Epidural Blood Patches in a Patient With Multi-level Cerebrospinal Fluid Leakage That Was Induced by Spontaneous Intracranial Hypotension

  • Spontaneous intracranial hypotension (SIH) is considered to be a very rare disorder. It is characterized by an orthostatic headache that is aggravated with the patient in the upright position and it is relieved by the patient assuming the supine position. SIH is caused by a spontaneous spinal cerebrospinal fluid leakage without the patient having undergone trauma, surgery or dural puncture or having any other significant medical history. An autologous epidural blood patch (EBP) is effective in relieving SIH. We report here on a case of SIH with cerebrospinal fluid leakage at the upper cervical vertebral level and the middle thoracic vertebral level. The points of leakage were identified by radionuclide cisternography, and this patient was successfully managed by injecting an EBP at each level of leakage.

The Effects of Alphabet Exercise with SSP Electrotherapy on Lateral Ankle Sprain (족관절 염좌에 SSP 전자침 요법을 병행한 동기침법의 효과)

  • The multiple acupuncture techniques have been used to relieve the pain of lateral ankle sprain. Recently, early functional rehabilitation is emphasized in rehabilitative concepts using active ROM, facilitated proprioception, alphabet exercise and so forth. Similarly, in oriental rehabilition medicine, traditional movement technique called 'Dong-Ki' has been used to relieve pain of ankle sprain. So we used 'Dong-Ki' technique with SSP(silver spike point) electrotherapy transformed from alphabet exercise and evaluated the effects in method of randomized controlled trials. 27 outpatients with lateral ankle sprains were randomized into two groups : 15 samples and 12 controls. Sample group was treated with 'Dong-Ki' technique(writing his name with foot) with SSP electrotherapy to relieve pain during additionally. 'Dong-Ki'. Outcomes were measured by 10cm VAS and ankle circumference(cm). In terms of VAS, Sample group had inclination of pain relieving but not significantly meaningful. In terms of ankle circumference measurement, Sample group showed significantly improved.

  • The purpose of this study was to assess the pain-relieving effect of iontophoretically applied prednisolone on the patients with temporomandibular joint (TMJ) pain. 30 TMJ pain patients participated in this study. Inclusion criteria for subject selection were 1) tenderness of TMJ on palpation and 2) visual analog scales (VAS) of above 20 mm. The patients with polyarthralgia, rheumatic arthralgia, and systemic diseases were excluded for this experiment. The patients were randomly assigned to one of three groups; (1) Group 1 for administration of 2% prednisolone, (2) Group 2 for administration of 1% prednisolone, and (3) Group 3 for administration of saline. Phoresor PM600(Motion Control Utah, U.S.A) was used for administration of drugs by iontophoresis. Baseline and post-medication pain levels were recorded by VAS, pressure pain threshold (PPT), and palpation index (PI). Post-medication data were compared with baseline data for each group. The results were as follows : 1. All the groups showed significant decrease of VAS scores after treatment. 2. PPT was significantly increase only in Group 1 and 2 not in Group 3. 3. PI was significantly decreased only in Group 1 and 2 not in group 3.

The effects of cold therapy on pain related to chest tube removal in patients with coronary artery bypass graft surgery (냉요법 적용이 관상동맥 우회술 환자의 흉관 제거시 통증에 미치는 효과)

  • Purpose: Patients who underwent a coronary artery bypass graft surgery(CABG) experienced the unpleasant emotions and discomfort when their chest tube was removed. The purpose of this study was to evaluate the effects of cold therapy on pain related to chest tube removal(CTR) in CABG patients. Methods: Fifty adult patients undergoing CABG were recruited in a prospective, double blinded study. Subjects were divided into the experimental group and the control group considering their sex and age. The pretest data were obtained 20 minutes before CTR. Patients in the experimental group, received cold therapy for 10 minutes before CTR. Pain sense and intensity were determined immediately after CTR and at 10 minutes after CTR. Results: The total score of pain sense immediately after CTR of the experimental group was significantly lower than that of the control group(t=-3.703, p=.003). And scores of pain intensity immediately after CTR in the experimental group were significantly lower than that of the control group(t=-3.073, p=.001). But, there was no significant difference in the score of pain intensity 10 minutes after CTR between the experimental and the control group(t=1.759, p=.085). Conclusion: The cold therapy would be recommended as an effective and nonpharmacologic nursing intervention for relieving pain in patients undergoing CTR.

  • Objective : This study is to report the effect of Scolopendrid Herbal Acupuncture on a patient suffering from the pain of Hip Adductors after operation for hip fracture. Methods : First we had treated the patient with the pain of Hip Adductors by acupuncture and herb medication(活絡湯), but we did't get remarkable improvement in condition of patient. So we added to treat his pain site with Scolopendrid Herbal Acupuncture. And then we evaluated his pain by VAS(Visual Analog Scale). Results : 1. Scolopendrid Herbal Acupuncture had a very excellent effect on relieving pain and inflammation of Hip Adductors after operation for hip fracture. 2. It appears that Scolopendrid Herbal Acupuncture is very effective against Trigger Point of muscle and pain site fixed.

  • Ulnar nerve compression in the cubital tunnel is a common entrapment syndrome of the upper limb. Pulsed radiofrequency lesioning (PRFL) has been reported as a treatment method for relieving neuropathic pain. Since the placement of the electrode in close proximity to a targeted nerve is very important for the success of PRFL, ultrasound seems to be well suited for this technique. A 36-year-old woman presented with complaints of numbness and pain on the medial aspect of the elbow and the pain radiated down to the $4^{th}$ and $5^{th}$ fingers for 10 years after she suffered an elbow contusion, we then scheduled this woman for the ultrasound guided PRFL of the ulanr nerve. The initial ultrasound examination demonstrated a swollen nerve, loss of the fascicular pattern and an increased cross sectional area of the ulnar nerve. After confirmation of the most swollen site of the nerve via ultrasound, two sessions of PRFL were performed. The postprocedural 10 cm visual analog scale score decreased from 8 to 1 after the two sessions of PRFL.

Effect of Ultrasound on the Patients with TMJ Pain (초음파가 악관절동통 환자에 미치는 영향)

  • Author could get the result as below after ultrasound treatment, one of the pain relieving method, to 40 patients in whom author could confirm the objective improvement of symptoms among the patients who visited SNUH between April first, and October 31st. 1984, because of TMJ pain. 1. The average number of treatment were 6.5 times/pt, and treatment duration was 15minutes. 2. The severities of pain before the treatment were distressing (50.0%), discomfort (25.0%) mild (15.0%) and horrible (10.0%), but the percentages of them became changed to discomfort (45.0%), mild (30.0%), distressing (20.0%), and none (5.0%) after treatment. 3. The activities of mouth opening before treatment were severely restricted (50.0%) moderately restricted (37.5%) and slightly ristricted (12.5%), but the percentages of them became changed to slightly restricted (50.0%), moderately estricted (25.0%) severely restrictely(12.5%) and normal (12.5%) after treatment. 4. The effect of the pain upon one's personality before treatment were severely upset (55.0%), moderately upset(37.5%) and slightly upset(7.5%), but the percentages of them became changed to slightly upset(50.0%), normal(25.5%), moderately upset(12.5%) and severely upset (12.5%) after treatment.

Change of Pain, Lumbar Sagittal Alignment and Multifidus after Sling Exercise Therapy for Patients with Chronic Low Back Pain

  • Purpose: This study examined the effects of sling exercise therapy on vertebral alignment, VAS, muscle activity, and multifidus of patients with chronic low back pain. Methods: Simple random sampling was used to divide the patients (n=116) into the sling exercise therapy group (SETG) and conservative physical therapy group (CPTG), with each group provided a intervention program in 3 sessions a week for 12 weeks. The lumbar lordosis angle (LLA), lumbar intervertebral disc angle (LIVDA) for vertebral alignment, lumbar muscle activity, and multifidus atrophy were measured before and after the intervention. Results: SETG showed significant changes in LLA, LIVDA of rate of change (delta score), and in relieving pain. The right-left balance gap for the lumbar dynamic muscle activity decreased after the intervention. The SETG showed significant changes in the grade of lumbar multifidus atrophy. Conclusion: The sling exercise therapy program is an effective exercise therapy method on vertebral alignment, muscle activity, recovery from multifidus atrophy, and pain relief for patients with chronic low back pain.

Effects of Distraction by a Cellular Phone on Pain and Fear During Venipuncture Procedure for Hospitalized Preschool Children (휴대폰을 이용한 관심전환이 학령전기 입원 아동의 정맥주사시 통증과 두려움에 미치는 효과)

  • Purpose: The purpose of this study was to investigate effect of distraction by use of a cellular phone on pain and fear during venipuncture procedure for hospitalized preschool children. Method: Fifty four preschool children aged 3-6 were recruited from one university affiliated hospital in Daegu. Participants were assigned to the control(n=27) to receive usual care or intervention group(n=27) to receive distraction plus usual care. For the assessment of children's pain, FACE scale and mothers' perceptions of their own children's pain, visual analogue scale(VAS) were used. For the assessment of fear, Procedure Behavioral checklist developed by Lebaron and Zeltzer was used. Results: Results showed that intervention group demonstrated significantly less pain scores on FACE(t=2.19, p<.03) as rated by children and on VAS(t=2.78, p<.01) as rated by their mothers. Intervention group showed also significantly less fear scores(t=2.30, p<.02) as rated by the researcher. Conclusion: Distraction with cell phone for relieving pain and fear during venipuncture procedure was effective for the hospitalized preschool children.

Pilot Study for Perceived Effectiveness of Music Therapy for Pain and Music Preference in Women following Gynecological Surgery in a Local Area (일개 지역 부인과 수술환자가 인식하는 음악요법의 통증 조절효과 및 음악 선호도 : Pilot study)

  • Purpose: The purposes of this pilot study were to describe the perceived effectiveness of music therapy for pain control and to identify music preference for postoperative pain and anxiety control in Korean women who had a gynecological surgery. Sample: A convenience sample of 52 women was recruited from a gynecological unit at P University hospital and they were interviewed to assess their perception of music therapy and music preference using a structured study questionnaire. Results: Fifty-six percent of women reported that music therapy would be effective and 96% perceived it would be effective if combined with analgesics. The best music chosen for relieving postoperative pain and anxiety were Korean old pop and ballad song/music followed by Korean religious music, American pop songs, piano music, Korean classic songs, and Orchestra music. Conclusion: There was a cultural difference in music preference for the relief of post-operative pain in this sample. Therefore, effects of music for postoperative pain control need to be determined after considering musical preferences of postoperative women in Korea.
