Sleep disorder in chronic obstructive pulmonary disease (COPD) is common and typically is associated with oxygen desaturation. The mechanisms of desaturation include hypoventilation and ventilation to perfusion mismatch. Despite the importance of sleep in patients with COPD, this topic is under-assessed in clinical practice. Impaired sleep quality is associated with more severe COPD and may contribute to worse clinical outcomes. Recent data have indicated that specific respiratory management of patients with COPD and sleep disordered breathing improves clinical outcomes. Clinicians managing patients with COPD should pay attention to and actively manage symptoms of comorbid sleep disorders. Management of sleep-related problems in COPD should particularly focus on minimizing sleep disturbance.
Obstructive sleep apnea (OSA) is a common sleep-breathing disorder associated with significant comorbidities and perioperative complications. This narrative review is aimed at comprehensively overviewing preoperative risk evaluation and perioperative management strategies for patients with OSA. OSA is characterized by recurrent episodes of upper airway obstruction during sleep leading to hypoxemia and arousal. Anatomical features, such as upper airway narrowing and obesity, contribute to the development of OSA. OSA can be diagnosed based on polysomnography findings, and positive airway pressure therapy is the mainstay of treatment. However, alternative therapies, such as oral appliances or upper airway surgery, can be considered for patients with intolerance. Patients with OSA face perioperative challenges due to difficult airway management, comorbidities, and effects of sedatives and analgesics. Anatomical changes, reduced upper airway muscle tone, and obesity increase the risks of airway obstruction, and difficulties in intubation and mask ventilation. OSA-related comorbidities, such as cardiovascular and respiratory disorders, further increase perioperative risks. Sedatives and opioids can exacerbate respiratory depression and compromise airway patency. Therefore, careful consideration of alternative pain management options is necessary. Although the association between OSA and postoperative mortality remains controversial, concerns exist regarding adverse outcomes in patients with OSA. Understanding the pathophysiology of OSA, implementing appropriate preoperative evaluations, and tailoring perioperative management strategies are vital to ensure patient safety and optimize surgical outcomes.
Sleep related breathing disorders(SRBDs) are a group of diseases accompanied by difficulties in respiration and ventilation during sleep. Central sleep apnea, obstructive sleep apnea(OSA), sleep-related hypoventilation, and hypoxemia disorder are included in this disease entity. OSA is known to be the most common SRBDs and studies show its significant correlation with general health problems including hypertension, arrhythmia, diabetes, and metabolic syndrome. The diagnostic process of OSA is composed of physical examinations of the head and neck area and also the oral cavity. Radiologic studies including cephalography, CT, MRI, and fluoroscopy assist in identifying the site of obstruction. However, polysomnography(PSG) is still considered the gold standard for the diagnosis of OSA since it offers both qualitative and quantitative recording of the events during a whole night's sleep. The dentist who is trained in sleep medicine can easily identify patients with the risk of OSA starting from simple questions and screening questionnaires. Diagnosis is the first step to treatment and considering the high rate of under-diagnosis for OSA the dentist may play a substantial role in the diagnosis and treatment of OSA which will eventually lead to the well-being of the patient as a whole person. So the objective of this article is to assist dental professionals in gaining knowledge and insight of the diagnostic measures for OSA including PSG.
Purpose : Smoking is a major factor in chronic obstructive pulmonary disease (COPD), but the effect of electrical cigarette smoking on COPD development is still uncertain. This study aimed to compare the functions of airways and lungs exposed to combustible cigarettes and electrical cigarettes based on the pulmonary function test (PFT) results from the Korean National Health and Nutrition Examination Survey (NHANES). Methods : This study used data from 8,942 participants with PFT results out of 47,309 total subjects from the 6th to 8th Korean NHANES (2014-2015, 2016-2018, and 2019, respectively). Individuals with diseases such as cancer, ex-smokers, and dual tobacco users were excluded. The PFT results were analyzed according to the COPD diagnostic criteria. After adjusting for confounding variables, a complex sample generalized linear model ANOVA test was performed to investigate the association between PFT results and combustible smoker or electrical cigarette user groups. Results : In an analysis based on the obstructive ventilatory disorders (forced expiratory volume in 1 second[FEV1]/forced vital capacity[FVC]<.7), combustible cigarette smokers showed a 3.46 times higher risk of COPD compared to non-smokers, while electrical cigarette smokers exhibited no significant difference in terms of COPD-related risks compared to non-smokers. FEV1 showed a negative relation with combustible cigarette smokers as reported elsewhere (B=-.07, p<.001). FEV1/FVC was negatively related to both combustible cigarette smokers (B=-.03, p<.001) and electrical cigarette smokers (B=-.02, p<.001). Conclusion : FEV1/FVC decreases were observed in the long-term exposure to both combustible and electrical cigarettes. The lower FEV1 in the combustible cigarette group implies the worsening of the severity of COPD, suggesting more damage to the airways and lungs in the short term. Therefore, the temporary electrical cigarettes use for the transition period in order to smoking cessation potentially aids to reduce the harmful effect of combustible cigarettes in COPD development.
저자는 상기도부의 동적변화를 관찰하여 수면시의 협착부위와 그 정도를 확인하며, 하악의 전방이동이 상기도부에 미치는 영향과 부위를 조사하고자 하였으며 총 9명의 건강한 피험자를 대상으로 임상적 검사, 방사선학적 검사 및 간이수면다원검사를 실시하여 코골이 및 수면무호흡증 등의 수면장애가 없음을 확인한다. 각성시 및 수면시, 하악의 안정위 및 전방이동시에서 각각 전자선 단층촬영(EBT)을 시행하여 각 조건하의 구인두의 부위별 최대 최소 단면적 및 허탈지수를 구하였다. 이때 수면의 유도를 위해 Dormicum$^{(R)}$을 정맥투여하였다. 그 결과, 각 조건에 따라 비교하였을때 수면 및 하악 전돌에 따른 상부, 중간부, 하부 상기도간의 최소 단면적 및 허탈지수의 유의한 차이는 없었다. 반면 하악 안정위에서 각성 및 수면상태간의 비교시에 구인두의 하부에서 단면적의 유의성 있는 차이가 관찰되었다. 한편, 각성상태에서 하악 안정시와 전돌시간의 비교시에는 중간부에서 단면적의 유의성 있는 차이과 관찰되며 수면상태에서는 하악 안정시와 전돌시 단면적의 변화율을 나타내는 허탈지수에 있어 유의한 차이는 없었다.
연구배경: 본원을 방문한 만성 폐성심 환자를 대상으로 만성폐성심의 원인 질환과 생존율 및 예후 인자에 대해 밝히고자 하였다. 방 법: 1990년 1월부터 1999년 3월까지 부산대학교 병원을 방문한 만성 폐성심 환자를 대상으로 병력지의 검토를 통해 후향적 조사가 이루어졌으며 만성 폐성심의 원인이 될 수 있는 기저 폐질환을 확인하고, 임상적으로 급성 폐성심이 의심되는 경우는 조사대상에서 제외하였다. 만성 폐성심의 진단은 일차적으로 심초음파 검사를 시행하여 우심실의 확장 또는 비대가 관찰되는 경우로 하였다. 그와 내원 당시의 임상 증상, 진찰 소견 및 각종 검사 수치를 정리 분석하였다. 결 과: 대상 환자는 총 103명이었고 남녀비는 약 1.9:1로 남자에서 많았다. 연령은 평균 $55.8{\pm}14.2$세였으며, 남자 $57.9{\pm}14.0$세, 여자 $52.0{\pm}14.1$세로 남자에서 연령이 높았다. 원인 질환으로는 폐결핵이 61명(59.2%)으로 가장 많았고, 만성폐쇄성 폐질환이 29명(28.2%)으로 다음을 차지하였다. 생존율은 1년 생존율 57%, 2년 생존율 45%, 3년 생존율 34%였다. 생존율에 미치는 변수들에 대해서는 단변량 분석 결과 최대 자발성 호흡량, 노력성 폐활량, 1초간 노력성 호기량, 혈청 나트륨, 폐활량, 혈청 알부민, 최대 호기량 등이 통계적으로 유의하였고, 다변량 분석을 통해서는 혈청 알부민(p=0.0144) 과 폐활량(p=0.0078)만이 통계적인 유의성을 가지는 변수들로 판명되었다. 결 론: 본 연구를 통해 만성 폐성심의 원인 질환으로서 폐결핵이 가장 중요하며, 만성 폐성심의 생존율은 1년 생존율 57%, 2년 생존율 45%, 3년 생존율 34%였다. 혈청 알부민(p=0.0144)과 폐활량(p=0.0078)이 통계적으로 의미있게 생존율에 영향을 미치는 변수들로 판명되었다.
연구배경 : 만성폐쇄성폐질환과 기관지 천식은 만성 기도 염증과 기류 장애를 특징으로 하고 있으며, 일부에서는 합병되어 나타나기도 한다고 알려져 있으나 그 정도에 대한 연구는 미미한 실정이다. 저자는 두 질환의 병태 생리의 차이와 합병 정도를 평가하기 위하여 본 연구를 시행하였다. 방 법 : 만성폐쇄성폐질환과 기관지 천식의 진단은 미국 흉부학회의 지침서 기준에 의하였고, 대상 환자들에게는 폐기능 검사, 최대호기유속 측정, 속효성 ${\beta}_2$-항진제에 대한 기관지 반응성, methacholine 유발 반응 검사, 객담내 호산구치 및 ECP, 혈청내 총 IgE치, 알레르기 피부반응 검사 등을 시행하였다. 결 과 : 대상 환자 59명중 남자가 49명, 평균 연령은 66.46세, 흡연자가 42명이었다. 속효성 ${\beta}_2$-항진제에 대한 양성 반응을 보인 경우는 37.3%였으며, 최대 호기유속 측정상 일중 변이성을 보인 경우는 47.5%이었고, 두 가지 검사에 모두 양성을 보인 경우는 27.1%이었다. 만성폐쇄성폐질환과 기관지 천식이 합병된 환자의 유도객담은 호산구와 ECP농도의 유의한 상승을 보였다. 그러나 기관지 천식이 합병되지 않은 만성폐쇄성폐질환 환자의 유도 객담에서는 중성구의 유의한 상승을 관찰할 수 있었다. 또한 중증의 만성폐쇄성폐질환 환자에서 최대호기유속의 변화율은 현저히 감소하였지만 천식의 합병과 만성폐쇄성폐질환의 중증도간에는 상관관계를 보이지 않았다. 결 론 : 이상의 결과를 요약하면 중등도와 중증의 환기장애를 동반한 만성폐쇄성폐질환 환자의 약 27% 정도에서 기관지 천식이 동반되었으나, 환기장애의 중증도와 기관지 천식의 합병률 간에는 유의한 차이가 없었다.
