Paraplegia following spinal epidural anesthesia is extremely rare. Various lesions for neurologic complications have been documented in the literature. We report a 66-year-old female who developed paraplegia after left knee surgery for osteoarthritis under spinal epidural anesthesia. In the recovery room, paraplegia and numbness below T4 vertebra was checked. A magnetic resonance image (MRI) scan showed a spinal thoracic intradural extramedullary (IDEM) tumor. After extirpation of the tumor, the motor weakness improved to the grade of 3/5. If a neurologic deficit following spinal epidural anesthesia does not resolve, a MRI should be performed without delay to accurately diagnose the cause of the deficit and optimal treatment should be rendered for the causative lesion.
Purpose :The present study invesigated the effect of changes in transversus abdominis thickness using ultrasonography during a hip adductor contraction. Methods : This study was carred out in a volunteer sample of adults (N=30) without a history of low back pain or injury. In standing position, muscle thickness measurements of transversus abdominis(TrA) were measured using ultrasonography at rest and during a hip adducor contraction. Results : TrA thickness were influenced a hip adductor during a voluntary contraction in people without LBP. TrA showed significantly greater thickness changes on a hip adductor contraction.(p=0.000) Conclusion : The results from this study showed that the hip adductor contraction improves the ability to increase change in TrA thickness. These results can be a good evidence to prevent low back pain due to hip adductor weakness or genu varum deformity of knee osteoarthritis.
The purpose of this case report is to introduce a case of immediate results with TMJ balancing therapy (TBT) in the treatment of chronic patellofemoral pain (PFP). A 17-year-old female patient with 4-month-old chronic knee pain that was unresponsive to conventional treatment was treated with TBT for a total of 5 sessions. The primary outcomes were evaluated using the Numeric Rating Scale (NRS) and the Korean version of the Western Ontario and McMaster Universities Osteoarthritis Index (KWOMAC). Following the first treatment, the NRS decreased rapidly, and the KWOMAC improved after a total of five treatments. These findings suggest that TBT can be an effective pain management option for chronic PFP patients with temporomandibular joint malposition. However, as this study is a report of a single case, further research is necessary.
High tibial osteotomy (HTO) is a widely accepted treatment for unicompartmental osteoarthritis of the knee and other lower extremity deformities, particularly for young and active patients. However, it is generally recognized as a technically demanding procedure. Thus the limitations of conventional surgical methods have been reported. Currently, the use of computer-aided surgery (CAS) system for successful surgery was increased. However it has been reported many problems such as expensive equipment, infection or fracture caused by inserting half-pin and delaying surgery. Therefore we propose a novel method which can be tracked by using tracking-pin inserted in arbitrary locations of the femur and tibia, and fluoroscopic images obtained from G-arm of antero-posterior and lateral-medial.
Arthritis may be broadly classified as degenerative - related to defects in cartilage and other joint constituents, often age-associated - or inflammatory disease. Inflammatory arthritis called as rheumatoid arthritis (RA) is a chronic inflammatory arthropathy and characterized by a destructive arthritis. RA encompasses infectious arthritis, arthritis caused by intra-articular deposits of crystalline material (gout), syndromes associated with genetic defects (familial Mediterranean fever), and the immune-mediated inflammatory arthropathy. Degenerative arthritis called as osteoarthritis (OA), which is most frequently occurring, causes degenerative figures of knee, waist and knuckle, and accompanies severe pain around the cartilage. Also, it may cause morning stiffness, gelling effect, tenderness, bone swelling, crepitus, and motion disorders.
In this review, we searched for clinical and experimental studies related to acupuncture-related therapy (ART) on the microbiome in musculoskeletal disorders (MSDs) through the electronic databases of MEDLINE via PubMed, EMBASE, and Oriental Medicine Advanced Searching Integrated System up to May 2023, without language restriction, and after the selection/exclusion process, the study design, target disease, intervention details, treatment period, outcomes, and study results were extracted. A total of 8 articles were selected. Two randomized controlled trials and 6 animal studies evaluated knee osteoarthritis, rheumatoid arthritis, spinal cord injury, ankylosing spondylitis, and osteoporosis. ART, including electroacupuncture, thread-embedding acupuncture, and moxibustion, affected microbiome modulation in MSDs. The results reveal that ART could be a potential treatment for regulating the microbiome in MSDs. However, further high-quality studies are needed.
Objectives The purpose of this study is to compare the effects between the KL-Grade and improvement of knee pain treated by Korean Medicine therapy. Methods 114 patients who received inpatient treatment from July 2014 to May 2017 in the Daejeon Jaseng of Korean Medicine Hospital were divided into 5 groups by the KL-Grade. All patients received a combination of treatment including acupunture, pharmacopunture, herbal medication. They were compared and analyzed on the basis of improvement between measuring Numeric Rating Scale (NRS), Western Ontario and McMaster Universities Arthritis Index (WOMAC Index), EuroQol-5 Dimension Index (EQ-5D Index) as they were hospitalized and as they were discharged. The statistically significance was evaluated by SPSS 23.0 for windows. Results After treatment, KL-Grade 0 group's Numeric Rating Scale (NRS), Western Ontario and McMaster Universities Arthritis Index (WOMAC Index), EuroQol-5 Dimension Index (EQ-5D Index) improvement was $2.02{\pm}1.69$, $7.50{\pm}9.67$ and $0.11{\pm}0.15$ respectively. KL-Grade 1 group's improvement was $2.09{\pm}1.23$, $11.75{\pm}13.99$ and $0.12{\pm}0.13$ respectively. KL-Grade 2 group's improvement was $1.60{\pm}1.07$ and $14.70{\pm}14.19$ respectively. But In this group, EQ-5D Index has decreased by $0.01{\pm}0.10$. KL-Grade 3 group's improvement was $1.88{\pm}1.31$, $7.81{\pm}13.35$ and $0.13{\pm}0.20$ respectively (p<0.034). In the case of KL-Grade 4, the population was not statistically significant (N=2) and therefore excluded from statistical significance. And there was no statistically significance between 4 group's improvement after treatment (p>0.05). Conclusions The above study showed that Korean medicine treatments showed significant therapeutic effects on knee pain and degenerative knee joints, but there was no significant difference in the effectiveness of degenerative arthritis (KL-Grade).
목적: 고정형 슬관절 단일 구획 치환술의 5년 추시 결과로 임상적 및 방사선적 결과, 합병증 발생 빈도를 알아보고자 하였다. 대상 및 방법: 2003년 5월부터 2011년 8월까지 고정형 슬관절 단일 구획 치환술을 시행한 25명, 26예를 대상으로 하였다. 평균 연령은 63.5세였으며 남자가 3명(3예), 여자가 22명(23예)이었으며 술 전 진단은 골관절염 23예, 골괴사 3예였다. 평균 추시기간은 평균 67개월(60-149개월)이었다. 임상적으로 술 전과 술 후의 미국슬관절학회 슬관절 점수 및 기능점수, 관절 운동 범위를 조사하였고, 방사선적으로 슬관절 기립 전후방 사진과 측면 사진, 투시기 촬영을 통하여 술 후의 정렬상태와 골용해 여부를 조사하였다. 결과: 임상적 결과로 슬관절 점수는 평균 42.0점에서 87.9점으로, 기능점수는 57.5점에서 85.0점으로 향상되었다(p<0.001). 술 전 관절 운동 범위는 평균 $132.9^{\circ}$에서 술 후 $132.5^{\circ}$로 큰 차이를 보이지 않았다. 방사선적 결과로 대퇴경골각은 술 전 평균 내반 $0.5^{\circ}$에서 술 후 외반 $2.2^{\circ}$로 측정되었다. 방사선 투과성 선(radiolucent line)이 관찰되는 경우가 2예 발견되었으며 그 중 1예는 삽입물이 안정적이었으나 다른 1예는 단일 구획 치환술 시 대퇴슬개관절의 관절염을 확인하였고, 추시상 슬관절 내측 관절선의 동통 및 슬관절의 미만성 통증이 있어 슬관절 전치환술을 권유하였다. 그 외 골용해, 감염, 술 후 강직, 탈구 등의 합병증은 없었다. 결론: 고정형 슬관절 단일 구획 치환술의 중기 추시 결과는 임상적 및 방사선적으로 만족스러운 결과를 보였다.
