Xerostomia and halitosis : A review and current concepts (구강건조증과 구취 : 최신 지견의 고찰)

  • Xerostomia is usually caused by a reduced salivary flow or by changes in the biochemical composition of saliva. Halitosis or oral malodor is an offensive odor usually originating from the oral cavity. Halitosis can lead to anxiety and psychosocial embarrassment. The occurrence of halitosis closely related with intraoral conditions including the presence of xerostomia. Especially, the relationship between xerostomia and halitosis is prominent in elderly patients receiving polypharmacy with at least two systemic diseases. This study is a review of the update literature of xerostomia and halitosis. A large number of papers have been searched and identified using the words , , , , , , and . Papers not relevant to the issue were removed reducing the entries to 79 only. Most of identified papers were systematic reviews, non-systematic reviews, and observational studies. With a proper diagnosis, identification of the etiology and timely referrals certain steps are taken to create a successful individualized therapeutic approach. It is significant to highlight the necessity of an interdisciplinary method for the treatment of xerostomia and halitosis to prevent misdiagnosis or unnecessary treatment. This article concisely focuses on the development of a systemic flow of events to come to the proper treatment of the xerostomia and halitosis.

Antibacterial Effect of Colloidal Silver on Some Oral Bacteria (콜로이드상 은이 수종의 구강 세균에 미치는 항균 효과)

  • The maintenance of good oral health in adults is often hindered by oral malodor and periodontal diseases which are known to be commonly caused by some species of Gram-negative anaerobic bacteria, with low sensitivity to common synthetic antibiotics or antibacterial chemical agents. Therefore the development of a nonharmful natural antibacterial oral rinsing remedy against the causative bacteria is thought to be very important. The purpose of this study is to obtain the basic data for development of a nonharmful natural antibacterial oral rinsing remedy using colloidal silver. The author applied colloidal silver solution with concentration of 10, 30, 50, 80 ppm to some strains in species of Prevotella intermedia, Porphyromonas gingivalis, Fusobaterium nucleatum, and evaluated the effects of colloidal silver on the growth of experimental bacterial strains in aspects of minimal inhibitory concentration (MIC), minimal bactericidal concentration (MBC) and growth pattern after incubation for 24, 48, 72 hours. The obtained results were as follows: MIC of colloidal silver solution against experimental strains was 30 ppm in P. intermedia, 10 or 30 ppm in P. gingivalis, and 30, 50, or 80 ppm in F. nucleatum. And MBC of colloidal silver solution against experimental strains was 30 ppm in P. intermedia, 30 or 50 ppm in P. gingivalis, 30 or 80 ppm in F. nucleatum. Therefore it was concluded that colloidal silver exhibited bacteriostatic or/and bacteriocidal effects against some experimental strain. And the inhibition of growth of experimental strains were markedly or considerably exhibited under 30 ppm$\sim$50 ppm of colloidal silver solution for 48 hours$\sim$72 hours in P. intermedia, 10 ppm$\sim$30 ppm for 24 hours$\sim$48 hours in P. gingivalis, 30 ppm for 24 hours in F. nucleatum. These results indicate that the colloidal silver inhibited effectively the growth of some species of Gram-negative anaerobic bacteria by exhibition of bacteriostatic or/and bacteriocidal effects, and can be used as a possible major ingredient of the nonharmful natural antibacterial oral rinsing remedy to oral malodor and periodontal diseases.

A research study on the necessity of school oral health education (학교구강보건교육의 필요성에 관한 조사 연구)

  • The purpose of this study was to examine the influence of oral health education experience on needs for oral health education in children and adolescents in elementary and secondary schools. A self-administered survey was conducted on the students in elementary, middle and high schools located in the city of Mokpo, South Jeolla Province, from October 1 to 31, 2018. The data that were collected from 327 students were analyzed by SPSS 21.0. Statistical data on frequency, percentage, mean and standard deviation were obtained, and t-test, one-way ANOVA, correlation analysis and logistic regression analysis were carried out. The findings of the study were as follows: First, the subjects got 2.25 on a three-point scale in self-rated oral health status. This score was above average, which indicated that the students thought they were in good oral health. As for problems with oral health, dental caries was given the highest score of 2.48 on a five-point scale, followed by oral malodor with 2.35. Second, the students who experienced oral health education accounted for 69.1 percent, and the students who felt the need for this education represented 82.6 percent. As for educational content desired, the biggest group of 57.8 percent hoped to learn about how to prevent and manage dental caries, and the second largest group of 17 percent wanted to learn about how to take care of oral health during orthodontic treatment. Third, as a result of investigating the state of oral health education by the grade of school, the elementary school students had more oral health education experience than the middle and high school students, and the middle school students placed the most importance on the necessity of oral health education. The differences were statistically significant. Fourth, as a result of analyzing the correlation between oral health education experience and the necessity of oral health education, the students who had more oral health education experience asked more for this education, which implies that there was a statistically significant positive correlation. The findings of the study ascertained that oral health education should be provided for students in childhood and adolescence to boost the level of their oral health knowledge and change their oral health attitude in a positive manner. If oral health practice programs that connect schools, local communities and families with one another are developed to guide the oral health behaviors of teenagers in the right direction, it will make a contribution to the promotion of oral health.

A Case Report on the Effect of Yin Deficiency Medicine on Halitosis Patients by Using OralChromaTM (구취 환자의 음허지제 치료에 대한 연속 증례보고 : OralChromaTM를 이용하여)

  • Objective: This study investigated the effect of Yin deficiency medicine on halitosis patients and the relationship between subjective recognition and objective symptoms of oral malodor. Methods: We administered Yin deficiency medicine, such as Jaeumganghwa-tang, and conducted the Halitosis Associated Life-quality Test (HALT) and $OralChroma^{TM}$. Subjective recognition of halitosis was evaluated using HALT. Objective symptoms of halitosis were evaluated using $OralChroma^{TM}$. In addition, before and after administering medicine, we determined the halitosis according to percentages at the patient's word. Results: The total gas and HALT were not related to each other after Maekmoondong-tang or Saengmaek-san were administered to 10 halitosis patients. Conclusion: Yin deficiency medicine is effective in treating halitosis. Moreover, the subjective recognition and objective symptoms of halitosis are not related to each other.

Application of Periodontal Splint for Severely Mobile Incisor Teeth in Two Dogs (개에서 periodontal splint를 이용한 치아유동성의 치료)

  • A 2-year-old, intact and a 10-year-old, castrated male Maltese were referred for treatment of progressive tooth mobility and periodontal disease. The first case was presented with tooth mobility of mandibular incisors (Grade 2-3) and the second case was also presented with tooth mobility of maxillary incisors (Grade 2-3) by periodontal disease. The treatment plan included supragingival scaling, closed root planing, subgingival curettage and removable-fixed periodontal splinting of the mandibular (case 1) and maxillary (case 2) incisors to stabilize them. Three months after therapy, oral examinations were performed for evaluation of success of therapy. In both cases, oral malodor, periodontal disease and tooth mobility were resolved and periodontal splints were remained rigidly.

Clinical Characteristics of Patients with Halitosis (구취 환자의 임상적 특징)

  • Objective : Halitosis, or bad breath, is a common concern for many people. The aim of this study was to investigate clinical characteristics of halitosis patients and correlation with their various associated elements. Methods: We surveyed 169 halitosis patients by reviewing questionnaires from the Halitosis Clinic in the Hospital of Oriental Medicine, Kyunghee University from January 2004 to March 2006. The questionnaires contained various items including sex, age, history of smoking and alcohol intake, duration of symptoms in a week, time and condition of severe halitosis, suggestive origin of halitosis, taste abnormality, oral hygiene, self-assessed halitosis severity, dry mouth, postnasal drip, tonsillolith, globus pharyngeus, reflux sensation, too much gas and Winkel tongue coating index. Volatile sulfur compounds were measured with a $Halimeter^{\circledR}$. Results : The halitosis patients actively carried out management methods such as non-smoking, moderation in drink, teeth brushing and tongue scraping in order to decrease their oral malodor. Tongue coating significantly affected the $Halimeter^{\circledR}$ reading score, and tongue coating and dry mouth significantly affected self-assessed halitosis severity. Conclusions: Our results suggest that tongue coating and dry mouth should be treated to improve satisfaction in halitosis patients.

Factors Related to Patients' Satisfaction Level of Treatment Outcome of Oral Malodor (구취치료 후 만족도에 영향을 미치는 예견인자)

  • 본 연구에서는 환자들에 의해 구취와 관련이 있다고 일반적으로 믿어지고 있는 여러 요인들이 치료 후의 환자의 만족도에 미치는 영향에 대해 알아봄으로써 환자의 만족도의 예후를 판단할 수 있는 자료를 마련하고자 하였다. 서울대학교 치과병원 구강진단과에 구취를 주소로 내원한 환자 194명을 대상으로 설문지를 이용하여 구취와 관련성이 있는 것으로 알려져 있는 항목들을 조사하였다. 조사 항목으로는 1) 본인확인가능여부, 2) 타인확인가능 여부, 3) 구취의 기간, 4) 구취의 빈도, 5) 축농증, 6) 비염, 7) 코수술 병력, 8) 콧물이 목뒤로 넘어감, 9) 코막힘, 10) 비호흡 곤란, 11) 구강건조감, 12) 편도선부종, 13) 가래, 14) 신물의 역류, 15) 소화불량, 16) 흡연여부가 포함되었다. 구취의 평가는 portable sulfide monitor인 $Halimeter^{(R)}$(Interscan Co., Chatsworth, CA, USA)를 이용하여 치료 전, 후에 각각 측정하였고, 혀배면의 세정, 치면 세균막 조절, 0.25% $ZnCl_2$ 양치액의 사용을 내용으로 하는 구취의 치료를 시행한 뒤 자신의 구취개선정도에 대해 %로 질문하여 치료 후 만족도를 조사하였다. 각 조사 항목과 치료 후 만족도 사이의 상관관계 및 실제 $Halimeter^{(R)}$ 수치의 개선정도와 만족도 사이의 상관관계를 분석하였으며, Symptom Checklist - 90 Revised(SCL-90R)의 분석결과에 따라 환자를 두 그룹으로 분류하고 두 그룹 사이의 치료 후 만족도의 차이를 조사한 결과 다음과 같은 결론을 얻었다. 1. 본인 및 타인 확인가능여부, 구취가 항상 나는지 여부, 구취의 기간, 축농증, 비염, 코수술 병력, 코막힘, 비호흡 곤란, 콧물이 목뒤로 넘어감, 흡연여부에 따른 치료 후 만족도의 유의한 차이는 없었다. 2. 신물의 역류(p=0.003), 소화불량(p=0.007), 구강건조감(p=0.016), 편도선 부종(p=0.018), 가래(p=0.033)에 따른 치료 후 만족도는 통계적으로 유의한 수준의 차이를 보였다. 3. $Halimeter^{(R)}$ 수치의 개선정도와 만족도 사이의 유의한 상관관계는 관찰되지 않았다(r=0.092, p=0.201).

Relation of Self-Perception of Halitosis According to Some Dental Hygiene Students's Oral Care Habits and Dietary Habits (일부 치위생(학)과 학생들의 구강관리습관과 식이습관의 구취자각 관련성)

  • Purpose. Most of halitosis is found by people than by themselves in their life. There are some people who are suffering from oral malodor psychasthenia although they do not have halitosis which may disturb social life by sociophobia. Methods. The subjects of this survey were the dental hygienic college students in Busan city and Gyeongnam province from December 1 to December 20, 2013, we use the 281 parts except for the 19 parts of a bad faith response of 300 parts. Results. As a response to the awareness of the halitosis of college students, "the smell became sometime" was highest in 63.3%, and tooth brushing of halitosis, In according to about awareness of halitosis brushing the number of cases showed a 73.4% increase when you answered "no" in case one day the number of brushing at least three times less than a minute halitosis awareness conducted brushing (p<0.05),in case of less than 1 minute brushing time, this was a statistically significant increase halitosis awareness (p<0.05). Conclusions. This study assessment the association between halitosis awareness and oral care habits was to provide students with the basis for education and counseling halitosis in clinical.

Changes in concentration of VSCs after home oral care interventions based on community care in older adults (커뮤니티케어 기반 방문구강관리 중재에 의한 노인의 구취 농도 변화)

  • Objectives: This case study was conducted to assess changes in the concentration of volatile sulfur compounds (VSCs) of older adults following home oral health care interventions based on community care. Methods: The participants were three elderly people with the halitosis. An oral health intervention programs including oral massage, oral hygiene care, and oral muscle training strenghtening were conducted for 12 weeks. Halitosis was measured using an oral malodor-checking device. Results: The program showed positive effects on changes in halitosis. The concentration of VSCs of the first case decreased rapidly from 44.5 Refres Oral Volume (ROV) on pre-test to 15.5 ROV on the first post-test. In the second case, the score decreased from 14.5 ROV on pre-test to 12 ROV on 2nd post-test. In the third case, the score decreased slightly from 6.5 ROV on pre-test to 6 ROV on the first post-test. Conclusions: Oral health care interventions contributed to decreasing the concentration of VSCs and improving the quality of life of older adults. The active promotion and customization of these programs are required.

Effect of the tooth pick of the disabled person oral treatment (Toothpick method(와타나베법)을 이용한 장애인 구강건강관리 효과)

  • This study conducted a 'specialist tooth brushing' method against the severely disabled once every other week. The purpose of this study is to analyze and validate the effectiveness of maintaining healthy periodontal management when consistent oral hygiene lasts with minimum stimuli for 24 - months. The conclusions were as follows. Table 2 compares the difference between group 1 and 2 from the 1st to 11th management and verifies the actual difference in measurement of each index. Group 1 used a professional tooth brush while Group 2 used a general brush. There were found many significant differences in dental index. As for PHP, Oral Malodor index and gingival bleeding, it was found that one or two times of dental care can make substantial differences in dental health condition. These results show that as the number of toothbrush method has been increased, gingival bleeding and periodontal index are significantly improved, not to mention the improvement of gingivitis and periodontal disease. This is because periodontal tissue is affected by brushing method, brushing time and consistency of dental care. During the 6-month period, 20 minutes of active periodontal care was intensively conducted, and for 18-months the effect of consistent care was verified by 10-minute periodontal care once every other week. As a result of the test, it was found that there was no complete regression in the basic periodontal treatment and the periodontal health condition had been maintained for 24 months. Also this test shows that, despite of inconsistent dental hygienic care, regular plague control can prevent dental diseases and maintain the dental health. This study proved that periodontal condition can be maintained by periodontal care once every other week as the tooth brushing properly stimulates the gums with positive effect. Therefore, professional dental healthcareworkers should be designated for each facility for the disabled, and dental health of the disabled should be professionally cared on a regular basis, and consistent and repetitive management by the dental care specialist are required.
