Effect of Anterior Guidance Change on the Condylar Path in Skeletal Class I Young Adult Women Using a Splint with Flat or Steep Anterior Guidance

  • Purpose: To investigate the effects of anterior guidance (AG) change on the working (WCP) and non-working condylar paths (NWCP), and lower incisor path (LIP) using a splint with flat (FAG) or steep AG (SAG). Materials and Methods: The samples consisted of six young adult women (mean age=$23.5{\pm}3.3$ years). Inclusion criteria were skeletal Class I and normodivergent pattern, normal overbite/overjet, minimal slide from retruded cuspal position to intercuspal position, no temporomandibular disorder signs and symptoms, mutually protected occlusion, and minimal tooth wear. After the values of natural AG (NAG) were obtained as a reference for each patient, two types of splints ($15^{\circ}$ flatter and steeper than NAG) were made. After insertion of the splints with FAG or SAG, the WCP, NWCP, and LIP were recorded five times for each patient using an ultrasonic AQR (SAM, Munich, Germany) and statistical analysis was subsequently performed. Result: NAG exhibited postero-superior movement in the WCP and did not show a noticeable immediate side shift (ISS) or difference between the eccentric (EP) and returning paths (RP) in the NWCP. FAG was associated with an irregular and excessive WCP, an increase in ISS, and a difference between EP and RP in the NWCP. Exosomes from Tension Force-Applied Periodontal Ligament Cells Promote Mesenchymal Stem Cell Recruitment by Altering microRNA Profiles

  • Background and Objectives: To investigate the role of exosomes from periodontal ligament cells (PDLCs) in bone marrow mesenchymal stem cell (BMSC) migration. Methods and Results: Human PDLCs were applied cyclic tension stretching. Exosomes were extracted from cultured PDLCs by ultracentrifugation, then characterized for their size, morphology and protein markers by NTA, TEM and western blotting. The process that PKH26-labeled exosomes taken up by BMSCs was assessed by confocal microscope. BMSC migration was examined by Transwell assay. Exosomes derived from PDLCs were identified. Cyclic tension stretch application on PDLCs can enhance the migration ability of BMSCs through exosomes. The exosomal miRNA expression profiles of unstretched and stretched PDLCs were tested by miRNA microarray. Four miRNAs (miR-4633-5p, miR-30c-5p, miR-371a-3p and let-7b-3p) were upregulated and six (miR-4689, miR-8485, miR-4655-3p, miR-4672, miR-3180-5p and miR-4476) were downregulated in the exosomes after stretching. Sixteen hub proteins were found in the miRNA-mRNA network. Gene Ontology and KEGG pathway analyses demonstrated that the target genes of differentially expressed exosomal miRNAs closely related to the PI3K pathway and vesicle transmission. Conclusions: The exosomes derived from cyclic tension-stretched PDLCs can promote the migration of BMSCs. Alternation of microRNA profiles provides a basis for further research on the regulatory function of the exosomal miRNAs of PDLCs during orthodontic tooth movement.

치아이동 후 고정방법이 성견 치주인대 섬유의 재배열에 미치는 영향 (The Effect of Splinting Methods on the Rearrangement of Periodontal Fibers after Tooth Movement in Adult Dogs)

  • 실험적 치아이동 후 보정기간 동안 고정방법이 치주인대 섬유의 재배열에 미치는 영향을 알아보기 위하여, 체중 20kg 내외의 성견에서 상악 제 3절치와 견치 사이에 견인 스프링을 이용하여 120 gm의 교정력을 3주간 가한 후 좌측에는 유연성 고정장치로 0.0215 인치 multistrand wire를, 우측에는 견고성 고정장치로 폴리 에틸렌 리본을 레진 접착제로 각각 장착한 다음 4주, 8주, 또는 12주 후 희생시켜 치주조직의 변화를 방사선 사진, 광학 현미경 관찰을 통하여 분석한 바 다음과 같은 결과를 얻었다. 1. 치아이동 후 압박측에서는 치주인대 공간의 압축과 함께 치주인대섬유가 굵어진 소견을 보였으며 인장측의 치주인대섬유는 신장되어 보였다. 2. 보정 4주 후 유연성 고정군에서는 치주인대섬유의 재배열이 관찰되었으나 견고성 고정군에서는 관찰되지 않았다. 3. 보정 8주 후부터는 두 군 모두에서 치주인대섬유의 재배열 소견이 관찰되었으며, 두 군간 치주인대섬유의 재배열 차이는 보이지 않았다. 4. 보정기간 동안 치주인대섬유의 재배열은 압박측 보다 인장측에서 빠른 소견을 보였다. 이상의 결과는 치아이동 후 견고하게 고정한 보정방법에서도 치주인대섬유의 재배열이 일어남을 보여주었고, 치아 보정시 폴리에틸렌 리본으로 고정한 보정장치도 유용하게 사용될 수 있음을 시사하였다.

상악 전치부 견인시 치근 흡수 발생 양상에 관한 연구 (A STUDY ON THE PATTERN OF THE ROOT RESORPTION FOLLOWING MAXILLARY INCISOR RETRATION)

  • 본 연구는 상악 전돌증을 보이는 환자에게서 상악 제 1 소구치를 발치한 후 견인시 bracket 종류에 따른 치근흡수 차이, 이동양상에 따른 치근 흡수 차이, 각 치아간에 따른 전치부의 치근 흡수 정도를 연구하기 위해 단국 대학교 부속 치과 병원 교정과에 내원한 남녀 환자 32명을 대상으로 연구하여 다음과 같은 결과를 얻었다. 1. 상악 전치 후방 견인시 치근 흡수를 보이지 않는 것은 31.77%, 약간의 흡수는 33.33%, 치근의 1/4이하 흡수는 23.96%, 1/4이상은 10.94%를 보였다. 2. Apical tipping, lingually bodily movement, ant-post apical displacement 의 세가지 이동 양상은, 치근 흡수와의 상관 관계에 유의한 차이를 보이지 않았다. 3. Standard br.을 사용한 환자는 많은 치축 변화를 보이고 치근첨의 이동은 적었으나, straight br.을 사용한 환자는 적은 치축 변화를 보이고 치근첨의 이동량은 많았다. 4. Standard br.과 straight br.의 치근흡수에서 standard br.을 사용한 환자가 더 많은 치근 흡수를 보였다. 5. Standard br.을 사용시 치근 흡수정도는 2>1>3=0순을 보이고, straight br.을 사용시에는 0>1>2>3으로 나타났다.

Mechanical stress가 골조직세포군에 미치는 영향 (THE EFFECTS OF MECHANICAL STRESS ON CULTURED BONE CELL POPULATIONS)

  • The movement of teeth during orthodontic treatment requires bone remodeling process of bone formation and bone resolution. To find out the changes occuring in the cell itself, mechanical stress was applied to the cell populations involved in the bone metabolism. Bone tissue cell populations were isolated from fetal rat calvaria and divided into OC and OB groups. Following results were obtained from measuring the changes in acid & alkaline phosphatease activity, cyclic AMP and $PGE_2$ production in time lapse after the application of mechanical stress. 1. In case of the marker enzyme of specific bone tissue cell, acid phosphatase activity was high in OC group and alkaline phosphatase activity was high in OB group. 2. After the mechanical stress was applied, acid phosphatase activity was decreased in both OC and OB groups and alkaline phosphatase activity was increase in OB group. 3. When the mechanical stress was applied for 15, 30 and 60 minutes, the production of $PGE_2$ increased in both OC and OB groups, as the time span increased. 4. When the mechanical stress was applied for 20 and 40 minutes, the production of $PGE_2$ increased in both OC and OB groups, as the time span increased.

Case series of cleidocranial dysplasia: Radiographic follow-up study of delayed eruption of impacted permanent teeth

  • This report describes 3 cases of cleidocranial dysplasia (CCD) and presents relevant findings on long-term follow-up radiographic images of impacted permanent teeth with delayed eruption. Radiographic images of 3 CCD patients were reviewed retrospectively. These images were mainly composed of panoramic and skull radiographs, and the follow-up periods were 3, 13, and 13 years, respectively. The distinct features revealed by the images were described, and the eruption state of impacted permanent teeth was evaluated. The features common to the 3 cases were multiple supernumerary teeth, the presence of Wormian bone, underdevelopment of the maxilla and the maxillary sinus, and clavicular hypoplasia. The eruption of impacted permanent teeth was not observed without proper dental treatment in adult CCD cases, even after long time periods had elapsed. When proper orthodontic force was applied, tooth movement was observed in a manner not significantly different from the general population.

Static tensional forces increase osteogenic gene expression in three-dimensional periodontal ligament cell culture

  • Orthodontic tooth movement results from the combinational process of both bone resorption and formation in the compressive and tension sides, respectively. However, the genes responsible for new bone formation in tension sides have not been determined. In this study, we used DNA microarray and real-time RT-PCR to identify genes in human periodontal ligament (PDL) cells that undergo significant changes in expression in response to static tensional forces (2 or 12 hours). The genes found were alkaline phospatase (ALP), matrix metalloproteinases (MMPs), vascular endothelial growth factor (VEGF), and several collagen genes. Furthermore, an ELISA evaluating the expression of VEGF, type IV collagen and MMP-2 found levels significantly increased after 24 and 72 hours (P < 0.05). ALP activity was also increased after 24 hours (P < 0.05). Collectively, we found the genes up-regulated in our study by the static tensional force are related to osteogenic processes such as matrix synthesis and angiogenesis.

  • Tooth impaction represents the stop of eruption by clinical and radiographical disturbance in eruption path or the dislocation of tooth germ. The most common factor in tooth eruption disorders are spacial deficiency with other causes reported to be odontogenic tumors, periodontal ligament injury, etc. Impaction of the mandibular second molar is relatively rare and reported in about 3 out of 1,000 people. Because the second molars tend to erupt in a mesial direction, this situation can lead to serious problems if untreated, including dental caries, periodontal disease and root resorption of the first molar. Treatment of this problem includes, surgical repositioning and orthodontic forced eruption. Because each procedure have the definite advantages and disadvantages, and influenced by circumferential environment, these have limits for successfu1 recovery as independent treatment. In a case at St. Mary's Hospital, we performed successful correction of a horizontal impacted mandibular second molar using a miniplate skeletal anchorage system. We introduce this treatment as a valid method for an impacted second molar and consider a oromaxillofacial surgeon's role in tooth movement treatment.

  • Background: Treatment planning the correction of a transverse maxillary occlusal plane cant often involves a degree of qualitative "eyeballing", with the attendant possibility of error in the estimated judgement. A simple chair side technique permits quantification of the extent of asymmetry and thereby quantitative measurements for the correction of the occlusal plane cant. Methods: A measuring instrument may be constructed by soldering the edge of a stainless steel dental ruler at 90° to the flat surface of a similar ruler. With the patient either standing in natural head position, or alternatively seated upright in the dental chair, and a dental photographic retractor in situ, the flat under-surface of the horizontal part of this measuring instrument is placed on a unilateral segment of a bilateral structure, e.g. the higher maxillary canine orthodontic bracket hook. The vertical ruler is held next to the contralateral canine tooth, and the vertical distance measured directly from the canine bracket to the flat under-surface of the horizontal part of the measuring instrument. Results: This vertical distance quantifies the overall extent of movement required to level the maxillary occlusal plane. Conclusions: This measuring instrument and simple chair side technique helps to quantify the overall extent of surgical levelling required and may be a useful additional technique in our clinical diagnostic armamentarium.

  • The tissue reactions concerned in alveolar bone remodelling at the pressure zones of rat molar periodontium associated with the application of force (15 gm) to the maxillary first molar teeth of the albino rats were studied by the transmission electron microscopy. Osteoclasts referrable to bone resorption were observed thereafter 3 hour survival period and undermining resorption was generated thenceforth 2 day survival period. Bone resorption, reversal zone and new bone formation were simultaneously observed adjacent to the zone of undermining resorption in the 7 day survival period. Osteoclasts with well developed primary lysosome, ruffled border, clear zone, granules and Golgi apparatus were detected at the zone of the bone resorption, and dark and bright cells adjacent to the osteoclasts as well. Mononuclear cells and perpendicularly arranged collagenous fibers were observed in the reversal zone and, on the other hand, osteoblasts with well developed Golgi apparatus and rough endoplasmic reticulum were detected at the zone of bone formation.
