The Influences of Swallowing Function on Swallowing-Quality of Life and Activity of Daily Living of Inpatients in Geriatric Hospital (노인요양병원 입원환자들의 삼킴기능이 삶의 질과 일상생활동작에 미치는 영향)

  • This study was to evaluate the influences of swallowing function on quality of life and activity of daily living of inpatients in geriatric hospital. The subjects were consisted of elderly patients over the age of 65, 50 people in geriatric hospital of gwangju and jeonnam area. The results are as follows: First, swallowing function score of the subjects is $29.90{\pm}5.27$, SWAL-QOL score is $158.12{\pm}33.36$, ADL score is $80.40{\pm}22.91$. Second, swallowing function, SWAL-QOL details and ADL details of each other showed a statistically significant correlation. Third, the items of swallowing function affecting on quality of life are 'oral cavity in residue', 'to eat solid food', and 'phlegm obstructs the throat'. In conclusion, this study showed that swallowing function of inpatients in geriatric hospital is closely associated with the quality of life and the activity of daily living. Therefore, future development of programs for the improvement of swallowing function is required in order to improve the quality of life of inpatients in geriatric hospital.

  • Objectives: The aim of this study was to use four sets of success criteria to evaluate the outcomes of arthrocentesis treatment with hyaluronic acid injection in patients with internal derangement (ID) of the temporomandibular joint (TMJ). Materials and Methods: The study included 40 patients diagnosed with unilateral Wilkes stage III TMJ dysfunction. Clinical parameters, including maximum mouth opening (MMO) and pain during function, were evaluated preoperatively, 6 months, and 1 year after TMJ arthrocentesis. Outcomes were assessed and compared using four sets of success criteria from the following: the American Association of Oral and Maxillofacial Surgeons (AAOMS; MMO ${\geq}35mm$ and visual analogue scale [VAS] score ${\leq}3$), Murakami et al.'s criteria (MMO >38 mm and VAS score <2), Emshoff and Rudisch criteria (MMO ${\geq}35mm$ and >50% pain reduction), and patient self-reports (self-evaluation of treatment as successful or unsuccessful). Results: Significant improvements in MMO and pain reduction during function were observed between the preoperative period and 6 months and 1 year postoperatively (P<0.01). The success rates of treatment determined using AAOMS (52.5%), Emshoff and Rudisch criteria (57.5%), and self-reported patient criteria (40.0%) were similar. Application of the Murakami et al. criteria reported the lowest success rate (12.5%). Conclusion: The AAOMS and Emshoff and Rudisch criteria are consistent with patient expectations and can be used to assess treatment efficacy.

The Effects of Vocal Loudness on Nasalance Measures of Normal Adults (음의 크기가 정상성인의 비음도에 미치는 영향)

  • This study examined the effect of vocal loudness on nasalance measures, under the conditions of three sentence patterns (i.e., Oral sentences, Mixed sentences, Nasal sentences). The vocal loudness level was classified into soft voice (55 dB), medium voice (65 dB) and loud voice (75 dB). The participants in the present study were 30 normal adults (male: female =1:1). Kay's Nasometer 6200 was used to measure nasalance and Sound level meter was used to adjust the loudness level. The results of the present study are as follows. Firstly, the change in vocal loudness is in the following. In the Oral sentence stimuli, the loud voice for both male and female showed the highest nasalance degree, and the medium voice the lowest level. In the Mixed and Nasal sentence stimuli, however, male participants showed the highest degree of nasalance in the soft voice, and the lowest degree in the loud voice, and female showed the highest degree of nasalance in the soft voice and the lowest in the medium voice. Secondly, when each subject's nasalance scores were ranked in a ordered manner, noticeable tendency. Lowest nasalance score occurred in the loud voice and the highest nasalance score was recorded in the soft voice during participants' reading of the Nasal sentences. However, it was hard to find such pattern in the Oral sentences. It is assumed that velopharyngeal function could be related to these findings. Furthermore, the findings associated with vocal loudness may have diagnostic as well as clinical implications.

Comparison of Nasalance Score Between Glottal and Oral Articulation in Children with Velopharyngeal Insufficiency (연인두 폐쇄부전 아동의 보상조음과 정조음에서의 비음치 비교)

  • Background and Objectives: Nasometry is an easy, noninvasive method to obtain objective data regarding the function of velopharynx. However, because articulation errors may affect the results of nasometry, the examiner should interpret the nasalance score based on appropriate speech stimuli. The purpose of this study is to examine the difference of nasalance score between glottal and oral articulations in patients with velopharyngeal insufficiency (VPI). Materials and Method: Nineteen children between 3.4 and 12.1 years of age (mean age 5.7 years) with a confirmed VPl showing hypernasality and articulation errors (glottal stops) were included. Nasalance scores were obtained for two speech patterns of glottal and oral stops. In addition, the velopharyngeal functions were analyzed in four subjects using video nasopharyngoscopy. Results: The $mean{\pm}S.D$ nasalance scores of the glottal stops and oral stops were $42.54{\pm}16.26%$ and $25.47{\pm}16.51%$ respectively (p=.000). Six of 19 patients achieved normal nasalance scores when glottal stops changed to oral stops by the trial speech therapy. Video nasopharyngoscope confirmed that large velopharyngeal gaps can be decreased into tiny gaps or complete closure when compensatory articulations were corrected for some cases. Conclusion: Compensatory articulation errors must be corrected for the reliable interpretation of the nasalance scores that are obtained in children with velopharyngeal insufficiency, which would facilitate to make a better decision for further management of these patients.

Association Between Cognitive Impairment and Oral Health Related Quality of Life: Using Propensity Score Approaches (인지기능과 구강건강관련 삶의 질의 연관성에 대한 연구: 성향점수 분석과 회귀모델을 중심으로)

  • Objective : This study analyzed the correlation between cognitive function and oral health-related quality of life (OHQoL). Methods : Demographic and clinical characteristics were extracted and utilized for subjects aged 45 years or older who participated in the 8th Korean Longitudinal Study on Aging in 2020. The dependent variable was the Geriatric Oral Health Assessment Index, and the independent variable was the level of cognitive function classified by the Mini-Mental State Examination scores. The analysis method used inverse probability of treatment weighting (IPTW). Then, the association between cognitive function and OHQoL was analyzed by multiple regression analysis. Results : Among the participants, 4,367 (71.40%) had normal cognition, 1,155 (18.89%) had moderate cognitive impairment, and 594 (9.71%) had severe cognitive impairment. As a result of analysis by applying IPTW, there was a negative correlation between the cognitive function group and OHQoL (normal vs. moderate: β = -2.534, p < .0001; normal vs. severe: β = -2.452, p < .0001). Conclusion : After propensity score matching, mild cognitive impairment showed a more negative association than severe cognitive impairment. Therefore, patients with cognitive impairment require oral health management education to improve OHQoL regardless of the level of cognitive impairment.

Evaluation of Sex and Age Factors Contributing to the Diagnosis of Oral Frailty in Community-Dwelling Older Adults

  • Background: With increasing interest in health in old age, aspects of oral aging are being considered. The Korean Academy of Geriatric Dentistry recently proposed the diagnostic criteria for oral frailty in older adults in Korea. This study aimed to conduct a cross-sectional survey of factors related to oral frailty among community-dwelling older adults and identify differences in oral frailty status according to age and sex. Methods: Among 217 older adults aged ≥60 years who visited a senior center in Wonju, 206 completed all tests for oral frailty. Among them, data from those with a Korean Version of the Modified Barthel Index score ≥90 were used in the final analysis. After evaluating oral frailty diagnostic factors such as chewing ability, occlusal force, tongue pressure, oral dryness, oral cleanliness, and swallowing function, oral hypofunction was determined according to the oral frailty diagnostic criteria. Subsequently, the evaluation results were compared based on sex and age. Results: Significant differences in chewing ability, maximum occlusal pressure, and maximum tongue pressure were observed between sexes. However, these differences did not affect oral frailty diagnosis. All diagnostic factors of oral frailty, except for the risk of oral dryness and swallowing dysfunction, showed significant differences with age. However, no significant difference was observed in the prevalence of oral frailty. Additionally, this study found no relationship between sex and oral frailty factors using the oral frailty diagnostic criteria. However, it also found that age plays a significant role as an oral frailty diagnostic indicator, in addition to oral dryness and swallowing function. Conclusion: Sex and age did not affect oral frailty diagnosis. However, patients' chewing ability, occlusal force, and tongue pressure were affected by sex and age. Therefore, sex and age should be considered when diagnosing and intervening in oral frailty in the future.

Effect of Oral Health Care on Swallowing Function and Swallowing-Quality of Life in Patients with Dementia (치매환자의 구강관리가 삼킴기능과 구강관련 삶의 질에 미치는 영향)

  • The purpose of this study was to evaluate the status of oral health care in dementia patients living in long-term care facilities and to investigate the effects of this on swallowing function and swallowing-quality of life. The subjects were 60 demented patients who were admitted to long-term care facilities in Gwangju and Jeonnam provinces. Their Oral Health Assessment Tool(OHAT), Clinical Dysphagia Scale(CDS), and Oral Health Impact Profile 14(OHIP-14) were measured. The results of this study were as follows: First, the effect of oral health care on dentition patients using long-term care facilities on swallowing function and swallowing-quality of life was significantly different according to oral care score (p<.05)(p<.01). Second, the relationship between oral health care and swallowing function of dementia patients and swallowing-quality of life. was examined. As a result, oral health care of dementia patients was affected by swallowing function (r=.405, p<.01) the swallowing-quality of life (r=.462, p<.01) and swallowing function were significantly correlated with swallowing-quality of life (r=.351, p<.01). Therefore, the oral health care of users of long-term care facilities was closely related to swallowing function and swallowing-quality of life.

Association between masticatory ability, oral health-related quality of life and cognitive function in the elderly population using structural equation modeling (구조방정식을 이용한 대도시 일부 노인들의 저작능력 및 구강건강관련 삶의 질과 인지기능)

  • Objectives: The aim of this study was to investigate the association between masticatory ability, oral health-related quality of life, and cognitive function in an elderly population using structural equation modeling. Methods: A total of 308 subjects, aged 65 years and over, were recruited from the senior citizen center in Daegu, South Korea and provided consent for inclusion in the study. The Korean version of the Mini-Mental State Examination (K-MMSE) was used to assess the level of cognitive function. Masticatory ability was evaluated through chewing tests, patient-perceived masticatory function and, measurement of relative occlusal forces on molar teeth using the T-Scan $III^{(R)}$ system. All data were analyzed using ${\chi}^2$ tests, t-tests, and one-way ANOVA in SPSS version 23.0 for windows. Structural equation modeling was performed using SPSS AMOS version 22.0. Results: There was a significant association between cognitive function and molar teeth occlusal force. Ability of food chewing score and patient-perceived masticatory function could affect oral health-related quality of life (P>0.05). Higher occlusal forces on molar teeth, compared to anterior teeth, positively correlated with higher cognitive function. Conclusions: These results suggest that an individual's oral health condition could affect molar teeth occlusal forces and may be related to a risk of developing dementia. Therefore, there is a need for implementing nation-wide policies to improve oral health, such as masticatory function, in the elderly population.

COMPARATIVE STUDY OF PANORAMIC MANDIBULAR PARAMETERS IN POSTMENOPAUSAL OSTEOPOROTIC WOMEN (폐경후 골다공증 여성환자에 있어서 파노라마상 하악골 지표에 관한 비교연구)

  • Osteoporosis has recently been recognized as a major health problem in the elderly population. The disorder is manifested as a loss of bone mass accompanied by structural alteration of bone and increased incidence of fracture. Mandible also may be affected. So, I evaluated panoramic views of 66 postmenopausal women for finding the possibility of useful diagnostic mandibular parmeters of osteoporosis. To know the correlationship between skeleton and mandible, the average of the bone mineral density of lumbar from 2nd to 4th by the dual energy X-ray absorptiometry(DEXA, LUNAR DPZ. USA), and age and mandibular parameters, that is, the number of residual teeth, alveolar ridge resorption ratio, panoramic mandibular index (PMI), mandibular cortical width (MCW), angular cortical thickness (ACT), ramus cortical thickness (RCT), morphology of mandibular inferior cortical (MIC) were compared. And I divided the all tested women to the osteoporotic group and non-osteoporotic group by the use of T-score -2.0, which was derived from skeletal bone mineral density (BMD). To find the correlationship of the each group with mandibular parameters, t-test and discriminant analysis were done. The results of the t-test were that all parameters were highly related with 2 groups (p<0.05). Especially ACT, MIC, age have had even higher correlationship than others (p<0.001). The results of the discriminant analysis by the use of these ACT, MIC and age were that the discriminant function was Z = -2.973+(-1.447)$\times$(ACT)+1.131$\times$(MIC score)+(0.052)$\times$(age), the cutting score was 0.257 and the classification accuracy was 84.8%. Therefore I suggest that the consideration of the angular cortical thickness (ACT), the age of patient and the morphology of mandibular inferior cortical(MIC) may help find the osteoporosis.

The Effects of Swallowing with Oropharyngeal Sensory Stimulation in Nasogastric Tube Insertion in Stroke Patients (구강인두 감각자극이 비위관 삽입 뇌졸중환자의 연하기능에 미치는 효과)

  • Purpose: This study was to identify the effects of oropharyngeal sensory stimulation on a functional oral intake scale, ore-pharyngeal swallowing functions, and aspiration pneumonia symptoms with nasogastric tube insertion in stroke patients. Method: This study was a nonequivalent control group non-synchronized design. The subjects or the study were 32 patients who were hospitalized in Kosin Gaspel hospital. The experimental subjects were stimulated for $10{\sim}20$ minutes, 1 time per day. The intervention was implemented for 2 weeks. Result: Participants in the experimental group significantly received a higher score in ore-pharyngeal swallowing function than those in the control group. However the participants in the experimental group only got a high score in the function oralintake scale which doesn't imply a statistical significance. In addition, they didn't geta remarkably higher score in aspiration pneumonia symptoms than those in the control group. Conclusion: This study demonstrates that oropharyngeal sensory stimulation is effective in promoting recovery oro-pharyngeal swallowing function of nasogastric tube insertions in stroke patients.
