상지에 발생한 악성 및 침윤성 양성골종양에 대한 사지 구제술 (Limb Salvage in the Treatment of the Upper Extremity Bone Tumors)

  • 1986년 3월부터 1993년 12월까지 연세대학교 의과대학 정형외과학 교실에서 상지에 발생한 악성 및 침윤성 양성 골종양에 대하여 사지구제술을 시행하여 다음과 같은 결과를 얻었다. 1. 총 13명의 상지에 발생한 악성 및 침윤성 양성골종양 환자에 대해 사지 구제술을 시술 하였다. 2. 사지 구제 술의 내용은 8 례가 Tikhoff -Linberg 수술, 2 례가 분절절제 및 재접합술, 2 례가 종양삽입물 치환술 그리고 1 례가 분절절제술 및 유리혈관부착 생비골이식술이었다. 3. 13 례중 3 례가 골육종, 4 례가 연골육종, 3 례가 거대세포종, 1 례가 병적 골절을 동반한 유잉육종, 1 례가 연골아세포종, 1 례가 전완부 건 및 근육과 원위요골 및 척골을 동시에 침범한 평활근 육종이었다. 4. 추시기간은 술수 1년에서부터 7년 5개월로 평균 4년 5개월이었다. 5. 총 13명의 환자 중 1 례의 상완골에 발생한 병적 골절을 동반한 유잉 육종의 환자에서 국소재발 및 다발성 골전이가 나타나 수술 후 4년 4개월만에 사망하였고 나머지 12 례의 환자는 국소재발이나 원격전이의 소견은 없었다. 6. 상지에 발생한 악성 및 침윤성 양성 골종양의 치료로 여러방법의 사지구제술은 병의 치료 면이나 기능적인 면에서 만족스런 결과를 가져왔다.

익산 왕궁리유적 후원(後苑)의 조성 시설에 관한 고찰 (Perspectives on the Composition Facilities of The Royal Garden at Wanggung-ri site, Iksan)

  • 본고의 검토는 익산 왕궁리유적에서 조사된 후원 조사 결과로서, 후원을 조성하고 있는 시설들에 대해 살펴보는 것을 목적으로 하였다. 백제 사비기에 조성된 궁성 유적인 익산 왕궁리유적의 정원시설과 후원에 대한 조사 결과를 통해 백제의 궁내원에 대한 연구의 기초가 마련되었다. 익산 왕궁리유적의 후원을 구성하는 시설은 1) 석축시설, 2) 도수시설 : 환수구(동 서 북측 및 출수구 암거시설 등 출수시설) 및 곡수로(1~6 및 집수시설), 3) 방형초석 건물지로 나눌 수 있다. 본고에서는 후원 시설들의 조성과 배치에 대한 조사 결과를 정리하여 백제 사비기 도성에서의 궁원의 원형 복원을 위한 기초를 마련하고자 한다. 먼저, 석축시설은 물의 집수 혹은 경계의 역할을 담당하기 위한 것으로 판단된다. 특히 말각장방형석축시설은 이음부를 통해 환수구까지 물이 이동하였던 것으로 생각되며 조경, 집수 및 도수의 역할을 복합적으로 담당했던 것으로 보인다. 위의 두 시설은 궁성이 조성될 때 후원의 대표적인 시설이었다는 것을 확인하였다. 서사면의 석렬시설은 아래쪽에 위치하는 공방시설 등에 물로 인한 피해를 막기 위해 조성된 것으로 추정된다. 환수구 역시 동측궁장(성벽)과 북측궁장(성벽), 서측 경사면 아래쪽을 경계짓고 보호하는 역할을 하였다. 환수구는 물을 집수 혹은 치수하는 역할을 하였고, 동시에 관상을 위한 조경의 역할도 담당하였던 것이다. 백제~통일신라시대에 이르는 동안 환수구는 물에 의한 내부 퇴적의 진행이 이루어지며 지속적으로 사용되었던 것으로 추정된다. 이후, 환수구 내부가 완전히 퇴적된 시기에, 환수구와 동일한 필요성으로 서측 경사면에 곡수로가 새롭게 조성되었다. 구릉정상부의 방형초석 건물지는 주변의 백제 기와무지 등의 존재를 통해 백제 사비기에 처음 조성되어 이후 개축을 거치며 방형의 초석이 설치된 건물로 사용되었던 것으로 보인다.

경기도 광주시 일대의 지질구조와 변형사 (Geological Structure and Deformation History in the Gwangju area, Gyeonggi-do, Korea)

  • 경기육괴 중앙부의 광주시 일대에 분포하는 선캠브리아기의 경기편마암 복합체는 흑운모호상 편마암 및 석영장석질 편마암으로 구성되어 있다 지질구조 분석결과 연구지역은 적어도 다섯 번의 변형작용을 받은 것으로 밝혀졌다. 첫 번째 변성작용(M1) 및 변형작용(Dl)중에 퇴적암(층리; S0)이 변성되어 흑운모호상 편마암으로 되었으며, 성분엽리(S1)가 발달되었다. 심성암이 관입 후 두 번째 변성작용(M2) 및 변형작용(D2)이 일어났으며, 이때 심성암이 엽리(S2)가 발달된 석영장석질 편마암으로 변하였다. 세 번째 변형작용(D3) 중에 수십 cm 규모의 밀착습곡 및 등사습곡(F3)이 발달하였으며, 축면방향으로 성분엽리(S3)가 발달하였다. 변위작용에 의해 뿌리 없는 습곡(F3)이 S3 엽리와 평행하게 발달되어 있다. 네 번째 변형작용(D4) 중에 남북방향의 압축력에 의해 약 100m의 축면분리를 가지는 F4 습곡이 동서 방향으로 발달하였다. 다섯 번째 변형작용 중에 동서 방향의 압축력에 의해 축면분리가 약 3km 규모의 남북 방향의 F5 습곡이 발달하였다. F5 습곡의 중심부에서는 F4 습곡에 F5 습곡작용이 중첩되어 F5 습곡의 습곡축면과 평행한 남북방향으로 F4 습곡이 발달되어 있으며 날개 사잇각 및 축면분리가 작아졌다. 반면에 F5 습곡의 서쪽 날개 지역에서는 북동방향으로, 동쪽 날개 지역에서는 북서방향으로 F4 습곡이 회전되었다. 연구지역에 발달되어 있는 엽리의 궤적은 주로 F4 습곡 및 F5 습곡에 의해 지배되어 있다. F4 습곡작용 동안에 취성변형작용에 의해 절리 및 단층이 발달하였으며 F5 습곡의 양쪽 날개부에는 부채꼴 절리가 우세하게 발달하고, 힌지대에는 be-절리가 우세하게 발달하였다 그 이후에 북서 방향의 주향이동 단층이 지질도 규모로 발달하였다. 제4기 동안에 발생한 산사태가 일부 절리 및 엽리의 방향을 반시계방향으로 약 $15^{\circ}$ 회전시켰다.

진주시 산지골 유역내 땅밀림지 특성에 관한 연구 (Study on the Characteristics of the Slow-moving Landslide (Landcreep) in the Sanji Valley of Jinju)

  • 본 연구는 진주시 집현면에서 발생한 땅밀림지의 지질, 지형 및 물리적 특성을 분석하여 땅밀림 발생 피해방지를 위한 기초자료를 제공하기 위하여 실시하였다. 대상지는 1982년에 1차 땅밀림 이후에 2019년 2차 땅밀림이 발생하였는데, 2차 땅밀림 피해면적은 1차보다 약 11.5배로 나타났다. 주 구성암석은 풍화가 비교적 심해 땅밀림이 쉽게 발생하는 퇴적암 지역이었다. 땅밀림으로 붕괴된 지역은 활동면으로 추정되는 면과 슬라이딩 방향으로 층리와 수직으로 발달한 절리의 조합에 의해 암반이 작은 블록으로 나뉘고 이완되면서 활동면을 따라 슬라이딩이 발생하였고, 암반이 분리되면서 분리된 틈으로 지표수 및 토사가 유입하는 것으로 조사되었다. 아울러 셰일 내의 사암이 층리와 절리의 조합에 의해 블록으로 나뉜 후 지하수 등의 풍화작용에 의해 모서리 부분부터 내부로 풍화가 진행되는 양파구조(Onion Structure)를 보여주는 노두가 나타나고, 이런 과정을 거쳐 핵석이 만들어진 것으로 판단된다. 조사대상지는 지하수에 의한 전형적인 땅밀림형 산사태 지역으로 유형은 凸상미근형으로 붕적토 땅밀림형 산사태에 해당하였다. 땅밀림 대상지의 산사태위험등급은 3~5등급에 해당되어 일반적인 산사태와는 차이가 있었다. 지하수는 북동(NE)방향으로 유동하는 것으로 나타났으며, 지형 경사를 따라 유동하며 땅밀림으로 붕괴된 방향과 일치하였다. 땅밀림지로부터 능선부는 땅밀림 영향을 받지 않은 토양으로 땅밀림으로 붕괴된 하단부보다 토양용 적밀도는 높았으며, 붕괴로 인해 토양의 와해와 교란으로 인해 토양용적밀도는 급격히 낮아지는 것으로 나타났다.

Transfer of Marketing Knowledge within Multinational Corporations and Its Impact on Performance: Moderating Effects of Absorptive Capacity, Socialization, and Local Knowledge

  • Knowledge1 is considered to be a key element of understanding how organizations gain and sustain competitive advantages. But very few firms are capable of creating the requisite knowledge and thus, firms should acquire and exploit new knowledge through knowledge transfer processes. The empirical part of this study involves examining relationships among adaptability of knowledge and knowledge transfer and marketing performance and testing the moderating roles of absorptive capacity, socialization and local marketing knowledge. This study is organized as follows: (1) Previous literature on knowledge, knowledge transfer and absorptive capacity is summarized, followed by the development of hypotheses derived from the knowledge-based view and absorptive capacity. (2) The hypotheses are tested with data collected from MNCs' subsidiaries performing marketing activities in Korea.Thestudyisclosedwithfindings,implications,andconclusions. Following six research hypotheses are drawn from literature review in related areas: H1: Adaptability of knowledge transferred from the MNCs' headquarters and other subsidiaries is positively associated with knowledge inflows into the receiving subsidiary. H2: The level of marketing knowledge transferred from the MNCs' headquarters and other subsidiaries is positively associated with marketing performance of the receiving subsidiary. H3: Increases in potential absorptive capacity will enhance the relationship between adaptability of knowledge and the level of marketing knowledge transfer. H4: Increases in realized absorptive capacity will enhance the relationship between the level of knowledge transfer and marketing performance of the receiving subsidiary. H5: Increases in socialization activity among the headquarters and subsidiaries will enhance the relationship between adaptability of knowledge and the level of marketing knowledge transfer. H6: Increases in the level of locally developed marketing knowledge will enhance the relationship between the level of knowledge transfer and marketing performance of the receiving subsidiary. The research framework that illustrates the proposed hypotheses is presented in figure 1. The unit of analysis for this study is knowledge transfer from the MNCs' headquarters and other subsidiaries to their subsidiaries operating in South Korea. The population for this study consists of subsidiaries established either as joint ventures or as wholly-owned subsidiaries. A group of 603 foreign firms were drawn from diverse industry organizations and business societies. After personal contact, telephone, fax, and e-mail to request that the respondents complete the questionnaire, 282 valid questionnaires from 133 initial sample companies were collected. The results of the empirical analyses significantly support all of the proposed hypotheses except hypothesis 3. Adaptability of external knowledge promotes knowledge transfer and the relationship is moderated by a firm's potential knowledge absorptive capacity. On the other hand, knowledge transfer improves a firm's marketing performance and a firm's realized knowledge absorptive capacity and local marketing knowledge moderate the relationship. The theoretical and practical implications of the findings in this study are as follows: (1) firms must take seeking, transferring, sharing and exploiting of external knowledge into serious consideration, while simultaneously creating knowledge to support the necessary business operations, remain competitive, and achieve superior performance. (2) Firms should continuously seek to develop their knowledge absorptive capacity (both potential and realized capacity) to absorb, learn and utilize valuable external knowledge. (3) Firms should emphasize not only absorptive capacity, but also development of local knowledge. Firms with strong absorptive capability and local knowledge can learn and transfer more external knowledge, which can be translated into greater levels of competence and performance.

팔맥교회혈(八脈交會穴) 중(中).열결(列缺) 조해(照海)의 배합(配合)에 관한 문헌(文獻) 연구(硏究) (A Literary Study on Combination of Yeolgyeol $(LU_7)$ and Johae $(KI_6)$ of Eight Confluent Acupoints)

  • Objectives : The aim of this study was to analyze how to treat various symptoms through the combination of Yoelgyoel $(LU_7)$ and Johae $(KI_6)$, according to reviewing the contents and data since Ling Shu (靈樞經) to recent literatures including thirty-five medical books. Methods : It was arranged and considered that the location, needling, and symptoms of each acupoint were described in various literatures before the publication of Chim Kyung Ji Nam (鍼經指南). Through various literature since the Publication of Chim Kyung Ji Nam, it was examined how to be recognized and be referred about Yoelgyoel $(LU_7)$ and Johae $(KI_6)$. Results and Conclusions : The location of Yoelgyoel is the superior 1.5cun at wrist joint striation, medial of extensor carpi radialis longus; the location of Johae is the depression part under foot medial condyle. Yoelgyoel is often used for respiratory organ disease, urinary organ disease, neuopsychiatory disease, musculoskeletal system disease; Johae is often used for urinary organ disease, circulatory organ disease. At Chim Kyung Ji Nam, Yoelgyoel is often used for thoraco-abdominal Pain, gynecological disease, digestive organ disease; Johae is often used for abdominal pain, gynecological disease, digestive organ disease. Therefore, these points are used together for general internal disease. As well, these are not directly continuous with Previous literatures from Chim Kyung Ji Nam. The combination of Yoelgyoel and Johae have been developed to the three categories as follows; it is quoted from as it is; it is reconstructed in the form of song; it has new symptoms enlarged. Consequently, the combination of Yoelgyoel and Johae was not bind to the rule of Up-Bottom harmony (上下配合), but asserted for the rule of Ju-Eng harmony (主應配合), which add specific acupoints to e combination of Yolgyol and Chohae as the complication of the symptoms.

한국 프로야구투수들의 부상 발생원인 및 분류에 관한 연구 (The study about the cause of the Korean professional pitchers' injury and its classification)

  • Objectives: We did research in the cause of the Pitchers' injury and their recovery process to make a detailed injury list for the purpose of finding the cause of the Korean professional pitchers' injury and its classification. We drew the conclusion through the results as following. Methods: We posed a question to the 80 pitchers playing in the first team of the eight Korean professional baseball team and analyzed the 62 pieces of question paper except the paper having a mistake. We used SAS/PC statistical package in analyzing the data. Results: In the frequency of the pitchers' shoulder injury in the last three years, the injured of all the players were 61.3$\%$ and the injury free players were 28.7$\%$. The cause of the injury was 45.2$\%$ wrong pitching motion, which was the highest value. For the shape of a pain when injured, the reverberation ache feeling when he is hit in the weight commanded an absolute majority as 19.4$\%$. Those who had muscular pain were 17.7$\%$, which was felt mostly at the pitching motion. The most trouble name of the injured shoulder was bicepstendinitis as 16.1$\%$ while the injury of shoulder joint was the lowest as 1.6$\%$. As the most widely used treatment, 25.8 percent of all the players had taken an electronical thraphy after injury. 14.5 percent of the players who had an injury to the shoulder told that they have an operation and 85 percent of them didn't. As a sort of the operation, a repairing of labrum was 44.4 percent, which is the highest value and the 77.8 percent pitchers are performing a normal pitching through rehabilitation after the operation and 22.2 percent of them are undergoing rehabilitation training. Conclusion: The research have shown that the main cause of the injury, concerning the Korean professional pitchers throwing lots of ball in both matches and practices, is overuse syndrome, bad mechanism, muscle weakness and instability of balance. I think that the role of trainer, physical therapy, and team physician taking charge of the players' injury must learn physical test method by heart exactly to check up the state of the injury definitely at the initial phase. Moreover, when the cause of the injury part after a close examination is discovered, the scientific and good surgery is essential to the rehabilitation success and making a classification of shoulder instability is useful to make a operation plan as well as the players' rehabilitation, treatment.

식품의 미생물 규격기준의 국제적동향 (International Tendencies for Estavlishing a Microbiogical Standard for Food)

  • In 1962 the governing bodies of FAO and WHO approved the establishment of a joint FAO/WHO Food Standards Programme, the creation of a jointly sponsored body to be known as the Codex Alimentarius commission to implement the Programme. It can reasonably be claimed that the Commission has assumad the leading role in establishing internation food standards throughout the world. The Codex Committee of Food Hygiene has received much advice and assistance from other international organization which have been working in this field for a number of years. In particular, it has received valuable background documentation from the International Commission on Microbiological Specifications for Foods(ICMSF) which was set up by the International Association of Microbiological Societies(IAMS), and also from the International Organization for Standardization (ISO). Nevertheless, in spite of the information supplied by governments and research bodies in this field, microbiological standards have proved to be a highly controversial subject from the point of view of Codex standards. When it is decided to establish a microbiological standard for a food or class of foods, the following technical and administrative aspects must be considered: 1) The standard should be based on factual studies and serve one or more of the following objectives: (1) to determine the conditions of hygiene under which the food should be manufactured; (2) to minimize the hazards to public health; (3) to measure the keeping quality and storage potential of the food 2) The standard should be attainable under practicable operating and commercial conditions and should not entail the use of excessive heat treatment or the additions of extra preservatives. 3) The standard should be determined after investigation of the processing operation. 4) The standard should be as simple and inexpensive to administer as possible, the number of tests being kept to a minimum. 5) Details of methods to be used for sampling, examining and reporting should accompany all published microbiological standards. 6) In establishing tolerance levels for the permissible number of defective samples, allowance should be made for sampling and other variations due to differences in the laboratory methods. The following additional points should be kept in mind: 1) It is not satisfactory to establish one set of microbiological standards for a miscellaneous group of foods, such as“frozen foods”or“precooked foods”. 2) Microbiological standards should be applied first to the more hazardous types of food on the basis of experience of expected microbiological levels, taking into account variations in composition, processing procedures, and storage. 3) When a standard is established, there should be a definite relationship between the standard and the hazard against which it is meant to protect the public. 4) The sensitivity, reliability, and reproducibility of the sampling and analytical methods should be compared in different laboratories and the methods to be used should be specified in detail as part of the standard. 5) Tolerances should be included in the standard to account for inaccuracies of sampling and analysis. 6) Standards should be applied on a voluntary basis before compliance is made mandatory.

$Cu^{++}$ 촉매작용에 의한 과산화 현상이 Collagen 손상에 관여함과 Sodium Salicylate에 의한 보호 작용 (Involvement of $Cu^{++}$-Catalyzed Peroxidation in Degradation of Collagen and Protective Mechanism of Sodium Salicylate on this Peroxidative Reaction)

  • $Cu^{++}$ 촉매작용에 의한 과산화현상이 관절조직손상의 한 형태인 Collagen 손상에 관여할 수 있음을 알아보고, sodium salicylate의 항 염증기전의 일부를 설명해 보고자 sodium salicylate가 이 과산화반응에 미치는 효과를 검토하였다. 쥐피부로 부터 얻은 Collagen을 이용하여 Collagen gelation에 대한 $Cu^{++}$$H_2O_2$의 효과를 관찰한 결과 $Cu^{++}$ 또는 $H_2O_2$ 단독으로는 gelation에 영향을 미치지 못하였으나, $Cu^{++}$$H_2O_2$가 동시에 첨가된 경우 gelation이 억제되어 maximal turbidity가 감소되고, lag phase가 연장됨을 보였다. 그리고 같은 반응 조건에서 sodium salicylate 첨가에 의해 $Cu^{++}$$H_2O_2$에 의해 억제된 gelation이 회복됨을 볼 수 있었으며 회복정도는 salicylate 농도 증가에 의존적이었다. 한편 $Cu^{++}$에 의한 $H_2O_2$의 decomposition rate가 sodium salicylate에 의해 증가됨을 보였고, salicylate 농도 증가에 의해 점차 saturation되는 양상을 보였다. 이상의 결과로 부터 $Cu^{++}$ 촉매작용에 의한 과산화 현상은 collagen에 작용하여 구조적 또는 기능적인 변화를 초래함을 알 수 있었고, salicylate에 의해 이러한 과산화 현상이 억제되는 것은 $Cu^{++}$에 의한 $H_2O_2$의 decomposition rate를 증가시킨 결과임을 알 수 있었다. 그러므로 $Cu^{++}$ 촉매작용에 의한 과산화현상은 만성염증 반응 특히 rheumatoid arthritis에서 나타나는 관절조직 손상에 관여할 수 있으며, sodium salicylate는 이 과산화반응에 작용하여 항 염증효과를 나타낼 수 있으리라 믿어졌다.

Contrast reference values in panoramic radiographic images using an arch-form phantom stand

  • Purpose: The purpose of this study was to investigate appropriate contrast reference values (CRVs) by comparing the contrast in phantom and clinical images. Materials and Methods: Phantom contrast was measured using two methods: (1) counting the number of visible pits of different depths in an aluminum plate, and (2) obtaining the contrast-to-noise ratio (CNR) for 5 tissue-equivalent materials (porcelain, aluminum, polytetrafluoroethylene [PTFE], polyoxymethylene [POM], and polymethylmethacrylate [PMMA]). Four panoramic radiographs of the contrast phantom, embedded in the 4 different regions of the arch-form stand, and 1 real skull phantom image were obtained, post-processed, and compared. The clinical image quality evaluation chart was used to obtain the cut-off values of the phantom CRV corresponding to the criterion of being adequate for diagnosis. Results: The CRVs were obtained using 4 aluminum pits in the incisor and premolar region, 5 aluminum pits in the molar region, and 2 aluminum pits in the temporomandibular joint (TMJ) region. The CRVs obtained based on the CNR measured in the anterior region were: porcelain, 13.95; aluminum, 9.68; PTFE, 6.71; and POM, 1.79. The corresponding values in the premolar region were: porcelain, 14.22; aluminum, 8.82; PTFE, 5.95; and POM, 2.30. In the molar region, the following values were obtained: porcelain, 7.40; aluminum, 3.68; PTFE, 1.27; and POM, - 0.18. The CRVs for the TMJ region were: porcelain, 3.60; aluminum, 2.04; PTFE, 0.48; and POM, - 0.43. Conclusion: CRVs were determined for each part of the jaw using the CNR value and the number of pits observed in phantom images.
