Effects of Diabetes Education on Diabetic Management in Non-Insulin-Dependent Diabetics Mellitus Patients (당뇨교육이 당뇨환자의 식사요법에 대한 지식과 실천 및 혈당에 미치는 영향)

  • The purpose of this study was to examine the effect of diabetes education on diabetic management by determining the changes of the knowledge and practice for diet therapy and blood glucose level pre-training and post-training. Statistical data analyses were completed using the SPSS 11.0 program. The results can be summarized as follows : The average age of the subjects was 52.8 years old, the period of suffering from diabetes was 7.9 years and 31% of the patients had a history of diabetes in their family members. In life style for self-management, they showed lower levels in drinking post-training, and significantly higher exercise levels post-training (P<0.05). Regarding the level of knowledge for diet, they showed significantly higher levels post-training in eight items such as importance of diet therapy for diabetes (p<0.005), principle of diet therapy (p<0.005), nutrient composition of foods (p<0.005), carbohydrate composition of foods (p<0.005), calorie prescribed to themselves (p<0.001), exchange units prescribed to themselves (p<0.005), exchange food items and exchanges units of cereal & grains (p<0.005) and exchange food items and exchanges units of fruit & juices (p<0.005). Regarding the practices of diet, they showed significantly higher levels of practice in keeping permitted meal size (p<0.005), using food exchange list (p<0.005), keeping exact meal times (p<0.001) and restricting most foods to eat (P<0.01) post-training. When measuring their bodies, average weight was lower post-training. Obesity was significantly lower post-training (p<0.01), and blood pressure both in systolic and diastolic was lower. Postcardinal-2hour blood glucose level decreased significantly from 268.4$\pm$98.9 pre-training to 180.9$\pm$48.4 post-training (p<0.001).

The Clinical Summary of the Coronary Bypass Surgery (심장 관상동맥 외과)

  • It was my great nohour that I can be exposed to such plenty materials of the coronary bypass surgery. Here, I am summarizing the xoronary bypass surgery, clinically. The material is serial 101 patients who underwent coronary bypass surgery between July 17, 1979 to November 30, 1979 in Shadyside Hospital, University of Pittsburgh. 1. Incidence of the Atherosclerosis is frequent in white, male, fiftieth who are living in industrialized country. It has been told the etiologic factor of the atherosclerosis is hereditary, hyperlipidemia, hypertension, smoking, drinking, diabetes, obesity, stress, etc. 2. The main and most frequent complication of the coronary atherosclerosis is angina pectoris. Angina pectoris is the chief cause of coronary bypass surgery and the other causes of coronary bypass surgery are obstruction of the left main coronary artery, unstable angina, papillary muscle disruption or malfunction and ventricular aneurysm complicated by coronary artery disease. 3. The preoperative clinical laboratory examination shows abnormal elevation of plasma lipid in 82 patint, plasma glucose in 40 patient, total CPK-MB in 24 patient stotal LDH in 22 patient out of 101 patient. 4. Abnormal ECG findings in preoperative examine were 29.1% myocardial infarction, 25.8% ischemia and injury, 14.6T conduction defect. 5. Also we had done Echocardiography, Tread Mill Test, Myocardial Scanning, Vectorcardiography and Lung function test to get adjunctive benefit in prediction of prognosis and accurate diagnosis. 6. The frequency of coronary atherosclerosis in main coronary arteries were LAD, RCA and Circumflex in that order. 7. The patients' main complaints which were became as etiologic factor undergoing coronary bypass surgery were angina, dyspnea, diaphoresis, dizziness, nausea and etc. 8. For the coronary bypass surgery, we used cardiopulmonary bypass machine, non-blood, diluting prime, cold cardioplegic solution and moderate cooling for the myocardial protection. 9. We got the grafted veins from Saphenous and Cephalic vein. Reversed and anastomosed between aorta and distal coronary A. using 5-0 and 7-0 prolene continuous suture. Occasionally we used internal mammary A. as an arterial blood source and anastomosed to the distal coronary A. and to side fashion. 10. The average cardiopulmonary bypass time for every graft was 43.9 min. and aortic clamp time was 23 minute. We could Rt. coronary A. bypass surgery only by stand by the cardiopulmonary machine and in the state of pumping heart. 11. Rates by the noumbers of graft were as follow : 21.8% single, 33.7% double, 26.7% triple, 13.9% quadruple, 3% quintuple and 1% was sixtuple graft. 12. combined procedures with coronary bypass surgery were 6% aneurysmectomy, 3% AVR, 1% MVR, 13% pacer implantation and 1% intraaortic ballon setting. 13. We could see the complete abolition of anginal pain after operation in 68% of patient, improvement 25.8%, no change in 3.1%, and there was unknown in 3%. 14. There were 4% immediate postoperative deaths, 13.5% some kinds of heart complication, 51.3% lung complications 33.3% pleural complications as prognosis.

Comparison of the Estimations of Body Fat by Bioelectrical Impedence Analysis(BIA) and Anthropometric Measurements of Elementary School Students in Daejeon (대전 시내 초등학생의 생체 전기 임피던스치와 신체계측치에 의한 체지방율 비교 연구)

  • Body fat proportion is a major issue in health. The prevalence of obesity in childhood has been increasing on thesedays. A series of indirect estimates of body fat have been developed. There are hydrodensitometry, BIA and anthropometry equation for body fat. Hydrodensitometry has been regarded as a common criterion method. BIA made it possible to analyze body fat mass more related to hydrodensitometry than anthropometry. The purpose of this unvestigation were to compare body fat and other body composition between boys & girls and to know which anthropometry equation for body fat was most closely associated with BIA in boys & girls respectively. The subjects were 148 4th grade elementary school students(male:70, female:78). They underwent BIA(InBody 3.0, Biospace Korea)to determine body fat and other body composition. Anthropometric measurements were taken of height, weight, skinfolds and circumferences. The results obtained are summarized as follow: 1) Weight, BMI and Rohrer index were $39.9{\pm}7.2kg$, $20.5{\pm}8.0$, $138.2{\pm}17.0$ for boys and $35.9{\pm}6.7kg$, $18.2{\pm}2.6$, $96.6{\pm}13.6$ for girls. 2) Total body fat and percent body fat determined by BIA were $10.3{\pm}4.3kg$, $25.4{\pm}6.6%$ for the boys group and $9.3{\pm}3.8kg$, $25.1{\pm}6.0%$ for girls. 3) Mean triceps skinfold thickness was $20.5{\pm}6.1mm$ in boys, $17.0{\pm}5.1mm$ in girls and mean WHR was $0.88{\pm}0.4$ in bodys, $0.82{\pm}0.4$ in female and the values were significantly different by sex. 4) Correlational nanlyses showed that estimation of body fat by BIA measurement seemed to be closely associated with Rohere index(boys r=0.854, girls r=0.909). Kim's equation was highly correlated with the body fat in girls(r=0.910) but less correlated in boys(r=0.710). We need to develop specific anthropometric equations based on age and sex to determine body fat for children.

Injecheonggeumdan reduces body weight gain and blood lipid profiles in high fat diet-fed mice (고지방식이 마우스 비만모델에서 인제청금단에 의한 체중감량과 혈중 지질대사의 개선효과)

  • Objectives : We investigated the effects of Ingecheonggeumdan(ICD) on body weight and examined whether blood lipid levels and visceral fat are inhibited by it in high fat diet-fed obese male mice. Methods : 8 weeks old, high fat diet-fed obese male mice were divided into 6 groups: C57BL/6N normal, control, ICD-1(150mg/kg), ICD-2(300mg/kg), ICD-3(600mg/kg) and orlistat(10mg/kg). After mice were treated with ICD and orlistat for 8 weeks, we measured body weight gain, food intake, feeding efficiency ratio, fat weight, plasma leptin and lipid levels. We also performed histological analysis for liver and fat on the mice. Results : Compared with controls, ICD and orlistat-treated mice had lower body weight gain and adipose tissue weight, the magnitudes of which were prominent in ICD-3. Compared with controls, ICD and orlistat-treated mice had lower blood leptin levels, the magnitude of which was prominent in ICD-3. Compared with controls, ICD and orlistat-treated mice had higher blood HDL-cholesterol and lower blood plasma LDL-cholesterol, free fatty acid and triglyceride levels, the magnitudes of which were prominent in ICD-3. Blood plasma AST and ALT concentrations were not changed by ICD and orlistat, indicating ICD and orlistat do not show any toxic effects. Consistent with their effects on body weight gain, the size of adipocytes and hepatic lipid accumulation were significantly decreased by ICD and orlistat. Conclusions : These results demonstrate that ICD and orlistat effectively reduce body weight gain, blood plasma LDL-cholesterol, free fatty acid and triglyceride levels and improves abdominal fat, the magnitudes of which were prominent in ICD-3.

Prevalence of Diabetes Mellitus and Associated Diseases in Yeungnam Province Area (영남지방에서의 당뇨병 유병율과 이에 관련돈 질환의 빈도에 관한 연구)

  • To know the prevalence of the diabetes mellitus and associated diseases, we analysed the data of the 3,088 subjects who were examined with the Computed Automated Medi-Screening Test System which consisted of 65 parameters including blood glucose determination fasting and one hour after 100g of oral glucose load. We grouped the subjects by the modified criteria of National Diabetic Data Group. Followings are the results of the various analysis : 1. The prevalence of diabetes mellitus and impaired glucose tolerance is 2.27% and 18.26% respectively. 2. The prevalence of diabetes mellitus is 2.63% In male and 1.66% in female. There is no statistically significant difference between male and female. 3. There is tendency of increasing prevalence of diabetes mellitus as the age increases. From second to eighth decade, the prevalence of diabetes mellitus Increases as 0.0, 0.45, 0.67, 2.28, 3.47, 5.36, 10.00% respectively. 4. There is no statistically significant difference of prevalence of obesity between normal and diabetes: that is, 18.03%, 22.86% respectively.($P{\geq}0.1$) 5. There is no statistically significant difference of prevalence of impaired glucose tolerance and diabetes between non-obese and obses group. ($P{\geq}0.1$) 6. There is statistically significant increases of frequency of proteinuria, azotemia, hypertension as the glucose tolerance decreases. ($P{\leq}0.05$)

Associations between Psychological Distress and Body Mass Index among Law Enforcement Officers: The National Health Interview Survey 2004-2010

  • Objectives: To investigate the association between psychological distress and obesity among law enforcement officers (LEOs) in the United States. Methods: Self-reported data on psychological distress based on six key questions were obtained from LEOs who participated in the National Health Interview Survey (2004-2010). We used Prochaska's cut-point of a Kessler 6 score ${\geq}5$ for moderate/high mental distress in our analysis. Mean levels of body mass index (BMI) were compared across three levels of psychological distress. Results: The average age of LEOs (n = 929) was 39.3 years; 25% were female. Overall, 8.1% of LEOs had moderate or high psychological distress; 37.5% were obese (BMI ${\geq}30$). Mean BMI increased with increasing psychological distress (no distress, BMI = $27.2kg/m^2$; mild distress, $27.6kg/m^2$; and moderate/high distress, $33.1kg/m^2$; p = 0.016) after adjustment for age, race, income, and education level among female officers only. Physical activity modified the association between psychological distress and BMI but only among male LEOs (interaction p = 0.002). Among male LEOs reporting low physical activity, psychological distress was positively associated with BMI ($30.3kg/m^2$ for no distress, 30.7 for mild distress, 31.8 for moderate/high distress; p = 0.179) after adjustment, but not significantly. This association was not significant among males reporting high physical activity. Conclusion: Mean BMI significantly increased as psychological distress increased among female LEOs. A longitudinal study design may reveal the directionality of this association as well as the potential role that physical activity might play in this association.

Effect of Complex Training on Inflammatory Markers and Homocysteine of Obese Men (복합트레이닝이 비만남성의 염증지표와 homocysteine에 미치는 영향)

  • The purpose of this study was the effect of complex training of obese men in their 30s on inflammatory markers and homocysteine, a risk factor for cardiovascular disease. The subjects consisted of obese men (n=12) with the body fat ratio of 25% or above in their mid 30s who had no medical conditions and can follow the exercise routine required by this study. To achieve the purpose of this study measured the maximum oxygen intake (VO2max) and 1RM of 5 kinds of machine(bench press, lat pull down, arm curl, leg press, squat) as a preliminary test; based on the results, performed 8 weeks complex training (aerobic exercise + weight training); and then analyzed the variation in body composition (body weight, body fat and BMI), blood inflammatory markers (IL-6, TNF-α and CRP), and homocysteine between before and after training. The results exhibited that 8 weeks complex training reduced weight, body fat and BMI significantly (p<0.01) and also reduced inflammatory marker CRP and homocysteine, a risk factor for cardiovascular disease, significantly (p<0.05). In conclusion, 8 weeks complex training confirmed the variation in body composition, and this variation has a positive effect on the inflammatory marker and the risk factor for cardiovascular disease.

Caffeine inhibits adipogenesis through modulation of mitotic clonal expansion and the AKT/GSK3 pathway in 3T3-L1 adipocytes

  • Caffeine has been proposed to have several beneficial effects on obesity and its related metabolic diseases; however, how caffeine affects adipocyte differentiation has not been elucidated. In this study, we demonstrated that caffeine suppressed 3T3-L1 adipocyte differentiation and inhibited the expression of CCAAT/enhancer binding protein (C/EBP)α and peroxisome proliferator-activated receptor (PPAR)γ, two main adipogenic transcription factors. Anti-adipogenic markers, such as preadipocyte secreted factor (Pref)-1 and Krüppel-like factor 2, remained to be expressed in the presence of caffeine. Furthermore, 3T3-L1 cells failed to undergo typical mitotic clonal expansion in the presence of caffeine. Investigation of hormonal signaling revealed that caffeine inhibited the activation of AKT and glycogen synthase kinase (GSK) 3 in a dose-dependent manner, but not extracellular signal-regulated kinase (ERK). Our data show that caffeine is an anti-adipogenic bioactive compound involved in the modulation of mitotic clonal expansion during adipocyte differentiation through the AKT/GSK3 pathway.

A Study on the Life-Style of Obese Children (비만 아동의 비만에 관련된 특성과 생활양상)

  • The purpose of this study was to find family characteristics, habit of dietary intake, physical exercise, and mothers' concern related to obesity through questionaires among 5th grade children of 5 emementary schools in Taegu city from May 29th to June 8th, 1989. The data used in this study were collected from 245 obese children and 327 normal children by Body Mass Index with their body weight and height, and measurement of Triceps Skinfold Thickness The findings were as follows: 1. This survey revealed that Triceps Skinfold Thickness was 11.0mm in the obese group and 7.5mm in normal group. Their parents body weight and BMI of the obese group were higher than those of the normal group. 2. Ending of weaning of the obese group had a faster tendency than that of the normal group. 3. It showed that the children of obese group had a hearty appetite during the evening meal and children of the normal group did so in lunch time. 4. The normal group was willing to take exercise but the obese group was not so. 5. Mothers of the obese group showed deeper concern at quantity of dietary intake and physical exercise than mothers of the normal group for their children's health.

Protective effects of Commiphora myrrha on acute pancreatitis (몰약(沒藥) 물 추출물의 급성 췌장염 보호 효과)

  • Objectives : Commiphora myrrha (CM) has been used in traditional medicine for treating disease such as obesity, hyperlipidemia, atherosclerosis, diabetes and osteoarthritis. However, the protective effects of CM on acute pancreatitis (AP) has not been reported. Thus, the aim of this study was to evaluate the protective effects of CM water extract on cerulein-induced AP. Methods : AP was induced in mice via intraperitoneal injection of supramaximal concentrations of the stable cholecystokinin analogue cerulein ($50{\mu}g/kg$) every hour for 6 times. Water extract of CM (0.1, 0.2, or 0.5 g/kg) was administrated intraperitoneally 1 h prior to the first injection of cerulein. The mice were killed at 6 h after the final cerulein injection. Pancreas was rapidly removed for morphologic and histochemical examination, myeloperoxidase (MPO) assay. Blood samples were taken to determine serum amylase and lipase activities. Results : Administration of CM significantly inhibited pancreatic weight/body weight ratio, pancreas histological injury. And CM administration inhibited the serum digestive enzyme elevation such as amylase and lipase on cerulein-induced pancreatitis. In addition, Pancreas MPO activity which indicates neutrophil infiltration was inhibited by CM extract on cerulein-induced pancreatitis. Conclusions : In conclusion, our results could suggest that pre-treatment of CM reduces the severity of cerulein-induced AP. Therefore, CM could be used as a protective agent against AP. Also, this study could give a clinical basis that CM could be a drug or agent to prevent AP.
