Diet intervention to improve cardiovascular risk factors among Iranian postmenopausal women

  • Cardiovascular disease is a leading cause of death and disability and remains so in the future. The aim of this study was to detect the impact of a 6-month diet intervention on cardiovascular risk factors in postmenopausal Iranian women. It was a randomized controlled trial that carried out in the East Health Clinic in Ahvaz-Iran. This study started on June 2007 and was completed on May 2008. A total 64 healthy postmenopausal women recruited and randomly assigned to the intervention group (35) and control (29). Over the six months follow-up, the intervention group received five educational sessions (two face to face and three lecture discussion classes with slide demonstration) at the first month. These sessions were about menopause, cardiovascular disease and healthy diet. Every participant in the intervention group received one face to face education session at the $3^{rd}$ month, and also received a telephone call at the end of each month starting with the second month to remain on the diet. Pre-intervention and post-intervention anthropometric measurements, blood pressure, blood lipids and dietary intake were assessed. Data analyzed using the SPSS version 15. Descriptive (means and SD), univariate (paired-t test, Chi-Square and Independent T-test) and multivariate (GLM Repeated Measure) statistics were used. Participants in the diet group had significantly lower weight (-0.9 kg), body mass index (-0.4 $kg/m^2$), and fasting blood sugar (-4.5 mg/dl). The diet group significantly increased their daily intake of fiber (+2.3 g, P=0.05), decreased their intake of sodium (-28 mg, P=0.04), and consumption of fruit and vegetable ${\geq}$ 5 serving a day (80%, P=0.03) compared to the control group. Post menopausal women are at a greater risk for cardiovascular disease. Healthy diet using educational intervention can be an effective means of reducing cardiovascular risk in postmenopausal women.

무선인터넷에서 신용카드기반의 안전한 소액 지불 프로토콜 (A Secure Micro-Payment Protocol based on Credit Card in Wireless Internet)

  • 최근 정보통신기술의 급속한 발달로 무선인터넷을 이용한 전자상거래 사용자가 폭발적으로 증가하고 유선에서 유/무선 통합 환경으로 변화함에 따라 보안상의 많은 문제점이 제시되고 있다. 특히 무선전자상거래에서는 무선 환경의 제한적 특정에 따라 경랑화된 보안기술, 종단간 보안 기술 및 프라이버시 보안 등에 관한 연구가 활발하게 진행되고 있다. 현재 무선 전자상거래에서는 주로 신용카드기반의 지불 프로토콜인 WPP와 ASPeCT에서 제안한 인증과 지불초기화를 위한 AIP프로토콜을 사용하고 있다. WPP에서 사용하는 보안 프로토콜 WAP는 무선과 유선을 연계하는 G/W에서 전달되는 데이터의 모든 내용이 누출되는 보안상의 취약점이 있어 종단간 보안도 제공하지 못하는 단점이 있고, AlP 프로토콜은 인증서 체인을 이용하여 인증을 수행하므로 계산량이 많은 단점과 인증서에서 사용자의 신원이 노출되어 프라이버시 보호를 위한 익명성이 보장되지 않는 단점이 있다. 이 논문에서는 기존 AIP프로토콜을 기반으로 초특이 타원곡선인 Weil Pairing을 적용한 ID 기반 공개키 암호기법을 사용하여 거래정보의 기밀성을 보장하고 은닉전자서명 기법을 통한 인증서를 사용하여 프라이버시 보호, 공개키와 사용자 인증 및 부인방지를 해결했으며 또한 두 객체만 공유하는 세션키를 사용하여 종단간 보안이 제공되는 특정 무선 플fot폼에 독립적이며 안전하고 효율적인 지불 프로토콜을 제안하였다. 또한 제안한 프로토콜은 사용자와 서비스 제공자간의 온라인 인증기관이 지불 프로토콜의 인증과정에 참여함으로써 다른 도메인에 존재하는 서비스 제공자에게도 효율적이고 안전한 서비스를 받을 수 있도록 하였다.

대상회전 기타 피질이 손상된 흰쥐들의 행동 변화 (Behavioral Changes of Rats following Cingulate or Other Cortical Damages)

  • A study was planned to evaluate the effects of removal of the cingulate cortex upon the occurrence of any behavior commonly displayed by the rat, and to compare the effects of cingulectomy with those of removal of the parietal, parieto-occipital, or occipital regions. The subjects were 54 male albino rats (Holtzman strain, body weight $200{\sim}330\;gm$) including 14 rats in which the cingulate gyri between splenium and genu of the corpus callosum were bilaterally ablated by suction (cingulate group), 9 animals which had their parietal cortices (chiefly area 7) partially removed (parietal group), 9 rats whose parietal and occipital regions (chiefly areae 7 & 17), 13 animals in which the occipital cortices (chiefly area 17) were removed bilaterally (occipital group), and 9 normal rats (normal control group). Eighteen observation cages, each of which housed a subject and was provided with food and water ad lib., were arranged in 6 rows on a rack and the behavior of each subject was scanned by an observer at a distance of 1.5 m from the rack. The observer scanned the first and second rows 6 times in 1 min, then proceeded to the 3rd and 4th rows, scanning for another 1 min, and finally to the 5th and 6th rows. The speed of scanning was such that behavioral observations of all of the 18 rats were completed in 3 min, each subject receiving 6 observations. The scanning was repeated every 3 min for 18 min, which constituted one observation session and was followed by a 72 minutes' recess. The whole procedure was repeated through 24 hours so that a total of 576 behavioral observations were made on each subject in 16 observation sessions. Behaviors checked were sleeping, lying, lying and sniffing, standing, standing and sniffing, exploring, eating, drinking, grooming (included were washing, licking, and scratching), and others. Results obtained were as follows: 1. The cingulate group ate significantly more often than the normal control, the parietal, and the parieto-occipital groups. 2. Exploration was significantly less frequent in the cingulate group than in the normal control, the parietal, and the occipital groups. There was, in the case of the cingulate group, a significant negative correlation between the occurrence of eating and the exploratory activity. 3. The general activity, as judged from the value obtained by adding the occurrence of exploration, eating, drinking, grooming, and standing and sniffing, was significantly increased in the cingulate group compared with those of any other groups including the normal control. 4. Though statistically insignificant, the cingulate group slept least often among all the animal groups tested. 5. The parieto-occipital group tended to groom less, and the parietal group to eat less often than the normal control group did, but the difference was not significant. There were no significant differences among all the groups except the cingulate group as regards other behaviors analyzed. Based on the above results, it was inferred that the cingulate cortex exerts an inhibitory influence upon the occurrence of eating and general activity, while it tends to facilitate the occurrence of sleep.

Extra-phase Image Generation for Its Potential Use in Dose Evaluation for a Broad Range of Respiratory Motion

  • Background: Four-dimensional computed tomographic (4DCT) images are increasingly used in clinic with the growing need to account for the respiratory motion of the patient during radiation treatment. One of the reason s that makes the dose evaluation using 4DCT inaccurate is a change of the patient respiration during the treatment session, i.e., intrafractional uncertainty. Especially, when the amplitude of the patient respiration is greater than the respiration range during the 4DCT acquisition, such an organ motion from the larger respiration is difficult to be represented with the 4DCT. In this paper, the method to generate images expecting the organ motion from a respiration with extended amplitude was proposed and examined. Materials and Methods: We propose a method to generate extra-phase images from a given set of the 4DCT images using deformable image registration (DIR) and linear extrapolation. Deformation vector fields (DVF) are calculated from the given set of images, then extrapolated according to respiratory surrogate. The extra-phase images are generated by applying the extrapolated DVFs to the existing 4DCT images. The proposed method was tested with the 4DCT of a physical 4D phantom. Results and Discussion: The tumor position in the generated extra-phase image was in a good agreement with that in the gold-standard image which is separately acquired, using the same 4DCT machine, with a larger range of respiration. It was also found that we can generate the best quality extra-phase image by using the maximum inhalation phase (T0) and maximum exhalation phase (T50) images for extrapolation. Conclusion: In the present study, a method to construct extra-phase images that represent expanded respiratory motion of the patient has been proposed and tested. The movement of organs from a larger respiration amplitude can be predicted by the proposed method. We believe the method may be utilized for realistic simulation of radiation therapy.

영어 약모음 /ə/ 교수에 있어서 명시적 Form-Focused Instruction의 효과 연구 (The Effectiveness of Explicit Form-Focused Instruction in Teaching the Schwa /ə/)

  • 본 연구는 명시적 형태 중심 교수법(FFI)이 영어 약모음 /ə/을 교실 상황에서 EFL 학생에게 교수하는 데 있어 얼마나 효과적인지 조사하였다. 25명의 고등학교 여학생이 13명은 실험집단에 12명은 통제집단으로 나뉘어 참여하였다. 또한, 미국인 여성 한 명도 비교 기준점을 위한 음성자료를 제공하였다. 실험집단 참여자는 한 달 반 동안 연구자의 발음과 텍스트를 음성으로 변환해주는 인터넷 프로그램의 발음을 따라 하고, 개인별로 피드백을 받았다. 처치 전, 후 참여자들은 14개의 2음절 이상 다음절 실험단어와 그 단어가 포함된 문장을 읽었으며 읽은 문장은 음성자료로 녹음되었다. 자료 분석을 위해 대응 표본 t 검증과 비모수 Wilcoxon signed-rank 검증이 사용되었다. 연구 결과에 따르면 실험군 참여자들은 사전 실험보다 사후 실험에서 영어 약모음을 약 40% 짧게 조음하였다. 하지만 모음 조음 공간에서 혀의 위치를 나타내는 F1/F2 formant에서 실험 참여자의 F1/F2 formant 분포형태는 이 연구의 기준점인 539 Hz (F1) × 1797 Hz (F2)와 상이했다. 이 연구의 결과는 반복적인 따라 하기와 적절한 피드백을 제공하는 명시적인 형태 중심 교수법(FFI)이 영어 발음 교수에 일부 효과가 있다는 것을 보여 주었다.

율동적 운동 프로그램이 여성노인의 생리적 지수, 생활만족, Calcium, Phosphorous, Osteocalcin, Deoxypyridinoline에 미치는 영향 (The Effects of Rhythmic Exercise Program on Physiologic Variables, Life satisfaction, Calcium, Phosphorous, Osteocalcin, Deoxypyridinoline in the Elderly Women)

  • Recently, the number of the elderly has increased according to the improvement of socioeconomic status and the efficient medical care system. In spite of the development of medicine, the elderly suffers from the various health problems caused by weakness of musculoskeletal system, cardiopulmonary function and immunologic dysfunction. Regular rhythmic exercise program is known to be one of the effective tools to enhance the health condition in the elderly. However, there has been few studies to evaluate the comprehensive effects of rhythmic movement program on the elderly. This study was focused to evaluate the indices of cardiopulmonary function, life satisfaction, calcium, phosphorous, osteocalcin and deoxypyridinoline which are the essential factors of health problems in the elderly women. Twenty six subjects, aged between 68 and 72, who can do the ordinary activities and do not have cardiovascular dysfunction and mental disorder, participated in this study. They were divided into two groups: 13 in the experimental group and 13 in the control group. The experimental group participated in the rhythmic movement program at the welfare center located in G-city. The program were consisted of three sessions a week during 10 weeks. Each session had three parts: warming up(10 minutes), main exercise(40 minutes), finishing(10 minutes). Heart rate, blood pressure and peripheral arterial oxygen saturation were measured for the evaluation of cardiopulmonary function. Serum calcium, phosphorous, osteocalcin and urine deoxypyridinoline were measured as the indices of bony metabolism. Data were analyzed with mean, standard deviation, $x^2$-test, t-test, paired t-test using SPSS PC+ program. The results of this study were as follows. 1) Heart rate of the experimental group showed significant decrease following the rhythmic movement program. Peripheral arterial oxygen saturation of the experimental group showed significant increase following the program. 2) The degree of life satisfaction of the experimental group showed significant increase following the program. 3) Calcium showed significant decrease following the program but remained within normal range. There was no significant difference of phosphorous between two groups. 4) Osteocalcin, the index of bone formation, showed no significant change following the program, but significant increase in the experimental group comparing with the control group. 5) Deoxypyridinoline, the index of bone resorption, in urine of the experimental group showed significant decrease following the program. In conclusion, the rhythmic exercise program in the elderly showed the improvement of physiologic function and favorable effects on life satisfaction and bony metabolism. According to the above results, the regular rhythmic movement program can be strongly recommended for the improvement of health in the elderly women.

Analysis of Al-Saggaf et al's Three-factor User Authentication Scheme for TMIS

  • 본 논문에서는 Al-Saggaf 등이 제안한 TMIS(Telecare Medicine Information System)를 위한 사용자 인증 기법을 분석하였다. 2019년에 Al-Saggaf 등이 제안한 인증 기법은 생체정보를 이용한 인증 기법으로, Al-Saggaf 등은 그들의 인증 스킴이 매우 적은 계산 비용으로 다양한 공격에 대한 높은 비도를 보장한다고 주장하였다. 그러나 본 논문에서 Al-Saggaf 등의 인증 기법을 분석한 결과, Al-Saggaf 등의 인증 기법은 서버에서 사용자의 ID를 계산해내기 위해서 필요한 난수 s가 DB에서 누락되었고, 서버의 ID SID를 사용자에게 전달하는 방식이 부재하여 인증 기법의 설계 오류가 존재하였다. 또한 Al-Saggaf 등은 그들의 인증 기법이 다양한 공격에 안전하다고 주장하였으나, 로그인 요청 메시지와 스마트카드를 사용한 패스워드 추측 공격, 세션키 노출, 내부자 공격 등에 취약하였다. 또한 공격자는 추측한 패스워드를 사용하여, DB에 패스워드로 암호화하여 저장된 사용자의 생체정보까지 복호화할 수 있었다. 생체정보의 노출은 사용자의 개인정보에 대한 아주 심각한 침해이고, 이로 인하여 공격자는 사용자 가장 공격에 성공할 수 있다. 게다가 Al-Saggaf 등의 인증 스킴은 ID 추측 공격에도 취약하여, 그들이 주장했던 것과 달리 TMIS에서 중요한 사용자 익명성을 보장하지 못한다.

Effects of Aquatic Exercise on Upper Extremity Function and Postural Control During Reaching in Children With Cerebral Palsy

  • Background: Despite the fact that aquatic exercise is one of the most popular alternative treatment methods for children with cerebral palsy (CP), there are few research regarding its effectiveness. Objects: The purpose of this study was to examine the effects of aquatic exercise on upper extremity function and postural control during reaching in children with CP. Methods: Ten participants (eight males and two females; 4-10 years; Gross Motor Function Classification System levels II-IV) with spastic diplegia were recruited to this study. The aquatic exercise program consisted of four modified movements that were selected from the Halliwick 10-point program to enhance upper extremity and trunk movements. The participants attended treatment two times a week for 6 weeks, averaging 35 minutes each session. The Box and Block Test (BBT), transferring pennies in the Bruininks-Oseretsky Test (BOT), and pediatric reaching test (PRT) scores were used as clinical measures. Three-dimensional motion analysis system was used to collect and analyze kinematic data. Differences in BBT and BOT values among pre-treatment, post-treatment, and retention (after 3 weeks) were analyzed using a Friedman test. In addition, the PRT scores and variables (movement time, hand velocity, straightness ratio, and number of movement units) from the three-dimensional motion analysis were tested using a Wilcoxon signed-rank test. The significance level was established at p < 0.05. When the results appeared to be statistically significant, a post-hoc test for multiple comparisons was performed with the Wilcoxon signed-rank test. Results: All clinical measures, which included BBT, transferring pennies of BOT, and PRT, were significantly increased between pre-intervention and post-intervention scores and between pre-intervention and retention scores after treatment (p = 0.001). Three-dimensional motion analysis mostly were significantly improved after treatment (p = 0.001). Conclusion: Aquatic exercise may help to improve body function, activity, and participation in children with varying types of physical disabilities.

모바일 애드 혹 네트워크에서 패킷 버스팅을 이용한 혼잡 해결 및 성능향상 기법 (A Solution for Congestion and Performance Enhancement using Dynamic Packet Bursting in Mobile Ad Hoc Networks)

  • 모바일 애드 혹 네트워크상에서 DSR, AODV등 대부분의 on demand 라우팅 프로토콜들은 경로 탐색 과정에서 트래픽 로드를 고려하고 있지 않다. 최근 혼잡을 해결하고 트래픽 로트 밸런싱을 이루기 위해서 여러 알고리즘들이 제시되었으나 대부분 경로 탐색과정에서 단순히 대체 경로를 찾거나 혼잡이 발생된 노드를 회피하여 라우팅하는 기법들이었다. 본 논문에서는 이러한 이슈들에 대한 성능을 향상시키기 위해 혼잡이 발생된 노드에서 패킷 버스팅 기법을 사용하여 혼잡을 해결하고자 한다. 패킷 버스팅 기법은 IEEE 802.11e QoS 동작에서 소개되었으며 한번의 채널획득으로 여러 패킷을 보낼 수 있도록 한다. 이로써 혼잡이 발생한 노드는 버퍼링된 패킷을 신속하게 전송할 수 있으며, 병목현상을 막을 수 있다. 또한 정확하고 동적으로 혼잡상태를 결정하기 위하여 두 가지의 임계값을 정의한다. 하나는 인터페이스 큐길이이며, 다른 하나는 버퍼링 시간이다. 마지막으로 실험을 통하여 네트워크 트래픽이 많을 때 제안된 알고리즘이 기존의 일반적인 on demand 프로토콜보다 더 효율적이고 우수한 성능을 가짐을 보인다.

에비안스-함베르크 스트레칭이 아급성 뇌졸중 환자의 상지기능과 일상생활활동에 미치는 영향: 단일 사례연구 (The Effects of Evjenth-Hamberg Stretching on Upper-Extremity Function and Activities of Daily Living in Subacute Stroke Patients)

  • 목적 : 본 연구는 에비안스-함베르크 스트레칭이 아급성 뇌졸중 환자의 환측 상지기능과 일상생활활동에 미치는 영향에 대해 알아보고자 단일 사례연구를 실시하였다. 연구방법 : 본 연구의 대상자는 대구광역시에 소재한 K재활병원에서 입원치료를 받고 있는 만 39세 남성 뇌졸중 환자이다. 에비안스-함베르크 스트레칭을 4주간 주 5회 1일 1회 30분간 적용하여 중재 전후 상지기능과 일상생활활동을 비교하였다. 상지기능의 평가는 뇌졸중상지기능평가(manual function test; MFT), 운동활동측정표(motor activity log; MAL)를 사용하였고, 일상생활활동 평가는 기능적 독립 척도(functional independence measure; FIM)를 사용하였다. 결과 : 대상자는 중재 후 평가에서 MFT는 향상 없이 유지되었으며, MAL에서는 사용빈도와 움직임의 질에서 각각 3점씩 향상되었다. FIM은 73점에서 94점으로 21점이 향상되었다. 결론 : 본 연구의 결과를 통해 에비안스-함베르크 스트레칭이 아급성 뇌졸중 환자의 상지기능과 일상생활활동을 증진시키는데 효과가 있음을 알 수 있었으며, 기존의 작업치료와 에비안스-함베르크 스트레칭을 적절히 활용한다면 환자의 기능 회복에 많은 도움이 될 것이라 사료된다.
