Xylitol Sensitivity among Oral Streptococci

  • Xylitol is a five-carbon sugar alcohol that inhibits the growth of oral streptococci, including Streptococcus mutans. In this study, we tested xylitol sensitivity among the oral streptococci. We also compared nucleotide homology of putative fructose phosphotransferase system (PTS) and xylitol sensitivity, since xylitol is transported via the fructose PTS. Among the tested Streptococci, S. pneumonia showed the highest resistance to xylitol while S. gordonii and S. sanguinis showed the most sensitive growth inhibition. These streptococci could be grouped according to their xylitol sensitivity. S. mutans and S. salivarius showed similar bacterial growth inhibition by xylitol. S. mitis, S. oralis, S. pneumonia, S. intermedius and S. anginosus showed relatively low sensitivity to xylitol. When the genetic homologies of five fructose PTSs were compared among the tested streptococci, closely related streptococci showed similar sensitivity to xylitol. Taken together, fructose PTSs may mediate the sensitivity to xylitol in oral streptococci.

Effects of Active Oral Play and Ayres Sensory Integration (ASI®) for Child With Oral Defensiveness: Single Subject Research (구강방어아동을 위한 능동적 구강놀이와 ASI적용효과 : 단일사례연구)

  • Objective : The purpose of the present study is to investigate the effects of Ayres Sensory Integration (ASI®) with play-centered oral defensiveness reduction activities on the oral sensitivity of a child with oral defensiveness. Methods : The subject was a three-and-a-half year-old boy with a developmental delay. The study was performed from August to September, 2018. The research design used in this study is the design for single-subject research. The experimental period was divided into three phases: three sessions for the initial baseline phase (A), 15 sessions for the treatment phase (B), and three sessions for the final baseline phase (A'). Ayres Sensory Integration (ASI®) with active oral play was conducted in the treatment phase. The Measuring Oral Sensitivity in Practice was used to test changes in the oral sensitivity level. The Korean Children's Eating Behavior Inventory and Food Checklist were used to assess the difference in functions related to oral sensitivity before and after the intervention. Results : In comparison with the baseline phase, the Measuring Oral Sensitivity in Practice and Korean Children's Eating Behavior Inventory were improved during the treatment phase, but the Food Intake checklist score was not changed after the treatment phase. Conclusion : The use of Ayres Sensory Integration (ASI®) with active oral play produced positive results and reduced the child's oral sensitivity and eating problems.

Comparison of Waters′ radiography, panoramic radiography, and computed tomography in the diagnosis of antral mucosal thickening (상악동의 점막비후에 대한 WATERS방사선사진과 파노라마방사선사진 및 전산화단층사진의 진단능 비교)

  • With the CT findings as gold standard, the sensitivity, the specificity, and the diagnostic accuracy of Waters' radiography and Waters' radiography with panoramic radiography were compared in the diagnosis of antral mucosal thickening of 16 patients. Three oral radiologists and three non-oral radiologists interpreted the Waters' radiographs and after 4 weeks, interpreted the Waters' radiographs and panoramic radiographs simultaneously. The interpretation point was the existence or the non-existence of the mucosal thickening on the medial, the posterolateral, the floor, and the roof of maxillary sinus. The obtained results were as followed : 1. In oral radiologist group, the sensitivity, specificity and diagnostic accuracy of Waters' film were 0.7250, 0.8489 and 0.7578 respectively. 2. The sensitivity and the diagnostic accuracy in oral radiologist group were higher than those of non-oral radiologist group (P<0.05), but there was no significant difference between two groups in the specificity (P>0.05), 3. There was no significant difference of the diagnostic abilities except the specificity in oral radiologist group between Waters' radiography and Waters' radiography with panoramic radiography (P>0.05). 4. The sensitivity and the diagnostic accuracy were the highest in the case of medial wall interpretation, the specificity was the highest in the posterolateral wall. 5. In the posterolateral wall and the floor, the sensitivity and the diagnostic accuracy of oral radiologist group were higher than those of non-oral radiologist group (P<0.05).

Antibiotic sensitivity and resistance of bacteria from odontogenic maxillofacial abscesses

  • Objectives: This study investigated the types and antibiotic sensitivity of bacteria in odontogenic abscesses. Materials and Methods: Pus specimens from 1,772 patients were collected from affected areas during incision and drainage, and bacterial cultures and antibiotic sensitivity tests were performed. The number of antibiotic-resistant bacteria was analyzed relative to the total number of bacteria that were tested for antibiotic susceptibility. Results: Bacterial cultures from 1,772 patients showed a total of 2,489 bacterial species, 2,101 gram-positive and 388 gram-negative. For penicillin G susceptibility tests, 2 out of 31 Staphylococcus aureus strains tested showed sensitivity and 29 showed resistance. For ampicillin susceptibility tests, all 11 S. aureus strains tested showed resistance. In ampicillin susceptibility tests, 46 out of 50 Klebsiella pneumoniae subsp. pneumoniae strains tested showed resistance. Conclusion: When treating odontogenic maxillofacial abscesses, it is appropriate to use antibiotics other than penicillin G and ampicillin as the first-line treatment.

The Effects of Oral Health Belief on Oral Disease Prevention and Quality of Life (구강건강신념이 구강병 예방 및 삶의 질에 미치는 영향)

  • Objectives : This study analyzed the influencing relationship among oral health belief, prevention of oral diseases, and quality of life subject in adults over 20 years of age. Methods : This study enrolled 237 participants who visited Y hospital located in the Gyeongnam district as the final subjects for the analysis. From the collected data, multiple regression analysis was conducted to identify the relative influence of oral health belief on each parameter using SPSS 18.0. Results : Based on the regression analysis results, it was found that the factor in oral health belief with the strongest influence on the intention to prevent oral disease was importance. The behavior for prevention of oral disease was influenced by sensitivity, and the quality of life was affected by self-efficacy. Conclusions : These results, suggest that in order to promote oral health in adults, continuous support from oral health professionals is required to make patients recognize the importance and the sensitivity of oral health and reduce sensitivity.

Microbial isolates and antibiotic sensitivity in patients hospitalized with odontogenic infections at a tertiary center over 10 years

  • Objectives: This study investigated causative strains and their antibiotic sensitivity in patients who were hospitalized for maxillofacial odontogenic infections at a tertiary center in South Korea over the past 10 years with the aim of providing guidelines for the selection of appropriate empirical antibiotics. Materials and Methods: Patients with head and neck fascial space abscesses due to odontogenic infections who underwent incision and drainage surgery with pus culture tests between 2013 and 2022 at the Department of Oral and Maxillofacial Surgery, Dankook University Hospital were included. The bacterial isolates and antibiotic sensitivity of each strain were analyzed for 2013-2022, 2013-2017, and 2018-2022. The affected fascial spaces were classified into primary, secondary, and deep neck spaces. Results: In the 192 patients included in this study, 302 strains were detected. Viridans streptococcus had the highest frequency (51.7%), followed by Prevotella spp. (16.9%), Staphylococcus spp. (5.6%), and Klebsiella pneumoniae (4.6%). The identification rate of viridans streptococcus significantly increased from 41.8% in 2013-2017 to 60.9% in 2018-2022. Viridans streptococcus showed an antibiotic sensitivity of 80.5% to ampicillin; the sensitivity to penicillin antibiotics decreased over the study period. Antibiotic susceptibility was approximately 94% for third-generation cephalosporins. K. pneumoniae, which was identified at a high percentage in patients with deep neck space infection, showed increasing antibiotic resistance to most antibiotics over the study period. Conclusion: Viridans streptococcus was identified in head and neck fascial space abscesses with the highest frequency. Empirical antibiotics should be effective against this strain; penicillin antibiotics are considered inappropriate. For effective treatment of deep neck space abscesses, bacterial culture and antibiotic sensitivity tests performed as soon as possible are essential.

Factors related to oral health management for child in mothers of multicultural families (다문화가정 모친의 자녀 구강건강관리 관련요인)

  • Objectives: The purpose of this study was to identify the factors affecting the oral health behavior of mothers in multicultural families and the oral health management of children. Methods: The subjects were 303 mothers in multicultural families having children in childhood. To verify the validity of the outcome, a factorial analysis was conducted. To examine differences in the outcome according to socio-demographic characteristics, a one-way ANOVA was used, and the hypothesis was tested through a multiple regression analysis. Results: According to the results, it was found that severity, self-efficacy, sensitivity, and multicultural disability that show attitudes had significant impact on the mother's oral health behavior. Moreover, it was found that self-efficacy, sensitivity, and the mother's oral health behavior that show attitudes had significant impact on the children's oral health management. Conclusions: Based on the results of this study, it would be necessary to increase the interest in mother-and-child oral health, developing a mother-and-child oral health education program targeted toward the oral health of the mothers and children in multicultural families. Moreover, it is essential to educate mothers in multicultural families in order to increase their sense of responsibility toward their children's oral health management, aiming to promote knowledge and to change attitudes and behavior.

The value of panoramic radiography in assessing maxillary sinus inflammation

  • Purpose : To evaluate the value of panoramic radiography in diagnosing maxillary sinus inflammation. Materials and Methods : A total of 214 maxillary sinuses from 114 panoramic radiographs were assessed in this study. Two independent experienced oral radiologists evaluated the images in random order for sinus inflammation. Using Cone beam CT images as the gold standard, the sensitivity and specificity of panoramic radiography were calculated, and inter- and intraobserver agreement for panoramic interpretation were obtained. Results : The mean sensitivity and specificity of panoramic radiography were 81.0% and 85.6%, respectively. The weighted kappas for inter- and intraobserver agreement of panoramic radiography were 0.56 and 0.60, respectively. Conclusion : Panoramic radiography is a reasonably accurate method for diagnosing maxillary sinus inflammation and can be used for screening. However, additional examinations should be considered in patients with potentially significant pathology. (Korean J Oral Maxillofac Radiol 2008; 38: 215-8)

Comparison of effectiveness on knowledge improvement before and after the oral health education: focusing on foreign students (구강보건교육 전·후 지식향상 비교효과 연구: 온라인과 오프라인에서 외국인 유학생들을 대상으로)

  • Objectives: The purpose of this study was to investigate comparison of the effectiveness on knowledge improvement before and after the oral health education focusing on foreign students. Methods: A self-reported questionnaire was filled out by 40 foreign students in Sun Moon University from June 2 to September 30, 2014. The experimental group had an online oral health education and the control group had an off line education. The questionnaire consisted of oral health(10 items), toothbrushing(10 items), dental caries(10 items), periodontal disease(5 items), tooth sensitivity(5 items), oral cancer(6 items), and smoking(5 items). The data were analyzed by SPSS Win 18.0 program. Results: Knowledge scores of online education increased to average 1.257 points, and that of offline education was average of 2.56 points. There were statistically significant results for toothbrushing, dental caries, periodontal disease, tooth sensitivity, oral cancer and TMD except for smoking in offline education group. Conclusions: This study suggests that the offline oral health education is more effective than online oral health education.

ROC Analysis of Acid Demineralized Artificial Caries (인공치아 우식병소 진단의 ROC 분석)

  • 조직학적으로 유용성이 입증된 산탈회법을 이용한 인접면 비교적 초기 치아 우식의 병소를 형성하여 진단율을 조사하였다. 산 용액을 이용하여 20개 인접면 치아우식을 20개 소구치에 형성하였고, 37개 인접면 치아우식을 30개 대구치에 형성하였다. 건전한 소구치 20개, 대구치 30개를 포함하여 총 96개 치아를 4개씩 나누어 24개의 블록을 형성하였고, 각각 2개 블록의 교합면을 교합시켜서, 교익촬영을 하였다. 촬영 결과를 36명의 치과의사들이인접면 치아우식의 유무를 기록하고, 동시에 및 ROC 분석을 위한 5 개 범주의 판독 기준으로 판독하여 기록하였다. 인접면 치아우식증 유, 무만으로 판독한 결과 진단의 sensitivity는 0.71, specificity는 0.78 이였다. ROC 분석 한 결과의 곡선도표 아래부분의 평균 면적은 약 0.806 이였다. 치아우식증 유무만으로 진단한 결과는 특정한 sensitivity와 specificity 만을 나타내지만, ROC 분석 결과는 주관적 진단 기준과 구별되는 고유의 진단 능력을 표시하는 1-specificity(False Positive)의 변화에 따른 sensitivity(True Positive)의 변화를 연속적으로 나타내어 주었다.
