편평세포암종으로 오인된 협부에 발생한 간질 호산구 증다증을 동반한 외상성 궤양성 육아종: 증례보고 (Traumatic ulcerative granuloma misjudged as oral squamous cell carcinoma (SCC) on the buccal cheek: case report)

  • Traumatic ulcerative granuloma with stromal eosinophilia (TUGSE) is an ulcerative lesion on oral mucosa featuring as a benign mass of self-limiting growth. It can be easily misdiagnosed as squamous cell carcinoma (SCC) due to its long healing period and elevated or rolled-up margin. A 57-year old male patient who visited our department was diagnosed as SCC according to the clinical features, results of positron emission tomography (PET) and magnetic resonance imaging (MRI) of the oral lesion. However, after performing incisional biopsy, histopathologically, there were no atypical cells, but eosinophil and CD 30+ T-cells were clustered in subcutaneous and muscle tissue. It is very significant to consider this reactive lesion in the field of oral and maxillofacial surgery because of its possibility of mistaken as malignant disease. Here, we will report a couple of cases of TUGSE with references and limit of radiographic tools used for diagnosis of head and neck cancer.

Stability and Effectiveness of Laser and Liquiband Treatments in the Rat Model of Oral Ulcer

  • Background and Objectives This study was undertaken to assess the stability and efficacy of laser therapy (808 nm), Liquiband (a commercial topical skin adhesive product), and a combination treatment, for application in oral ulcers. Materials and Methods The oral ulcer rat animal model was used to determine the efficacy of photobiomodulation, Liquiband, or combination therapy. Ulcers were induced by injecting 60% acetic acid in the oral mucosa. Three days after ulcer confirmation, the chemically induced ulcers were treated with either laser (808 nm), Liquiband, or a combination of both, every two days for 8 days (4 sessions). Combination therapy was performed by first treating with laser followed by application of the Liquiband. Reduction in ulcer area size was subsequently determined, and animals were sacrificed at 5 and 10 days after the last administration, for histological observation of the extracted oral ulcer tissue samples. Results In this study, treatment with either laser (808 nm) or commercial Liquiband product as well as the combined treatment significantly enhanced oral ulcer healing in the rat animal model. The effect of laser treatment is mainly attributed to collagen synthesis, whereas application of the Liquiband promotes vascularization and formation of granulation tissue. Our results indicate that further optimization of the combined therapy could synergistically and significantly enhance oral ulcer healing. Conclusion Laser or Liquiband treatment of oral ulcer in the rat animal model exert different outcomes, but both methods confirm significant effects relative to the untreated group. The combined treatment group with laser and Liquiband showed marked oral ulcer healing, but further optimization is required to get highly significant results and establish the potential synergistic effect of the combination therapy, as compared to the individual enhancing effects of healing oral ulcers.

  • 뇌성마비(cerebral palsy)는 비진행성의 정신, 운동 장애를 총칭하는 용어로, 소아 장애의 흔한 원인이 된다. 자해 행위란(self-injurious behavior) 자살 의도없이 자신의 신체 일부를 고의적으로 손상시키는 것으로, 종종 반복적인 행동으로 나타난다. 소아에서의 이러한 자해 습관은 정상적인 어린이에서는 드물며, 증후군, 유전질환, 정신지체 어린이 등에서 그 발생률이 높게 보고된다. 구강 내 자해 행위의 가장 흔한 유형은 혀나 입술 혹은 구강 점막을 물어뜯는 것이다. 이러한 자해 행위를 조절하기 위하여 행동 수정법, 약물 치료, 신체 속박술, 치과적 장치의 적용, 외과적 수술 또는 치아 발치술 등의 다양한 방법들이 제시되었다. 이 중 마우스가드 등의 치과적 장치를 이용한 방법은 구강 내 자해 행위의 감소와 조직의 보호를 위해 가장 보존적이며 적합한 방법이라고 사료된다. 본 증례에서는 자해 습관에 의해 하순과 협점막에 궤양성 병소를 가진 뇌성마비 환아에 있어서, 변형된 마우스가드를 이용하여 자해에 의한 손상을 방지하고 만족할 만한 치유 양상을 보였기에 이를 보고하는 바이다.

구강점막의 창상치유에 대한 식나무(Aucuba japonica) 추출물의 효과 (Effects of Aucuba japonica Extract on Oral Wound Healing)

  • Aucuba japonica has variable pharmacological effects such as hepatoprotective, choleretic, hemodynamic, antimicrobial, and anti-inflammatory activities. This study was performed to investigate the effects of Aucuba japonica extract on oral wound healing. Aucubin was extracted from Aucuba japonica, and injected on either side of buccal mucosa of male mice. Artificial full thickness wounds were made on the site with 1.5 mm biopsy punch under sterile technique. The specimens had taken on day 1, 3, and 5 with 4 mm biopsy punch. Light microscopic examination and quantitative histologic analysis were performed for reepithelization, inflammatory cell infiltration. Reepithelization of the aucubin (0.1%) group was earlier than the control group. And the number of inflammatory cells of the aucubin group was lesser than the control group. In view of the results so far achieved, the aucubin extracted from Aucuba japonica may be useful for oral wound healing and it can be applied as a topical agent on the oral wound. Further research should be performed on the mechanism of aucubin on oral wound healing and proper formulation for effective topical agents.

A critical assessment of the medication-related osteonecrosis of the jaw classification in stage I patients: a retrospective analysis

  • Objectives: It is unclear whether the extent of intraoral mucosa defects in patients with medication-related osteonecrosis of the jaw indicates disease severity. Therefore, this study investigated whether mucosal lesions correlate with the true extent of osseous defects in stage I patients. Materials and Methods: Retrospectively, all patients with stage I medication-related osteonecrosis of the jaw who underwent surgical treatment between April 2018 and April 2019 were enrolled. Preoperatively, the extent of their mucosal lesions was measured in clinical evaluations, and patients were assigned to either the visible or the probeable bone group. Intraoperatively, the extent of necrosis was measured manually and with fluorescence. Results: Fifty-five patients (36 female, 19 male) with 86 lesions (46 visible bone, 40 probeable bone) were enrolled. Intraoperatively, the necrotic lesions were significantly larger (P<0.001) than the preoperative mucosal lesions in both groups. A significant (P<0.05) but very weak (R2<0.2) relationship was noted between the extent of the mucosal lesions and the necrotic bone area. Conclusion: Preoperative mucosal defects (visible or probeable) in patients with medication-related osteonecrosis of the jaw do not indicate the extent of bone necrosis or disease severity.

Obstructive Sialadenitis associated with Injectable Facial Fillers

  • Obstructive sialadenitis, one of the diseases that most frequently causes swelling and pain in the salivary glands, is mainly caused by structural obstructions. Sialolithiasis is the most frequent cause of the disease, and other causes include calculus formation, duct strictures, foreign bodies, and anatomical variations. Although there is a possibility that facial fillers directly block the salivary ducts, no cases of obstructive sialadenitis associated with them have been reported yet. We report the case of a 34-year-old female patient who complained of recurrent swelling and pain in the left buccal mucosa. She had undergone facial filler injection procedures on her facial area for cosmetic purposes several years before. Based on the findings of magnetic resonance imaging (MRI) and MR sialography, she was diagnosed with obstructive sialadenitis due to facial fillers. Through this case, we should remember to obtain a thorough history including filler treatments in the case of parotid gland swelling. We also suggest proper utilization of advanced imaging such as MRI in evaluating the location of facial fillers.

  • Because the implant is regarded as a common treatment. It is clinically important that systemic and local risk factor of threatening peri implant mucosa should be considered during the process. The most risk factors are detected in clinical diagnosis, but it might be difficult and not clear to recognize systemic or combined factors. This article reviews risk factors of peri-implantitis. Local factors are biomechanics, periodontal soft tissue characteristics, infected site and oral hygiene. Systemic factors are alcohol, smoking and genetic traits.

  • We experienced a case of Sturge-Weber Syndrome in a 32 years old female. The diagnosis was established by clinical features of Sturge-Weber Syndrome including unilateral facial portwine nevus, vascular hyperplasia of oral mucosa(espectially 1st & 2nd division of trigeminal nerve). But, plain radiographys of the skull revealed no evidence of calcification. A brief review of related literatures was made. Common Clinical findings in Sturge-Weber Syndrome and specific signs & symptoms manifested by this patient were discussed.

  • Four water monitors at Zoological Garden, Chang Gyeong Won, Seoul, died within a week after signs of anorexia, lethargy, and discharge from eyes, nasal and oral cavities. The autopsy findings of the four animals were similar. As a main lesion, the liver was congested and diffuse necrosis was observed. The terminal portions of the rectum were studded with numerous small ulcers causing rectal stenosis. Histopathologically, massive hepatic necrosis preceded by fatty changes were evident. The rectal lesions manifested coagulative necrosis and thrombosis in the mucosa and submucosa.

  • The purpose of this study was to investigate the effect of quercetin on the bioavailability of paclitaxel orally coadministered in rats Paclitaxel is reported to be metabolized by cytochrome p-450(CYP3.A,)in both the liver and epithelial cells of small intestine and also absorption of paclitaxel is inhibited by p-glycoprotein efflux Pump in the intestinal mucosa. This resulted in poor orall bioavailability of paclitaxel. Area under the plasma concentration-time curve (AUC) of paclitaxel in combination with quercetin were significantly (p< 0.01) higher than those of control. (omitted)
