The purpose of this study is to evaluate the effects of mechanical and thermal fatigue stress on the shear, tensile and shear-tensile combined bond strengths(SBS, TBS, CBS) in various orthodontic brackets bonded to human premolars with chemically cured adhesive(Ortho-one, Bisco, USA). Five types of commercially available metal brackets with various bracket base configurations of Photoetched base(Tomy, Japan), Non-Etched Foil Mesh base(Dentaurum, Germany), Micro-Etched Foil Mesh base(Ortho Organizers, USA), Chessboard base(Daesung, Korea), and Integral base(3M Unitek, USA) were used. Samples were divided into 3 groups, the first group was acted with shear-tensile combined loads($45^{\circ}$) of 200g for 4 weeks(mechanical fatigue stress), the second group was subjected to the 5,000 thermocycles of 15 second dwell time each in $5^{\circ}C\;and\;55^{\circ}C$ baths(thermal fatigue stress), and the third group was the control. Bond strengths were measured at the crosshead speed of 0.5mm/min. The cross-section of bracket base/adhesive interface and the fracture surface were examined with the stereoscope and the scanning electron microscope. The resin remnant on bracket base surface was assessed by ART(Adhesive Remnant Index). The obtained results were summarized as follows, 1. In static bond strength, Photoetched base bracket showed the maximum bond strength and Integral base bracket showed the minimum bond strength(p<0.05). In all brackets, shear bond strength(SBS) was in the greatest value and shear-tensile combined strength(CBS) was in the least value(p<0.05). 2. After mechanical fatigue test, Photoetched base bracket showed the maximum bond strength and Integral base bracket showed the minimum bond strength(p<0.05). The aim of this study were to measure and compare the stress level on three type brackets and each other material (stainless steel, ceramic) with tipping and torquing forces by using the finite element analysis and to design biomechanically favorable brackets. For this study, three kinds of brackets were selected(A:Transcend-RMO, B:Signature-Unitek, C:PAW: plain archwire appliance-applied for a patent in Yonsei Udiversity). The slot size of bracket was 0.022inch and the size of archwire was 0.0175x0.025inch and taper shaped archwire was used in PAW. Loading force in tipping was 4.27N and torquing force was 32.858N applied by archwire torsion with 19.7degree and 11.3 degree in C type bracket. The conclusions were that (1) The finite element method proved to be a useful tool in the stress analysis of orthodontic bracket subjected to various forces. (2) With tipping, the stresses were concentrated at the gingival wall of the wire slot where it meets the mesial bracket surface and the incisal wall of the wire slot where it meets the distal bracket surface and with torquing, the stresses were concentrated at the junction of the gingival or incisal wall and base of the slot. (3) The maximum stress value was higher in torquing force than tipping force and therefore it is desirable to design on the basis of torquing force. (4) It was considered that the change in material might be affect on the diminish of stress value in the place of stess concentration. (5) The maximum stress value was highest on PAW bracket when the tipping and torquing force was applied and therefore it would be desirable to use mechanically favorable material on PAW bracket.
Objective: The aim of this study was to compare the shear bond strengths (SBS) of orthodontic brackets bonded to enamel with a self-etching primer after bleaching, desensitizer application and combined treatment. Methods: Forty-eight premolars were randomly divided into four groups, each with n = 12 premolar samples. The four groups were; Group1: 15% hydrogen-peroxide office bleaching agent (Illumin$\acute{e}$ Office-IO), Group 2: IO + BisBlock Oxalate Dentin-Desensitizer, Group 3: Bis Block Oxalate Dentin-Desensitizer, Group 4: No treatment (control). Twenty-four hours after bonding, the specimens were tested in SBS at a crosshead speed of 5 mm/min until the brackets debonded. The failure mode of the brackets was determined by a modified adhesive remnant index. Results: Bleaching, bleaching and desensitizer treatment, and desensitizer treatment alone all significantly reduced SBS of the orthodontic brackets ($p$ = 0.001). No statistically significant difference was found between Group 1, Group 2 and Group 3 (Group 1-Group 2, $p$ = 0.564; Group 1-Group 3, $p$ = 0.371; Group 2-Group 3, $p$ = 0.133). The predominant mode of failure for the treatment groups (Group1, Group 2 and Group 3) was at the enamel-adhesive interface leaving 100% of the adhesive on the bracket base. Conclusions: Bleaching and desensitizer treatment should be delayed until the completion of orthodontic treatment.
Although ceramic brackets have been used widely for improved esthetics during treatment, ceramic brackets have some inherent problems; brittleness, attrition of the opposing teeth and high frictional resistance. This study was performed to understand the frictional resistance of the ceramic brackets, as well as to be a helpful reference for finding the solutions to the problem of frictional resistance. Three different kinds of brackets were used; metal bracket, polycrystalline ceramic brackets with a metal slot to reduce the high frictional resistance and monocrystalline ceramic brackets. The brackets were tested with a $.019{\times}.025$ stainless steel wire with a second order angulation of $0^{\circ}\;and\;10^{\circ}$, and the static and kinetic frictional forces were measured on the universal testing machine. The results of this study showed that the ceramic brackets, especially the monocrystalline ceramic bracket without a metal slot, generated higher frictional resistance than the metal bracket, and the frictional resistance was increased as the angulation between the bracket slot and the wire increased. Therefore, the development of the ceramic bracket with reduced frictional resistance and the prevention of excessive crown tipping during orthodontic treatment will lead to the simultaneous attainment of more efficient and improved esthetic treatment goals.
Objective: Different methods have been utilized to prevent enamel demineralization and other complications during orthodontic treatment. However, none of these methods can offer long-lasting and effective prevention of orthodontic complications or interventions after complications occur. Considering the photocatalytic effect of $TiO_2$ on organic compounds, we hoped to synthesize a novel bracket with a $TiO_2$ thin film to develop a photocatalytic antimicrobial effect. Methods: The sol-gel dip coating method was used to prepare $TiO_2$ thin films on ceramic bracket surfaces. Twenty groups of samples were composed according to the experimental parameters. Crystalline structure and surface morphology were characterized by X-ray diffraction and scanning electron microscopy, respectively; film thickness was examined with a surface ellipsometer. The photocatalytic properties under ultraviolet (UV) light irradiation were analyzed by evaluating the degradation ratio of methylene blue (MB) at a certain time. Antibacterial activities of selected thin films were also tested against Lactobacillus acidophilus and Candida albicans. Results: Films with 5 coating layers annealed at $700^{\circ}C$ showed the greatest photocatalytic activity in terms of MB decomposition under UV light irradiation. $TiO_2$ thin films with 5 coating layers annealed at $700^{\circ}C$ exhibited the greatest antimicrobial activity under UV-A light irradiation. Conclusions: These results provide promising guidance in prevention of demineralization by increasing antimicrobial activities of film coated brackets.
Precise bracket positioning is essential in modem orthodontics. However, there can be alterations in the vertical position of a bracket due to several reasons. The purpose of this study was to evaluate the effect of variations in the vertical bracket position on the crown inclination in Korean patients with normal occlusion. From a larger group of what was considered to be normal occlusions obtained from the Department of Orthodontics, College of Dentistry, Seoul National University, each of the final 10 subjects (6 males and 4 females, with an average age of 22.3 yews) was selected. The dental models of each of the subjects were scanned three-dimensionally by a laser scanner, and measurements drawn from these were made on the scanned dental casts of the subjects were input into the computer program. From this the occlusal plane and the bracket plane were determined. The tooth plane was then constructed to measure the crown inclination on the bracket plane of each tooth. From a practical standpoint, information was obtained on the extent to which the torque of a tooth would be changed as the bracket position was to be moved vertically (in ${\pm}0.5mm,\;{\pm}1.0mm,\;{\pm}1.5mm$) from its ideal position. A one way analysis of the variance (ANOVA) was used to compare each group of the different vertical distances from the bracket plane on a specific tooth. Duncan's multiple comparison test was then performed. There were statistically significant differences in the crown inclination among the groups of different vertical distances for the upper central incisor, upper lateral incisor, upper canine, upper first and second molars, lower first and second premolars, and lower first and second molars (p<0.05). On the upper anterior teeth, upper molars, lower premolars and lower molars, the resultant torque values due to the vertical displacement of the bracket were different depending on the direction of the displacement, occlusal or gingival. This study implies that the torque of these teeth should be handled carefully during the orthodontic treatment. In circumstances in which the bracket must be positioned more gingivally or occlusally due to various reasons, it would be useful to provide the chart of torque alteration of each tooth referred to in this study with its specified bracket prescription.

The purpose of this study was to evaluate the tooth color changes of resin bonding sites and their adjacent sites on orthodontic bracket bonding. Sixty extracted sound premolars were used and the tooth color was recorded according to the CIE $L^*a^*b^*$ color system using a spectrophotometer. The tooth colors of the twenty premolars were measured and compared before bracket bonding and after removal. On a further twenty premolars, the tooth color was measured before and after only primer application. In the change of $L^*$ values, according to the bracket bonding and primer application, the lightness was decreased, and in the change of $a^*\;and\;b^*$ values, the color was changed into a more yellowish color The color differences $({\Delta}E^*)$ were calculated from the $L^*a^*b^*$ values and compared with the standard value of clinical detection $({\Delta}E^*=3.7)$. The color differences between before the bracket bonding and after removal noted exceeded the standard value and those of between before and after the primer application were not larger than the standard value. Toothbrushing was performed after application of the primer to evaluate the color changes according to the primer abrasion. As a control, toothbrushing was performed on the last twenty premolars. The color differences noted were larger than the standard value after toothbrushing. Also, to evaluate the color changes of the tooth which is exposed to sun irradiation after bracket removal, additional photoaging was performed and the color was measured for all teeth. The additional color differences after photoaging were smaller than the standard value. The above results suggest that the tooth color changes after fixed orthodontic treatment.
Frictional force between the orthodontic bracket and arch wire during sliding tooth movement is related to many factors, such as the size, shape and material of both the bracket and wire, ligation method and the angle formed between the bracket and wire. There have been clear conclusions drawn in regard to most of these factors, but as to the effect of bracket width on frictional force there are only conflicting studies. This study was designed to investigate the effect of bracket width on the amount of frictional forces generated during clinically simulated tooth movement. Three different widths of brackets $(0.018{\times}0.025'\;standard)$ narrow (2.40mm), medium (3.00mm) and wide (4.25mm) were used in tandem with $0.016{\times}0.022'$ stainless steel wire. Three bracket-arch wire combinations were drawn on for 4 minutes on a testing apparatus with a head speed of 0.5mm/min and tested 7 times each. To reproduce biological conditions, dentoalveolar models were designed with indirect technique using a material with similar elastic properties as periodontal ligament (PDL). In addition, to minimize the effect of ligation force, elastomer was used with added resin, which was attached to the bracket to make up for the discrepancies of bracket width. The results were as follows: 1. Maximum frictional force for each bracket-arch wire combination was: Narrow (2.40mm): $68.09\pm4.69gmf$ Medium (3.00mm): $72.75\pm4.98 gmf$ Wide (4.25mm): $72.59\pm4.54gmf$ 2. Frictional force was increased with more displacement of wire through the bracket slot. 3. The ANOVA psot-hoc test showed that the bracker width had no significant effect on frictional force when tested under clinically simulated conditions(p>0.05).
A device called incisor inclination indicator to control the axial inclinations of the incisors in the diagnostic setup is introduced. It is used to control the retraction of the maxillary and mandibular incisors. In this article, we describe the use of the incisor inclination indicator to prescribe adequate torque into the anterior lingual brackets and evaluate the results of treatment in a bimaxillary dentoalveolar protrusion case that underwent orthodontic treatment with the pretorqued anterior lingual brackets. Retraction of the maxillary and mandibular incisors was achieved with careful control of the axial inclination. It is indicated that the use of the incisor inclination indicator is an effective adjunctive laboratory procedure for anterior torque control during retraction in lingual orthodontic treatment.
Periodontal disease and/or loss of teeth brings pathologic tooth migration that can result in esthetic and occlusal problems. Diastema and general spacing of the teeth, particularly in the anterior segments of the dentition are frequently developed in individuals with advanced periodontal disease. Thus, the overall treatment plan for a patent with advanced periodontal disease often involves periodontal orthodontic combined therapy. Orthodontic treatment in adults with periodontal disease is restricted to tooth alignment with special caution. Indirect bonding can achieve accurate bracket placement. A 38 year old woman with adult periodontitis was treated by periodontal therapy. Subsequently, her diastema was orthodontically corrected by indirect bonding technique. It must be an appropriate case report of periodontal-orthodontic combined therapy.
