Benign osteoblastoma of the mandible: a case report

  • Osteoblastoma is a rare benign neoplasm formed by osteoid tissue and well-vascularized bone that occurs mainly in children and adolescents. It appears primarily in the long bones, vertebral column, and small bones of the hands and feet, and not typically in the skull and maxillary bones. The purpose of this study is to present the case of an 8-year-old girl with a diagnosis of right mandibular osteoblastoma and a review of the relevant literature. The goals of treatment were to preserve dental occlusion, masticatory function and facial symmetry while minimizing the effects on patient body image and quality of life. Osteoblastoma, although it is benign, can be aggressive, and its treatment will depend on the timing of diagnosis, size and location. Early diagnosis is essential to avoid not only radical surgery as in the case presented, but also to help minimize the risk of possible relapse and potential malignancy of a benign osteoblastoma.

대퇴골두 부위에 발생한 양성 골 종양에 대한 경피적 고주파치료 (증례보고) (Percutaneous Radiofrequency Therapy of Benign Bone Tumors in the Femoral Head)

  • 서론: 대퇴골 근위부에 발생하는 종양은 양성일지라도 수술적 병변의 제거시 관절내 도달이 필요하거나 대퇴골 경부에 손상을 줄 수 있는 등 주위 고관절의 구조 및 기능을 위협할 수 있어 그 예후가 좋지 않을 수 있고 치료에 어려움이 있으나 고관절부의 동통을 유발하는 양성 골종양의 치료는 비관혈적 수술이 현재로는 거의 불가능하다. 최근 CT등을 이용한 경피적 절제술, 경피적 고주파 열치료등 비교적 덜 침습적인 치료법에 대한 관심과 연구가 증가하고 있으나, 아직까지는 대퇴골 근위부 종양중 비교적 흔한 유골골종(osteoid osteoma)에 국한된 연구가 대부분이다. 이에 저자는 대퇴골두내 발생한 내연골종 및 피질골내 연골종을 경피적 고주파 열치료를 통해 치료한 후 이를 보고하는 바이다. 증례: -증례1 - 31세 여자환자로 1년 전부터 시작된 우측 둔부 및 서혜부 외측의 통증을 주소로 내원하였다. 사무직 여성으로 과거력 및 가족력 상 특이 소견 없었으며 통증의 양상은 간헐적으로 야간에 심해지는 양상으로 통증 발현 시에는 체중 부하하기 힘든 정도였다. 통증에 대해 수개월간 대증적 치료를 시행하였으나 수일에서 수주 후 재발되는 양상이었으며 비스테로이드성 소염 진통제에 비교적 효과가 있었으나 내원 시에는 증상이 점차 심해지며 지속적인 양상으로 소염진통제가 계속적으로 필요하였다. 이학적 소견상 우측 고관절의 운동제한은 없었으나 Patrick 검사 양성이었으며 내회전시 불쾌감을 호소하였다. 단순 방사선 검사상 우측 대퇴 골두 부위에 주변으로 경화상을 보이는 방사선 투과성 병변이 관찰되었으며, MRI상 T1WI와 T2WI에서 낮은 신호강도를 보이면서 1.5${\times}$1cm 크기로 경계가 비교적 분명하였고 주변에 반응성 부종이 관찰되어. 유골골종의 가능성이 큰 것으로 보아 고주파열치료를 시행하기로 하였으며 전신마취하에서 먼저 CT시행 하에 세침 생검을 실시한 후 Radionics$^{(R)}$ 고주파침을 삽입하여 병변 중앙에 위치시킨 후 $80^{\circ}C$ 평균모드으로 6분간 고주파 열 치료 시행하였으며 시행 중 다른 합병증은 발생하지 않았다. 환자는 마취에서 의식이 돌아온 직후와 술 후 14시간 후에 통증 전혀 호소하지 않았으며 술후 1일째 체중 부하시 통증 없는 것 확인 후 퇴원 조치 하였으며 술 후 3주 뒤 MRI로 병변 부위 관찰하였으며, 이때 병변부위 소실된 것 확인할 수 있었다. 술 후 15개월째 환자는 일상 생활에 지장없었으며 통증 호소하지 않았다. 조직 검사결과는 피질골내 연골종으로 확인되었다. -증례 2- 56세 여자환자로 3개월 전부터 시작된 좌측 둔부 및 대퇴부 외측의 통증을 주소로 내원하였다. 통증은 주로 밤에 악화되었으며 1달 전부터 통증 심해지고 지속되는 양상으로 타 병원에서 아스피린으로 통증 조절되었고 유골 골종 의심 하에 본원으로 전원되었다. 과거력이나 가족력상 특이사항 없었으며 이학적 검사상 고관절 운동 범위 제한은 없었으나 외전과 내회전시 좌측 서혜부에 통증을 호소하였다. 단순 방사선 소견상 좌측 대퇴골두 하방 내측으로 석회화를 동반하고 주변 경화상을 보이는 방사선 투과성 병변이 관찰되었으며 MRI상 T1WI와 T2WI에서 비교적 낮은 신호강도를 보이는 양상으로, 유골골종의 가능성이 높은 것으로 보아 고주파 열치료를 시행하기로 하였으며 전신 마취 후 CT시행 하에 먼저 세침 조직 검사를 실시한 후 Radionics$^{(R)}$ 고주파침 삽입 후에 평균 모드 $80^{\circ}C$로 6분간 열 치료 시행하였고 환자는 마취에서 회복 후 즉시 통증 감소 보였으며 시술 후 1일째 이학적 검사 및 체중 부하 시행하였을 때 통증이 없는 것을 확인한 후 퇴원 조치 할 수 있었으며 술 후 1달째 MRI검사를 통해 병변이 사라지고 성공적으로 시술을 시행하였음을 확인할 수 있었으며 이후 시술 3개월째로 통증호소없었고 이학적소견상 정상이었다. 조직검사결과는 내연골종으로 확인되었다. 결론: 저자들은 수술적으로 치료가 비교적 어려운 대퇴 골두 부위에 발생한 피질골내 연골종과 내연골종에 대해 경피적 고주파열치료를 시행한 후 이를 보고하는 바이며 동통을 유발하는 유골 골종 이외의 다른 양성골 종양에 대해 치료 계획시 그 도달이 어렵거나 술 후 주위 구조물과 관절의 불안정성등의 합병증이 유발될 수 있는 경우 그 적응증의 확대를 신중히 고려해야 할 것이다.

개의 유선에서 발생한 혼합성 골육종 및 샘포 상피세포의 전이성 암종 (Mixed Osteosarcoma with Metastatic Alveolar Carcinomatous Appearance in Canine Mammary Gland Tumor)

  • 본 증례는 개의 유선에서 발생한 골형성성의 악성 혼합 유선 종양으로 샘포의 암종성 변화를 동반하고 있다. 종양은 12년령 암캐의 좌측 5번째 유선에서 절제되었으며 직경 $2{\sim}2.5cm$의 단단한 mass로 절단 시 경도가 높은 골성의 구조를 가지고 있었다. 현미경학적 관찰 시, 골유사 물질이 미네랄 침착되고 있었으며 다수의 골형성 세포와 일부 파골세포가 골생성 종양 기질 전반에 걸쳐서 관찰되었다. 이러한 골유사 병변은 높은 밀집도를 보이는 근상피 세포와 연접하고 있었으며 이러한 세포들은 수 개의 유사분열상을 나타내고 있었다. 이와 더불어, 유선세관과 샘포의 암종성 변화를 보이는 세포들이 인접 기질로 침습하고 있는 모습이 관찰되었고 이것 역시 증가된 근상피 세포들로 둘러싸여 있었다. 본 증례에서 볼 수 있는 이러한 세포들의 출현은 동시 발생된 악성 종양의 형태를 제시할 수 있으며 종양의 기원은 상피 유래의 암종성 조직과 중간엽 유래의 육종성 연골 및 골 조직으로 구별할 수 있겠다.

흡수성 차폐막과 동종탈회동결건조골 이식에 의한 치조골 재생의 병용효과 (Alveolar Bone Formation in Dogs using Vicryl Absorbable Mesh(Polyglactin 910) and Decalcified Freeze-Dried Bone Grafting)

  • The purpose of this study was to evaluate new bone formation following guided bone regeneration by resorbable and nonresorbable membrane. Six adult mongrel dogs were used. The first, second, third, fourth premolars in the mandible of each dog were extracted. Two months after tooth extraction, a buccal dehiscence defect was surgically created on each edentulous area. The experimental sites were divided into three groups according to the treatment modalities ; Group I-a: surgical treatment only ; Group I -b: allogenic decalcified freezed dried bone grafting ; Group II-a : e- PTFE membrane placement only ; Group II-b : allogenic decalcified freezed dried bone grafting and e-PTFE membrane placement ; Group III-a : Vicryl(R) mesh placement only ; Group III-b : allogenic decalcified freezed dried bone grafting and Vicryl(R) mesh placement . The animals were sacrificed at 8 weeks after operation and the specimens were prepared for histologic and histometric examination. The results were as follows : Clinically, all defect sites were healed without exposure of barrier membrane after the eight weeks. In Group I-a, dense connective tissues were impinged in the bony defect area. Well vascularized and fibrous bone marrow indicated that bone formation was still taking place was found. In Group I-b, in areas closer to the periphery, lamellation of the newly formed bone would found. In Group II-a, beneath the e-PTFE membrane a dense layer of connective tissue covering the most external portions of the regenerated tissue was seen. The new bone surfaces were lined with osteoid and osteoblast. In Group II-b, a dense layer of connective tissue covering the most external portions of the regenerated tissue was observed beneath the e-PTFE membrane. A notable amount of alveolar ridge regeneration was seen with new rigdes with well-contoured form. In Group III-a, the new bone surface were lined with osteoid and osteoblast, indicating active bone formation. A clear demarcation could not be noted between the host bone and new bone. In Group III-b, a notable amount of alveolar ridge regeneration was seen with new ridges assuming wellcontoured form. In areas closer to the periphery, lamellation of the newly formed bone would found. As histometric examination, the amount of bone formation was gained from $12.8mm^2$ to $26.3mm^2$. It was significantly greater in group II-b and group III-b compared to other groups(p<0.05) . These results suggest that Vicryl(R) mesh after DFDB grafting used in guided bone regeneration could create and sustain sufficient space for new bone formation.

치아회분(齒牙灰粉)과 도재(陶材) 복합(複合) 매식체(埋植體)에 관(關)한 광학현미경(光學顯微鏡) 급(及) 주사전자현미경적(走査電子顯微鏡的) 연구(硏究) (A Light and Scanning Electron Microscopic Study on the Implant of Tooth Ash-Porcelain Mixture)

  • The purpose of this study was to investigate whether the ashed tooth powder is utilized as an alternative material of the implant to recovery the bony defect. For this purpose its biocompatibility was evaluated comparing to the synthetic calcium phosphate compounds, such as Syntograft and Calcitite, as well as the vacuum firing porcelain (Ceramco Inc.) which is anticipated to use as a matrix to aid sintering. Bony defects to exposure the bone marrow, $3{\times}5$ mm in size, were created in the right and left tibias of fifteen rabbits, and then the ashed tooth powder at $950^{\circ}C$, the porcelain powder, Syhtograft and Calcitite were inserted in the defects of twelve rabbits of the experimental group and the blood clot only was filled in the defects of three rabbits of the control group. The experimental and control rabbits were sacrificed at 1st, 2nd 3rd week after implantation and the histologic examination was performed. The ashed tooth powder in order to make the needed form of the implant was molded using the cylindrical mold 1 cm high, 1 cm in diameter under the pressure of $1000kg/cm^2$ and the ashed tooth powder was sintered at $1100^{\circ}C$ for 1 hour and the mixture of the porcelain powder and the ashed tooth powder at the weight ratio of 7:3, 6:4, 5:5, 4:6 were molded in the same manner and were sintered at $925^{\circ}C$. From this sintered material, square shaped implants were prepared in the dimension of $2{\times}4{\times}6mm$. The prepared implants were surgically placed in the subperiosteum of lateral surfaces of the right and left mandibular bodies. The dogs were sacrificed at 4 weeks, and then the specimens were examined using the light and scanning electron microscopes. The results of this study were obtained as follows: 1. Any inflammatory response was not noted after implanting of the ashed tooth powder, Syntograft, Calcitite and the porcelain powder during the whole experimental period after implantation. 2. Induction of the new bone formation was significantly shown in the ashed tooth powder, Syntograft and Calcitite. 3. The more the porcelain powder was contained in the implants, the more the porosity was and the bigger the pore size was under the scanning electron microscope. And there was ingrowing of the fibrous connective and the osteoid tissue. 4. The osteoid tissues were found to be directly fused to the implant of the ashed tooth powder, and the mixture implant of the porcelain powder and the ashed tooth powder at the weight ratio of 4:6 under the light and scanning electron microscopes.

탈회동결건조골과 합성골이식재가 치조골 결손부 재생과정에 미치는 영향 (THE EFFECTS OF DECALCIFIED FREEZE-DRIED BONE AND SYNTHETIC BONE GRAFTS ON REGENERATION OF ALVEOLAR BONE DEFECTS IN DOGS)

  • The purpose of this study was to compare effects of various bone grafts on periodontal regeneration of alveolar bone defects in dogs. Seven adult dogs aged 12 to 18 months were used in this study. Experimental alveolar bone defects were created surgically with a #1/2 round bur at the furcation area of the buccal surface of the mandibular 3rd, 4th premolars and 1st molar. Each experimental alveolar bone defects were grafted with dense hydroxyapatite, natural coral, and decalcified freeze-dried bone, and respectively divided into DHA, NC, DFDB group. An area without bone graft was divided into control group. At 1,2,4,6, and 12 weeks, dogs were serially sacrificed and specimens were prepared with Hematoxylin-Eosin stain and Mallory stain for light microscopic evaluation. The results of this study were as follows : 1. In control group, the matrix change of granulation tissue was observed at 1 week. And in experimental groups, the appearance of connective tissue around graft materials was loosely formed at 1 week, but densely formed at 2 weeks. 2. In every group, the slight formation of new trabecular bone was seen from remaining bone at 1 week. 3. The DHA and NC particles were gradually encapsulated by new trabecular bone from remaining bone, and the osteoid tissue was directly induced from DFDB particles. 4. The presence of osteoblasts was first observed at 1 week in control group and at 2 weeks in NC group, but at 6 weeks in DHA group. 5. In DHA group, the resorption of particles was not observed until 12 weeks. But in NC and DFDB group, the particles were resorbed at 6 weeks and replaced by new bone. And the amount and size of particles were reduced, and their border represented irregular form. In summary, in three experimental groups the inflammatory or foreign body reaction were slight, but the regeneration of new osteoid tissue and the matrix change of dense connective tissue fiber were observed. Especially, NC and DFDB materials were considered as the biocompatible graft materials which were effective in the regenertion of new bone.

가토에 이식된 Titanium plasma Sprayed IMZ 임프란트와 골의 계면 접촉 양상에 따른 결합력에 관한 연구 (A STUDY ON SHEAR BOND STRENGTH OF INTERFACE BETWEEN BONE AND TITANIUM PLASMA SPRAYED IMZ IMPLANT IN RABBITS)

  • In recent years immediate implantation has been tried by a few clinicians. This study placed IMZ implants in the rabbit femur with and without bony defects around the implant for simulating fresh extraction site. And one group with bony defects used porous hydroxyapatite ganules(HA) to fill if and the other group left the bony defects around the implant. The purpose of this study was to compare the shear bond strength and the bony contact and formation around the implant. Fifteen rabbits were divided into three groups and placed 10 IMZ implants to each group. Implant sites were surgically prepared with IMZ drills kit and implants were placed(Control), artificial bony defect was created with Apaceram drills kit around the implant sites and implants were placed(Experimental I), bony defect was filled with porous hydroxyapatite granules(Experimental II). Thereafter, rabbits were sacrificed at 8th week and specimens were prepared and pushout tested for shear bond strength of bone-implant interface immediately. Undecalcified and decalcified specimens were prepared with Vilanueva and hematoxylin-eosin stain for light microscopic finding. The results of this study were as follows. 1. In the control group, mean shear strength of bone-implant interface was $2.614{\pm}0.680$ MPa, experimental I was $0.664{\pm}0.322$ MPa, and experimental II was $2.281{\pm}0.606$ MPa. There was significant difference between control and experimental I, between experimental I and experimental II, but did not show significant difference between control and experimental II statistically. 2. In the bony formation surrounding IMZ implant of the three groups, that of cortical bone is more advanced than cancellous bone area. 3. In the histological findings of undecalcified specimens, control and experimental II showed more than 50% of bony or osteoid formation at the bony-implant interface. 4. In the histological findings of undecalcified specimens, experimental I showed less than 50% of bony or osteoid formation at the interface, and observed partial bony defect in the coronal zone. 5. In the experimental II group, were observed direct bony contact to hydroxyapatite granules, and infiltration of a few giant cells. 6. No inflammatory responses were seen around the titanium implants and the hydroxyapatite granules.

치아재식시 골형태형성단백이 치주조직 치유에 미치는 영향 (EFFECTS OF BONE MORPHOGENETIC PROTEIN ON THE HEALING OF PERIODONTIUM AFTER TOOTH REPLANTATION OF THE RAT)

  • The purpose of this study is to evaluate the influences of the bone morphogenetic protein (BMP) on the healing of periodontal ligament and alveolar bone after replantation of tooth, and to examine the possibility of its clinical application. 45 Sprague Dawley rats weighted about 100 gram were divided into 3 experimental groups by different dose of BMP. All the upper right and left 1st molar were extracted after 5 days feeding of 0.4% ${\beta}$-aminopropionitrile, and right molar were used as experimental group and left molar were used as control group. The root surface of experimental molar were treated with 25,50 and l00ng/ml of human recombinant Bone morphogenetic protein-4 (rh-BMP-4) with micropipet, and 1M Sodium hypochloride were used on control root surface. All the experimental animals were sacrificed as 1, 2, 4, 7 and 14 days after autoreplantation of upper 1st molar into their own position. The maxilla were disected included both side of 1st molar. The collected tissue were processed from demineralization to paraffin embeding as usual procedure, and the specimens were prepared with Hematoxylin-Eosin stain for the light microscopic evaluation. The results of this study were as follows : 1. There was no significant differences between control and experimental site on 1 and 2 days after replantation of tooth. In the case of 4th days, the evidence of tissue regeneration were observed on experimental site to compare the controls. New osteoid were revealed on high concentration of BMP at 7 days after replantation, and it became more obvious at 14 days, 2. The effect of the rh-BMP-4 coated on root surface was revealed slight influences for the prolifertion of cells of periodontium and tissue regeneration as dose-dependent pattern. 3. Bony ankylosis was observed between alveolar bone and root surface due to the remarkable amount of osteoid formation on the 14 days after replantation of root. In the conclusion, it was suggested that topical application of the rhBMP-4 on the root surface has influence on the periodontal ligament and alveolar bone. The application method of BMP on the root should be designed with calculation of proper concentration.

발생중인 흰쥐에 전신적으로 투여된 불소가 골형성에 미치는 영향 (The Effects of Fluoride Administered Systemically during Rat Development on Forming Bone)

  • 본 연구는 전자현미경을 이용하여 음용 불소가 발생중인 경골의 성장변화와 골모세포의 조직학적 특성에 미치는 영향을 관찰하고 에너지분광분석기를 사용하여 불소의 투여 농도에 따른 골기질 성분변화를 알아보고자 하였다. 전자현미경 관찰 결과, 대조군에서는 입방형의 전형적인 골모세포가 관찰되었다. 100 ppm 불소투여군의 경우, 활성이 강한 골모세포에 의해 생성된 여러층의 반전선과 새로 형성된 골성조직이 관찰되었다. 또한 골모세포의 세포질은 잘 발달된 조면세포체와 미토콘드리아가 관찰되었고 세포막 주위에는 많은 분비소포들이 관찰되었으며, 일부는 세포막과 융합되어 분비물질이 세포 밖으로 방출되었다. 200 ppm 불소투여군에서는 활성이 저하된 골모세포가 관찰되었는데, 세포질내에는 미토콘드리아가 팽창되어 있었고 수조의 형태가 파괴되어 나타났다. 또한, 조면소포체의 표면에는 리보솜이 탈락되어 관찰되었다. 300 ppm의 고농도 불소투여군에서는 골내막과 인접한 골모세포는 불규칙하게 배열되어 있었고, 세포막은 파괴되어 세포성분이 세포 밖으로 용출되어 관찰되었다. 한편, 에너지분산분광분석 결과는 대조군에 비해 100 ppm 불소투여군에서 골기질 성분인 인과 칼슘이 증가하였으나 200 ppm 및 300 ppm 불소투여군에서는 더 이상 증가하지 않았다. 따라서 본 연구결과, 투여된 불소에 의해 골모세포의 활동이 활발해지고 왕성한 골기질 합성이 관찰되었으나, 고농도의 불소 투여는 골모세포를 파괴하고 활동을 억제한다는 것을 확인하였다.

거골 체부에 발생한 양성 종양 (Benign Tumors of the Talar Body)

  • 목적: 거골 체부에 발생하는 양성 종양은 매우 드물어 치료 방법에 따른 결과를 비교하기 어렵다. 이 연구에서는 거골 체부에 발생한 양성 종양 8예를 후방 도달법과 후 돌기를 통한 소파술로 치료한 결과를 보고하고자 한다. 대상 및 방법: 1986년 2월부터 2001년 8월 사이에 저자들이 경험한 거골 체부에 발생한 양성 골종양 환자 8명을 연구 대상으로 하였다. 종양의 발견 당시 평균 연령은 22.1세(10세~41세)이었고, 남자 4명, 여자 4명이었다. 종양의 종류는 거대세포종 2례, 유골 골종 2례, 연골모세포종 1례, 골내 모세혈관종 1례, 단순 골 낭종 1례, 골연골종 1례이었다. 모두 후방도달 법으로 접근하였고, 2례의 유골 골종과 1례의 골연골종 환자는 절제술을, 나머지 5례는 후 돌기를 통한 소파술을 시행하였다. 평균 추시 기간은 7.5년(1년~16년)이었다. 결과: 추시 기간 중에 재발한 환자는 없었고, 1례에서 감염증이 발생했다. 이 경우를 제외한 모든 경우에서 체중 부하시 통증은 없었고, 관절 운동 범위의 제한이 발생한 경우도 없었다. 결론: 거골 체부에 발생한 양성 종양은 후방 도달법으로 주위 조직의 손상 없이 안전하게 접근할 수 있으며, 거골의 후 돌기를 통한 피질골 창으로 만족할 만한 소파술이 가능함을 알 수 있었다.
