하였다. 연구결과 응급구조학과 학생들의 학습 역량 강화를 위해서 Borich 요구도가 높은 '구급차량 관리 교육', '졸업 후 전문성 유지를 위한 교육', '병원 내 환자 감시 교육'과 LF 모델의 최우선 영역에 해당하는 '의료지도, 인계 교육', '내과적 응급환자 처치 교육', '병원 내 환자 감시교육', '병원 내 응급환자 진료보조 교육'에 해당하는 교과과정의 보강이 필요하다고 판단된다.

한국의 환자중심 의사 역량 연구 (Patient-Centered Doctor's Competency Framework in Korea)

  • With increasing demands for medical care by society, the medical system, and general citizens and rapid changes in doctor's awareness, the competencies required of doctors are also changing. The goal of this study was to develop a doctor's competency framework from the patient's perspective, and to make it the basis for the development of milestones and entrustable professional activities for each period of medical student education and resident training. To this end, a big data analysis using topic modeling was performed on domestic and international research papers (2011-2020), domestic newspaper articles (2016-2020), and domestic social networking service data (2016-2020) related to doctor's competencies. Delphi surveys were conducted twice with 28 medical education experts. In addition, a survey was conducted on doctor's competencies among 1,000 citizens, 407 nurses, 237 medical students, 361 majors, and 200 specialists. Through the above process, six core competencies, 16 sub-competencies, and 47 competencies were derived as subject-oriented doctor's competencies. The core competencies were: (1) competency related to disease and health as an expert; (2) competency related to patients as a communicator; (3) competency related to colleagues as a collaborator; (4) competency related to society as a health care leader (5) competency related to oneself as a professional, and (6) competency related to academics as a scholar who contributes to the development of medicine.

당뇨병 환자를 위한 엠파워먼트 프로그램 개발 및 당뇨병 환자의 엠파워먼트 과정 경험 -근거이론 방법론 적용- (Development of Empowerment Program for the Diabetes Patients and the Experiences of Diabetes Patient's Empowerment Process - A Grounded Theory Methodology Approach)

  • The purposes of this study were to develop the empowerment education program, to describe the experiences of diabetes patient's empowerment process and to develop a theoretical model of the diabetes patient's empowerment process. Method 1. : The development of the empowerment program for the diabetes patients: The strategies of the empowerment education program were enhancement of problem - solving, decision making, self-efficacy, self-control. participation and mutual support. Method 2. : According to the grounded theory methodology of Strauss and Corbin, the qualitative data was collected with in depth interviews and participants observations until its saturation when the 25 consented subjects were participating and interacting with the other subjects in the empowerment education program. Results: With the analysis of the data, 29 categories were generated. The core category generated, which was a central phenomenon of the empowerment process, was named powerlessness. The intervening conditions facilitating or impeding the empowerment process were discovered as supportive systems through the participation of group meeting, problem solving dialogue, and the knowledge deficit of self-care. The action/interaction strategies were developed as the paricipating, dialoguing, questioning, supporting system, self-controlling, self efficacy, enhancing self-esteem. stress relaxing and instillation of hope.
