Association of Type II Diabetes Mellitus with Hepatocellular Carcinoma Occurrence - a Case Control Study from Kathmandu Valley

  • Objective: To assess associations of Type II DM with hepatocellular carcinoma occurrence in Nepal. Materials and Methods: This case control study was carried out using data retrieved from the register maintained in the Department of Biochemistry of Nepalese Army Institute of Health Sciences between 1st January, 2012, and 31st August, 2012. The variables collected were age, gender, HbA1c. All biochemical parameters were analyzed in the Central Laboratory of our hospital by standard validated methods. One way ANOVA was used to examine the statistical significant difference between groups with the LSD post-hoc test for comparison of means of case groups. Odds ratios (OR) were calculated using simple logistic-regression analysis. Results: Etiological factors for HCC were HBV, HCV, alcohol and cryptogenic cirrhosis. The highest age group belonged to the etiological category of HCV with a mean of $71.9{\pm}3.6$ (CI 69.3, 74.5) years and the lowest age group to the etiological category of HBV with $61.7{\pm}5.3$(CI 57.9, 65.5) years. The main imperative basis of HCC in present study was HCV (39.5%) and second most significant cause of HCC was alcohol (26%). Glycated hemoglobin was found to be more in males with HCC (7.9%) as compared to females (7.3%). The percentage of Type II diabetes mellitus was greater in HCC patients when compared to controls. This difference was statistically significant with an odd ratio of 4.63 (p<0.001). Conclusion: Type II DM influences incidence, risk of recurrence, overall survival, and treatment-related complications in HCC patients.

Number of existing permanent teeth is associated with chronic kidney disease in the elderly Korean population

  • Background/Aims: The aim of this study was to assess the association between the number of existing permanent teeth and chronic kidney disease (CKD) in a representative sample of the elderly Korean population. Methods: A total of 2,519 subjects who participated in the Korean National Health and Nutrition Examination Survey were cross-sectionally examined. The number of existing permanent teeth was evaluated by clinical oral examination. CKD was defined based on definition and classification by Kidney Disease: Improving Global Outcomes (KDIGO) 2012 guidelines. Multivariable logistic regression analyses were performed controlling for age, gender, income, education, tooth-brushing frequency, periodontitis, state of dentition, smoking, alcohol consumption, hypertension, obesity, diabetes mellitus, and hypercholesterolemia. Subgroup analyses by age and gender were also performed. Results: The number of teeth was significantly associated with CKD after controlling for all potential confounders (adjusted odds ratio [AOR], 1.67; 95% confidence interval [CI], 1.04 to 2.70 for lower number of teeth; AOR, 1.59; 95% CI, 1.14 to 2.23 for moderate number of teeth). In the subgroup analyses, the association was highlighted in females aged 75 years over (AOR, 2.55; 95% CI, 1.05 to 6.20 for lower number of teeth; AOR, 1.95; 95% CI, 1.01 to 3.80 for moderate number of teeth). Conclusions: Our findings suggest that the number of existing permanent teeth may be associated with CKD among Korean elderly.

Functional Movement Screen as a Predictor of Occupational Injury Among Denver Firefighters

  • Background: The Functional Movement Screen (FMSTM) is a screening tool used to assess an individual's ability to perform fundamental movements that are necessary to do physically active tasks. The purpose of this study was to assess the ability of FMS to predict occupational injury among Denver Fire Department firefighters. Method: FMS tests were administered from 2012 to 2016. Claim status was defined as any claim occurrence vs. no claim and an overexertion vs. no claim/other claim within 1 year of the FMS. To assess associations between FMS score and claim status, FMS scores were dichotomized into ≤ 14 and > 14. Age-adjusted odds ratios were calculated using logistic regression. Sensitivities and specificities of FMS predicting claims at various FMS score cut points, ranging from 10 to 20 were tested. Results: Of 581 firefighters (mean ± SD, age 38 ± 9.8 y) who completed FMS between February 2015 and March 2018, 188 (32.4%) filed a WC claim in the study time frame. Seventy-two of those (38.3%) were categorized as overexertion claims. There was no association between FMS score and claim status [odds ratio (OR) = 1.27, 95% confidence interval (CI): 0.88 - 1.83] and overexertion claim vs. no claim/other claim (OR = 1.33, 95% CI: 0.81 - 2.21). There was no optimal cutoff for FMS in predicting a WC claim. Conclusions: Although the FMS has been predictive of injuries in other populations, among this sample of firefighters, it was not predictive of a future WC claim.

Association between Diabetes Mellitus and Fatty Liver Based on Ultrasonography Screening in the World's Highest Cholangiocarcinoma Incidence Region, Northeast Thailand

  • Fatty liver disease (FLD) can be a precondition for other liver pathology including cholangiocarcinoma (CCA). Diabetes mellitus (DM) has been suggested in some studies to be a risk factor for FLD as well as cancers, including cholangiocellular carcinoma; however, there are currently very few studies on FLD in DM subjects, although the rate of FLD continues to increase annually. To determine the association between DM and FLD ultrasonographic data were analyzed from the Cholangiocarcinoma Screening and Care Program (CASCAP), in northeast Thailand. DM was reported by the subjects based on the CASCAP health questionnaire. Factors that were associated with FLD were determined by prevalence, odds ratio (ORs) and its 95% confidence intervals (CIs) using multiple logistic regression. There were 45,263 subjects with a mean age of 53.46 (${\pm}9.25$) years. FLD was found in 36.3% of DM subjects but only in 20.7% of non-DM subjects. The association between DM and FLD was adjusted for all other factors including gender, age, education level, relatives diagnosed with CCA, smoking, alcohol consumption, and hepatitis B and C. The risk of DM in subjects having FLD was highly significant compared with the non-DM subjects (OR 2.13; 95%CI: 1.92 to 2.35; p-value < 0.001). Thus DM is significantly associated with FLD which in turn may facilitate the development of several diseases including CCA. DM should be taken into consideration in future ultrasonic investigations of FLD and CCA.

한국 성인의 육가공품 섭취 빈도와 대사증후군과의 관련성: 2007~2008년 국민건강영양조사 자료 분석 (The Association between Consumption of Processed Meat and Prevalence of Metabolic Syndrome Among Korean Adults: Based on 2007-2008 Korean National Health and Nutrition Examination Survey)

  • Recent studies have shown that high consumption of processed meat may be associated with increasing risk of metabolic syndrome, which have been suggested as a predictor of diabetes and cardiovascular disease. However, limited studies have investigated this association in Korean population. The purpose of this study was to investigate the cross-sectional association between processed meat/unprocessed (beef, pork, chicken) intakes and the prevalence of metabolic syndrome. Using data from 2007-2008 Korean National Health and Nutrition Examination Survey (KNHANES), we analyzed data including 5,545 men and women who were aged older than 20 years, and who were free of chronic disease such as hypertension, type 2 diabetes and cardiovascular disease. Subjects who frequently consumed processed meat tended to be younger and more likely to be current smokers. In addition, men were more likely to consume processed meat than women. Although higher processed/unprocessed meat intakes were significantly associated with the lower risk of metabolic syndrome in a crude model, these associations were no longer significant after adjustment for potential confounding factors. For example, comparing subjects in the highest intake quartile of processed meat with the lowest intake group, the multivariate-adjusted odds ratio (OR) of metabolic syndrome was 0.92 (95% CI: 0.58-1.46) for processed meat, 1.09 (95% CI: 0.76-1.56) for beef, 1.10 (95% CI: 0.74-1.62) for pork and 0.75 (95% CI: 0.51-1.12) for chicken. In conclusion, we found no evidence of any adverse effects of frequent processed or unprocessed meat intakes on the prevalence of metabolic syndrome among Korean adults at the exposure levels seen in this study.

Predictive Factors for Recurrence after Burr-Hole Craniostomy of Chronic Subdural Hematoma

  • Objective : Chronic subdural hematoma is a common and relatively benign disease. However, recurrence is common after surgical treatment, and the recurrence rate varies from 5% to 33%. The aim of this study was to investigate the predictive factors for recurrence of chronic subdural hematoma. Methods : We analyzed data from 248 patients with chronic subdural hematoma who were treated by burr-hole craniostomy with a closed drainage system for hematoma evacuation in this five-year retrospective study. Results : Thirty-one (12.6%) patients underwent re-operation for recurrence of chronic subdural hematoma. Univariate analysis revealed that anticoagulation (p=0.0279), headache (p=0.0323), and preoperative midline shifting (p=0.0321) showed significant differences with respect to recurrent chronic subdural hematoma. We performed a multivariate logistic regression analysis and found that diabetes mellitus (odds ratio [OR], 2.618; 95% confidence interval [CI], 1.0899-6.2898; p=0.0314), anticoagulation (OR, 6.739; 95% CI, 1.1287-40.2369; p=0.0364), headache (OR, 2.951; 95% CI, 1.1464-7.5964; p=0.0249), and preoperative midline shifting (OR, 1.0838; 95% CI, 1.0040-1.1699; p=0.0391) were independent predictive factors for recurrence of chronic subdural hematoma. Conclusion : We showed that diabetes mellitus, anticoagulation, headache, and preoperative midline shifting were independent predictors of recurrence of chronic subdural hematoma.

40세 이상 성인 남녀의 대사증후군 관련 심리사회적 요인 (Psychosocial Factors Associated with Metabolic Syndrome among Korean Men and Women Aged over 40 Years)

  • Objectives: This study identified biopsychosocial factors associated with metabolic syndrome among Korean men and women aged over 40 years. Methods: Secondary data from the 2010-2016 Korean National Health and Nutrition Examination Survey were used for this study. Based on the biopsychosocial model, psychosocial factors (stress, depression, smoking, binge alcohol consumption, physical activity) were assessed with control of biomedical (age, body mass index, family history of hypertension, dyslipidemia, type 2 diabetes mellitus, and cardiovascular disease) and biosocial factors (educational level and economic status). Data from 8,624 men and 7,321 women were analyzed by logistic regression analysis using a complex sample procedure. Results: Among men, current or past smoking habits (Adjusted Odds Ratio [AOR]: 1.349; 95% Confidence Interval [CI]: 1.155-1.575, p<.001) and binge alcohol consumption (AOR: 1.570, CI: 1.389-1.774, p<.001) were associated with metabolic syndrome. In addition, moderate (AOR: 1.205, CI: 1.038-1.400, p=.014) and low levels of physical activity (AOR: 1.296, CI: 1.109-1.514, p=.001) were associated with metabolic syndrome. Among women, low level of physical activity (AOR: 1.276, CI: 1.017-1.602, p=.036) was associated with metabolic syndrome. Conclusion: Gender specific interventions such as encouraging physical activity and prevention of smoking and excessive alcohol drinking are important to prevention of metabolic syndrome.

혈청 Gamma-Glutamyltransferase에 따른 복부비만과 제2형 당뇨병간의 관련성: Korean Genome and Epidemiology Study (The Association of Central Obesity with Type 2 Diabetes among Koreans according to the Serum Gamma-Glutamyltransferase Level: Korean Genome and Epidemiology Study)

  • Objectives : This cross-sectional study was performed to examine if the serum gamma-glutamyltransferase (GGT) level that is within its normal range is associated with the risk of type 2 diabetes and if the association between the waist hip ratio (WHR) and type 2 diabetes is different depending on the serum GGT levels. Methods : The study subjects were 23,436 persons aged 40 years or older and who participated in regular health check-ups at 11 hospitals (males: 5,821, females: 17,615). The gender-specific quintiles of the serum GGT and WHR were used to examine the associations with type 2 diabetes. Results : The serum GGT levels within their normal range were positively associated with type 2 diabetes only in women. The adjusted odds ratios (ORs) were 1.0, 1.0, 1.4, 2.1, and 2.5 according to the quintiles of the serum GGT ($p_{trend}$<0.01). The WHR was more strongly associated with the prevalence of diabetes among the women with a high-normal serum GGT level as compared with those with a low-normal serum GGT level (p for interaction=0.02). For example, the adjusted ORs for women with a low normal serum GGT level were 1.0, 1.2, 1.5, 2.2, and 2.4 according to the quintiles of the WHR, while those figures were 1.0, 2.4, 3.6, 5.0, and 8.3 among the women with a high normal serum GGT level. However, in men, the serum GGT was very weakly associated with type 2 diabetes and the association between the WHR and type 2 diabetes was not different depending on the serum GGT level. Conclusions : Serum GGT within its normal range was positively associated with type 2 diabetes, and central obesity was more strongly associated with the prevalence of type 2 diabetes when the serum GGT level was highnormal. However, these associations were observed only in women, which is different from the previous findings. The stronger relation between central obesity and type 2 diabetes among women with a high-normal serum GGT level can be useful for selecting a group that is at high risk for type 2 diabetes irregardless of whatever the underlying mechanism is.

초회다제내성 결핵의 위험요인 (Risk Factors for Primary Multidrug Resistant Tuberculosis)

  • 목 적 : 결핵약으로 치료 받은 적이 없으면서 최소한 아이소니아지드와 리팜핀에 동시에 내성이 있는 결핵균에 감염된 결핵을 초회다제내성 결핵이라 하며 이는 다제내성결핵균에 감염되어 발병한다. 근래에 초회내성결핵은 결핵관리 프로그램의 수행에 있어 중요한 지표로 사용되고 있다. 저자들은 초회다제내성 결핵의 위험요인을 알아내기 위해 환자대조군 연구를 시행하였다. 방 법 : 2001년 1월 1일부터 2003년 6월 30일 동안 국립마산병원에 입원한 29명의 초회다제내성결핵 환자들을 대상으로 환자군을 설정하였고, 대조군은 같은 기간 동안 본원에 입원한 모든 약제에 감성인 결핵환자들을 대상으로 하였다. 초회다제내성 결핵에 대한 의심되는 위험요인들의 교차비를 계산하였다. 결 과 : 다변량로지스틱회귀분석 결과 당뇨병이 초회다제내성 결핵과 통계적으로 유의한 연관성이 있었다(교차비 2.68; 95% 신뢰구간 1.05-6.86). 결 론 : 초회다제내성 결핵의 위험인자로서 당뇨병을 의심할 수 있었으며 향후 추구 연구가 요구된다.

대한민국 성인에서 알부민뇨와 요산 대 고밀도 지단백 콜레스테롤 비율의 관련성 (Relationship between Albuminuria and Uric Acid to High-Density Lipoprotein Cholesterol Ratio in Korean Adults)

  • 본 연구는 한국 성인에서 요산 대 고밀도 지단백 콜레스테롤(uric acid to high density lipoprotein cholesterol, UA/HDL-C) 비율과 요 중 마이크로알부민 대 크레아티닌 비율(uACR)의 연관성을 평가하기 위해 수행되었다. 2019년 국민건강영양조사 자료를 이용하여 20세 이상 성인 5,845명을 대상으로 연구를 수행하였다. 본 연구에서 몇 가지 중요한 발견이 있었다. 첫째, 관련 변수(연령, 성별, 고혈압, 비만, 제2형 당뇨병 및 만성 신장 질환)를 조정한 후, UA/HDL-C ratio의 사분위수가 증가함에 따라 uACR수준이 증가하였다(P=0.002). 둘째, 관련 변수를 조정한 후, UA/HDL-C ratio의 사분위수 1 (Q1)과 비교하여 알부민뇨(uACR≥30.0 mg/g)의 odd ratio (OR)은 Q2(OR, 1.015; 95% confidence interval [CI], 0.757~1.361)와 Q3 (OR, 1.090; 95% CI, 0.811~1.465)에서는 유의하지 않았다. 그러나 Q4 (OR, 1.416; 95% CI, 1.044~1.921)에서 알부민뇨의 발생률은 Q1에 비하여 유의하게 높았다. 한국 성인에서 UA/HDL-C ratio는 알부민뇨와 양의 상관관계가 있었고, UA/HDL-C ratio는 대한민국 성인에서 알부민뇨를 평가하는 유용한 지표로 사용될 수 있을 것으로 사료된다.
