Surgical Treatment of Idiopathic Unilateral Chylothorax - 1 case report - (특발성 유미흉 치험 1례 보고)

  • A 51-year-old woman was transferred from a private hospital for persistent massive left pleural effusion. Available examination methods did not reveal the cause of the disease. The pleural effusion was confirmed as chylothorax by thoracentesis and chest computed tomography. Previous tube drainage and pleurodesis had failed. Therefore we decided on an operative approach. A left thoracotomy revealed nothing abnormal except for the oozing lymph from the mediastinal pleura, which was sutured by 4-0 prolene. Decortication and pleurodesis were done at the same time. Postoperative course was uneventful and no recurrent pleural effusion was recognized for 3 months. Adult Idiopathic unilateral chylothorax with unknown etiology is rare, but this case was successfully treated with an operative method.

  • Impacted supernumerary anterior teeth (mesiodens) usually are removed surgically with drug sedation and local anesthesia. After extraction of mesiodens, the wound are sutured and removable resin plate is then applied. In this operation, the postoperative bleeding and infection is likely to occur owing to postoperative accumulation of hematoma & seroma, psychologic stress and other contaminated factors (resin plate, poor oral hygiene, etc). So, the authors established the immediate rubber & iodoform gauze drainage into the sutured wound of mesiodens extraction for the prevention of postoperative bleeding and infection. The removable resin splint are not used because of the poor oral hygiene and economic factor. The results were more favorable without the postoperative blood oozing & wound infection in the dentistry (OMFS) of Wonju Christian Hospital.

Anesthetic Management of the Dental Treatment in a Child with Liver Failure Scheduled for Liver Transplantation - A case report - (간이식 예정인 간부전 환아의 치과치료 시 마취관리 -증례 보고-)

  • Special anesthetic considerations were required for children with acute or chronic liver disease. We experienced a case of dental treatment to control infection under general anesthesia in the 2-year-old girl with liver failure. She was also scheduled for liver tansplantation. Her preanesthetic results of liver function test, electrolytes, and coagulation panel were unstable and out of normal ranges. Uneventful anesthetic induction using isoflurane and atracurium and nasotracheal intubation were carried out. General anesthesia was maintained with isoflurane for 2 hours. Oozing from multiple extraction sites was sustained, so the transfusion of platelet concentration 1 units, fresh frozen plasma 1 unit, and packed red blood cell 1 unit was done. She was recovered without complication but was transferred to pediatric intensive care unit for wound care with her endotracheal tube kept. She was transferred to a ward without noticeable complications next day. So we report this successful case of anesthetic management for dental treatment in a child with liver failure.

A Clinical study of Atopic Dermatitis treated by external application with Gynostemma Pentaphyllum MAKINO in cosmetics (아토피 피부염 환자에서 칠엽담(七葉膽)을 함유한 화장품의 임상적 연구)

  • Objectives : This study was conducted to test patients with Atopic Dermatitis by external application with Gynostemma Pentaphyllum MAKINO in cosmetics. Methods : We gave scores to 59 patients who visited the Dept. of Oriental Medical Opthalmology & Otolaryngology & Dermatology of Daegu Hanny Oriental Medical Center from July 1st, 2005 to December 31th. We checked the clinic index score(extent. intensity : the degree of erythema, induration or papulation, puritus, excorination, lichenification, scaling or dryness, erosion or oozing or weeping. the degree of itching), Eosinophil count and IgE. Results : There are 46 patients we could check by clinic index score, and 44 patients by laboratory. 1. In the 46 cases clinic index score was significantly decreased between the before- and afterexternal application treatment periods(P<0.01). 2. The 46 cases was devided into two types (moist heat type and blood dryness type) by intensity. External application of Gynostemma Pentaphyllum MAKINO in cosmetics did not significant change by two types. 3. In the 44 laboratory cases, the number of eosinophil counts was decreased but the number of IgE was significantly increased. Conclusions : Considering the above results, we have concluded that cosmetics with Gynostemma Pentaphyllum MAKINO have the remedical effect for Atopic Dermatitis.

Angiodysplasia in a Child with Chronic Renal Failure: Endoscopic Hemostatic Therapy (소아 만성 신부전 환자에서 발생한 위혈관 이형성증의 Hemoclipping에 의한 지혈 치료 1례)

  • Angiodysplasia is the most common vascular abnormality of the gastrointestinal tract and probably the most frequent cause of recurrent lower intestinal bleeding in otherwise healthy elderly patients. Also, it is an important cause of hemorrhage in chronic renal failure observed in up to 19~32% of patients. Bleeding due to gastric angiodysplasia is treated by various endoscopic approaches, including argon and Nd : YAG laser photocoagulation, monopolar or bipolar electrocoagulation, heater probe, injection sclerotherapy, band ligation or hemoclipping. A 15-year-old boy, who had undergone hemodialysis for chronic renal failure for about 10 years, was admitted due to melena and progressive anemia. A gastroduodenoscopy revealed a cherry red and fern-like lesion with oozing on the posterior wall at junction of gastric body and fundus. Endoscopic hemoclipping therapy was performed. However, melena recurred four days later. Argon plasma coagulation and hemoclipping therapy were performed again. Since then, no recurrence of bleeding has been observed.

  • We report a 10-year-old girl with the blue rubber bleb nevus syndrome (BRBNS) who had chronic severe anemia caused by chronic occult bleeding in the gastrointestinal (GI) tract. The patient was admitted to the hospital frequently for recurrent pallor and fatigue since the age of 7 years. Gastroduodenoscopy and capsule endoscopy revealed multiple venous malformations with blood oozing in the stomach, small bowel and colon. The patient was treated by aggressive surgical resection of the 23 vascular malformations in the GI tract. The patient is well without anemia 15 months post surgery.

Temporary Closure for Sternotomy in Patient with Massive Transfusion Might Be Lethal

  • A 58-year-old male visited our emergency room for multiple traumas from explosion. On initial evaluation, hemopneumoperitoneum with liver laceration (grade 4) and colon perforation was identified. Hemopericardium with cardiac tamponade was also identified. Shrapnel was detected in the right ventricle. Damage control surgery was planned due to condition of hypotension. In operation room, control over bleeding was achieved after sternotomy, pericardiotomy, and laparotomy. Massive transfusion was done during operation. After gauze packing, operation was terminated with temporary closure (TC). Sanguineous fluid was drained profusely. Disseminated intravascular coagulopathy was confirmed through laboratory findings. No extravasation was discovered at hepatic angiogram. On re-operation, there was no active bleeding but oozing from sternotomy site was identified. Bone bleeding was impossible to control. Finally, reoperation was ended after gauze packing and TC all over again. The patient could survive for only a day after re-operation.

  • Although most clinicians now agree that sinusitis can afflict children of all ages, appropriate diagnosis remain controversial. Sinusitis is one of the most challenging diagnoses for a clinician, because there is a lack of validated diagnostic criteria for acute rhinosinusitis. Symptoms generally include nasal congestion, purulent nasal discharge, and cough .The physical examination is often unsuccessful in confirming the diagnosis. If purulent discharge is seen oozing from the middle meatus, the diagnosis of acute rhinosinusitis is almost certain. Purulent drainage may also be seen in the posterior pharynx and accompanied by halitosis. Absence of light by transillumination may indicate acute rhinosinusitis. Imaging studies are not necessary to confirm the diagnosis of sinusitis in children younger than 6 years and should be used sparingly in children older than 6 years. Sinus radiographs still have several advantages over CT scanning including the relatively low cost and the ability to obtain films without the use of sedation in younger children. Positive findings of sinusitis on plain film include air-fluid levels, complete opacification, or mucosal thickening greater then 4 mm. The CT scan however, is significantly more sensitive then plain films in detecting these abnormalities. Although recovery of bacteria from a sinus aspiration is considered reference standard for diagnosis in pediatric rhinosinusitis, its routine used by pediatricians is not practical and therefore is not recommended.

  • Objective : The purpose of this study is to report the effect of Chijageongang-tang on nummular eczema. Methods : We treated a 21-year-old female patient who have nummular eczema by Korean medicine. This treatment was enforced from 19.8.26 to 19.12.30. And we have confirmed the improvement of itching, rashes and oozing. We recorded photos that includes change of symptoms. Results : After the treatments was done, the grade of VAS was decreased. The VAS grade was decreased from grade 6 to grade 0. And we checked DLQI, all questions of index were improved. Conclusion : After administer Chijageongang-tang, the lesions are clearly improved The itchness was decrease. The scabs disappeared and changed into pigment lesions. Thus, It is able that Chijageongang-tang works on skin desease like nummular eczema.

Improvement of Chronic Bleeding in the Patient with Unresectable Advanced Gastric Cancer Using the Decoction of Notoginseng Radix - a Case Report

  • Objectives: A 74-year-old male patient with unresectable advanced gastric cancer (clinical initial stage T3N+, Borrmann type III) admitted due to gastric bleeding at tumor site. On first admission day, hemoglobin level was 5.7g/dl and performance status was grade 3 according to Eastern Cooperative Oncology Group Performance Status(ECOG-PS). After performing red blood cell transfusion as an emergency treatment, hemoglobin level was increased up to 9.5g/dl. However, bleeding of oozing site was continued. For hemostasis, decoction of notoginseng radix (30g/day) was administered since day 7 after admission. The dose was elevated to 40g/day after hemoglobin level was decreased to 6.5g/dl on day 11. Since then, melena was stopped and hemoglobin level was maintained over 9.1g/dl. This case shows the hemostasis effect of decoction of notoginseng radix on gastric bleeding in unresectable advanced gastric cancer.
