사이버 치과 진료 상담을 통해 살펴본 구강내과 질환의 중요성에 대한 고찰 (Consideration for the Importance of Diseases Associated with Oral Medicine by Analyzing the Dental Cyber Consultation)

  • 현대화 사회로 갈수록 현대인들은 대도시 중심의 생활환경의 변화와 함께 일상적 스트레스가 증가함에 따라 측두하악장애, 구강점막 질환, 만성 신경병변성 통증 등의 유병율이 증가하고 있으며, 이에 대한 환자들의 의존도가 높아지고 있는 추세이다. 이에 강릉대학교 치과병원 홈페이지의 사이버 진료실에 게재된 질문들의 내용들을 검토해서 치과 진료과별로 분석해 보고, 이를 통해 구강내과 영역에 해당되는 질환에 대한 중요성에 대해 고찰해 보고자 하였다. 전체 2000여 개의 질문 중 구강내과 진료에 대한 상담에 속하는 질문은 20.92%이었고, 구강악안면외과 진료에 속하는 질문이 16.87%, 치과보존과 16.67%, 치과교정과 14.02%, 치과보철과 12.25%, 치주과 8.36%, 소아치과 4.93%, 예방치과 2.08%, 기타 질문이 3.90%로 나타났다. 구강내과 진료에 대한 상담에 속하는 403개의 질문 중 구강점막 질환에 대한 질문이 44.17%, 측두하악장애에 관한 질문이 41.19%, 구취에 관한 질문이 4.47%, 구강 건조와 관련된 질문이 3.23%, 다른 구강안면통증에 관련된 질문이 2.23%, 법치의학과 관련된 질문이 1.49%, 기타 질문이 2.98%로 나타났다. 구강내과 영역 질환에 대한 질문의 비율이 타과 진료 영역 질환과 관련된 질문에 비해 상대적으로 높게 나타난 것으로 판단해 볼 때, 구강점막 질환과 측두하악장애를 포함한 구강내과 질환에 대한 일반인들의 관심이 상당히 높은 것으로 판단된다.

  • Multiple myeloma is malignant neoplasm of plasma cells. Mandible lesions are rarely the first symptoms of multiple myeloma. While sensory dysfunction and pain are the main symptoms of traumatic trigeminal neuropathy, the same oral symptoms can appear in cases of multiple myeloma with developed mandible lesions. In addition, if the radiological osteoporosis or lytic lesion is seen in older patients, further examination is required to find the cause of the symptoms. In this paper, we present a case that was tentatively diagnosed as traumatic trigeminal neuropathy after left third molar extraction, but later confirmed as multiple myeloma.

Evaluation of Clinically Effective Doses of Triamcinolone Acetonide for Intralesional Injection in Oral Lichen Planus

  • Purpose: The aim of this study was to evaluate the optimal doses of intralesional triamcinolone acetonide (TA) in the treatment of oral lichen planus (OLP). Methods: A randomized clinical trial was performed. Sixty-two lesions of OLP were received 12 mg (group A) or 20 mg (group B) of TA intralesionally weekly for 2 weeks. Subjective symptoms, lesion size, favorable conversion of clinical subtypes, and clinical response were evaluated at weeks 0, 1, 2, and 4. Results: After two consecutive injections of TA, group B showed significant reduction in burning sensation and reticular area (p<0.01). Favorable conversion and complete response were greater in group B. Mild oral candidiasis was developed in group B (10.7%). Conclusions: A 20-mg injection of TA was much more effective compared with 12-mg injection of TA in the treatment of OLP.

  • Whereas there has been developed relatively definitive treatment for the oral mucous membrane diseases with well known etiology, but not developed definitive treatment for those with unknown etiology. And we have thought it should be recommended to further investigate etiology of oral mucous membrane diseases for development of more definitive pharmacologic treatment.

  • 전문의는 일반의(General Practitioner)와는 차별된 전문성을 가지고 증상의 난이도가 높은 환자를 처리하는 능력이 요구되는 진료과목의 필요성을 기반으로 하고 있다. 또한 전문의제도는 국민의료적인 측면에서 임상 각 분야에 있어 단일과목을 전공하는 의사를 양성하여 그들로 하여금 의료 전 과목에 관한 기본적 이론과 실기를 교육받은 일반의사의 능력에서 벗어나는 진료기능을 의료전달체계에서 담당하도록 하여 국민의료의 향상을 기하는 데 있다. 한편, 구강내과는 악안면 통증, 연조직 질환, 법의치과분야, 구강진단분야를 다루는 전문과목으로서 그 전문성이 매우 중요한데, 구강내과학적 측면에서 비추어 볼 때, 현대화 사회로 갈수록 현대인들은 대도시 중심의 생활환경의 변화와 함께 일상적 스트레스가 증가함에 따라 측두하악장애, 구강내 연조직 질환, 삼차신경통과 같은 신경병변 등의 유병율이 증가하고 있으며, 이에 대한 환자들의 의존도가 높아지고 있는 추세이다. 이에 본 연구는 2004년 8월부터 2005년 4월까지 연세대학교 치과대학병원 구강내과 외래에 내원한 초진환자 3,707명을 대상으로 하여, 의뢰율 및 내원경로를 중심으로 분석한 결과 다음과 같은 결론을 얻었다. 1. 구강내과에 의뢰된 전문 진료분야로는 구강안면통증, 구강 연조직 질환, 법의치과분야, 구강진단분야로 나타났다. 2. 구강내과에 의뢰된 환자의 비율은 58.51%로 과반수를 넘는 환자들이 의뢰되었다. 3. 의뢰환자 중 의뢰기관의 영역분석을 시행한 결과, 치과에서 의뢰된 경우가 83.23%로 가장 많았고 의과 및 한의과 영역에서 의뢰된 경우는 16.78%를 보였다. 4. 자의내원한 환자 중 인터넷 및 매스미디어에서 구강내과 전문분야에 대한 사전 검색 및 주변의 권유에 의해 내원한 환자들이 응답자의 30.52%를 차지하였다. 이상의 결과를 종합해 볼 때, 구강내과는 대부분 의뢰된 환자를 진료하고 있으며, 치과영역 뿐 아니라 의과영역에서의 의뢰율이 높은 것으로 보아 치과영역에서는 일반의들이 치료할 수 없는 수준의 진료영역을 담당하고 있는 것으로 볼 수 있으며 의과학 분야에서는 구강내과학적 전문성을 요하는 질환을 구강내과와 협진하에 해결하려는 것으로 사료되어 전문과목으로서 구강내과학의 역할과 비중이 매우 높음을 확인할 수 있었고, 이는 향후 전문치의제도 인력수급 및 이의 기반이 되는 전공의의 정원책정에서 시장적 접근 및 규범적 접근의 기초자료로 활용되어야 할 것이다.

  • Desquamative gingivitis (DG) is a gingival manifestation of systemic mucocutaneous disorders such as mucous membrane pemphigoid, oral lichen planus, and pemphigus vulgaris. The lesion is very painful, so affects the patient's ability to do proper oral hygiene practices. This may be a potential risk factor for long-term periodontal health. However, there is some controversy about the relationship between the existence of DG and periodontal status. Although the correlation between DG-associated diseases and periodontal status is not to be certain, early diagnosis and appropriate treatment including adequate plaque control and removal of local factors is very important for preventing the progression of diseases and destruction of periodontal tissues.

Non-Odontogenic Toothache Caused by the Fungal Ball of Maxillary Sinus: Case Reports

  • A fungal ball (FB) of the paranasal sinuses is a chronic, non-invasive fungal sinusitis defined as the accumulation of dense aggregation of fungal hyphae in a sinus cavity. A patient with FB infection in a sinus cavity has usually non-specific symptoms such as post-nasal drip, nasal congestion, headache. However, facial pain and toothache can be developed if FB infection is in maxillary sinus. The aim of this case report is to present two cases of FB of the maxillary sinus which caused toothache in the upper molar region. It is also to make dental practitioners consider the non-odontogenic origins of toothache and to pay special attention to avoid unnecessary dental treatment.

Oral Hairy Leukoplakia in Patient with Human Immunodeficiency Virus Infection

  • Oral hairy leukoplakia occurs on the lateral surface of the tongue that clinically as an asymptomatic white lesion. It is mainly found in patient with human immunodeficiency virus infection. However, it rarely outbreak immunosuppressed patients after organ transplantation, or the patients taking steroids or immunosuppressants. It is the result of proliferating Epstein-Barr virus in the oral epithelium. Most of human immunodeficiency virus infected patients with oral hairy leukoplakia are highly contagious and possible to progress acquired immunodeficiency syndrome. Therefore, the early diagnosis of oral hairy leukoplakia is very important. Taking a thorough history and human immunodeficiency virus screening test is highly recommended in case oral hairy leukoplakia is detected. In this case, a 29-year-old man presented with whitish lesion on lateral border of tongue is diagnosed as oral hairy leukoplakia and human immunodeficiency virus infection.

  • Soft tissue calcification is a pathological condition in which calcium and phosphate salts are deposited in the soft tissue organic matrix. This study presents an unusual calcification noted in the cartilaginous portion of the Eustachian tube. A 67-year-old woman presented for dental treatment, specifically for implant placement, and cone-beam computed tomography (CBCT) was performed. The CBCT scan was reviewed by a board-certified oral and maxillofacial radiologist and revealed incidental findings of 2 distinct calcifications in the cartilaginous portion of the Eustachian tube. To the authors' knowledge, no previous study has reported the diagnosis of Eustachian tube calcification using CBCT. This report describes an uncommon variant of Eustachian tube calcification, which has a significant didactic value because such cases are seldom illustrated either in textbooks or in the literature. This case once again underscores the importance of having CBCT scans evaluated by a board-certified oral and maxillofacial radiologist.

Oral Appliance as a Sensory Trick to Manage Tardive Dyskinesia of Tongue: A Case Report

  • Tardive dyskinesia (TD) is continuous, repetitive movement disorder of tongue, lip or jaw, induced by medication. It causes pain and dysfunction of oral structures but also interferes with dental treatment and overall social life of patients. As a dentist, it is imperative to recognize and manage TD, although currently, there is no definitive treatment for TD. This article reports a patient with TD of tongue, successfully managed with an oral appliance mimicking sensory tricks. Considering the limited treatment options for TD, an oral appliance, a simple and conservative approach, can be a meaningful treatment for some patients with orofacial dyskinesia.
