소양인(少陽人) 지황백호탕(地黃白虎湯)으로 호전된 경증(輕症) 파킨슨 환자에 대한 임상보고(臨床報告) (A clinical report on the mild case of Parkinson's disease improved with Soyangin-Gihwangbeakho-tang)

  • Parkinson's disease(PD) is characterized by chronic progress of mesencephalic dopaminergic neuronal death. Diagnostic criteria for PD require at least two of three motor sign: tremor, rigidity, or bradykinesia. Levodopa and the dopamine agonists are considerd first-line drug therapy. In the book ‘dongyi soose bowon(東醫壽世保元)’, Soyangin Gihwangbeakho-tang(地黃白虎湯) is used at Soyangin Interior-overheated-disease. This case is patient who is 69 years old lady, suffered by the tremor of jaw and a slight rigidity, bradykinesia etc. This patient was classified as Soyangin by features, somatotype and emotional patterns. She improved in the tremor of jaw and others with Gihwangbeakho-tang for 67days. The result revealed that Soyangin-Gihwangbeakho-tang was effected on the tremor of jaw and others with Parkinson's disease patient.

뇌경색환자의 혈중 homocysteine 및 d-dimer 농도와 한의변증유형에 대한 연구 (Study for Blood Homocysteine Levels and d-dimer Levels of Cerebral Infarction Patients and Pattern Identification)

  • The aim of this study was to investigate the differences in blood homocysteine levels and blood d-dimer levels of cerebral infarction patients categorized by Pattern Identification. We studied hospitalized patients within 4 weeks after the onset of stroke who were admitted to the Oriental Internal Medical Department at Semyung University Chungju Oriental Medical Hospital from May 2008 to September 2009. We analyzed risk factors and blood homocysteine levels and blood d-dimer levels accordings to Pattern Identification in Cerebral infarction patients. A total of 49 patients were included in the trial. No statistical significance was noted for any characteristics except body weight and body mass index. Body weight and body mass index were significantly higher Dampness-Phlegm pattern. On past history of patients, prevalence of DM was significantly higher in Fire-Heat pattern than that of other patterns. There was no significant difference of blood homocysteine levels and blood d-dimer levels among Pattern Identification. This study investigated the differences in blood homocysteine levels and blood d-dimer levels of cerebral infarction patients categorized by Pattern Identification. The correlation in homocysteine and d-dimer levels and Pattern Identification was not clarified.

동태손상(動態損傷) 증후군(症候群)에 관한 연구 현황 고찰 (A Review Study on Movement System Impairment Syndromes)

  • Objectives: Sahrmann, an American physical therapist, has developed a set of movement-related diagnoses on musculoskeletal pain syndromes (MPS). We reviewed articles for studies of objectification and quantification of Oriental medicine diagnosis system about MPS. Methods: The authors reviewed a total of 12 studies found on PubMed to obtain movement system impairment (MSI)-based classification categories. Results: The MSI system has been developed for objectification and quantification of physical therapists' MPS diagnoses. The MSI system of classification is based on the basic premise that loss of precise movement is the result of specific repetitive movements and positions in everyday life. The MSI system defines 8 categories in the shoulder region, 5 in the lumbar region, and 7 in the knee region. Treatment involves (1) educating the person about the specific directions of alignment and movement that appear to be contributing to the musculoskeletal problem, (2) modifying the direction - specific alignment and movement patterns during daily activities, and (3) exercises to address the impairments. Conclusions: The authors propose that MSI studies could help to advance an Oriental medical diagnosis system on musculoskeletal pain syndromes, because MSI shares similarity with Oriental medicine in terms of holism, views of constant motion, and mi-byung (subhealth) treatment.

중풍환자의 화열변증 진단지표에 관한 연구 (Study on the Diagnostic Indicators of Fire-heat Pattern Identification in Stroke Patients)

  • The purpose of this study was to evaluate the diagnostic indicators which are used for the identification of fire-heat pattern in stroke patients. For evaluation of diagnostic indicator, we analyzed the indicators which are composed of symptoms and signs collected from stroke patients based on the clinical records using case report form (CRF). Patients had a first-ever stroke within 1 month after the onset of stroke. Pattern identification was performed and decided by two independent physicians. Two patient groups that consist of fire-heat pattern and the other patterns were compared to isolate important indicators affecting the fire-heat pattern identification of stroke patients. The 8 indicators among 16 fire-heat pattern indicators were significantly more frequent compared with non fire-heat pattern group. Logistic regression analysis revealed that 5 indicators among fire-heat indicators were significantly sensitive indicators being capable of identification of fire-heat pattern. But two of them was from the indicators of dampness-phlegm pattern and yin deficiency pattern. Therefore, further studies are required for the development of Korean standard indicators of Fire-heat pattern identification.

당뇨병의 한의학적 치료에 대한 최신 연구 동향 - 2008년부터 2013년까지 국내 학술논문을 중심으로 - (Recent Research Trends in Korean Medicine Treatment of Diabetes Mellitus - Focusing on Domestic Articles from 2008 to 2013 -)

  • Objectives : This study was performed to research trends in Korean medicine treatment of diabetes mellitus (DM). Methods : We reviewed 73 studies about diabetes mellitus, which had been published from 2008 to 2013. The article search was performed on the Korean institute of oriental medicine (KIOM) database. Results : 1. Patterns of study design were as follows : 14 review studies, 44 experimental studies, 7 clinical studies, 8 case reports. 2. From the 14 review studies, we noted that literature studies about definition, pathologic mechanism, treatment, pathologic pattern identification of DM and previous study analyses were done. 3. From the 44 experimental studies that are the most common type of DM studies, various Korean medicine treatments such as single medicinal herbs, herbal complexes, diets and pharmacopuncture were used. Anti-diabetic effects were demonstrated in most of the studies. 4. 7 clinical studies about acupuncture, electroacupuncture, herbal medicine, etc. were reported. 5. 8 cases about DM complications and comorbidities were reported. Clinical symptoms improved in all of studies, blood sugar level decreased in four studies. Conclusions : Further clinical and case study based on experimental researches should be performed.

경도인지장애와 경도치매 단계에서의 한의 변증 및 간 혈액지표 연관성 탐색 연구 (Association of Korean Medicine Pattern Identifications and Liver Blood Markers with the Severity of Cognitive Impairment in an Elderly Population)

  • Objectives: This study aimed to comprehend the characteristics of Korean medicine patterns in relation to varying degrees of cognitive impairment in an elderly population. Methods: The dataset included 127 elderly individuals with cognitive impairment obtained from three Korean medicine hospitals between 2018 and 2021. The participants were categorized into two groups based on Clinical Dementia Rating-Sum of Boxes (CDR-SB) scores: those with questionable impairment (QI) and those with very mild dementia (VMD). A diagnostic framework for Korean medicine patterns encompassing Qi deficiency, Yin deficiency, Phlegm dampness, and Heat-fire was employed. Liver blood markers, including aspartate aminotransferase (AST), alanine aminotransferase (ALT), and the AST/ALT ratio, were also analyzed. Results: The scores of the cognitive assessment tools (MoCA-K, MMSE-DS, and K-IADL) of the QI group significantly differed from those of the VMD group. CDR-SB exhibited a positive correlation with the scores of each pattern of Qi deficiency, Yin deficiency, Phlegm dampness, and Heat-fire, whereas ALT and AST values displayed negative correlations. Binomial logistic analysis, controlling for potential confounders, such as age, education years, body mass index, the presence of chronic disease, and the presence of medication, verified that the VMD group showed higher pattern scores and lower ALT and AST values than the QI group. Conclusions: Increases in pattern scores along with decreased AST and ALT levels might be indicative of more severe cognitive impairment.

출류(朮類) 한약재의 외.내부형태와 이화학패턴 연구 (A Study on External.Internal Morphology and Pattern Analysis of Atractylodes Rhizomes)

  • Objectives : To determine the discriminative criteria for Atractylodes rhizomes, the experiment of externalinternal characteristics and physicochemical pattern analysis were performed. Methods : External characteristics was observed using stereoscope. The sectioned materials which were covered with parffin were stained by Ju's method. Physicochemical patterns were analyzed using HPLC/DAD. Results : 1. External shape of original plant : Atractylodes maaocephala and A. japonica had relatively long petioles and 3-5 parted leaves. A. macrocephala had big purple flowers whereas A. japonica had relatively small white flowers and pinnate bracts. A. lancea had sessile leaves and white flowers, and the end parts of degenerated stamens were bent. 2. External shape of herbal medicine: A. macrocephala which was fist-shaped rhizome had pa-pillate processes and the cross section was light gray and sulcate. A. japonica and A. lancea were connected-beady or tubercular rhizomes, and the cross sections were both yellow-colored white. However, the cross section of A. japonica was fibrous, the width of cortex was narro-wer than that of stele, and radial shape in cortex was rare, whereas the width of cortex in A. lancea was similar to that of stele in size, and radial shape in cortex was obvious. 3. Internal shape of herbal medicine: A. macrocephala and A. lancea did not have lignified fascicles in cortex. However, the vascular bundles and vessels of A. macrocephala were wedge shaped and radial arrangement, and vascular bundles were densely populated in stele whereas those of A. lancea were repeatedly arranged and thinly extended to cortex. A. japonica had lignified fascicles in cortex and the width of vascular bundles was conspicuously thick with narrow intervals. 4. Physicochemical pattern analysis : A. macrocephala and A. lancea contained atractylenolide I and atractylenolide Ill whereas A. japonica contained atractylenolide I, atractylenolide Ill. diacetyl-atractylodiol, compound-4, compound-5. The three species of Atractylodes rhizomes showed different chromatogram patterns. Conclusions : The results could be used as discriminative criteria for Atractylodes rhizomes and as fundamental materials to researches of further pattern analysis and biological reaction.

호흡변이도와 POMS(profile of mood states)의 상관성 연구 (Correlation Analysis of Respiratory variability and POMS factors)

  • Background and purpose: Health is characterized by variability and a loss variability is associated with aging and disease. Normal healthy respiratory patterns are characterized by breath to breath variability. The aim of this study is to analyze Correlation Analysis of 10 respiratory indices(include breath to breath variability) and K-POMS factors score. Method: Respiratory data were measured during 15 minutes and transferred to text file. We calculated 10 respiratory parameters in use of MATLAB7.1. After then do correlation analysis of 10 respiratory indices and K-POMS factors score. Result and conclusion: Respiratory variability is showed significant correlation with vigor factor. Respiratory rate is showed significant correlation with depression factor.

그래프를 이용한 성격 설문지의 사상체질 특성 분석 (Analysus of Constitutional Characters of Personality Questionnaire using Graph)

  • Personality is an important factor in diagnosing Sasang constitutions, and has been studied by researchers with various statistical methods. Using these statistical methods, we obtain several clinical factors including significant p-value. In this paper, we applied a graph for analyzing personality questionnaires. The graph can well represent pairwise relations among items from the collected clinical information. In our analysis, we can find several meaningful personality patterns according to Sasang constitutions.
