  • Obesity is an independent risk factor fur coronary artery disease in the postmenopausal women, which may be mediated by alteration of blood lipid metabolism. We are aimed to evaluate the effects of low energy diet, restriction of high fat foods and exercise education on weight, fat mass and blood lipid profile. Fifteen postmenopausal obese women were studied. Subjects received detailed advice about how to achieve a reduction of weight by a low-energy diet (1,200kcal), restriction of high fat food and increased exercise in every 4 wks during 8 wk. To evaluate the effectiveness for education programs, dietary fat habit and daily nutrient intakes, exercise were tested before and after intervention. Anthropometry, computerized tomography, and blood lipid profile were assessed before and after intervention. According to the nutrition education, energy intake (from 1776.1 ${\pm}$ 28.2 kcal to 1268.7 ${\pm}$ 115.2 kcal, p < 0.001) and percent of energy from fat (21.9%, p < 0.01) were significantly decreased. However, the index of nutritional quality was over 1.0. On the contrary, exercise (from 341.3 ${\pm}$ 222.1 kcal to 569.4 ${\pm}$ 309.8 kcal, p < 0.05) was increased and dietary fat habit scores (from 30.8 ${\pm}$ 5.2 to 36.1 ${\pm}$ 3.0, p < 0.01) were improved. Also, body weight (4.2%, p < 0.001), BMI (4.5%, p < 0.001), body fat mass (3.4%, p < 0.05) and waist to hip ratio (1.1%, p < 0.05) were decreased. Among abdominal fat, visceral fat (26.1%, p < 0.05) and subcutaneous fat (14.8%, p < 0.01) were decreased. But there was no difference in visceral fat to subcutaneous fat ratio. We observed HDL-cholesterol increase (11.7%, p < 0.05), triglyceride reduction(14.8%, p < 0.05) and atherogenic index improvement (from 2.7 ${\pm}$ 0.7 to 2.3 ${\pm}$ 0.7, p < 0.05) However, there was no change in LDL-cholesterol and total cholesterol. Theses results showed that low energy diet, restriction of high fat foods and exercise education could result in reduction of body weight, fat mass, visceral and subcutaneous fat, and in improvement of blood lipid profile in the postmenopausal obese women.

  • Leptin, the product of the obese gene, is produced by adipose tissue and is known to be a hormone concerned with regulation of appetite and metabolism. Recent reports have shown that leptin is associated not only with obesity but also with female reproduction, but it has not yet been ascertained whether leptin acts directly on the ovaries or indirectly via the hypothalamus or pituitary pathway. The object of this study is to determine the expression of leptin and its receptor in the ovaries of 3 and 8 weeks old rats by immunohistochemistry and RT-PCR. In the ovaries of 3 and 8 weeks old rats, leptin was stained in the theca cells and portions of granulosa cells of atretic follicles, whereas leptin receptors was stained in interstitial cells and ova of preantral follicles. The RT-PCR results showed that leptin receptor mRNA was expressed in the ovaries of both immature and adult rats, while leptin mRNA was not. In conclusion, leptin mRNA was not expressed in the ovaries, however, leptin was detected by immunohistochemistry. Compared to leptin itself, leptin receptors in the ovaries were ascertained by both RT-PCR and immunohistochemistry. These results suggest that leptin is related to the regulation of the physiological functions of the ovaries.

  • Hypoparathyroidism has abnormally decreased secretion of parathyroid hormon which responds to the blood calcium level. Wherease, Pseudohypoparathyroidism has normal activity of thyroid hormon, but end-organs, such as urinary tract and osteoclast, do not respond to parathyroid hormon. The cause of this disease is due to the mutation of Guanine stimulating(Gs) protein regulating Gs gene, which is the receptor to this hormon. Pseudohypoparathyroidism is usually noted before 20 years old on average of 8-9 years old. The clinical features of this disease includes delayed growth and development, round face, obesity, soft tissue calcification, ectopic ossification, shortening of metacarpals and metatarsals by epiphyseal closure in advance of age. The mutation of Gs gene which are found in brain, endocrine organs, and chondrocytes is the cause of those features. Reaction to Glucagon, gonadal hormon, and thyroid stimulating hormon is not expected in both cases. The common dental manifestations include enamel hypoplasia, delayed eruption, agenesis of tooth, hypodontia, dysplastic short roots, widened pulpal space, microdontia, intrapulpal calcification, and malocclusion are also often reported. This case which is diagnosed to Pseudohypoparathyroidism showed short and under-developed root of permanent troth, delayed eruption, and non-eruption of premolars and molars. And morphogenesis imperfecta of first and second premolars were also found.

  • This study was performed to investigate the effects of very low calorie diet (VLCD) using meal replacements that contain the wild grass extracts that are consisted of mainly Samul-tang ingredients on weight reduction and health promotion in the obese women (BMI$\geq$25 kg/$m^2$) for four weeks. Fifty six premenopausal women participated in this experiment. Each subject was either classified as part of the control group (C group, n=28) that consumed 3 regular meals a day or the meal replacement group (MR group, n=28) that consumed 1 regular meal and 2 meal replacements within 600 kcal/day. Anthropometric measurements, body composition, health related factors, and psychological factors were assessed before (the initial) and after (the 4$^{th}$ week) the study. Obesity was significantly decreased after the diet intervention. Anthropometric measurements such as weight, waist and hip circumference, and BMI and body composition such as body fat, and fat mass were significantly decreased in all groups after the diet intervention. Anthropometric measurements and body composition of the MR group were significantly decreased more than those of the C group. Serum total cholesterol was significantly decreased in the MR groups but not in the C group and serum triglyceride concentration was significantly decreased in all groups after the diet intervention. All the biochemical measurements were within normal ranges after the diet intervention. Psychological factors such as stress and depression were significantly decreased in two groups after the diet intervention. Therefore, VLCD using meal replacements is very effective for weight reduction in the obese premenopausal women.

  • Purpose The cold hypersensitivity is a subjective symptom and it is very difficult to evaluate the severity. It is possible to detect cold hypersensitivity by measuring the skin temperature on DITI, but there is limitation only using DITI to find the objective grade of the symptom. To set a new objective standard for the diagnosis of cold hypersensitivity, we examined the relationship between the Visual Analogue Scale (VAS) score for the cold hypersensitivity and the change of skin temperature on hands by cold stress test Method 23 patients with symptom of cold hypersensitivity were participated as subjects who visited the women medical center of Kangnam Kyunghee Korean Hospital, Kyung Hee Univ. from May 1, 2002 to August 31, 2002. There were all carefully examined to rule out other disease such as obesity, skin diseases, spinal nerve lesions and external wounds. Thermographic observations for this study were made using DITI. We performed cold stress test three times to compare with the results from thermographic observations by DITI: first, after 15 minutes-resting, second, right after 1 minutes soak in $20^{\circ}C$ water, the third for last, 10 minutes after the soak. VAS score was chosen to determine the severity of cold hypersensitivity. Result 1 male and 22 female patients were participated ranging in age from 22.17 to 45.21. There was a significant negative correlation between the recovery rate of finger skin temperature after cold stress test and the VAS score. And there was a significant positive correlation between the difference of finger skin temperature and the back and palm of hands after cold stress test and the VAS score. Conclusion In cold hypersensitivity patients, the cold stress test combined with DITI could be a accurate method for the objective evaluation of cold hypersensitivity, especially good at deciding the severity by numeric values. Using a more strict criterion, as diagnosing of cold hypersensitivity, and longer follow-up may improve the validity of the results attained in clinical trials.
