사암침법 간정격과 간승격의 최근 연구 동향 분석 (Recent Study Trends of the Liver-tonification and Liver-sedation of Saam Acupuncture)

  • Objectives : This study was aimed to investigate current status researches and to provide source in setting development strategy on Liver-tonification acupuncture(LTA) and Liver-sedation acupuncture(LSA). Methods : We searched databases of Pubmed, KISS, OASIS, Google scholar and CNKI up to February 2017. We included articles that using LTA or LSA as main treatment, using the partial acupoints combination of LTA or LSA, or using each acupoint of LTA or LSA. We analyzed disease, details of acupuncture and control treatment and clinical outcomes from included studies. Results and Conclusions : Among the included clinical studies, case report was the most used methodology. LTA used to treat obesity, myopia, bartholinitis, tinnitus, hiccup and sleep disorder, prostate cancer and tremor. LSA used to treat panic disorder, gastroesophageal reflux disease and irritable bowl syndrome, herpes zoster and neuralgia. Obesity and tremor were the most studied diseases using LTA or LSA. Amyotrophic lateral sclerosis was the only disease which LTA or LSA have no therapeutic effect. Each acupoints was not used as a meaning of Saam acupuncture but as affiliated meridian acupoint. Since animal researches were not fully conducted, mechanism of LTA and LSA was hard to be proved. Thus, more clinical studies and basic animal research are needed.

Bee Venom Acupuncture for Shoulder Pain: A Systematic Review and Meta-analysis of Randomized Controlled Trials

  • Objectives: Our review aimed to summarize and evaluate evidence on the effectiveness of bee venom acupuncture (BVA) in the treatment of shoulder pain. Methods: Randomized controlled trials (RCTs) evaluating the effectiveness of BVA on shoulder pain were searched up to October 2019 in 11 electronic databases (Medline, Embase, CENTRAL, CiNii, CNKI, VIP, Wanfang, Kmbase, NDSL, RISS, OASIS). The methodological quality of the included RCTs were evaluated using Cochrane Risk of Bias tool and a meta-analysis was performed. Results: Seven studies were included in the review, and four studies were included in the meta-analysis. Comparing BVA plus conventional therapy (CT) with saline injection plus CT, it showed an effect in favor of BVA plus CT in visual analog scale (VAS) and pain rating scale (PRS) (p = 0.02, p = 0.009, respectively). Comparing BVA plus physiotherapy (PT) with saline injection plus PT, it showed that there was no significant difference in VAS and verbal rating scale (VRS) between the two groups. Conclusion: This systematic review and meta-analysis suggest that BVA could be beneficial as an adjuvant treatment for shoulder pain.

한의학의 치매관련 임상연구 동향분석과 제안 (Literature Review and suggestions : dementia clinical studies in Korean Oriental Medicine)

  • Objectives : This study was aimed to review the trend of dementia clinical studies in Korean Oriental Medicine and to suggest better clinical studies. Method : We collected 26 papers on dementia clinical studies in the internet site OASIS using the keyword 'Dementia'. Then we analyzed them. Results : There were 26 clinical study papers that were published between 1996 and 2010. Observational studies account for 61% of total clinical studies, whereas experimental studies account for only 31%. Effects of treatment in experimental studies was evaluated mainly by MMSE-K and K-DRS. Treatment period of experimental studies was longer than 100days. Conclusions : High quality experimental studies like randomized clinical trial(RCT) and intervention studies which use syndrome differentiation(辨證) have not yet been published. It is necessary to research and develop duration of treatment effect and tools for evaluating treatment effects.

여성 복압성 요실금에 대한 전침치료 연구 동향 (A Review of Research on Electroacupuncture for Female Stress Urinary Incontinence)

  • Objectives: The aim of this study is to establish a base for further research by reviewing studies on electroacupuncture (EA) for stress urinary incontinence (SUI). Methods: Clinical studies concerning the effects of EA for SUI, were obtained from Cochrane Library, PubMed, CNKI, RISS, NDSL, KISS and OASIS. Results: Forty-five studies met the criteria, which included 33 RCTs, 1 pilot RCT, 4 non-randomized Clinical Trials, 6 Case studies and 1 Orthogonal design study with 3638 patients. There was only one article published in Korea. In these study, the most common primary outcome measurement was the pad test. Most of the studies showed the group treated by EA effects compared to the control group. Also, many interventions that combined with EA were found and all complex therapy group had significantly better than control group. 7 studies observed adverse events (AEs), four of which referred to EA related AEs among them. And 4 studies reported no AEs associated with EA. Conclusions: Despite several limitations, various studies to prove limited yet effective EA on SUI provides much significance. Subsequent studies conducted by the complementary systematic review of the studies and well-designed clinical trials using the methodological quality will be needed to more firmly validate the therapeutic effect of EA on SUI.

위축성 질염의 좌훈 치료 연구에 관한 체계적 문헌 고찰 (Systematic Review of Fumigation Therapy for Atrophic Vaginitis)

  • Objectives: The purpose of this systematic review was to overview and evaluate the effectiveness of fumigation treatment for women with atrophic vaginitis. Methods: We searched articles from Pubmed, Chinese Academic Journals (CAJ) and Oasis online databases. Searching keywords were ‘fumigation’, ‘atrophic vaginitis’, ‘vaginitis’, ‘阴道炎’, ‘膣炎’, ‘老年性 阴道炎’, ‘坐熏’, ‘熏洗’, ‘熏浴’, ‘좌훈요법’, ‘질염’, ‘위축성질염’. After searching the articles, we performed quality assessment using Cochrane risk of bias (RoB) tool. Results: Among the 142 articles were searched, 5 randomized controlled clinical trials (RCTs) were finally selected. All of 5 studies showed that fumigation treatment has significant effect on atrophic vaginitis in terms of the effectiveness rate, but 3 studies were not statistically significant. Conclusions: Our systematic review found encouraging but limited evidence of fumigation therapy for atrophic vaginitis. We recommend clinical trials which obtain stronger evidence without the demerits of trial design.

골반염의 침치료에 대한 최신 연구 동향 (Research Trends of the Acupuncture Treatment for Pelvic Inflammatory Disease)

  • Objectives: The purpose of this review is to overview the studies of acupuncture treatment for pelvic inflammatory disease. Methods: In order to investigate the effect of acupuncture treatment for pelvic inflammatory disease, we searched relevant studies in Pubmed, Cochrane, Oasis, Korean studies Information Service System up to March 2019. After searching studies, we analyzed selected studies. Results: Five studies were included in this review. There were 1 randomized controlled studies and 4 before and after studies. The outcome measurement of pelvic inflammatory disease was divided into effective rate, physical examination, response, hematological parameter, pain. The acupuncture treatment was effective in improving pelvic inflammatory disease. Conclusions: This study shows that acupuncture treatment is one of effective treatment methods for pelvic inflammatory disease. Further large randomized placebo controlled trials will be needed.

건선의 면역조직화학적 연구 결과에 대한 한의학적 접근을 위한 문헌적 고찰 (Literature Review of the Korean Medicinal Approaches to Immunohistochemical Research on Psoriasis)

  • Objectives : This study is aimed to analyse immunohistochemical research trend on psoriasis and consider the results in terms of Korean medicine. Methods : We analyse both immunohistochemical research papers and Korean medical papers on psoriasis, which I searched using KISS(Korean studies Information Service System) and OASIS(Oriental Medicine Advanced Searching Integrated System). Results : Psoriasis is characterized by hyperporliferation of the epidermis, inflammatory cell accumulation and increased tortuosity and dilatation of dermal papillary blood vessels. Recently, angiogenesis is considered to play a major role in pathogenesis of psoriasis. Conclusion : In Korean medicine, blood-tonifying, blood-cooling and blood-activating medicinal herbs have generally been used to treat psoriasis. I consider these herbs which also treat vessel problem can have an effect on angiogenesis of psoriasis.

사역산(四逆散)의 불면 치료 효과 : 체계적 문헌 고찰 및 메타 분석 (The Effect of Sayeok-san for Insomnia: A Systematic Review and Meta-Analysis)

  • Objectives: This study evaluated the effectiveness of Sayeok-san for treating insomnia. Methods: We conducted a search using keywords, such as "Sayeoksan", "Si-ni-san", and "insomnia" in eight databases (CNKI, Wanfang, Pubmed, Embase, Cochrane, NDSL, OASIS, RISS) on July 17, 2012. There was no limit placed on the publication period of the papers, and we used Cochrane's risk of bias tool to assess their quality. Meta-analysis was classified according to the outcome measurements, such as total effective rate and PSQI. Results: Nine RCTs were selected. Meta-analysis showed that the treatment group, which used Sayeok-san, had a higher total effective rate than did the control group that used only Western medicine. The treatment group also had a lower average of PSQI and TSS (TCM symptom score) than did the control group. Conclusion: This study suggests that Sayeok-san is effective in treating insomnia. However, these findings should be interpreted cautiously due to unknown or high risk of bias of the included trials.

탈모의 한의학적 치료에 대한 국내 현황 : 증례보고를 중심으로 (The Current Status of Korean Medicine Treatment of Alopecia : Focusing on Case Reports)

  • Objectives : This study is carried out to analyze the use of Korean medical treatments on alopecia in Korean medicine clinical studies published in South Korea. Methods : Based on OASIS and KTKP with the keyword 'alopecia', published in 1996-2019.4.23., total 23 studies have been found and analyzed. We analyzed Korean medical treatment used in each cases, and investigated the tendency of use for each treatment. Results : 369 cases in 23 research papers were selected. Herbal medicine, acupuncture, pharmacopuncture, external medicine and other therapy were used as treatment for Alopecia. Herbal medicine with a cooling effect was mainly used. The acupoints mainly used were GV20, ST8, GB5, ST36, HT8, PC6, BL66. Pharmacopunctures used to treat alopecia were CF, HP, HH, BV, etc. The average duration of Korean medicine treatment was 144 days. After Korean medicine treatment, 292 cases presented improvement or complete recovery. Conclusions : According to the results, we were able to discover the current status of Korean medicine treatment of alopecia. Based on this result, further studies will be needed to establish the primary treatment direction of alopecia.

외음부 통증(陰痛)의 침치료에 대한 최신 연구 동향 (Research Trends of the Acupuncture Treatment for Vulvodynia)

Methods: In order to investigate the effect of acupuncture treatment for vulvodynia, we searched relevant studies in Pubmed, Cochrane, Oasis, KISS up to October 2018. After searching studies, we analyzed selected studies. Results: Six studies were included in this review. There were 3 randomized controlled studies and 3 before and after studies. The outcome measurement of vulvodynia was divided into pain, sexual function, and quality of life. The acupuncture treatment was effective in relieving vulvar pain, dyspareunia and improving quality of life. Conclusions: This study shows that acupuncture treatment is one of effective treatment methods for vulvodynia. Further large randomized placebo controlled trials will be needed.
