The purpose of this study was to examine the state of infection control provided to members of Korean Dental Hygienists Association. The subjects in this study were dental hygienists who attended a symposium on July 1. 2006. after a survey was conducted, the answer sheets from 489 participants were analyzed, and the findings of the study were as follows: 1. Possession of disinfection room was being(72.7%), and person of infection control was zero(52.9%). Number of sterilizer was one(62.2%). 2. As a repetition choice, type of sterilizer was autoclave(97.9%), UV sterilizer(67.4%) and EO gas sterilizer(21.4%). As a repetition choice, infection materials was ethanol(84.1%). 3. Water tube of unit and chair was using of sterilized water(42.9%). Sterilizing of compressed air was no(69.0%). 4. Re-using of disposal was not using(62.5%), re-using disposal was suction tip(28.2%)(repetition choice) 5. In sterilization of instruments, hand-piece was every using time(28.4%), and reamer-file, bur, mirror, pincette, explorer, hand scaler and ultrasonic scaler were high in every using time. 6. Individual protection was high of using, cleaning of hands before treatment was every treatment(87.0%). Type of soap was liquid type in dental clinic(48.2%), infection soap in dental hospital(41.2%) and solid soap in public health center(50.6%). Answered that they need regular oral health education, and 82.9% respondents answered that they need oral health technicians in school. And 87.8% respondents needed individual oral health education for the benefit of better oral health.

This study was designed to reveal the correlationship between the internal derangement state of TMJ and clinical characteristics including pain and mandibular dysfunction. One hundred and twenty five subjects with TMJ signs and symptoms were chosen for two years. The level of pain and madibular dysfunction were evaluated by Visual Analog Scale(VAS) and Craniomandibular Index(CMI). The diagnostic categories of TMJ internal derangement were determined by arthrography and they included normal disc position, anterior disc displacement with reduction(ADDR), rotational disc displacement with reduction (RDDR), anterior disc displacement without reduction(ADDNR), and rotational disc displacement without reduction(RDDNR). Also disc perforation was used as a criteria to divide the diagnostic subgroups. The obtained results were as follows; 1. The patient distribution of each group was 5 in normal disc position(4%), 40 in ADDR(32%), 30 in RDDR(24%), 34 in ADDNR(27%), and 16 in RDDNR(13%). 2. Perforation was observed in 8% of ADDR, 10% of RDDR, 32% of ADDNR, and 19% of RDDNR. 3. CMI of non-reduction group was higher than that of reduction or normal group(p<0.05), but V AS showed no significant difference.4. CMI of perforation group was higher than that of non-perforation group in reduction group (p<0.05). 5. There were no significant differences of CMI and VAS between anterior disc displacement group and rotational disc displacement group in both reduction and non-reduction group. 6. CMI of RDDNR group was higher than that of RDDR group(p<0.05). 7. There were no significant difference of CMI and VAS between bilateral involvement group and unilateral involvement group(p<0.05).

In order to examine the effects of advanced periodontitis on the dental pulps, 38 extracted human teeth were examined histologically. The 38 teeth had a positive or negative state in the electric pulp test(EPT). In addition, almost of the 38 teeth had a deep pocket and severe mobility, and floating state. A medical and dental history was elicited. The extracted teeth fixed in 10% neutral formalin solution. The general tissue processing method was followed. The tissue block including the teeth was prepared for optical microscopy using hematoxillin-eosin staining. Among the 38 periodontally involved teeth, the dental pulps were respectively intact in 12(31%), and a pulp stone(or linear calcifications) was found in 18 teeeth(47%). In addition, 17 teeth(44%) had pulps exhibiting inflammatory reactions with varying intensities, such as hyperemia, pulp abscess, pulp necrosis. Among the 38 periodontally involved teeth, 37 teeth tested a positive to the EPT, and 7 teeth tested negative. The EPT positive 37 teeth had various histological features such as 7 normal pulp(18%), 17 pulp stone(44%), 1 hyperemia (2%), 9 pulpitis(23%), 5 root resorption(13%), 3 pulp abscess(7%), and 3 pulp necrosis(7%), In conclusion, it is suggested that in the EPT positive teeth, advanced periodontally involved teeth can cause inflammation of the dental pulp.

Background: Conscious sedation is a minimally depressed level of consciousness that retains the patient's ability to maintain the airway continuously and to respond appropriately to physical stimulation and verbal command at any time, produced by a pharmacologic or nonpharmacologic method or a combination thereof. In dental anesthesia, a state of sedation in which the conscious patient is rendered free of fear, apprehension, and anxiety through the use of pharmacologic agents. Midazolam is a useful drug of benzodiazepine on minor operation of dentistry. The purpose of this study is to estimate the appropriate dosage protocol of midazolam to the patient receiving intravenous sedation at the department of oral and maxillofacial surgery. Methods: Total subject was fifty-one patients and fifteen patients of this subject were decided to control group (Group I) that they were not received intravenous sedation. The experimental group was randomly subdivided into two groups and each of those groups was administrated to 0.05 mg/kg (Group II) and 0.08 mg/kg (Group III) each other. All patients of the subject were surveyed with postoperative one day questionaire to evaluate the level of sedation. Results: In view of antegrade amnesia, 1. Group II and III were statistically different compared to Group 1, respectively. 2. Group II and III were not statistically different. Conclusions: Apparent antegrade amnesia was found in both groups 0.05 mg/kg and 0.08 mg/kg intravenously administered. So we present the protocol of the department of oval and maxillofacial surgery, Kangnung National University Dental Hospital. But further studies concerning patient's age, body weight, and general condition will be carried out.

Mobile oral exam at the facilities for the disabled in Seoul is to improve the oral health of the disabled who can hardly approach dental clinic. We analyzed 1609 oral exam records at 24 facilities (11 special-education schools, 10 living facilities, 2 mental hospitals, 1 health care facility for the elderly) in 2011. The purpose of this study is to figure out the oral health state of the disabled in facilities in Seoul and to compare with the non-disabled in National Survey 2010 and 2008. 1. Special-education school : DMFT index of age 15 is 5.4 which is higher than 3.6 (DMFT index of age 15) of National Survey in 2010. 2. Living facility : DMFT index of ages 35~44 is 8.4 which is higher than 5.2 (DMFT index of ages 35 ~44) of National Survey in 2008. DT rate is higher (31.3% vs 19.2%) and MT rate is lower (7.6% vs 15.5%). 3. Mental hospital : DMFT index of ages 35~44 is 11.3 which is higher than 5.2 (DMFT indext of age 35~44) of National Survey in 2008. DT rate is significantly higher (50.7% vs 19.2%) and FT rate is lower (35.1% vs 65.3%). 4. Health care facility for the elderly : DMFT index of ages 65~74 is 7.1 which is slightly lower than 8.7 (DMFT index of ages 65~74) of National Survey in 2008. The number of existing natural teeth is similar (16 vs 18).

Mortality associated with maxillofacial infection is relatively low due to the development of antibiotics, and improved oral care. However, inappropriate treatment, delayed treatment, old age, underlying systemic disease, and drug-resistant microorganisms can potentially result in life threatening situations such as cavernous sinus thrombosis, mediastinitis, and sepsis. Sepsis is the most dangerous state with high mortality, ranging from 20~60%. The treatment of sepsis involves properly monitoring vital functions, fluid resuscitation, surgical drainage, and empirical use of high doses of antibiotics until culture results are available. Ventilatory support maybe be required as well. We encountered a 64-year-old patient who died from sepsis that developed as the result of an odontogenic infection. The initial diagnosis was right temporal, infraorbital, buccal, pterygomandibular space abscess. Despite surgical and medical supportive care, the condition progressed to sepsis and after four days the patient died due to multiple organ failure.
Purpose : The purpose of this study was to examine the oral health state of disabled people with mental retardation in an attempt to pave the way for oral health care planning geared toward the disabled. Subjects and Method : The subjects in this study were 46 mentally retarded people who attended rehabilitation centers for the disabled in the city of Wonju, Gangwon Province. After a survey was conducted, the collected data were analyzed with SPSS 13.0 program, and frequency analysis, percentage, ANOVA and t-test were utilized. Results : The findings of the study were as follows: 1. The simplified oral hygiene index of the mentally retarded people was 1.32, which was on the average. 2. The decayed teeth index and decayed teeth rate of the mentally retarded people were respectively 13.48 and 48.13 percent, which were above the average. 3. The decayed surface index and decayed surface rate of the mentally retarded people were respectively 27.17 and 17.39 percent. 4. The simplified debris index of the mentally retarded people significantly varied with gender(p<.01), and missing teeth index(p<.05) and missing surface index differed significantly with age. Filled surface index was significantly different according to the region. Conclusion : The findings of the study illustrated that their indexes related dental caries experience were high. In order to promote the oral health of the disabled with mental retardation, prolonged research should be implemented, and a dental checkup should be carried out on a regular basis by specialists. Preventive care and early treatment should be provided, and the development of customized oral health education programs tailored to different sorts of disabilities and oral health control methods is required.
Tracheostomy may be used to assure airway protection in various clinical situation. It, as a known operation, has a history spanning 2000 years. The first clear account of a successful tracheostomy was recorded in 1546 by Brasavola. Until 1718 the term "bronchotomy" was used to describe the procedure. Heister then introduce the term "tracheotomy and this was later adopted and popularized by Trousseau about 1830. The term "tracheostomy" appeared in medical literature after 1820 and the two terms "tracheostomy" and "tracheotomy" are used interchangeably today. Indications include relief of upper airway obstruction, facilitation of pulmonary toilet, diminution of dead space and need for prolonged mechanical ventilation. The extent of indication of tracheostomy has a tendency to increase, thus oral and maxillofacial surgeons have some opportunities to face a situation that require tracheostomy. So, we reported retrospective study of 31 cases of tracheostomy patient in oral and maxillofacial surgery with reference review to reveal the significance of surgical skill and management capability of emergercy state maxillofacial surgery patients.

Purpose: This study aims to assess the psychological characteristics of patients with iatrogenic damage of the trigeminal nerve using Symptom Checklist-90-Revised (SCL-90-R). Methods: A total of 22 patients who visited the clinic of Department of Oral Medicine, School of Dentistry, Chosun University were evaluated in the months from December 2012 to December 2014. Patients were included in the study if they presented with reported sensory changes due to iatrogenic trigeminal nerve injury. A total of 37 control group who has no symptoms of pain and sensory change were included in the study. All participants filled out the SCL-90-R instrument to assess psychosocial symptoms. Results: The T-scores of most of the SCL-90-R symptom dimensions were within normal range (less than 55) in the patients and the control group. The patients group showed higher T-scores of SCL-90-R than those of the control group, with statistically significance of somatization, anxiety, psychoticism, and positive symptoms distress. Female patients group had higher Tscores of SCL-90-R than those of male patients group, but there was no statistical difference, except for the positive symptom global index. The chronic patients group showed higher Tscores of SCL-90-R than those of the acute patients group, but there was no statistical difference, except for the Positive symptom global index. Conclusions: According to the psychological assessment of iatrogenic trigeminal nerve injury patients using SCL-90-R, the patients group, as compared to the control group shows a significantly higher score for somatization, anxiety, and psychoticism. Therefore, assessment of psychological state would be considered to treat patients with iatrogenic trigeminal nerve injury.
Purpose: The prevalence of supernumerary teeth has been reported to be between 0.1% and 3.8%. The aim of this study was to determine the prevalence, clinical significance, and associated pathologies of fourth molars based on a retrospective study and a literature review. Materials and Methods: A 5-year retrospective prevalence study was conducted at the Department of Oral Diagnosis and Dentomaxillofacial Radiology of Okayama University Hospital, Okayama, Japan. The study involved extracting data from the digital records of patients from January 1, 2013 through December 31, 2017. The sampling frame included all patients who had panoramic radiographs, cone-beam computed tomography (CT), and multislice CT images during the period under review. Results: A total of 26,721 cases were reviewed and 87 fourth molars were identified. The prevalence of fourth molars in the 5-year study at Okayama was calculated as 0.32%. The mean age of patients with a fourth molar was 30.43 years, and the male-to-female ratio was 1:0.98. The vast majority of cases were in the maxilla (92%) and had normal shapes(89.7%); furthermore, 82.8% of cases were unerupted. Conclusion: The prevalence of fourth molars in the study population was found to be 0.32%, and fourth molars occurred with approximately equal frequency in males and females. Fourth molars were more common in the maxilla and were predominantly unerupted and small.
