  • The function of the thumb is critical to overall hand function. The thumb enables motions such as pinch, grip, fine manipulation and allows to circumduction and opposition. It's loss is a serious problem not only from cosmetic point of view but also functional. Therefore, we should make every effort on thumb reconstruction. Many methods of thumb reconstruction from simple osteoplasty to complex microsurgical reconstruction have been reported. We should understand merits and demerits of each method and choose proper method on case by case. When both hands are injured and there is no option but to amputate one hand and the thumb of another hand is lost, spare part flap from the thumb of the amputated hand to another hand can solve cosmetic problem, functional problem and donor morbidity. We report a case of amputated thumb which has been reconstructed with amputated ring finger of the contralateral hand using spare part flap concept.

  • Careful attentions should be paid on the renovation of historical building, not only because it conservations architectural heritages by affording new roles of the present days, but also determine their successive value in the stream of the preservation of cultural properties. Among drastically-increasing interest in the remodeling of the modem architecture in Korea, it is meaningful to research the cases in Japan since there are precedent, accumulative, and ample example of lengthened time. In this paper, two examples with different renovation concept, 'harmony' and 'confrontation' are compared. 'Minami-za' is the typical example to harmonize the present requirement with the original purpose and the memories of the buildings, where the use and the design are preserved. On the contrary, 'Shinpu-kan' is exemplified as confrontation where everything except the facade and the structure was remodeled. It is suggested to understand 'harmony' and 'confrontation' as a way to the coexistence of 'new' and 'old' on the basic of the historical meaning of architectures, not only as a design trend.

  • In this study, the desirable direction of residential environment improvement was presented by comparing the areas in Korea and Japan that are difficult to develop due to regulations and low profitability. The study results showed that a new plan should be presented by autonomous governments to promote the participation of residents and make the most of the local characteristics, in addition to the existing comprehensive nationwide redevelopment. The project must be gradually progressed without haste, from the residents' point of view. The basic project outline and the method must be determined by considering the local characteristics in terms of city planning. In conclusion, it is necessary that the designation of the housing redevelopment district should be cancelled if the project cannot be progressed due to the constraints such as city planning and residents' opposition, and a legal basis should be stipulated so that different project methods, including maeul-mandeulgi, can be introduced.

  • Solid waste production increases due to population and consumption increments. Landfill is the ultimate destination for all kinds of municipal solid waste; and is the most convenient waste disposal method in developing countries. To minimize investment and operational costs and society's opposition towards locating landfills nearby, proper landfill sizing and siting are essential. In this study, solid waste forecasting using Autoregressive Integrating Moving Average (ARIMA) was integrated with government future plans and waste composition to estimate the required landfill area for the state of Selangor, Malaysia. Landfill siting criteria were then prioritized based on expert's preferences. To minimize ambiguity and the uncertainty of the criteria prioritizing process, the expert's preferences were treated using integrated Median Ranked Sample Set (MRSS) and Analytic Hierarchy Process (AHP) models. The results show that the required landfill area is 342 hectares and the environmental criteria are the most important; with a priority equal to 48%.

  • This study analyzed the policy making process by finding factors of a back-in phenomenon appeared in contracting out of public library using advocacy coalition framework. The coalitions were divided into 'agreement of direct management', 'opposition of direct management' and 'keep the contract out'. Considering their belief and activity, to share core belief could make a change of secondary belief. It suggests that activating public sphere is necessary for enforcement of their strategies throughout the library policy.

The Impact of Diagnostic Imaging Fee Changes to Medical Provider Behavior: Focused on the Number of Exams of Computed Tomograph (영상진단 수가 변화가 의료공급자 진료행태에 미치는 영향: 전산화단층영상진단 검사건수를 중심으로)

  • Background: Diagnostic imaging fee had been reduced in May 2011, but it was recovered after 6 months because of strong opposition of medical providers. This study aimed to analyze the behavior of medical providers according to fee changes. Methods: The National Health Insurance claims data between November 2010 and December 2012 were used. The number of exams per computed tomography was analyzed to verify that the fee changes increased or decreased the number of exams. Multivariate regression model were applied. Results: The monthly number of exams increased by 92.5% after fee reduction, so the diagnostic imaging spending were remained before it. But medical provider decreased the number of exams after fee return. After adjusting characteristic of hospitals, fee reduction increased the monthly number of exams by 48.0% in a regression model. Regardless type of hospitals and severity of disease, the monthly number of exams increased during period of fee reduction. The number of exams in large-scaled hospitals (tertiary and general hospital) were increased more than those of small-scaled hospitals. Conclusion: Fee-reduction increased unnecessary diagnostic exams under the fee-for-service system. It is needed to define appropriate exam and change reimbursement system on the basis of guideline.

In opposition to the Olympic Games, Delphic Games have attracted little attention. Moreover, There are no comprehensive monograph and research about Delphic Games in ancient and modem era. On that account, this paper will focused on the historical context of Delphic Games, review contents of it, and discuss visions to introducing and announcing the Delphic Games to the academic world.

A Case Study for Site Selection of the Waste Treatment Facilities (폐기물처리시설 입지선정에 따른 사례연구)

  • This study is to investigate the present condition of waste disposal establishment and to analysis problems which could be produced at location selection formalities of waste disposal establishment. It proposed building methods of waste disposal establishment to lead spontaneous participation of local resident according to case analysis of waste disposal establishment. There are research results; i) Opposition of inhabitants was the majority of reason at the business abandonment or delay of waste disposal establishment. Therefore agreement formation course with local inhabitants is most important position. ii) Many estimate have been needed for waste disposal establishment, but support estimate of government was 30-50% that is really low compare with other environmental establishment. So that it need to increase of government estimate. iii) Location collection is carried out based on law and final collected location must be executed without delay of relation business as soon as possible. iv) Standard of location collection has to divide into small, middle and large size and to apply with same rule according to divided location. v) It must be change public subscription before and location selection after and maintain continuance of information offer to local inhabitants and offered information. vi) after building of waste disposal establishment for solving distrust of waste disposal establishment. It must be planed and carried on useful support countermeasure to local inhabitants in actuality.

  • In some American English dialects, the vowels /a:/ or /c:/ have been replaced with /a:/, as in caught /ka:t/ dog /da:g/ that were pronounced in /kc:t/, and /dc:g/ previously. General American does not have /a:/ in its vowel system. But in East American and Western Pensylvania, cot and caught are homophones, /ka:t/, and similarly with other pairs such as collar vs. caller, stock vs. stalk, don vs. down, knotty vs. naughty. The use of /a/ or /c/ is quite unstable, and is a well-known diagnostic for distinguishing the northern speech area of the United States from the midland and southern area. For an increasing number of Americans, however, entirely lack the opposition between /a/ and /c/, merging to /a/, referring to 'both' of the vowels of lot. This paper investigates the use of /b:/ in American dialects and its relationship with /a:/, /c:/ in AE, and with short /a/ in RP. Examining the isoglosses of the use of /a:/ in various databases of the phonological atlas of North America, this paper discusses the use, position, and trend of merger to /a:/ from the vowels of /a:/ and /c:/ in Current American English.
