종합병원 소아청소년과 내원 초등학생의 비만도와 관련 요인 분석 (Obesity Index and Related Factors among Elementary School Students Visiting Pediatric Department of General Hospital)

  • The present study was conducted to investigate dietary habits and biochemical indices according to the obesity index of elementary school students visiting a hospital for diagnosis. The subjects of this study were 150 elementary school students and their mothers. The overweight rates of child subjects were 37.5% for males and 40.9% for females, whereas the rates of underweight children were 35.0% for males and 14.5% for females. Children's obesity index significantly decreased with an increase in the average monthly income of children's families. Children's obesity index, mothers' weights, BMI, and percent ideal body weight were all positively correlated. Blood biochemical values in children (hemoglobin, hematocrit, albumin, globulin, glucose, total cholesterol, and triglyceride levels) and their obesity index also showed positive correlation. For the underweight group, the frequency of iron-deficiency anemia was significantly high, whereas for the overweight group, the frequency of dyslipidemia was high. There was a significant increase in children's obesity index and a decrease in frequency of regular exercise with time spent on the TV/computer. The normal weight group recorded the highest total scores for mothers' nutritional knowledge, whereas the overweight group recorded the lowest total scores. Therefore, to maintain healthy weight in children, nutritional education programs for mothers and children should be developed.

비만아동과 그 부모에 대한 영양교육 효과의 평가(II)-영양지식, 식행동, 식습관과 영양소섭취를상태를 중심으로- (Effect of Nutrition Education Program in Obese Children and Their Parents(II) - Focus on Nutrition Knowledge, Eating Behaviors, Food Habit and Nutrient Intakes -)

  • The present study was conducted to investigate the effect of nutrition education program on nutrition knowledge, eating behaviors, food habit, nutrient intakes in obese children and their parents who live in Gumi city. The subjects were 16 obese boys, 20 obese girls with obesity index over 130% and 36 of their parents. The nutrition education for the subjects was carried out by the professional personnel such as doctors, professors, and dietitians. The children were provided with well-balanced lunch meals and had 40 min-lectures on the reasonable weight management, 40 min-games and also had regular exercises (stretching, swimming) for 90 mins everyday during 2 weeks of the program. The parents had 90 min-lectures on childhood obesity, diet therapy, behavior modification, and exercise for 6 times. The nutrition knowledge, eating behaviors and food habits were surveyed by using questionnaires before and after the education. The nutrition intakes of the children were surveyed before and after the education by 3-day food record method. The nutrition intakes of the parents were surveyed before and after the education by using semi-quantity questionnaires. After the education, the mean nutrition knowledge scores were significantly improved compared with the pretest scores in both children and parents. After the education, all subjects' eating behaviors were significantly changed positively and calorie and carbohydrates intakes were significantly decreased and vit. C was significantly increased in obese children. The food habits of the parents were significantly improved after the education. These findings show that the well-designed nutrition education program for obese children and their parents can be an effective approach to help them to improve their nutrition knowledge and to establish desirable food habits and eating behaviors.

A Study on Weight Control Program for 4th and 5th Grade Obese Children in Elementary School

  • This study examined the proper roles of dietitian and nurse-teacher in the weight control program (WCP) in schools and the effect of the WCP on subjects with respect to anthropometric measurements, nutrition knowledge, dietary attiude, and behavior changes. The program consisted of six sessions of nutrition education and frequent face-to-face nutrition counseling. Subjects were 22 obese children in the 4th and 5th grade who underwent counseling and 18 obese children in another school who served as a control group. After two months of WCP, obesity index such as .elative body weight (RBW, from 135.7 to 132.5), tricep skinfold thickness (TSFT, from 34.9 to 32.8 mm), and body fat content (from 32.0 to 29.8%) had decreased significantly in the experimental group, while the control group showed no significant differences in these indices. The reductions in obesity indices were maintained in the experimental group except for fat content (32.6%), which returned to its original value within six months. The control group significantly increased fat content in the same period (from 31.2% to 36.2%). Both groups decreased RBW, TSFT, and fat content while midarm circumference and waist/hip ratio remained the same after one year. Subjects' nutrition knowledge was improved with average test scores from 15.1 to 16.7 while nutrition attitude and behavior test scores remained unchanged suggesting that behavior modification may require more time than knowledge acquirement. These results suggest that proper nutrition counseling can initiate weight reduction. However, the maintenance of controlled weight requires changes in attitude and behavior which have not been achieved by the present WCP. The role of school dietitian for WCP in this study was limited to assisting the nurse-teacher in nutrition education. Expansion of dietitian's role in nutrition education and counseling is needed.

체중조절 프로그램에 참여한 성인여성의 비만도 및 영양지식 수준의 변화 (Changes in BMI and Nutrition Knowledge Levels of Korean Adult Women in Body Weight Control Program)

  • This study was conducted to investigate the effectiveness of a nutrition and exercise education program on weight control and nutrition knowledge. The nutrition-exercise educational period was 12 weeks. The subjects were 160 adult women with an average age of 48.1 years. We investigated general characteristics, diet history, eating habits, and intake frequencies of six food groups by survey. A total of 94 women among the subjects participated in the pre and post nutrition knowledge evaluations and anthropometric data assessment. After the program, body weight, body mass index (BMI), and waist circumference significantly decreased. Further, % of body fat decreased, but not significantly. BMI significantly decreased (p<0.001) from 25.14 to $24.80kg/m^2$, and waist circumference significantly decreased (p<0.001) from 79.72 to 77.63 cm. The nutrition knowledge scores of subjects significantly increased (p<0.001) from 8.01 to 9.03. However, there was little change in employment. These results suggest that the nutrition-exercise education program may improve obesity index and nutrition knowledge. However, it is necessary to develop a specialized weight control program for workers.

학령기 비만아동을 위한 체중조절 프로그램의 실시 및 효과평가 (A Study on Effects of Weight Control Program in Obese Children)

  • The enforcement and its effect of weight control program including nutrition education, exercise and behavioral modification therapy during 20 weeks were studied in primary school obese children. The sample consisted of treatment group(n=19) and control group (n=23). To evaluate the effects of weight control program, treatment group took pretest and posttest for nutrition knowledge, attitude and food habits. Nutrient intakes, serum lipid levels and anthropometric values were also measured. After weight control program, body weight of treatment group significantly decreased from 47.0$\pm$9.0kg to 45.9$\pm$8.5kg(p<0.001), but than of control group increased from 47.8$\pm$8.1kg to 49.0$\pm$8.7kg (p$\leq$0.05). Obesity index showed decline of 8.6% for treatment group and gain of 0.7% for control group. After self-control period, body weight and relate index in both groups were increased. For the nutrition knowledge test results, the posttest mean scores of treatment group showed the significant increase(p<0.001). For the attitude toward nutrition and food habit results, the posttest mean scores increased significantly (p<0.05, p<0.01). There was a decline of 28% in serum triglycerides, whereas serum total cholesterol increased 11% of baseline. These findings suggest that well-combined weight control program for obese children may result in a substantial loss of body weight during 14 weeks. In addition, these reuslts indicate that weight redution through weight control program can be a viable approach to help normalize serum lipids in obese children. Finally, these data suggest that it may be important to identify that affect weight loss and maintenance, and to examine how to manipulate these factors in weight control program.

Eating habits, physical activity, nutrition knowledge, and self-efficacy by obesity status in upper-grade elementary school students

  • BACKGROUND/OBJECTIVES: Childhood obesity has increased in recent decades in Korea. This study was designed to examine differences in the eating habits, physical activity (PA), nutrition knowledge, and self-efficacy of children by obesity status. SUBJECTS/METHODS: Subjects were 5th-grade children from 70 elementary schools in 17 cities nationwide. Two-stage stratified cluster sampling was employed. Survey questionnaire included items related to general characteristics, eating habits, PA, nutrition knowledge and self-efficacy. Excluding incomplete responses, 3,531 data were analyzed using SPSS. Subjects were categorized into overweight obesity (OW) and normal weight (NW) groups based on body mass index percentiles for age by sex. RESULTS: A total of 21.5% of subjects was overweight or obese. There were significant differences in gender, perceived stress, perception of body shape, body satisfaction, and interest in weight control between the OW and NW groups (P < 0.001). With respect to eating habits, the OW group ate breakfast (P < 0.05) and snacks (P < 0.01) less frequently, ate bigger meals (P < 0.001), and demonstrated less desirable behaviors during meals (P < 0.05 in boys) compared to the NW group. The OW group participated in less PA than the NW group, especially boys. OW boys spent less time walking during weekdays (P < 0.05) or the weekend (P < 0.001), spent more time being sedentary during weekdays or the weekend (P < 0.001), and exercised a fewer number of days (P < 0.01). For girls, the OW group spent more time being sedentary during the weekend (P < 0.01) and exercised a fewer number of days by walking or bicycle riding (P < 0.05) than the NW group. Nutrition knowledge was not significantly different between the OW and NW groups. Self-efficacy (P < 0.01 in boys), especially PA self-efficacy (P < 0.01), was significantly lower in the OW than NW group. CONCLUSIONS: This study revealed differences in eating habits, PA, and self-efficacy between OW and NW children. Obesity management programs for children need to focus on increasing self-efficacy, modifying eating habits, and increasing PA.

여고생들의 비만도 및 비만과 관련된 사회심리적 요인 분석 (A Study of the Obesity Index and Psychosocial Factors Influencing Obesity among Adolescent Girls)

  • The purpose of this study was to investigate the dietary intake, obesity index and psychosocial factors influencing obesity among 200 high school girls in Seoul. The Social Cognitive Theory provided the Conceptual basis for this study. A cross-sectional survey was conducted to examine factors related to obesity, including self-efficacy for controlling overeating, social support for eating behavior, perception of body image and weight control, nutrition knowledge, and attitudes toward obesity. The data were analyzed using t-test and multiple logistic regression. The results of this study are as follows : 1) The mean age of the girls was 16.4 years, and the rate of overweight and obesity(measured by obesity index) was 27.0$\%$. 2) The mean energy intake of subjects was 1832.3$\pm$384.0kcal. The energy derived from carbohydrates, proteins and fats was 62.7$\%$, 13.8$\%$, and 23.5$\%$, respectively. There was no significant difference between the obese and the comparison group in energy intake. 3) The result of multivariate analysis indicated that obesity had a significant relation to the perception of ideal body image, social support for eating behavior, and self-efficacy for controlling overeating(p〈0.01). As subjects preferred thinner body images(OR=0.39) and received less social support(OR=0.93), the odds of being classified as obese increased. The odds of being obese were also associated with self-efficacy, however, the relation was not strong(OR=1.04). 4) Specific social support was related to obesity among adolescent girls. As subjects received more support from family member, the odds of being obese decreased. The emotional support as well as family member's positive nutrition behavior plays a significant role. In addition, instrumental support from friends was associated with obesity. With repect to self-efficacy, the odds of being obese were increased as subjects felt less confident in controlling overeating when tempting food was placed in front of them or after an argument. In contrast, the obese group felt more confident in controlling overeating for the rest of the specific situations examined. These findings suggest that educational interventions for weight control should incorporate strategies to help participants realize their degree of obesity, to reduce the discrepancy between current and ideal body image, to elicit and maintain social support from friends and family, and to increase the self-efficacy for changing eating behaviors. 초등학교 어린이의 성별 및 비만도 수준에 따른 식생활인지.실천수준의 비교 (Comparison of Perception and Practice Levels of Dietary Life in Elementary School Children according to Gender and Obesity Status)

  • This study was performed to analyze children's perceptions and practice levels according to gender and obesity status using a dietary life safety index. A national survey was conducted on fifth grade children (n = 2,400), who were selected using three-stage stratified cluster sampling from 16 provinces. The average height was 144.8 cm, and weight was 38.8 kg. The average body mass index was 18.4 kg/$m^2$ and underweight, overweight, and obese children were identified using the 2009 KHNANES cutoff values, which were 5.3%, 10%, and 5.9%, respectively. The perception and practice scores for hand-washing prior to eating were high and the score for willing to buy at a clean store was also high. However, students answered that the hygiene level of food stores near the school was poor. More students skipped breakfast than lunch or dinner. The frequency scores for fruit and vegetables were significantly higher for girls than those for boys. Students had a good understanding of nutrition labeling but did not frequently check the label. Seventy-five percent of the students tried to avoid high calorie foods with low nutritional value, but only 40% had the appropriate knowledge about high calorie foods with low nutritional value. Girls had better dietary life perception and practice levels than those of boys. No differences in perception or practice levels were observed based on obesity status. Nutrition education on the importance of eating breakfast and having accurate knowledge on nutrition labeling and high calorie foods with low nutritional value is needed. Behavior-centered education should be implemented to improve the perceptions and practice level of student's dietary life.

삼척지역 일부 중등도 이상 비만 아동의 식생활관련 요인과 혈액성상에 관한 연구 (A Study on Dietary Related Factors and Blood Parameters of Obese Children Residing in Samcheok)

  • The purpose of this study was to investigate the dietary related factors and blood parameters of moderately or severely obese children residing in Samcheok. Anthropometric measurement, 24-hour recall for dietary intake, blood analysis and questionnaire response including nutritional knowledge, nutritional attitude, body image recognition and eating habits were conducted in 23 obese children(16 boys and 7 girls) and their mother. The mean age of subjects are 10.0 years. The average body weight, BMI, obesity index and percent body fat were 57.5㎏, 27.0㎏/$m^2$, 47.3 and 38.1% in boys and 51.0㎏, 24.8㎏/$m^2$, 40.3 and 43.9% in girls. The average nutritional attitude, the self-satisfaction and the nutritional knowledge scores in subjects were 29.9/40, 32.7/50 and 7.3/10, respectively. 34.8% of the subjects were unsatisfied with their body image, whereas, 81.8% of their mothers were unsatisfied with children’s body image. 60.9% of subjects had one or both obese parents. Average intake of energy were 90.6% of RDA in boys and 84.3% of RDA in girls. The ratio of energy from carbohydrate/protein/fat were 66/16/18 in boys and 66/17/17 in girls. The mean serum cholesterol concentration of the subjects was 191.8mg/dl and 78.3% of the subjects were above the normal serum cholesterol rage of children. In the results of correlation analysis, obesity index was negatively correlated with self and mother’s nutritional attitude scores. Body weight and BMI were positively correlated with vitamin C(p<0.05) and fruits(p<0.05) intakes. And there was a positive correlation between obesity index and BMI, and serum cholesterol(p<0.05).

중학생들의 비만도에 따른 체형인식, 영양지식, 영양소 섭취상태 (Body Image Recognition, Nutrition Knowledge and Nutrient Intakes of Middle School Students according to the Obesity Index)

  • 본 연구에서는 남녀 청소년들의 비만도에 따라 자신의 체형을 바르게 인식하고 영양소를 바르게 섭취하고 있는지를 조사하여 그들이 객관적으로 체형을 인식하고 올바른 영양지식을 가지고 자신에게 맞는 에너지를 섭취할 수 있도록 하는데 기초 자료를 제공하고자 시도하였다. 신체적 변화가 급속히 일어나는 일부 청소년을 대상으로 체형인식, 영양지식정도 및 영양소 섭취 상태를 비만도에 따라 조사하였다. 경주시 4개 중학교 남녀학생 총 622명이 본 연구에 참여하였다. 신장, 체중 및 BMI를 구하고, 체형에 대한 인식, 영양지식 및 영양소 섭취상태를 조사한 결과, 남학생의 비만도(17.1%)가 여학생 비만도(14.8%)보다 조금 더 많이 나왔으며, 조사대상자의 자신에 대한 체형인식에서 반 정도만 자신의 체형을 바르게 인식하고 있었고 정상군에서 38% 정도가 과체중 내지 비만이라고 인식하고 있는 것으로 나타났다. 체중조절 경험에 대해서는 여학생의 경우 정상군 및 과체중군 모두 80% 이상이 체중조절 경험이 있었고, 체중조절이 필요없는 저체중군과 정상군의 경우에도 체중조절을 시도했다는 것은 청소년들이 체형에 대해 지나친 관심을 보인 것으로, 성장기에 있는 이들이 정상적인 성장과 건강한 생활을 할 수 있도록 올바른 영양교육이 필요한 부분이라고 생각되어진다.
