건강보험 데이터를 이용한 14세 이하 소아청소년의 치아 우식 유병률 경향성 (Trends of Dental Caries Prevalence in Children Under 14-Year-Old Using a Health Insurance Database)

  • 이 연구의 목적은 건강보험심사평가원의 자료를 이용하여 만 14세 이하 소아청소년들의 치아 우식 유병률의 경향성과 치료의 현황을 파악하는 것이다. 이 연구는 2011년부터 2020년까지 연도별로 전체 소아청소년이 포함된 모집단에서 약 100만명을 무작위 표본 추출한 집단의 진료내역을 이용하여 분석을 시행하였다. 이 연구에서 K02 치아우식 주상병명을 진단받는 아이들은 연도별로 증가하였고, 전반적으로 모든 연령군에서 치아 우식 치료를 받는 아이들도 증가하는 경향성을 보였다. 반면 중증의 치아 우식으로 인해 치수 치료 또는 발치를 경험한 10 - 14세 어린이들은 감소하는 경향을 보였다. 2021 - 2022년 아동 구강 건강 실태조사에서도 5세와 12세의 우식경험지수는 정체되거나 소폭 증가하는 경향을 보였으나 우식 유병자율은 감소하였다. 치료적 관점에서 개인적, 지역적 환경은 과거에 비해 전반적으로 개선된 것으로 나타났으나 예방적 관점에서 치아 우식 유병 상태와 예방 진료가 지난 10년간 개선되지 않고 정체 상태로 머물러 있던 것으로 해석된다. 따라서 대한민국에서 소아청소년을 대상으로 구강건강 증진 및 예방을 위해 시행되고 있는 구강 보건 사업에 대한 홍보와 효용성에 대한 평가가 필요하다.

  • 기록학의 핵심에는 기록관리가 있다. 기록관리에 충실해야 한다. 그러나 기록관리에 충실할수록 발생하는 역설이 하나 있지 않을까. '책임있는 관리자가 되어, 관리적 차원에서 기록을 효율적으로 관리하는 일'이 그런 기록을 만들고 이용하는 인간이라는 사회적 존재에 대한 관심은 오히려 축소시킨다는 역설. 인간은 왜 기록을 생산하고 이용하는가. 그것은 인간이란 존재가 특이하게도 기록이라는 개념을 가지고 살아가는 존재이기 때문일 것이다. 개념은 '생각의 설계도 같은 것'이다. 기록관리가 발전할수록 가치 있는 기록이 더 많이, 보다 체계적으로 보존되며, 폭넓음과 유효적절함으로 서비스가 되는 것이어서 이 방향성을 의심할 필요는 없다. 그러나 기록관리가 아닌 인간의 시선에서 이런 상황을 관찰하게 되면, 기록관리에 등장하는 인간은 기록을 이용하는 대상으로 제한된다는 것을 알게 된다. 원점에서 재검토한다는 가설에 입각해 인간을 다르게 인식할 경우, 인간과 기록의 관계, 또는 기록과 인간의 관계에 대해 특이한 맥락을 접할 수 있다. 인간은 누구에게도 양도할 수 없는 존엄성을 지닌 존재, 행복을 추구할 권리를 가진 존재, 자유와 평등, 사회적 기본권을 향유하며 살아야 하는 존재라는 규범에까지 이르게 된다면, 요컨대, 헌법적 관점에서 인간을 인식한다면, 기록의 사회적 역할과 방향성에 대해 새롭게 인식할 수 있다. 국내외 헌법과 국내외 인권규범은 인간의 기본권을 최종 규범으로 문서화 하고 있으며, 이를 보장하고 실천하는 것이 국가의 의무임을 명확히 하고 있다. 헌법적 관점에서 기록의 역할은 인간의 기본권을 증식하는 기록실천이다. 또는 인간의 기본권을 옹호하고 지지하며 지원하는 일이다. 인간의 기본권을 증식하는 기록실천은 전문가에게 요구되는 시민의식이기도 하겠지만, 다른 한편으로는 기록학의 직업적 통로가 될 수 있다. 기록관리가 2차선 왕복도로라고 한다면, 기록관리와 인간의 기본권을 증식하는 기록실천이 상호작용하는 것은 4차선 왕복도로를 개척하는 일이라고 할 수 있겠다. 이 글은 헌법적 관점을 기록학의 관점으로 명확히 잡아, 그간 기록관리 안팎으로 전개된 인간의 기본권을 증식하는 기록실천을 점검하고, 이런 기조에서 기록학의 사회적 역할을 재검토한 글이다. 기록학의 사회적 역할에는 기록에 관한 새로운 언어적 규칙을 제공하는 것이 있다.

Prescription of antibiotics after tooth extraction in adults: a nationwide study in Korea

  • Objectives: This study aimed to understand the nationwide patterns of antibiotic prescription after tooth extraction in adult patients. Materials and Methods: This study analyzed dental records from the National Health Insurance Service-National Sample Cohort (NHIS-NSC) database on 503,725 tooth extractions performed in adults (≥19 years) during 2011-2015. Patient sex, age, household income, systemic disease (diabetes mellitus and hypertension), type of dental institution, region of dental institution, year of prescription, and type of tooth extraction procedure were considered. The antibiotic prescription rate and broad-spectrum antibiotic prescription frequency were analyzed using chi-squared tests. Factors affecting the prescription of broad-spectrum antibiotics were evaluated using multivariate logistic regression analysis. Results: The rate of antibiotic prescription after tooth extraction was 81.85%. Penicillin was most commonly prescribed (45.25%), followed by penicillin with beta-lactamase inhibitors (18.76%), metronidazole (12.29%), and second- to fourth-generation cephalosporins (11.52%). The proportion of broad-spectrum antibiotics used among all prescribed antibiotics was 45.88%. Conclusion: The findings of this study demonstrate that the rate of antibiotic prescription after tooth extraction is higher in Korea than in other countries. Furthermore, broad-spectrum antibiotics are used more frequently, which may indicate unnecessary drug prescription, an important contributor to antibiotic resistance.

원광대학교 대전치과병원 소아치과 환자에서의 정주 진정법 (Intravenous Sedation for Patients of Pediatric Dentistry in Wonkwang University Daejeon Dental Hospital)

  • Background: The purpose of this study was to evaluate the safety and effectiveness of intravenous ketamine-midazolam sedation in uncooperative children and disabled people undergoing dental procedures. Methods: From September 2009 to January 2012, total 114 intravenous deep sedation were carried out using midazolam and ketamine at pediatric dentistry in Wonkwang University Daejeon Dental Hospital. Patients' dental charts and sedation records were retrospectively reviewed. Results: 103 cases were enrolled, divided into four groups. There were 22 children under five in group 1, 14 children over six years with severe anxiety in group 2, 34 disabled person in group 3 and 33 children over six years with surgical procedure in group 4. Carries treatment (56 cases) was the most common dental treatment. The highest ketamine usage was 4.15 mg/kg, the longest duration of dental treatment was 41.6 minutes in children under five group. Nausea and vomiting (16 cases) was the most common side effect. Conclusions: Intravenous ketamine-midazolam sedation appears safe and effective for uncooperative children and disabled person undergoing dental procedures. But, emesis is a common side effect of ketamine occurring with increasing age.

Cytohistologic Discrepancy of High-Grade Squamous Intraepithelial Lesions in Papanicolaou Smears

  • Objectives: To evaluate the frequency of cytohistologic discrepancy of high-grade squamous intraepithelial lesions (HSILs) in Pap smears and associated factors. Methods: Medical records of 223 women with HSIL Pap smears who were treated at Thammasat University Hospital were reviewed. Data on age, parity, menopausal status, contraceptive use and colposcopic directed biopsy and loop electrosurgical excision procedure (LEEP) pathology results were recorded. Results: Mean (SD) age of patients was 38.0 (9.4) years. The majority were premenopausal (86.5%) and multiparous (83.9%). Cytohistologic discrepancy between the Pap test and colposcopic-directed biopsy histology was 45.7% and that between the Pap test and LEEP histology was 29.5%. Fifty-four (24.2%) women had no high-grade CIN on both colposcopic directed biopsy and LEEP. Nulliparity, postmenopausal status and having no oral contraceptive pills use were factors associated with cytohistologic discrepancy. Conclusion: The exact cytohistologic discrepancy rate was relatively high (24.2%). Factors associated with cytohistologic discrepancy were nulliparity and postmenopausal status and having no oral contraceptive pill use.

Relationship between Intersequence Pauses, Laying Persistency and Concentration of Prolactin during the Productive Period in White Leghorn Hens

  • Prolactin is considered to influence the taking of pauses in between ovulatory sequences in White Leghorn hens. Therefore modulating concentrations of prolactin using bromocriptine - a dopamine agonist during early life (17 to 36 weeks of age) could overcome the inhibitory effects of high concentration of prolactin on ovarian activity. The effect of modulation of prolactin concentration on egg production, sequence length and inter sequence pauses were studied by analyzing the oviposition records from 19 to 72 weeks were studied and compared with untreated controls. Bromocriptine administered subcutaneously (100 $\mu$g kg$^{-1}$ body weight or orally through feed (640 $\mu$g day$^{-1}$ bird$^{-1}$) resulted in a steady and sustained decrease in prolactin levels (p<0.01) during and after the withdrawal of treatment up to one reproductive cycle (72 weeks of age). The treated birds had comparatively longer sequences (p<0.01) and fewer pauses (p<0.01). Egg production increased (p<0.01) by fourteen per cent through subcutaneous administration and eleven per cent through oral feeding, over the control birds. It is concluded that the physiological pauses that occur during ovulatory sequences can be disrupted effectively using bromocriptine. Prolactin levels are modulated which may interfere with the follicular recruitment and subsequent oviposition thereby improve egg laying potential of the bird.

Condylar bony changes in patients with temporomandibular disorders: a CBCT study

  • Purpose: Diagnosis of osteoarthritis most commonly depends on clinical and radiographic findings. The present study attempted to observe the bony changes in temporomandibular joint (TMJ) patients from all age groups. Materials and Methods: The first-visit clinical records and cone beam computed tomography (CBCT) data of 440 TMJs from 220 consecutive TMJ patients were reviewed retrospectively. Results: The most frequent condylar bony change observed was sclerosis (133 joints, 30.2%) followed by surface erosion (129 joints, 29.3%), flattening of the articular surface (112 joints, 25.5%), and deviation in form (58 joints, 13.2%), which included 33 TMJs in a cane-shape, 16 with a lateral or medial pole depression, 6 with posterior condylar surface flattening, and 3 with a bifid-shaped condyle. Fifty-three joints (12.0%) showed hypoplastic condyles but only 1 joint showed hyperplasia. Osteophyte was found in 35 joints (8.0%) and subcortical cyst in 24 joints (5.5%), 5 of which had surface erosion as well. One hundred nineteen joints (27.0%) had only one kind of condylar bony change, 66 joints (15.0%) had two, 52 joints (11.8%) had three, 12 joints (5.0%) had four, and 6 joints (1.4%) had five kinds of condylar bony changes at the same time. Eighty-five (65.9%) of 129 joints with surface erosion had pain recorded at the chief complaint. Conclusion: With more widespread use of CBCT, more specific or detailed guidelines for osteoarthritis are needed.

치아종의 방사선학적 연구 (A Radiographic Study of Odontoma)

  • The purpose of this study was to obtain information on the clinical and radiographic features of the odontomas in the jaws. For this study, the authors examined and analyzed the clinical records and radiographs of 119 patients who had lesion of odontoma diagnosed by clinical and radiographic examinations, The obtained results were as follows: 1. Odontoma occurred the most frequently in the 2nd decade(45.4%) and occurred more frequently III males (60.5%) than in females(39.5%). 2. The most common clinical symptom was the delayed eruption of the teeth(34.2%). 3. The type of lesions was mainly observed as compound odontoma(80.8%), and internal pattern of the complex odontoma was unevenly radiopaque(73.9%). 4. The compound odontoma frequently occurred in anterior portion of the maxilla(57.7%) and mandible(30,9%), and complex odontoma frequently occurred in anterior portion of maxilla(34.8%) and posterior portion of mandible(30.5%). 5. The effects on adjacent teeth were impaction of teeth(71.7%) and prolonged retention of deciduous teeth (31.7%), 6. The impaction of the teeth occurred in anterior portion of maxilla (44.2%) and mandible ( 19.2%), but root resorption of the adjacent teeth were not seen, 7. The boundary to adjacent structure was well-defined, the lesions appear as radiopaque mass with radiolucent rim.

Clinical characteristics of trigeminal neuralgia in a dental hospital

  • Background: Neurovascular compression (NVC) is a well-known cause of trigeminal neuralgia (TN). However, patients with idiopathic TN (ITN) do not have evidence of NVC on magnetic resonance imaging (MRI), and other patients may remain asymptomatic despite evidence of NVC on MRI. This suggests that there may be additional risk factors for TN development other than NVC. Although epidemiological factors, such as age and sex differences, are useful for understanding the pathophysiology of TN, detailed statistics for each TN subtype are currently unavailable. Therefore, this study aimed to classify patients with TN into the following groups based on data extracted from past medical records: classical TN (CTN), secondary TN, and ITN. Methods: The characteristics of the groups and their differences were explored. Results: CTN was more common in women than in men, as previously reported, whereas ITN was more common in men than in women. The ratio of pain sites located on the right side of the face was high in all groups. Patients with CTN were also prone to NVC on the asymptomatic side. Conclusion: By investigating TN subtype, it may be possible to elucidate the pathophysiology of TN. This would greatly improve treatment outcomes.

A retrospective study of mandibular fractures in children

  • Objectives: The incidence and patterns of mandibular fractures vary by country and population age. This retrospective study evaluated the etiologies and patterns of mandibular fractures in children. Materials and Methods: The clinical records of 89 children (45 males and 44 females) aged 0 to 12 years who presented with mandibular fractures from July 2012 to June 2016 were retrospectively reviewed. The sex, patient age, site of fracture, etiology of trauma, and monthly variations of the fractures were recorded. Descriptive statistics, the z-test and chi-square test were used for statistical analysis and the P-value was set at <0.05. Results: Eighty-nine children (male-to-female ratio 1.02:1) sustained 131 mandibular fractures. Within the study sample, the 6 to 9 year age group most frequently experienced fractures (47.3%). Falls and road traffic accidents (RTA) were the two most common etiological factors that accounted for 44.9% and 24.7% of cases. The condylar fracture was the most common anatomical location (38.9%) followed by the angle (20.6%), parasymphysis (18.3%), body (15.3%), and symphysis (5.3%). A single fracture (51.7%) was more common than multiple fractures (48.3%). The month-wise distribution of mandibular fractures was fairly constant. Conclusion: The condylar region was the most common anatomic site for fractures; in addition, a fall and RTA were the major etiological factors for mandibular fractures. A single fracture was observed in 51.7% of patients while multiple fracture lines accounted for 48.3% of cases.
