Evaluation of the efficacy of nutritional screening tools to predict malnutrition in the elderly at a geriatric care hospital

  • BACKGROUND/OBJECTIVES: Malnutrition in the elderly is a serious problem, prevalent in both hospitals and care homes. Due to the absence of a gold standard for malnutrition, herein we evaluate the efficacy of five nutritional screening tools developed or used for the elderly. SUBJECTS/METHODS: Elected medical records of 141 elderly patients (86 men and 55 women, aged $73.5{\pm}5.2years$) hospitalized at a geriatric care hospital were analyzed. Nutritional screening was performed using the following tools: Mini Nutrition Assessment (MNA), Mini Nutrition Assessment-Short Form (MNA-SF), Geriatric Nutritional Risk Index (GNRI), Malnutrition Universal Screening Tool (MUST) and Nutritional Risk Screening 2002 (NRS 2002). A combined index for malnutrition was also calculated as a reference tool. Each patient evaluated as malnourished to any degree or at risk of malnutrition according to at least four out of five of the aforementioned tools was categorized as malnourished in the combined index classification. RESULTS: According to the combined index, 44.0% of the patients were at risk of malnutrition to some degree. While the nutritional risk and/or malnutrition varied greatly depending on the tool applied, ranging from 36.2% (MUST) to 72.3% (MNA-SF). MUST showed good validity (sensitivity 80.6%, specificity 98.7%) and almost perfect agreement (k = 0.81) with the combined index. In contrast, MNA-SF showed poor validity (sensitivity 100%, specificity 49.4%) and only moderate agreement (k = 0.46) with the combined index. CONCLUSIONS: MNA-SF was found to overestimate the nutritional risk in the elderly. MUST appeared to be the most valid and useful screening tool to predict malnutrition in the elderly at a geriatric care hospital.

비만 초등학생의 토요 영양 교실의 운영 효과 평가 (Evaluation of Saturday Nutrition Classes for Obese Elementary Students in Chungnam Province)

  • This study was designed to evaluate Saturday nutrition classes for obese elementary students in Chungnam Province. Obesity index, nutrient intake, nutrition knowledge, nutrition attitude, and self-satisfaction of 36 obese elementary students were compared before and after attending nutrition classes every other Saturday for 3 months. The average age, height, weight, body fat, % body fat, and obesity index were 11.9 years, 145.5 cm, 53.3 kg, 24.5 kg, 45.7%, and 32.4%, respectively. Most of the subjects(94.5%) viewed themselves as fat. Sixty-three percent of children were discontent with their body-image. All answered that they have experience with weight control. The major methods of weight control were exercise and reduction of snacks. The mean serum cholesterol, blood glucose, GOT/GPT, and hemoglobin were 177.4 mg/dL, 90.4 mg/dL, 25.8/25.5 IU/L, 14.5 g/dL, respectively. After attending nutrition classes, the daily intakes of energy, plant protein, plant fat, carbohydrate, crude fiber, and cholesterol increased significantly. The scores for nutrition knowledge, nutrition attitude, and self-satisfaction also increased after nutrition classes, but there were no significant differences. Percent body fat decreased significantly after nutrition classes. In conclusion, nutrition education for elementary students is effective for the prevention and control of obesity. Further research is needed to develop a systematic program of nutrition education for obese children.

입원 환자 영양검색 지표 개발 (Development of Nutrition Screening Index for Hospitalized Patients)

  • Several studies about hospital malnutrition have been reported that about more than 40% of hospitalized patients are having nutritional risk factors and hospital malnutrition presents a high prevalence. People in a more severe nutritional status ended up with a longer length of hospital stay and higher hospital cost. Nutrition screening tools identify individuals who are malnourished or at risk of becoming malnourished and who may benefit from nutritional support. For the early detection and treatment of malnourished hospital patients , few valid screening instruments fur Koreans exist. Therefore, the aim of this study was to develop a simple, reliable and valid malnutrition screening tool that could be used at hospital admission to identify adult patients at risk of malnutrition using medical electrical record data. Two hundred and one patients of the university affiliated medical center were assessed on nutritional status and classified as well nourished, moderately or severely malnourished by a Patient-Generated subjective global assessment (PG-SGA) being chosen as the 'gold standard' for defining malnutrition. The combination of nutrition screening questions with the highest sensitivity and specificity at prediction PG-SGA was termed the nutrition screening index (NSI). Odd ratio, and binary logistic regression were used to predict the best nutritional status predictors. Based on regression coefficient score, albumin less than 3.5 g/dl, body mass index (BMI) less than $18.5kg/m^2$, total lymphocyte count less than 900 and age over 65 were determined as the best set of NSI. By using best nutritional predictors receiver operating characteristic curve with the area under the curve, sensitivity and 1-specificity were analyzed to determine the best optimal cut-off point to decide normal or abnormal in nutritional status. Therefore simple and beneficial NSI was developed for identifying patients with severe malnutrition. Using NSI, nutritional information of the severe malnutrition patient should be shared with physicians and they should be cared for by clinical dietitians to improve their nutritional status.

  • Objectives: The intention of this study was to find out the impact of mother's body mass index, eating habits, nutrition status and sociodemographic factors on the body mass index of the children. Methods: The study used original data of Korea National Health and Nutrition Examination Survey (2010-2011) and had 957 children in the ages of 1-5 years and 957 mothers of these children, a total 1,914 as subjects. The weights (body mass indexes) of the children, subjects under 5th percentile were classified into a underweight group, 5~84th percentile into a normal weight group, 85-94th percentile into an overweight group and more than 95th percentile into an obesity group based on the 2007 Korean children and adolescents growth chart. Results: We observed that when the body mass index of a mother increased by 1, the possibility that her child would belong to the overweight group increased by 6.5% (95% confidence interval: 1.01~1.13), and the possibility that the child would belong to the obesity group increased by 95% (confidence interval: 1.01~1.18). With regard to the number of deficient nutrients which were ingested according to estimated average requirements (EAR) amid nutrients ingested by the mother increased by 1, the possibility that the child would belong to the obesity group increased by 16.6% (95% confidence interval: 1.01~1.35). In case the birth weight of child increased by 1kg, the odds ratio that the child would belong to the obesity group was 2.022 (95% confidence interval: 1.08~3.77). Conclusions: We conclude that the body mass index, eating habits and nutrition status of the mother had significant effects on the body mass index of the child. Therefore, it is critical to recognize the importance of this observation and provide practical training for eating habits and nutrient intakes in order to achieve healthy growth and prevention of obesity among children.

한국인 상용 식품의 혈당지수 (Glycemic Index) 추정치를 활용한 한국 성인의 식사혈당지수 산출 (Establishing a Table of Glycemic Index Values for Common Korean Foods and an Evaluation of the Dietary Glycemic Index among the Korean Adult Population)

  • Recent studies have reported that the glycemic index (GI) has an effect on developing the risk for metabolic abnormalities such as diabetes, dyslipidemia, and obesity. As there are no reliable GI values for common Korean foods, only a few studies have been carried out using the dietary GI for Korean adults. The aim of this study was to establish a table of GI values for common Korean foods and evaluate dietary glycemic index (DGI) and dietary glycemic load (DGL) among the Korean adult population. International tables of GI values and other published values were used to tabulate GI values for common Korean foods. Among 653 food items, 149 (22.8%) were adapted from published data, 60 (9.2%) were imputed from similar foods, and 444 (68.0%) were assigned a zero. Data from 7,940 subjects aged 20 years and older in the 2007-2008 Korea National Health and Nutrition Examination Survey were obtained, and DGI and DGL were calculated. The average DGI was 60.0 and the average DGL was 182.5 when the reference food GI value was glucose. After adjusting for potential confounding variables, DGI and DGL increased significantly according to age group (p for trend < 0.001). The food group that contributed most to DGL was grain and its products supplying 85.3% of total DGL, whereas the mean GI value in grain and its products was 72.6. Fruits and potatoes also contributed to DGL (5.8 and 2.9%, respectively), and their GIs were high (67.7 for potatoes and 45.8 for fruits). For individual food items, white rice supplied 66.7% of total GI followed by glutinous rice (2.3%) and steamed white rice cakes (2.0%). In conclusion, a table of GI values for 653 common food items was established in which white rice was the most contributing item to DGL. Our results will be useful to examine the relationships between DGI, DGL, and metabolic abnormalities in the Korean population.

식품영양전공과 비전공 여대생의 식행동과 영양지식에 관한연구 (A Study on Dietry Attitudes Food Behaviors and nutriton Know-ledge of Food and ANutrition Major and Non-Major Female Students)

  • The purpose of this study was to investigate dietary attitudes food behaviors and nutrition knowledge of food nutrition major and non-major female students. The subjects of this study consisted of 98 food and nutrition majors and 98 non-majors. Questionnaires were completed by them. Food and nutrition majors gave a serious consideration of taste and nutrition in selecting foods while non-majors considered taste food price amount of food and preparing time. Food and nutrition majors got higher scores in hig-her level of nutrition knowledge than non-majors and both got about the same scores in basic nutrition knowledge. The mean body weights of both were about the same and mean height of non-majors was hig-her than food and nutrition majors. Accrding to obesity index ideal body weight was more in food and nutrition ajors and underweight was more in non-majors. Dietary attitudes food behaviors and nu-trition knowledge of food and nutrition majors were better than those of non-majors. Therefore nutrition non-majors need nutrition education in order to improve their food behaviors and nutrition knowledge.

인천지역 일부 여중생의 비만도에 따른 섭식장애경향과 식습관의 비교 (Comparisons of Dietary Habits and Eating Disorder by Obesity Index on One of Middle School Girls in Incheon)

  • On the general trends of weight control for slimness, this study surveyed and compared eating attitude influenced by eating disorder, eating habits, and nutrition knowledge by obesity index in middle school girls. The purpose of this study was to investigate the eating disorders and eating habits, and analyzed these data by degree of obesity. This will provide basic information of nutrition education for adolescents who need to have proper body shape and dieting habits. This survey was conducted to one of girls' middle school in Incheon and all the data was analysed by SAS(Statistical Analysis System) program. The average height, weight and PIBW were 158.7cm, 48.2kg, and 93.9% in students. However, 50.8% of students were underweight or severely underweight. Most of female students perceived that they were normal or overweight even though they had underweight or normal. There was a significant difference in experience and intention of weight control by degree of obesity. In the overweight group, EAT-26 score tendency was high, especially preoccupation with losing weight(factor 1). It shows that obesity index had meaningful correlation with EAT-26 score and factor1. In dietary habit, overweight group showed more irregular meals tendency, and overeat frequently. On the other hand, this group had lower frequency and of smaller amount snacks. The intake frequency of meals and snacks showed in the affirmative way regardless of degree of obesity. The average score for nutrition knowledge about calorie and weight control was low. Correct answer rates were increased in higher overweight group. In conclusion, teenage girls who concerned too much about their slim body image need to be educated about accuracy concept of the body image. Also, practical and systematic nutrition education should be done for the correct nutrition knowledge and its application ability of individuals.

충남 지역 주민들의 약수 이용 실태 및 무기질 함량에 관한 조사 분석 (Survey on the Local Residental Utilization and the Mineral Content of Mineral Water in Chung Nam Area)

  • Survey was done to total 250 people to analyze characteristics of water quality in 5 widely used mineral water springs-(Teajosan(a), Leechungmukong(b), Onju(c), Yeongin(d), Crown(e))-in Chung-nam area and local mineral water usage. And the content of mineral and physico-chemical properties were compared in 5 sites. The largest users of mineral water spring was $40{\sim}50's$ of ages. Among those mineral water spring users, 52.5% had regular dietary habit and 32% showed improvement in lift habit after using mineral water spring. The mineral water spring users were engaged in student, housewife, professional, self-employed, etc. Those who were highly interested in health were likely to use mineral spring water more often regardless of age and occupation. Five mineral water springs differed in contents of mineral. The average content of minerals was $Ca\;28.5mg/{\ell},\;K\;1.5mg/{\ell},\;Mg\;5.3mg/{\ell},\;Na\;14.3mg/{\ell}$. 'K index' was an indicator of healthy water, and 'O index' was an indicator of tasty water. According to K indek and O index, waters of a b, c and e area were classified as 'tasty and healthy water', and water of e area had the highest values in both K and O index d area was classified as 'tasty water.'
