심박변동신호에 의한 자율신경 기능 해석시스템의 설계 (The desitgn of autonomic function analysis system by using heart rate variability signal)

  • In this paper, the autonomic function analysis system was designed to noninvasively assess the autonomic function of cardiovascular system. Orthostatic stress protocol was designed to stimulate the autonomic nervous system. designed protocol and analysis algorithm were evaluated by experiments for 25 normal subjects and 22 hemiplegia patients. Data were processed by usign the power spectral analysis. Nwe indexes of autonomic function, LF$_{N}$ and HF$_{N}$, were proposed and were compared with LF/HF ratio. New indexes of the sympathetic and parasympathetic activity, respectively. The IST and the DPT are balanced and have positive value for normal subjects during orthostatic stress but not for hempilegia patients. This result suggest that the IST and the DPT are used as new criteria of normal autonomic function during orthostatic stress.ess.

  • Postural orthostatic tachycardia syndrome (POTS) is characterized by increased heart rate with preserved blood pressure on orthostatic stress. Many patients with postural orthostatic tachycardia syndrome can be misdiagnosed as neurosis, chronic fatigue or anxiety disorder. We report a patient with POTS who presented chronic dizziness and fatigue. In approaching to a patient with orthostatic or nonspecific chronic dizziness, the postural heart rate as well as blood pressure should be checked not to miss the diagnosis.

  • Orthostatic stress in human cardiovascular system following spaceflight remains a critical problem in the current lifercience space program. The study presented in this paper is part of an ongoing effort to use mathematical models to investigate the effects of gravitational stresses on the cardiovascular system of normals and microgravity adapted individuals. We employ a twelve compartment lumped parameter representation of the hemodynamic system coupled to set-point models of the arterial baroreflex and the cardiopulmonary reflex to investigate the transient response of heart rate to orthostatic stress. We simulate current hypotheses concerning the mechanisms underlying postspaceflight orthostatic intolerance over a range of physiologically reasonable values and compare the simulations to astronaut stand-test data pre-and postflight.

Head-up Tilt상태에서 심박변동과 자율신경활동 균형의 관계에 관한 연구 (A Study on Relationship between Heart Rate Variabilities and Autonomic Balance during Head-up Tilt)

  • In this paper, the power spectral analysis of heart rate variability(HRV) was performed to evaluate effects of orthostatic stress with head-up tiIt on autonomic nervous system(ANS) for 25 healthy male subjects(age : 24 $\pm$ 5 yr.) and a new method was proposed to assess the autonomic balance. The ECG and respiration signals were recorded at tiIt angles of $0^{\circ}$, $45^{\circ}$, $90^{\circ}$and $0^{\circ}$ successively for 10 minutes per each stage under the condition of frequency controlled respiration(0.25Hz). Heat rate(HR) gradually increased ils the angle increased Similarly, according to the increment of angle, normalized low frequency component(0.05-0.15Hz) gradually increased, whereas normalized high frequency component (0.20-0.30Hz) was reduce4 From these results it is speculated that orthostatic stress head-up tiLt, results in the prevalence of sympathetic tone in autonomic balance with the increment of sympathetic tone and the decrement of parasympathetic tone, which seems to meanthat autonomic nervous system plays a major role in compensating for disturbances of cardiovascular system due to it.

  • Near-infrared spectroscopy (NIRS) monitoring has been used mainly to detect reduced perfusion of the brain during orthostatic stress in order to assess orthostatic intolerance (OI). Many studies have investigated the use of NIRS to reveal the pathophysiology of patients with OI. Research using NIRS in other neurological diseases (e.g., stroke, epilepsy, and migraine) is continuing. NIRS may play an important role in monitoring the regional distribution of the hemodynamic flow in real time and thereby reveal the underlying pathophysiology and facilitate the management of not only patients with OI symptoms but also those with various neurological diseases.

  • In this paper, the power spectral analysis of heart rate variability(HRV) was performed to evaluate effects of orthostatic stress with head-up tilt on autonomic nervous system(ANS) for 20 healthy male subjects(age : 245 yr.) and a new method was proposed to assess the autonomic balance. The ECG signals wore recorded for 3 minutes in both the supine and 70 head-up tilt positions, and then the HRV signals underwent power spectrum analysis at each position. The results of this study suggest that cardiac autonomic functions, such as sympathetic tone in autonomic balance with the increment of sympathetic tone and the decrement of parasympathetic tone which occur during head-up tilt position, arc not sufficient to overcome tile orthostatic stress arising in Cerebral Palsy.

  • Near-infrared spectroscopy (NIRS), a noninvasive optical method, utilizes the characteristic absorption spectra of hemoglobin in the near-infrared range to provide information on cerebral hemodynamic changes in various clinical situations. NIRS monitoring have been used mainly to detect reduced perfusion of the brain during orthostatic stress for three common forms of orthostatic intolerance (OI); orthostatic hypotension, neurally mediated syncope, and postural orthostatic tachycardia syndrome. Autonomic function testing is an important diagnostic test to assess their autonomic nervous systems for patients with symptom of OI. However, these techniques cannot measure dynamic changes in cerebral blood flow. There are many experimentations about study of NIRS to reveal the pathophysiology of patients with OI. Research using NIRS in other neurologic diseases (stroke, epilepsy and migraine) are ongoing. NIRS have been experimentally used in all stages of stroke and may complement the established diagnostic and monitoring tools. NIRS also provide pathophysiological approach during rehabilitation and secondary prevention of stroke. The hemodynamic response to seizure has long been a topic for discussion in association with the neuronal damage resulting from convulsion. One critical issue when unpredictable events are to be detected is how continuous NIRS data are analyzed. Besides, NIRS studies targeting pathophysiological aspects of migraine may contribute to a deeper understanding of mechanisms relating to aura of migraine. NIRS monitoring may play an important role to trend regional hemodynamic distribution of flow in real time and also highlights the pathophysiology and management of not only patients with OI symptoms but also those with various neurologic diseases.

  • In this paper, the power spectral analysis and fractral dimension analysis of heart rate variability(HRV) were performed to evaluate effects of orthostatic stress with head-up tilt on autonomic nervous system(ANS) for 24 young and healthy subjects(age: $24{\pm}5yr$.). The ECG and respiration signals were recorded at the tilt angle of $0^{\circ},\;15^{\circ},\;30^{\circ},\;45^{\circ},\;70^{\circ}$ and $90^{\circ}$ successively for 5 minutes per each stage under the condition of frequency controlled respiration (0.25Hz). Heart rate(HR) gradually increased as the angle increased. Similarly, according to the increment of angle, normalized low frequency(LF) component(0.05-0.15Hz) gradually increased, whereas normalized high frequency(HF) component (0.20-0.30Hz) was reduced. 정상인과 편마비 환자의 심박변동신호의 특성에 관한 연구 (A Study on the Characteristics of Heart Rate Variabilities In Nornal Subjects and Hemiplegic patients)

  • In this paper, the power spectral analysis and the fractal analysis of heart rate variability(HRV) were performed to evaluate the effects of brain lesion on cardiovascular system and autonomic function for 24 normal subjects and 22 hemiplegic patients. The ECG and respiration signals were recorded at tilt angles of $0^{\circ}$ and $70^{\circ}$ for 5 and 6 minutes successively under the condition of frequency controlled respiration (0.25Hz). For normal subjects, HR, LF component, HF component and fractral dimension of HRV were distinctly changed after orthostatic stress, whereas, for hemiplegic patients, those were little changed. Complexity and variability of heart rate of patients were smaller than those of normal subjects. Sympathetic tone of patients was higher than that in normal subjects. All of these results support that autonomic disorder and cardiovascular disturbance accompanied by brain lesion could be assessed by the power spectral analysis and fractal analysis of HRV.

대학생의 성별, 스트레스, 불안 및 우울 정도에 따른 심박변이도 차이: Neurovisceral integration model에 기반하여 (Differences in Heart Rate Variability Depending on Sex, Level of Stress, Anxiety, and Depression among College Students: on the Basis of Neurovisceral Integration Model)

  • Purpose: This descriptive study aimed to investigate heart rate variability (HRV) according to sex and to elucidate the influence of negative emotion such as levels of stress, anxiety and depression on HRV among Korean college students based on a neurovisceral integration model. Methods: A descriptive study design was used. Eighty-six healthy college students participated in the study. Resting HRV and standing HRV on orthostatic stimulation were measured for 5 minutes during 4-6 p.m. in the afternoon. Levels of stress, anxiety and depression were assessed using the Global assessment of recent stress, State-Trait Anxiety Inventory and Beck Depression Index, respectively. Results: Out of the 86 students, 47 (54.7%) were men and 39 (45.3%) were women. Root mean square of the differences between adjacent RR intervals (RMSSD) and normalized high frequency (nHF) on standing HRV were significantly lower in men than in women (p= .005, p= .019, respectively). Male gender (β= 0.30, p= .013), higher level of stress (β= -0.36, p= .009) and lower level of depression (β = 0.30, p= .044) exerted a significant influence on decreased nHF in the multiple regression analysis. Conclusion: We suggest that men are more vulnerable to having reduced vagal activity on HRV than women. Since male gender, higher level of stress and lower level of depression level influenced decreased vagal activity, strategies are needed to improve stress and depression rather than anxiety especially for men, which contribute to promoting HRV to prevent cardiac health diseases.
