수종 고정액의 상피세포에 대한 고정효과에 관한 연구 (A Study on the Fixing Effects of Several Fixatives on Oral Epithelial Cell)

  • The author thought that the results of cytologic tests could be changed according to the fixative methods. So the author has studied several methods of fixing exfolaiative cells collected from normal adult cheek mucosa with a solution of equal parts of ether and 95% ethylalcohol, 95 ethylalcohol and Isoprophyl alcohol, and compared the results with each other. The results were as follows : The effects of fixation, and staining efficiency of nucleus and protoplasm are similar in each method, but the staining efficiency of cytoplasm and the conversation of cytoplasmic membrane were best in 95% ethylalcohol.

구강내 면역계에 대한 기본 고찰

  • Oral health depends on the intergrity of the oral mucosa for prevention of the penetration of microbes and macromolecules that might be infectious, allergenic or antigenic. The intraoral immune systems include the tonsils, adenoids and nasopharyngeal-associated lymphoreticular tissue, or NALT. Mucosal inductive sites of the gastrointestinal tract(Peyer's patches and the appendix) and solitary lymph nodes collectively compose the gut-associated lymphoreticualr tissue, or GALT system. Both NALT and GALT are inductive regions where foreign antigens derived from viruses, bacteria, yeast and other molecules are encountered. The integration of tissues in NALT and GALT as part of the mucosal immune system, is very important to keep the oral immune system.

구강작열감 증후군 (Burning Mouth Syndrome)

  • Burning mouth syndrome(BMS) is a burning sensation in the oral mucosa and $doesn^{\circ}$Øt have any identifiable oral lesion and organic etiology. Diagnosis of BMS is mainly based on clinical features and serial exclusion of other possible causes. There is no specific examination for BMS and that could embarrasse the dentist. In this study, the characteristics, differential diagnosis and several treatments of BMS are presented so that dentists can better diagnose BMS to maintain a good relationship with the patients.

Lichenoid Dysplasia Misdiagnosed as Oral Lichen Planus: 3-Year Follow-up Case Report

  • Lichenoid dysplasia is a lichenoid features with epithelial dysplasia clinically and histopathologically similar to oral lichen planus. It can be clinically mistaken for oral lichen planus, but has histopathologic features of dysplasia and a true malignant predisposition. The clinician should be able to differentiate between oral lichen planus and lichenoid dysplasia for the proper management. We experienced a 75-year-old man with erosive, erythematous lesion on the left buccal mucosa previously diagnosed as oral lichen planus. He underwent surgical excision and the final histopathological result confirmed it to be lichenoid dysplasia with massive candidal infection. We report this case with a review of the related literature.

The effectiveness of optical coherence tomography for evaluating peri-implant tissue: A pilot study

  • Purpose: Optical coherence tomography (OCT) has been investigated as a novel diagnostic imaging tool. The utilisation of this equipment has been evaluated through several studies in the field of dentistry. The aim of this preliminary study was to determine through basic experiments the effectiveness of OCT in implant dentistry. Materials and Methods: To assess detection ability, we captured OCT images of implants in each of the following situations: (1) implants covered with mucosae of various thicknesses that were harvested from the mandibles of pigs; (2) implants installed in the mandibles of pigs; and (3) implants with abutments and crowns fixed with temporary cement. The OCT images were captured before cementation, after cementation, and after removing the excess submucosal cement. Results: If the thickness of the mucosa covering the implant body was less than 1 mm, the images of the implants were clearly detected by OCT. In the implants were installed in pigs' mandibles, it was difficult to capture clear images of the implant and alveolar bone in most of the samples. Remnants of excess cement around the implants were visible in most samples that had a mucosa thickness of less than 3 mm. Conclusion: Currently, OCT imaging of implants is limited. Cement remnants at the submucosal area can be detected in some cases, which can be helpful in preventing peri-implant diseases. Still, though there are some restrictions to its application, OCT could have potential as an effective diagnostic instrument in the field of implant dentistry as well.

  • Graft-versus-host disease (GVHD) is frequent complications of hematopoietic stem cell transplantation. In the chronic GVHD (cGVHD), the oral cavity is the most commonly affected region. The clinical manifestations include erythema, ulceration, lichenoid-hyperkeratotic change in oral mucosa, dry mouth, and limitation of mouth opening. The initial treatment strategy of oral cGVHD patients is topical corticosteroid therapy in various formulation. However, corticosteroid resistance appears in some patients. We report a case of a 25-year-old male patient with oral cGVHD, who has resistance to topical corticosteroid medication, treated with 0.03% tacrolimus ointment and low-dose doxycycline. The patient showed subjective and objective improvement without side effect.

  • Sodium hydroxide or caustic soda is a corrosive agent that can cause extensive damage to the oral mucosa, lips, and tongue when ingested either accidentally or intentionally. These injuries include microstomia, shallow vestibule, ankyloglossia, speech impairment, loss of teeth and impairment in facial expression. In the present article, we report a unique case of tongue adhesion to the mouth floor and its surgical management in a 66-year-old female patient, who had a history of caustic soda ingestion.

  • The purpose of this study was to investigate the irradiation effects on the palatal mucosa. For this study, Sprague-Dawley strain rats were irradiated to their head and neck region with the dose of 5Gy and l0Gy by 6MV X -radiation and sacrificed on the experimental periods after irradiation. The authors observed the histological changes of the hard and soft palatal mucosae. The results were as follows: In the light microscopic examination, hydropic change on the basal cells, increased cell size of the epithelium, and decreased epithelial cell layers were observed on the 3hours, 6hours, and 12hours groups after irradiation. But, basal cell hyperplasia, increased epithelial cell layers, and elongated rate pegs were observed on the 3days group after irradiation. After then, these changes were recovered in the mucosa of the hard palate on the 7days and 14days groups, and in the mucosa of the soft palate on the 14days and 2&lays groups after irradiation. And such changes were greater in the mucosa of the soft palate than in that of the hard palate, and more prominent in l0Gy irradiated groups than in 5Gy irradiated groups.

  • Recurrent Aphthous Stomatitis(RAS) is inflammatory ulcerative condition of the oral mucosa. The lesions of RAS are self-limited and persist for 1 to 2 weeks, resolved with or without scarring and recurred after periods of remission. It is known that nicotine may protect oral mucosa from aphthous ulcers by keratinization of the oral mucosa. After quitting smoking, a stroke patient who suffered from RAS, used to relapse into RAS unless he didn't gargle with nicotine extract water. We diagnosed his case as heat in the stomach and treated with Chungwie-San(Qingwei-San). After medication with Chungwie-San, RAS was subsided and he stopped smoking during admission period.

의료인을 위한 구강점막욕창 시각적 감별도구의 신뢰도 (Reliability of the Visual Discrimination Scale on Oral Mucosa Pressure Ulcer for Healthcare Providers)

  • 본 연구는 구강점막욕창의 시각적 감별도구에 대한 신뢰성을 평가하기 위한 연구이다. 연구의 절차는 개발단계와 평가단계로, 개발단계에서는 자료수집, 자문단의 타당도와 신뢰도 평가, 최종도구개발이 수행되었으며, 평가단계에서 대상자들에게 9장의 사진도구를 평가하게 하였다. 세 군에서 측정자 간 신뢰도를 구한 결과, 상처 전문간호사군에서는 Krippendorff's alpha 가 .75이었고, 중환자 전문간호사군에서는 .70이었다. 치과의사군에서의 Cohen's weighted kappa값은 .78 로 나타났다. 측정자 내 신뢰도를 살펴본 결과, 중환자실 일반간호사군들의 intra-rater reliability는 .73으로 나타났다. 신뢰도가 입증된 9장의 사진은 실제 중환자실과 같은 구강점막욕창 위험을 가진 환자를 돌보는 임상환경에서 교육용으로 활용될 수 있을 것으로 보인다.
