대구 시내 한방병원(韓方病院) 수련의의 스트레스 양상(樣相)에 관(關)한 조사(調査) 연구(硏究) (A Study on the stress condition of apprentice doctorf who were in Oriental Medical Hospital)

  • This paper is for studying stress condition of apprentice doctors who were in Oriental Medical Hospital. This study was done on 35 apprentice doctors who were in Kyungsang University Oriental Medical Hospital and Bulgy Oriental Medical Hospital. The Seven-Minute Stress Test of Thomas E. Sttats and 10 questions which were made by the present writer shows the following results.1. The Body stress scale was the highest point. In order of high percent, this shows Whole scale, Mind scale, Situation scale. 2. In comparison of the stress scale of men and women doctors, all the stress scale of women doctors were higher than men doctors. 3. In comparison of the stress scale of Interne and Resident doctors, Whole and Situation stress scale of Resident doctors were higher than Interne doctors, Body and Mind stress scale of Interne doctors were higher than Resident doctors. 4. The personal relation with other people was harmonious, the worst relation of the other colleague was the administrative staff, the next were nurses, senior apprentice doctors orderly. 5. In order of high percent of stressors, this shows personal relation, many works and troubles of care, the lack of private life, unsatisfactory administration system and equipment, the class system and overbearing atmospheres, education and continuous stress, bad conditions and fatigue, economic problems. 6. In order of high percent of systemic stress diseases, this shows musculo- skeletal disease, nervous disease, digestive disease, neurosis, urinary and genital disorder, opthalomo-otolaryngo disease, immunity disorder and vascular disease. In order of high percent of stress symptom, this show headache, fatigue, shoulder pain, back and leg pain, pantalgia and abdominal pain, diarrhea and dismenorrhea, stiffness of neck dizziness indigestion languor after a meal insomnia, neurasthenia lacking interest constipation menorrhalgia bloodshot eyes otitis media allergy thirst flushing edema. 7. In order of the seven mode of emotions in relation to stress, this shows anger, anxiety, isolation, melancholy, fright, sorrow, terror, overjoy.

연극성 성격장애를 지닌 신체화장애 환자 치험 1례 -최면치료, EFT 중심으로- (A Case Report of a Somatization Disorder Patient with Histrionic Personality Disorder)

  • Objectives : The histrionic personality disorder(HPD) is characterized by a pattern of excessive emotion and attention-seeking behavior, including a seductive behavior. It is known that HPD is closely related to a somatization disorder. The somatization disorder is characterized with various physical symptoms that have no pathologic manifestation. This report is on a case of 55 years old woman with HPD, who has been suffering from various physical symptoms that are coincided with diagnostic criteria for somatization disorder by DSM-IV. Methods : The patient was treated with oriental medical treatments (acupuncture, moxibustion, cupping therapy, and herb-medication), hypnotic therapy, and emotional freedom techniques. The effects of treatment were measured by VAS. Results : Chest pain was improved by hypnotic therapy and oriental medical treatments. But the patient's pantalgia and abdominal discomfort were controlled effectively by emotional freedom techniques. Conclusions : This result suggests that EFT might be effective for the defensive and dependent somatization disorder patient with HPD.

火病을 동반한 갱년기 환자 치험 5례 (5 Cases of Climacteric Syndrome Patients with Hwa-Byung)

  • Objectives: The purpose of this study is to evaluate the effects of Korean traditional treatments for Climacteric Syndrome Patients with Hwa-Byung. Methods: From February 2015 to May 2015, We treated 5 Cases Patients with Korean traditional treatments for a month. We measured treatment effects by Menopause Rating Scale (MRS), Beck's Depression Inventory (BDI), Visual Analogue Scale (VAS) and Digital Infrared Thermal Imaging (DITI). Results: After treatment, First, the scores of MRS, BDI were decreased. Second, the symptoms such as hot flush, pantalgia, palpitation, insomnia, fatigue were significantly improved. Also, the difference of 膻中 (CV17, Danjung) and 關元 (CV4, Guanyuan) temperatures were significantly decreased after treatment. Conclusions: This study suggests that Korean traditional treatments such as Gamicheonglijagam-hwan, acupuncture, moxibustion are effective on Climacteric Syndrome Patients with Hwa-Byung.

Review on Diagnostic Criteria of Neurasthenia : Suggesting Pathway of Culture-bound dieases

  • Objective: Neurasthenia is a disease which consists of increased fatigue or bodily weakness and exhaustion plus pantalgia, dizziness, headache and other symtoms relevant to autonomic nerve dysfunction. There are plenty of studies investigating the history of diagnostic criteria of neurasthenia, which is influenced by diverse cultural(or social) environment. The obejective of this study is to provide review of the previous studys on the changes of neurasthenia diagnoses in the context of local area to find meanings of these transition and improve health care for psychiatric patient. Methods: Literature review was conducted on studies demonstrating diagnostic criteria of neurasthenia with cultural(or social) environment. We investigated the literature reviews or observative studies which described alteration of diagnostic criteria of neurasthenia and assessed its significance. After selecting eligible studies, the authors read the articles and summarized the meaningful contents those were significant in clinical practice. Results: Transformation of Chinese Classification of Mental Disorder(CCMD) integrated with internationally utilized DSM-IV or ICD-10 is controversial about its significance in that it had limited effect on public health care due to the variables of sociocultural context, but primarily differentiated neurasthenia from other disorders. The latter one can be the directing point of the diagnostic criteria of other culture-bound diseases, which is the traits of not outstanding mood(or affect) than other neurotic disorders. Conclusion: As diagnostic criteria of neurasthenia varies, the significance of this variation is controversial, but could be the paragon of other culture-bound diseases.

사상체질에 따른 COVID-19 감염증상에 대한 연구 (A Study on COVID-19 Symptoms according to Sasang Constitution)

  • Objectives This study aims to identify differences in clinical manifestations of COVID-19 between different Sasang constitution. Methods Subjects were recruited from August 29, 2022, to July 11, 2023. COVID-19 clinical symptoms were self-reported via questionnaires. Sasang constitutional diagnosis was performed using the K-PRISM, and Sasang constitutional specialist. Results A total of 66 subjects were recruited for the study, and the Sasang constitutional distribution of the subjects was 19 soyangin, 25 taeeumin, and 22 soeumin. For most of the COVID-19 clinical manifestations, the study found that soyangin experienced symptoms at a higher rate than other constitutions. Among the symptoms observed in the study, sore throat, pantalgia, and cough were severe in all subjects regardless of constitution. Soeumin was more likely to experience abdominal pain, chest pain, and diarrhea, while soyangin was more likely to experience chest pain, nausea/vomiting, diarrhea, and cutaneous symptoms compared to other constitutions. Taeeumin had more sputm, nasal congestion, and skin symptoms, but fewer digestive symptoms. There were differences in the onset and duration of symptoms by constitution. Conclusions This study is an important contribution to our understanding of the differences in response to the COVID-19 virus among different Sasang constitutions. Symptomatic differences between constitutions may have important implications for prevention and treatment strategies for infectious diseases, and personalized treatment and management based on these differences may be needed in the future.

유방암 환자의 항암화학요법 후 한방병원 입원치료에 대한 일개 한방병원 환자 특성 및 치료 고찰 (Analysis of the Patients and Treatment of Korean Medicine Hospital after Chemotherapy in Patients with Breast Cancer)

  • Objectives: The purpose of this study is to analyse current status and treatment of the Korean medicine hospital after chemotherapy in patients with breast cancer. Methods: We investigated the medical records of 21 patients who admitted to Korean medicine hospital after chemotherapy in patients with breast cacner from March 1, 2017 to December 31, 2019. We searched medical records retrospectively and analyzed current status and treatment of Korean medicine hospital. Results: The average age of 21 participants was 52.81±8.38 years and 40s and 50s accounted for 85.6% of the total. After receiving chemotherapy, the average time to hospitalization was 1.87±3.13days and average hospital stay was 9.78±4.14 days. The subjects were classified as 28.6% of stage I, 52.4% of stage II, 9.5% of stage III, and 9.5% of stage IV. The analysis according to the presence of metastasis was 57.1% without metastasis, 33.3% with axillary lymph node metastasis, and 9.5% with distant metastasis. The main symptoms complained when hospitalized by 21 subjects were nausea (54.2%), fatigue (54.2%), and anorexia (50.8%) in over 50%, pantalgia (47.5%), and insomnia (47.5%), dizziness (44.1%), cold sweating (42.4%), lower extremity pain (40.7%), 37.5~37.9℃ fever (39.0%), headache (37.3%), hot flush (37.3%), pruritus (30.5%) are 30% or more. Korean medicine treatment was performed in 87.4% of all hospitalizations and Gwakhyangjunggi-san-gami was the most administered prescription. Extracts of Korean medicine was performed in 100.0% of all patients and Eunkyo-san was most administered extracts medicine. Acupuncture, moxibustion, and cupping treatments were performed in all 21 study subjects. Other treatments was performed at a frequency of hyperthermia (90.5%), lymph massage (23.8%), air compression therapy (23.8%), and Interference current therapy (19.0%) Conclusion: Korean traditional medicine can be used as a countermeasure for side effects after chemotherapy in breast cancer patients.
