Purpose: To determine the type and degree of effect that a hot footbath has on sleep quality and fatigue level in older Korean adults. Methods: A non-equivalent control group quasi-experimental design was used. Fifty participants from a long-term care facility in Kwangju, South Korea, were randomly selected and assigned to two groups: experimental group (27 participants) and control group (23 participants). The participants in the experimental group received hot footbaths in a temperature-controlled water tub of $42^{\circ}C$. They soaked their feet up to 20cm above the ankles for 30 minutes before going to bed for 3 nights. Actigraphy was used to measure their sleep patterns. The data were analyzed using the SAS program. Results: After the intervention, the total sleep satisfaction rate of the participants increased, while sleep latency and fatigue decreased significantly in the experimental group compared to the control group. Conclusion: The study results suggest that hot footbaths are beneficial for older Korean adults in enhancing sleep quality as well as reducing fatigue. Therefore, hot footbaths are recommended as a nursing intervention to improve sleep quality and to reduce fatigue in older Korean adults.
Purpose: This study was to determine the mediator or moderator role of social support in the relationship between stress and depression among family caregivers of older adults with dementia. Method: Sixty nine family caregivers were randomly selected from health care centers in P city and a face-to-face interview was conducted using questionnaires from January to May of 2002. Data were analyzed with descriptive statistics, Pearson correlation, and hierarchical multiple regression using SPSS program. Result: Family caregivers of older adults with higher dependency in ADLs and higher problematic behaviors, provided care to the older adults for a longer period of time, and perceived less social support reported higher depression. Social support showed mediating effects between stress and depression, while did not show moderating effects. Elderly dependency on ADLs and caregiving duration decreased perceived social support and decreased social support increased depression. Conclusion: To increase family and social support to the caregivers of more functionally impaired elderly, family education to increase emotional support and physical assistance to the caregivers and broader and flexible application of social support such as increasing accessibility to the elderly daycare service with lower price may prove beneficial.
Purpose: This study is to grasp factors influencing the quality of life in older adults after total knee replacement. Methods: This study was conducted with 165 older adults who had TKR at four orthopedic hospitals in D city. Data were analyzed using one-way ANOVA, independent t-test, Pearson's correlation, and stepwise multiple linear regression with SPSS 19.0 software. Results: Pain and depression were negatively correlated with range of motion, social support, while sense of coherence was positively correlated with quality of life. Sense of coherence (43%, ${\beta}=.40$), pain (8%, ${\beta}=-.30$), and depression (3%, ${\beta}=-.20$) on the Physical Component Summary in the quality of life have significant explanatory power of 54%. Sense of coherence (49%, ${\beta}=.44$), social support (6%, ${\beta}=.25$), and depression (3%, ${\beta}=-.22$) on the Mental Component Summary in the quality of life have significant explanatory power of 58%. Conclusion: This study suggests developing a program to improve the quality of life in older adults who had TKR, considering factors such as sense of coherence.
Purpose: The purpose of this study was to examine the relationships among perceived health status, exercise self-efficacy, social support, and exercise compliance and factors influencing exercise compliance in older adults in an area. Methods: The sample consisted of 154 older adults who attended a senior welfare center in D metropolitan city. Data were collected from the 25th to the 31th of January in 2012. Results: The mean score for perceived health status was 2.94, 911.69 for exercise self-efficacy, 46.99 for social support, and 6.83 for exercise compliance. The highest score on social support domains was emotional support, followed by self-esteem, material, and informational support. There were significant correlations between perceived health status and exercise self-efficacy, between perceived health status and exercise compliance, between exercise self-efficacy and social support, between exercise self-efficacy and exercise compliance, between emotional support and exercise compliance. Findings of multiple regression indicated that only exercise self-efficacy significantly explained exercise compliance. Conclusion: Health care providers may need to develop various intervention program to promote exercise self-efficacy in order to influence on exercise compliance and adherence among older adults.

Purpose : The purpose of this study was to determine risk factors and methods in balance assessment associated with fall in older adults. Methods : This article describes many of the tools that can be used to evaluate the physical parameters associated with fall risk in older adults. Results : Composite ratings of performance(Tinetti balance assessment, Guralnik test battery, Berg balance scale, modified-physical performance test) measures the score compounding the balance measure to determine fall risk. Static balance instruments are composed of FICSIT-4 that measures the ability of maintaining foot positions and CTSIB that measures postural stability. Dynamic balance instrument is composed of functional reach test. To measure walking velocity and mobility, 8-foot up-and-go test and walking around two cones are used. We can use 1-RM and to measure muscular strength, isokinetic dynamometery, and 30-second chair stand to measure lower extremity muscle strength. Conclusion : The described instruments are easy to use and widespread. To select and use these tool kits carefully is considered to be helpful in identifying those who are most likely to fall. The final part of the article includes a brief discussion of the potential role of exercise training interventions to improve these physical parameters and prevent falls.
Purpose: The purpose of this study was to identify the factors that influence medication adherence and self-care among low-income older adults with hypertension. Methods: A sample of 297 low-income older adults with hypertension was recruited from June 30 to July 30, 2010. Data collection was done using a face-to-face interview with structured questions. The data were analyzed using descriptive statistics, Pearson's correlation coefficient, and path analysis. Results: Subjective health status, duration of hypertension, number of drugs excluding antihypertensives, body mass index, knowledge about hypertension, sense of coherence, benefit, barrier, and self-efficacy were identified as significant predictors. Subjective health status and duration of hypertension, knowledge, depression, and self-care showed direct effects on medication adherence. Depression had the strongest direct influence on medication adherence. Body mass index, benefit, self-efficacy, and depression showed a direct effect on self-care. Sense of coherence was a strong predictor of depression which significantly influenced on medication adherence and self-care. Conclusion: For enhancing medication adherence and self-care, it is suggested that a psycho-education program reducing depression and increasing knowledge about hypertension should be provided into low-income older adults with hypertension.
Purpose: The purpose of this study was to examine the depression and cognitive function of community-dwelling older adults. Methods: 243 aged over 65 in Won-Ju in South Korea participated in this study. Data were collected from April to June 2013 through interviews. This study utilized the Geriatric Depression Scale Short Form, the Korean version of MMSE-DS (MMSE for Dementia Screening), and infirmity testing for basic health status. Descriptive statistics, ${\chi}^2$, t-test, ANOVA, $Scheff{\acute{e}}^{\prime}s$ test, and Pearson's correlation coefficient were used. Results: 75.3% of the participants belonged to the depression group, and the prevalence of cognitive impairment was 17.3%. There were significant differences in depression and cognitive function according to gender, educational level, type of insurance, subjective health, frailty, and whether to be a client of home care service. The level of depression was significantly higher and the level of cognitive functions was significantly lower in older adults in home care services than in community-dwelling older adults. Conclusion: This study is meaningful in that it included older adults in home care services to provide information to develop effective prevention programs for improving cognitive functions. The results of this study can be used to develop future community prevention and intervention.
Purpose: To compare the changes in activities of daily living (ADLs) in older adults with stroke in different modalities of long term care (LTC) services, which include home care and institutional care. Methods: This is a comparative study using secondary data from the Korean national LTC insurance. Home care (HC) services users (n=3,494) and institutional care (IC) users (n=1,428) were extracted and compared in terms of ADLs and changes in ADLs to investigate the effects of HC and IC services in LTC. Results: All of the ADLs and LTC services benefit levels for 2 years had improved in both HC and IC services. The ADLs of older adults with stroke who received HC improved, while those who received IC experienced deterioration. The LTC services benefit levels of the HC and IC groups were significantly different after 2 years. Conclusion: The study has found that HC services may lead ADLs better for older adults with stroke. We recommend LTC policy makers to further develop HC and IC service programs to deliver quality LTC services.
Purpose: The aim of this study was to compare the predictors of physical activity between urban and rural dwelling Korean older adults. Methods: This study was a cross-sectional descriptive survey. A self-report questionnaire or face to face interviews were used to collect data from 336 older adults (urban: 129, rural: 207) who visited public health centers or welfare centers in 2008. Results: About half of the participants (urban: 50.4%, rural: 47.3%) were classified as the minimally active group. Cognitive function (odds ratio [OR]=1.106, p=.004) and loneliness (OR=0.965, p=.044) were predicting factors for physical activity in rural elderly. Age (OR=0.326, p=.037), gender (OR=2.841, p=.021) and depression (OR= 0.799, p<.001) were significant factors predicting physical activity in urban elders. Conclusion: These findings provide information that is relevant in designing interventions to enhance physical activity in older adults. There is a need to develop effective mutifaceted physical activity interventions that include reducing psychological barriers such as depression, loneliness.

The purpose of this study was to investigate the effectiveness of elastic band resistance training for muscle strength among community-dwelling older adults. The systematic review and meta-analysis was conducted by following the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA). Data were pooled using fixed effect models. Sit to stand, arm curl, and grip strength were analyzed for main effects. Heterogeneity between studies was assessed using the I2 statistics and publication bias was evaluated by funnel plots. Twelves studies were included representing 611 participants. Elastic band resistance training was effective for lower (d=3.89, 95% CI: 3.03, 4.75) and upper extremity muscle strength (d=4.08, 95% CI: 2.94, 5.23). Heterogeneity was moderate and no significant publication bias was detected. Based on these findings, there is clear evidence that elastic band resistance training has significant positive effects on muscle strength among community-dwelling older adults. Further study will be needed to perform subgroup analysis using number of sessions and exercise intensity as predictors.
