Metastatic Bone Disease as Seen in Our Clinical Practice - Experience at a Tertiary Care Cancer Center in Pakistan

  • Aim: Metastatic tumor of bone is the most common malignancy involving bone and is an important predictor of prognosis in advanced cancers. The prognosis depends upon the primary site of origin and the extent of disease. In current study, we present the pattern and distribution of metastatic bone disease seen in the leading cancer care center of Pakistan, Shaukat Khanum Cancer Hospital & Research Center (SKMCH & RC), Lahore. Materials & Methods: All cases of bony metastatic disease were included that presented in the Pathology Department, from Jan 2005 to July 2011. Patients of all ages and both sexes were included. Primary bone tumors, lymphomas, sarcomas and other malignancies were excluded. The data were recorded and analyzed with SPSS 16.0. Results: A total of 146 cases of metastatic bone disease were included in the study. Out of the total cases, 79 were male and 67 were female. Age range 25-82 years (median 52). Hip bone was the most frequent bone involved, with femur and vertebrae as second and third in the list. The commonest bone involved in males was vertebrae with 23 cases and in females was hip bone with 22 cases. Regarding primary site, cancers of breast, prostate and gastrointestinal tract were at the top of the list with prostate and breast being the most frequent primary sites of metastasis in males and females respectively. Conclusion: Bone metastasis is an important entity to consider in the differential diagnosis whenever a bony tumor especially carcinoma present in older age. Our data are comparable with international findings and the literature available regarding the site and distribution of skeletal metastatic lesions. A slight deviation noted was more common bony metastatic lesions with ovarian primaries in females and gastrointestinal tract cancers in males in our study.

암환자에서 암발생부위와 생존기간에 따른 사망전 1년간의 의료비용 (Medical Expenses by Site of Cancer and Survival Time among Cancer Patients in the Last One Year of Life)

  • Objectives : To analyze medical expenses by cancer site and survival time among cancer patients in their last year of life. Method : The study subjects were 45,394 people that had died of cancers in 2002, were registered by the Korea Central Cancer Registry and received National Health Insurance benefit in the last year (360 days) of life. Personal identification data, general characteristics, dates of death and cancer incidence, and site of cancer were collected from the National Statistical Office and the Korea Central Cancer Registry, and merged with the data of the individual medical expenses of the Health Insurance Review Agency. Results : Average monthly cost curves were U-shaped with high costs near the time of diagnosis and death, and lower costs in between. Medical expenses in the last year of life were around 30.3, 16.7, 13.0, and 12.1 million won among leukemia, lymphoma, ovarian cancer, and breast cancer patients, respectively. Digestive organ cancers including stomach, esophagus, liver, pancreas, and colorectal cancers had relatively low medical expenses. Medical expenses in the last year of life were inverse U-shaped with high expenses near one year of survival. Average monthly cost in the 12 months before death among the patients who had survived $10{\sim}15$ years were more than two-fold greater than the cost before diagnosis among those who had survived for less than one year. Conclusions : Leukemia was the most expensive cancer. It is possible that once diagnosed as cancer, medical expenses do not return to the level before diagnosis. Further research will be needed to understand the magnitude and change of the medical expenses among cancer patients with long term follow up data.

  • Reactive oxygen species (ROS) are major sources of cellular oxidative stress. Specifically, cancer cells harbor genetic alterations that promote a continuous and elevated production of ROS. While such oxidative stress conditions could be harmful to normal cells, they facilitate cancer cell growth in multiple ways by causing DNA damage and genomic instability, and ultimately by reprogramming cancer cell metabolism. This review provides up to date findings regarding the roles of ROS generation induced by diverse biological molecules and chemicals in representative women's cancer. Specifically, we describe the cellular signaling pathways that regulate direct or indirect interactions between ROS homeostasis and metabolism within female genital cancer cells.

  • Paclitaxel isolated from the pacific yew tree, Taxus brevifolia, is microtuble-stabilizing agent that has a promising anticancer activity against a wide variety of tumors such as ovarian, breast and lung cancers. Molecular Docking Studies of p21-Activated Kinase-1 (PAK1) Inhibitors

  • The p21-activated kinase-1 (PAK1) has emerged as a potential target for anticancer therapy. It is overexpressed in ovarian, breast and bladder cancers. This suggests that PAK1 may contribute to tumorigenesis. 4-azaindole derivatives are reported as potent PAK1 inhibitors. The present work deals with the molecular docking studies of 4-azaindoles with PAK1. Probable binding mode of these inhibitors has been identified by molecular modeling. Docking results indicated that hydrogen bonding interactions with Glu345 and Leu347 are responsible for governing inhibitor potency of the compounds. Additionally, Val284, Val328, Met344 and Leu396 were found to be accountable for hydrophobic interactions inside the active site of PAK1.

A Novel Feature Selection Approach to Classify Breast Cancer Drug using Optimized Grey Wolf Algorithm

  • Cancer has become a common disease for the past two decades throughout the globe and there is significant increase of cancer among women. Breast cancer and ovarian cancers are more prevalent among women. Majority of the patients approach the physicians only during their final stage of the disease. Early diagnosis of cancer remains a great challenge for the researchers. Although several drugs are being synthesized very often, their multi-benefits are less investigated. With millions of drugs synthesized and their data are accessible through open repositories. Drug repurposing can be done using machine learning techniques. We propose a feature selection technique in this paper, which is novel that generates multiple populations for the grey wolf algorithm and classifies breast cancer drugs efficiently. Leukemia drug dataset is also investigated and Multilayer perceptron achieved 96% prediction accuracy. Three supervised machine learning algorithms namely Random Forest classifier, Multilayer Perceptron and Support Vector Machine models were applied and Multilayer perceptron had higher accuracy rate of 97.7% for breast cancer drug classification.

복강전이가 동반된 개 악성 난소암의 cisplatin 치료 (Intraperitoneal Cisplatin Chemotherapy in A Canine Ovarian Cancer with Peritoneal Metastasis)

  • 네 살령의 암컷 시츄견이 복부 팽만으로 내원하여 조직검사 결과 cystic papillary adenocarcinoma로 진단되었으며, 복강 장간막으로의 전이도 확인되었다. 난소 자궁 적출술을 실시하고 1달 후, 복수를 제거한 후 cisplatin을 (50 $mg/m^2$)을 0.9% 생리식염수(250 $ml/m^2$)에 희석하여 복강내로 주입하였다. 치료기간 동안 3번의 재발이 있었으나, 임상증상의 개선이 매우 뚜렷하였으며, 특별한 부작용을 보이지 않고 진단 후 33개월 간 건강 상태를 유지할 수 있었다. 그러나 11번의 항암 치료 이후에 복수와 더불어 흉수가 병발하였고, 신부전증도 동반하였다. 수액요법과 대증치료의 일환으로 복강, 흉강 천자도 실시하였으나 개선을 보이지 않고 환자는 폐사하였다. 복강 내 cisplatin의 투여법은 난소종양으로 인한 악성 복수, 흉수를 치료하는데 효과적인 완화요법으로 쓰일 수 있을 것으로 사료된다. 본 증례는 복강전이를 동반한 악성 난소 종양이 발생한 환자를 복강 내 cisplatin투여를 통해 효과적으로 치료한 한국에서의 첫 번째 보고이다.

난소암 세포에서 IEX-1과 MCL-1 단백질들의 세포 사멸 기능에 관한 상호작용 (Mutual Activities of IEX-1 and MCL-1 on the Apoptosis of Ovarian Cancer Cells)

  • 세포 사멸(apoptosis)은 세포의 항상성을 적절하게 유지하기 위한 중요한 메커니즘이다. BCL-2 family 단백질들은 세포의 생존과 세포 사멸을 조절하는 중요한 단백질이다. MCL-1 단백질은 세포의 생존을 촉진시키는 기능을 하는 pro-survival BCL-2 family member이며, 난소암, 혈액암, 자궁 경부암과 같은 다양한 암 조직에서 높게 발현하고 있다. 이전 연구에서, 본 연구진에 의해서 IEX-1 단백질은 MCL-1의 결합 단백질로 밝혀졌다. 본 연구에서는, 난소암 세포에서 IEX-1의 과다 발현에 의한 세포 사멸이 유도되는 것을 밝혔다. 더욱이, IEX-1 단백질은 MCL-1 단백질의 세포 생존을 위한 기능을 감소시켰으며, IEX-1에 의한 세포 사멸 효과는 MCL-1의 발현 정도의 변화에 의해서 그 기능이 영향을 받고 있었다. 그러므로 이러한 결과들은 IEX-1과 MCL-1은 세포 사멸과 생존을 조절하는 서로의 기능에 영향을 미치고, MCL-1의 기능에 대한 IEX-1의 세포 생존율 억제 효과는 항암치료 방법의 개발에 응용될 수 있는 가능성을 제시해 준다.

악성 흉막 삼출증의 조직학적 아형에 따른 빈도와 특성 (The Incidences and Characteristics of Malignant Pleural Effusions According to Histologic Types)

  • 연구배경: 악성 흉막 삼출증은 폐암에 합병되는 경우가 가장 흔하지만 그 외에 유방암, 난소암, 위암등에서도 호발하며 조직학적 아형에 따라 나누면 선암이 가장 흔하며 편평상피세포암이 악성 흉막 삼출을 일으키는 경우는 매우 드물다고 보고되어왔다. 연자들은 악성 흉막 삼출증의 조직학적 아형에 따른 빈도와 흉수의 특징을 분석하였다. 방 법: 1992년 1월부터 1997년 5월까지 서울대학교 병원에서 흉수의 세포진 검사나 경피적 혹은 흉강경을 이용한 흉막 조직 검사 결과 악성 흉막삼출증로 확인된 84예의 환자를 대상으로 하여 조직 표본과 의무기록을 통해 조직학적 아형을 구분하고 임상적 양상과 생화학적 지표 등을 조사하고 생존기간을 분석하였다. 결 과: 총 84명의 대상 환자 중 남자 52명, 여자 32명으로 남녀 성비는 1.6:1이었고 연령의 중앙값은 56세였다. 환자의 주 증상은 호흡곤란이 42명으로 가장 흔했고 기침과 객담, 흉막성 흉통 등의 순서였으며 평균 생존 기간은 6.4개월이었다. 그 중 혈성 흉수의 빈도는 66%. 림프구 우위성은 39%, 삼출성 흉수는 93%였고 조직학적 아형은 선암 54예(33예가 폐암), 편평상피세포암 10예 (8예가 폐암), 악성림프종 10예, 소세포암 8예 그리고 악성 중피종과 백혈병이 각각 1예였고 각각의 혈성 흉수, 림프구 우위성, 삼출성 흉수의 빈도는 별다른 차이가 없었으며 생화학적 수치의 차이도 관찰되지 않았다. 악성 흉막 삼출을 일으킨 원인 질환은 폐암이 49예로 가장 많았고 그외 원발 부위 불명확 악성 종양 7예, 위암, 유방암이 각각 5예, 담관암이 3예 그리고 난소암, 급성 백혈병, 악성 중피종이 각각 1예 씩 이었고 각각의 혈성 흉수, 림프구 우위성, 삼출성 흉수의 빈도, 생화학적 지표의 차이도 역시 관찰되지 않았다. 결 론: 같은 시기의 서울대학교병원의 폐암의 조직학적 아형의 빈도가 편평상피세포암 47%, 선암 33%, 소세포암 12%, 대세포암 2%인 것과 비교해보면 선암에서 악성 흉악 삼출증이 호발하며 편평 상피 세포암에서는 상대적으로 드물다는 것을 확인할 수 있었고 악성 흉막 삼출증 환자를 조직학적 아형이나 원발 악성 질환에 따라 분류하였을 때 혈성 흉수, 림프구 우위성, 삼출성 흉수의 빈도의 차이는 없었다.

Commonest Cancers in Pakistan - Findings and Histopathological Perspective from a Premier Surgical Pathology Center in Pakistan

  • Context: There are no recent authoritative data about incidence and prevalence of various types of cancers in Pakistan. Aim: To determine the frequency of malignant tumors seen in our practice and provide a foundation for building a comprehensive cancer care strategy. Materials and Methods: 10,000 successive cases of solid malignant tumors reported in 2014 were included. All cases had formalin fixed, paraffin embedded specimens available and diagnosis was based on histological examination of H&E stained slides plus ancillary studies at the Section of Histopathology, Department of Pathology and Laboratory Medicine, Aga Khan University Hospital, Karachi. The latest WHO classifications were used along with the latest CAP protocols for reporting and the most updated TNM staging. Results: There were 9,492 (94.9%) primary tumors while 508 (5.1%) were metastatic. Some 5,153 (51.5%) were diagnosed in females and 4,847 (48.5%) in males. The commonest malignant tumors in females were breast (32%), esophagus (7%), lymphomas (6.8%), oral cavity (6.7%) and ovary (4.8%), while in males they were oral cavity (13.9%), lymphomas (12.8%), colorectum (7.9%), stomach (6.9%) and esophagus (6.6%). Malignant tumors were most common in the 5th, 6th and 7th decades. About 8% were seen under 20 years of age. Conclusions: Oral cavity and gastrointestinal cancers continue to be extremely common in both genders. Breast and esophageal cancers are prevalent in females. Lung and prostate cancer are less common than in the west. Ovarian cancer was very common but cervix cancer was less so.
