Chronic progressive external ophthalmoplegia (CPEO) is a complex slowly progressive mitochondrial disorder characterized by extraocular muscle weakness with or without multisystem involvement. The mainstay of therapy in a patient with CPEO is supportive. However, in moderate cases, surgery might be indicated including surgeries for ptosis and strabismus. In this article, we report a Saudi patient with CPEO due to compound heterozygous variants in the DNA polymerase gamma (POLG) gene c.2246T>C p.(Phe749Ser) and c.1735C>T p.(Arg579Trp), which are classified as pathogenic. Proper diagnosis with genetic testing confirmation is important to guide the management and counsel the patient about the prognosis and the management options. The patient was successfully managed with bilateral frontalis sling and illustrates the importance of surgical intervention to improve vision and cosmetic appearance in patients with CPEO. We emphasize the importance of multidisciplinary care in the management of cases of mitochondriopathy, especially CPEO.
Alport syndrome (AS) is a progressive hereditary nephritis that is often accompanied by sensorineural hearing loss and ocular abnormalities. It is inherited in three modes of X-linked AS (XLAS), autosomal recessive AS (ARAS), and autosomal dominant AS (ADAS). XLAS is caused by pathogenic variants in COL4A5, while ARAS and ADAS are caused by those in COL4A3 or COL4A4. There is currently no curative treatment for AS; however, angiotensin-converting enzyme inhibitors (ACEi) can improve the outcome of AS. In the past decade, multiple studies have shown that early intervention with ACEi upon isolated microscopic hematuria or microalbuminuria could delay disease progression, and early diagnosis is crucial for early treatment. Therefore, a new classification of AS based on molecular diagnoses has been proposed, including the paradigm shift of re-classifying female "carriers" to "patients" and "thin basement membrane nephropathy" to "ADAS." In addition, with the detection of COL4A mutations in some patients with biopsy-confirmed IgA nephropathy, focal segmental glomerulosclerosis, and chronic kidney disease of unknown origin, it is suggested that the phenotype of AS should be expanded. In this review, we highlight the landmark studies and guidelines published over the past decade and introduce strategies for early diagnosis and treatment to improve the outcomes of AS.

Many monogenic neurodevelopmental disorders have been newly identified in recent years owing to the rapid development of genetic sequencing technology. These include variants of the epigenetic machinery - up to 300 known epigenetic factors of which about 50 have been linked to specific clinical phenotypes. Chromodomain, helicase, DNA binding 1 (CHD1) is an ATP-dependent chromatin remodeler, known to be the causative gene of the autosomal dominant neurodevelopmental disorder Pilarowski-Bjornsson syndrome. Patients exhibit various degrees of global developmental delay, autism, speech apraxia, seizures, growth retardation, and craniofacial dysmorphism. We report the first case of Pilarowski-Bjornsson syndrome in Korea, due to a de novo missense variant of the CHD1 gene (c.862A>G, p.Thr288Ala) in a previously undiagnosed 17-year-old male. His infantile onset of severe global developmental delay, intellectual disability, speech apraxia, and failure to thrive are compatible with Pilarowski-Bjornsson syndrome. We also noted some features not previously reported in this syndrome such as skeletal dysplasia and ichthyosis. Further studies are needed to discover the specific phenotypes and pathogenic mechanisms behind this rare disorder.
Dent disease is a rare inherited kidney tubulopathy caused by mutations in either the CLCN5 (Dent disease 1) or OCRL1 (Dent disease 2) genes, and which is often underdiagnosed in practice. A diagnosis is clinically suspected in patients with low-molecular-weight proteinuria, hypercalciuria, and one of the following: hematuria, nephrolithiasis, nephrocalcinosis, hypophosphatemia, or chronic kidney disease. Inheritance is X-linked recessive, meaning, these symptoms are generally only found in males; female carriers may have mild phenotypes. Genetic testing is only a method to confirm the diagnosis, approximately 25% to 35% of patients have neither the CLCN5 nor OCRL1 pathogenic variants (Dent disease 3), making diagnosis more challenging. The genotype-phenotype correlations are not evident with the limited clinical data available. As with many other genetic diseases, the management of patients with Dent disease concentrates on symptom relief rather than any causative process. The current treatments are mainly supportive to reduce hypercalciuria and prevent nephrolithiasis. Chronic kidney disease progresses to end-stage between the ages of the third to fifth decades in 30% to 80% of affected males. In this review, we aimed to summarize the literature on Dent disease and reveal the clinical characteristics and molecular basis of Korean patients with Dent disease.
서론: 페닐케톤뇨증은 방향족 아미노산의 하나인 페닐알라닌의 대사장애가 발생하는 유전성 대사질환이다. 양성 고페닐알라닌혈증은 페닐알라닌 수산화효소(PAH) 유전자 변이가 발견되는 페닐케톤뇨증 환자에서 저페닐알라닌 식사 치료가 필요 없이 정상적인 성장 및 발달이 가능한 질환이다. 최근 PAH 유전자의 R53H 변이는 병적이라기 보다 양성에 가까운 것으로 생각되었으며 저자들은 양성 고페닐알라닌혈증 환자들에서 R53H 유전자변이를 보인 10례를 경험하였기에 보고하는 바이다. 방법: 2010년 4월부터 2017년 5월까지 순천향대학교 서울병원에서 추적관찰하고 있는 양성 고페닐알라닌혈증 환자 중 R53H변이를 가진 10명의 한국인 환자의 의무기록을 검토하였다. 환자들은 혈중 아미노산 분석을 통해 고페닐알라닌혈증을 진단받았으며, 모든 환자들은 PAH 유전자 검사를 통해 유전자 변이를 진단받았다. 환자들의 의무기록 검토를 통해 환자의 동반 유전자 변이, 증상 및 혈중 페닐알라닌 농도, 치료여부 등을 분석하였다. 결과: 상기 환자들 중 5명은 R53H와 함께 "병적인" 유전자를 보였으며(R413P, R241C, $Y356^*$, c.442-1G>A, $Y325^*$), 2명에서는 "병적일 수 있는" 유전자 변이를 보였다($W187^*$, A259T). "중요도가 확실하지 않은" 유전자 변이를 가진 환자는 2명이었으며(R53H, G344D), 이 중 35세 남성 1명은 R53H의 동형접합자였다. 1명의 환자에서는 아직 밝혀지지 않은 인트론(intron) 변이가 발견 되었다. 환자들은 혈중 페닐알라닌 수치가 $103.1{\mu}mol/L$ 였던 R53H 동형접합체인 35세 남자 환자를 제외하고는 모두 양성 고페닐알라닌혈증으로 진단되었다. 모든 환자에서 정상 식사를 진행하였으며, 약물 치료는 시행하지 않았다. 모든 환자에서 페닐케톤뇨증의 증상은 없었다. 결론: 본 연구의 R53H 유전자 변이를 가진 환자 10명은 특별한 치료 없이도 정상적인 성장 및 발달을 보였다. 이는 이전에 양성 고페닐알라닌혈증으로 진단된 환자들의 R53H 변이가 정상 변이일 가능성이 높음을 의미하는 것이며 이형접합으로 병적변이를 가진 사람들이 환자가 아니라 보인자 또는 정상인일 수 있음을 의미한다. 따라서 현재 "중요도가 확실하지 않은"으로 분류되어있는 R53H변이는 "양성"으로 분류되어야 할 것이다.
당원병 III형은 GDE 결핍으로 분해되지 않은 당원이 간 또는 근육에 축적되는 유전대사이상질환이다. AGL 유전자 변이에 의해 발생하고 높은 유전적 이질성을 가지고 있다. 당원병 III형의 임상증상은 간 비대, 성장 지연, 저혈당 등이 있다. 이러한 임상 증상은 다른 타입의 당원병의 증상과 비슷하여 임상적으로는 구분하기가 어렵다. 저자들은 간 비대 주소로 내원한 11개월 환아에서 유전자 패널 검사로 진단된 당원병 III형 증례를 보고하고자 한다. 간 조직검사결과 간세포에 글리코겐이 축적되어 있어 당원병을 의심하였으며, 당원병 아형의 감별 진단을 위해 유전자 패널 검사를 시행하였다. 그 결과, AGL 유전자에서 이전에 보고된 바 없는 c.311_312del와 c.3314+1G>A 변이가 이형접합체로 발견되어 당원병 III형으로 진단하였다. 진단 후 생옥수수 전분가루를 복용하는 식이요법 시작하였고, 생후 35개월인 현재까지 급·만성 합병증 없는 상태이다. 또한, 가족 검사를 통해 부모가 각각 보인자임을 확인하였고, 임신된 동생의 융모막 검사에서도 동일한 변이가 확인되어 출산 후 재검 및 조기 식이요법을 시행할 예정이다. 유전자 패널 검사법은 당원병과 같이 임상적으로 구분이 어려우며 높은 유전적 이질성을 가진 질환의 감별 진단 시 시간과 비용을 아낄 수 있어 유용하다. 또한 정확한 분자 유전학적 진단은 환아와 가족에게 질환에 대한 정확한 정보 및 치료 예방, 산전 유전상담을 제공하는 데 도움이 된다.

Inherited peripheral neuropathies (IPN), which are a group of clinically and genetically heterogeneous peripheral nerve disorders including Charcot-Marie-Tooth disease (CMT), exhibit progressive degeneration of muscles in the extremities and loss of sensory function. Over 70 genes have been reported as genetic causatives and the number is still growing. We prepared a targeted gene panel for IPN diagnosis based on next generation sequencing (NGS). The gene panel was designed to detect mutations in 73 genes reported to be genetic causes of IPN or related peripheral neuropathies, and to detect duplication of the chromosome 17p12 region, the major genetic cause of CMT1A. We applied the gene panel to 115 samples from 63 non-CMT1A families, and isolated 15 pathogenic or likelypathogenic mutations in eight genes from 25 patients (17 families). Of them, eight mutations were unreported variants. Of particular interest, this study revealed several very rare mutations in the SPTLC2, DCTN1, and MARS genes. In addition, the effectiveness of the detection of CMT1A was confirmed by comparing five 17p12-nonduplicated controls and 15 CMT1A cases. In conclusion, we developed a gene panel for one step genetic diagnosis of IPN. It seems that its time- and cost-effectiveness are superior to previous tiered-genetic diagnosis algorithms, and it could be applied as a genetic diagnostic system for inherited peripheral neuropathies.
Purpose: The mutation of the SLC26A4 gene is the second most common cause of congenital hearing loss after GJB2 mutations. It has been identified as a major cause of autosomal recessive nonsyndromic hearing loss associated with enlarged vestibular aqueduct and Pendred syndrome. Although most studies of SLC26A4 mutations have dealt with hearing-impaired patients, there are a few reports on the frequency of these mutations in the general population. The purpose of this study was to evaluate the prevalence of SLC26A4 mutations that cause inherited deafness in the general Korean population. Materials and Methods: We obtained blood samples from 144 Korean individuals with normal hearing. The samples were subjected to polymerase chain reaction to amplify the entire coding region of the SLC26A4 gene, followed by direct DNA sequencing. Results: Sequencing analysis of this gene identified 5 different variants (c.147C>G, c.225G>C, c.1723A>G, c.2168A>G, and c.2283A>G). The pathogenic mutation c.2168A>G (p.H723R) was identified in 1.39% (2/144) of the subjects with normal hearing. Conclusion: These data provide information about carrier frequency for SLC26A4 mutation-associated hearing loss and have important implications for genetic diagnostic testing for inherited deafness in the Korean population.
Purpose: To evaluate the additive value of prenatal chromosomal microarray analysis (CMA) in assessing increased nuchal translucency (NT) (≥3.5 mm) with normal karyotype and the possibility of detecting clinically significant genomic imbalance, based on specific indications. Materials and Methods: Invasive samples from 494 pregnancies with NT ≥3.5 mm, obtained from the Research Center of Fertility & Genetics of Hamchoon Women's Clinic between January 2019 and February 2020, were included in this study and CMA was performed in addition to a standard karyotype. Results: In total, 494 cases were subjected to both karyotype and CMA analyses. Among these, 199 cases of aneuploidy were excluded. CMA was performed on the remaining 295 cases (59.7%), which showed normal (231/295, 78.3%) or non-significant copy number variation (CNV), such as benign CNV or variants of uncertain clinical significance likely benign (53/295, 18.0%). Clinically significant CNVs were detected in 11 cases (11/295, 3.7%). Conclusion: Prenatal CMA resulted in a 3% to 4% higher CNV diagnosis rate in fetuses exhibiting increased NT (≥3.5 mm) without other ultrasound detected anomalies and normal karyotype. Therefore, we suggest using high resolution, non- targeting CMA to provide valuable additional information for prenatal diagnosis. Further, we recommend that a genetics specialist should be consulted to interpret the information appropriately and provide counseling and follow-up services after prenatal CMA.
Purpose: Pompe disease (PD) is an autosomal recessive disorder caused by a deficiency of acid alpha-glucosidase resulting from pathogenic GAA variants. This study describes the clinical features, genotypes, changes before and after enzyme replacement therapy (ERT), and long-term outcomes in patients with infantile-onset PD (IOPD) and late-onset PD (LOPD) at a tertiary medical center. Methods: The medical records of 5 Korean patients (2 male, 3 female patients) diagnosed with PD between 2002 and 2013 at Samsung Medical Center in Seoul, Republic of Korea were retrospectively reviewed for data, including clinical and genetic characteristics at diagnosis and clinical course after ERT. Results: Common initial symptoms included hypotonia, cyanosis, and tachycardia in patients with IOPD and limb girdle weakness in patients with LOPD. Electrocardiography at diagnosis revealed hypertrophic cardiomyopathy in all patients with IOPD who showed a stable disease course during a median follow-up period of 10 years. Patients with LOPD showed improved hepatomegaly and liver transaminase level after ERT. Conclusion: As ERT is effective for treatment of PD, early identification of this disease is very important. Thus, patients with IOPD should be considered candidates for clinical trials of new drugs in the future.
