  • Various diseases of the orofacial region that are treated in the field of oral medicine not only are associated with local factors but may also be affected by systemic factors. Knowledge about laboratory medicine is needed to identify the systemic factors that can influence these diseases. Therefore, oral medicine specialists should be able to use diagnostic tests of laboratory medicine and interpret the results in diagnosing and treating diseases in the field of oral medicine. The aim of this article is to examine the diagnostic tests used in laboratory medicine that might be applied to assess the systemic aspect of diseases in the field of oral medicine and to interpret the significance of the findings.

Effects of Low Level Laser Therapy on Oral Mucositis Caused by Anticancer Chemotherapy in Pediatric Patients (소아 암 환자에서 항암제 치료 후 발생한 구내염에 대한 저출력 레이저의 효과)

  • Background: Oral mucositis is a common complication of anticancer chemotherapy. The sequelae of this consist of an increased risk of infection, moderate to severe pain, compromised oral function, and bleeding. This study was performed to evaluated the effects of the He-Ne laser and the Ga-Al-As laser on oral mucositis caused by anticancer chemotherapy in pediatric patients. Methods: There were 3 cases of osteosarcoma and 6 cases of leukemia. All patients received He-Ne laser (632.8 nm wavelength, power 60 mW) application on 400-600 Hz scanning for 5-20 minutes and Ga-Al-As laser (904 nm wavelength, power 40 mW) application by fiberoptic hand piece placed in immediate proximity to the tissue without direct contact with it for 30 seconds per point for 5 days per week. During the application patients wore wavelength-specific dark glasses and were instructed to keep their eyes closed. Results: The mean number of treatments with oral intake was $4.89{\pm}0.64$. The mean number of total treatments was $9.44{\pm}2.59$. There were no significant side effects during and after the laser treatments. Conclusions: He-Ne laser and Ga-Al-As (IR) laser treatment were well tolerated and reduced the severity and duration of chemotherapy-induced oral mucositis in pediatric oncologic patients.

Application of Fluoride for Dental Caries Prevention in Older Adults with Dry Mouth: a Clinical Review

  • Dry mouth is common among older adults and significantly affects the oral health-related quality of life. It is a significant risk factor for dental caries, particularly root caries, in older adults due to concurrent periodontal disease and age-related comorbidities. Clinicians managing patients with dry mouth must be aware of preventive measures against dental caries. This clinical review aims to update our knowledge on the use of fluoride for caries prevention in order to establish better strategies for the management of dry mouth in older adults.

GENE EXPRESSION AFTER THE APPLICATION OF THE FLUID-INDUCED SHEAR STRESS ON THE GINGIVAL FIBROBLAST (유체에 의해 유발된 전단력이 치은 섬유아세포 유전자 발현 변화에 미치는 영향에 관한 연구)

  • The oral cavity is humid environment mainly due to the continuous salivary flow. The reaction of oral mucosa to fluid flow is important for homeostasis and pathogenesis. The objective of this study is the screening the change of gene expression after the application of fluid induced shear stress (FISS) on the gingival fibroblast using cDNA microarray assay. The immortalized human gingival fibroblasts were grown and FISS was applied using a cone viscometer at a rotational velocity of 40 rpm, respectively for periods of 2 and 4 hours. The synthesis of cDNA was done from the extracted total RNA and cDNA microarray assay was done subsequently. The genes that showed over 1.6 in the Cy3/Cy5 or the Cy5/Cy3 value were regarded as genes influenced significantly by the FISS application ion (/M/>0.7). The " RUNX-1" was increased its expression in 2 hours group and " RUN and SH3 domain containing 1" was increased its expression in 4 hours group. The "CC020415", "cyclin L1", "interferon regulatory factor1", "early growth response 1", "immediate early response 2", and "immediate early response 3" genes were increased their expression in 2 and 4 hours after FISS application. In conclusion, we could find many genes that were probably related to the FISS application. Interestingly, most of them were placed in similar molecular pathways and these findings improve the reliability of chip data and usefulness in overall screening. From this experiment, we could find many items for further study and it will make improvement in the understanding of intracellular events in response to FISS.

Awareness and practice of dental caries prevention according to concerns and recognition for off-spring's oral health (자녀의 구강건강 관심도 및 인지도에 따른 치아우식예방법의 인식과 실천)

  • Objectives : The purpose of this study was to examine the awareness of mothers on their children's oral health and their concern for that by socio-demographic characteristics and the relationship of their awareness of methods of dental-caries prevention to their practice of the methods. Methods : The subjects in this study were 337 guardians of preschoolers at kindergartens and daycare centers. A self-administered survey was conducted from April 25 to May 27, 2011, and the collected data were analyzed by the statistical package SPSS 18.0. Results : 1.Self-rated concern for children's oral health, 87.7 percent and 12.1 percent replied, "So-so." Whether they were working or not and whether they were mainly responsible for child rearing made significant differences to that(p<.05). 2. As to subjective awareness of their children's oral health, the largest group of the mothers answered "So-so." (44.9%) The second replied that their children were in good oral health(40.5%), and the third group in poor oral health(14.2%). 3. The relationship between self-rated concern for their children's oral health and awareness of methods of caries prevention, statistically significant differences were found according to toothbrushing education and sealant(p<.05). There were no statistically significant differences in practice, but application of fluoride was the least. 4. The relationship between self-rated awareness for their children's oral health and awareness of the preventive methods of caries, there were statistically significant gaps in awareness of toothbrushing education(p<.05). In practice, statistically significant gaps were found in practice of toothbrushing education and sugar-intake restriction(p<.01). 5. In regard to the correlation between awareness and practice of the preventive methods of caries, awareness of all the factors involving toothbrushing education, sealant, application of fluoride and restriction of sugar intake had a significant positive correlation to practice of them. Better awareness led to better practice. Conclusions : In order to ensure children's successful oral health care, more authentic education of how to prevent dental caries should be offered by experts such as dental hygienists and dentists. Especially, detailed information on application of fluoride, restriction of sugar intake and pit and sealant should be provided for mothers to help their children to stay away from dental caries.

  • The definition and application of disability are becoming more important as society becomes more complex. The precise criteria for the assessment and classification of disabilities are primarily focused on certain medical conditions with a lack of guidelines for dental conditions. This study provides an evaluation and comparison of the current tables available for the assessment of disabilities in the dental field with the aim of identifying areas to be improved.

  • Periodontal regenerative therapy and tissue engineering on defects destructed by severe periodontitis need maintaining of space, which provides the environment for cell migration, proliferation and differentiation. Application of bone grafts may offer this environment in periodontal defects. This study evaluated bone graft materials, $MBCP^{(R)}$ and $Algipore^{(R)}$ , in surgically created i-wall periodontal intrabony defects of minipigs by histological analysis. Critical sized($4mm{\times}4mm$), one wall periodontal intrabony defects were surgically produced at the proximal aspect of mandibular premolars in either right and left jaw quadrants in four minipigs. The control group was treated with debridement alone, and experimental group was treated with debridement and $MBCP^{(R)}$ and $Algipore^{(R)}$ application. The healing processes were histologically observed after 8 weeks and the results were as follows. 1. In the control group, limited new bone formation was observed. 2. In MBCP group, more new bone formation was observed compared to other groups. 3. Histologically, dispersed mixture of new bone, biomaterial particles and connective tissue were shown and osteoblasts, osteoclasts and new vessels were present in this area. 4. Defects with Algipore showed limited new bone formation and biomaterial particles capsulated by connective tissue. 5. Histologically, lots of osteoclasts were observed around the biomaterial but relatively small numbers of osteblasts were shown. Within the limitation to this study protocol, $MBCP^{(R)}$ application in 1-wall intrabony defect enhanced new bone formation rather than $Algipore^{(R)}$ application.

  • A common method for treating oral mucosal diseases is taking medication by oral administration. The oral administration is the method of least resistance. Because large part of drugs is degraded by liver, it is necessary to take more drugs getting to appropriate level in blood stream. And there are so many side effects when patients take drugs by oral administration. In so many cases, the patients who suffer from oral mucosal problems have the other general diseases simultaneously. Willingly or not, some patients can't take the medicine by oral administration. Number of topical drugs for oral mucosal disease is less than that for skin diseases because the environment of oral mucosa prevents activity of medicine. In this paper, research on effects of topical type medication for treating oral mucosal diseases is conducted through investigating currently used medications and their effects. In addition, effects of dissolved oral medications with appropriate solvent are demonstrated if this medication is useful for patients clinically.

Exophytic Verrucous Hyperplasia of the Oral Cavity - Application of Standardized Criteria for Diagnosis from a Consensus Report

  • Verruco-papillary lesions (VPLs) of the oral cavity described in the literature involve a spectrum of conditions including squamous papilloma, verruca vulgaris, focal epithelial hyperplasia, condyloma, proliferative verrucous leukoplakia and verrucous carcinoma. A majority of the VPLs are slow growing, benign in nature and have a viral aetiology. Virus associated benign mucosal outgrowths are not too difficult to diagnose either clinically or by microscopy. Apart from virus-associated lesions, VPLs harboring malignant potential or behaviour such as verrucous carcinoma, proliferative verrucous leukoplakia, oral verrucous hyperplasia (OVH), oral papillary squamous cell carcinoma (PSCC) and oral conventional squamous cell carcinoma with papillary features (CSCC) need to be further clarified for better understanding of their predictable biologic behavior and appropriate treatment. Current understanding of potentially malignant VPLs is perplexing and is primarily attributed to the use of confusing and unsatisfactory terminology. In particular, the condition referred to as oral verrucous hyperplasia (OVH) poses a major diagnostic challenge. OVH represents a histopathological entity whose clinical features are not well recognised and is usually clinically indistinguishable from a verrucous carcinoma and a PSCC or a CSCC. A consensus report published by an expert working group from South Asia as an outcome of the 'First Asian Regional Meeting on the Terminology and Criteria for Verruco-papillary Lesions of the Oral Cavity' held in Kuala Lumpur, Malaysia, recognised the clinical description of these OVH as a new entity named 'Exophytic Verrucous Hyperplasia'. Previously described clinical features of OVH such as the 'blunt' or 'sharp' variants; and the 'mass' or 'plaque' variants can now collectively fall under this newly described entity. This paper discusses in detail the application of the standardized criteria guidelines of 'Exophytic Verrucous Hyperplasia' as published by the expert group which will enable clinicians and pathologists to uniformly interpret their pool of OVH cases and facilitate a better understanding of OVH malignant potential.

A Study on Oral Health Projects for the Disabled in public health center (보건소의 장애인 구강보건사업에 관한 연구)

  • Oral health projects that cater to the disabled should be more prevailing in order to ensure the maintenance and successful promotion of the oral health of disabled people. 70 public dental clinics that conducted oral health projects geared toward the disabled were examined to get a precise grip on their oral health projects. The findings of the study were as follows: 1. 31 out of 70 public dental clinics investigated(44.3%) were equipped with two or more dental hygienists who were professional human resources in charge of the oral health projects for the disabled. As for the age and disability type of the beneficiaries of the oral health projects, adolescents(74.3%) and people with mental retardation(87.1%) benefited most from the oral health projects. Concerning the most common implementation frequency of the projects, the projects were carried out once to three times a week(62.9%). 2. The most dominant oral disease treatment provided to disabled people was amalgam treatment and resin treatment(68.6%), which were the early dental caries treatment. The most common preventive treatment that was offered to improve their oral health was oral prophylaxis(82.9%). As for reform measures for the oral health projects, education of personnels in charge of the projects and their specialization(58.6%) were most emphasized. 3. Regarding factors related to the preventive oral health projects for the disabled, the implementation of oral prophylaxis and toothbrushing education was linked to the age of the beneficiaries. More oral prophylaxis was offered to teens, and more toothbrushing education was provided to preschoolers and adolescents. The age of the beneficiaries and the number of dental hygienists responsible for the projects had something to do with the application of fluorides. 4. Concerning the relationship of the preventive oral health projects for the disabled to the number of dental hygienists, one of the personnels in charge of the projects, the application of fluorides( 54.4%) and pit & fissure sealing(56.8%) were more prevalent when there were two or more dental hygienists. There was a statistically significant disparity in that regard(p<0.05). The above-mentioned findings illustrated that in order to boost the oral health of the disabled, dental hygienists who are responsible for the oral health projects for the disabled should put ceaseless efforts into fostering their professional knowledge and ability and offering quality service to disabled patients. Every public dental clinic should be equipped with plenty of professional personnels to enlarge the scope of treatment and ensure the efficiency of treatment and the preventive projects.
