A Comparison of Health Status and Health Behaviors between Korean Americans and Koreans (재미 한국인과 한국인의 건강수준 및 건강행태에 관한 비교)

  • Objectives: This study purports to comparatively study health condition, health behaviors, and health care utilization of Koreans living in Korea and in the US, and then, to analyze the factors influencing them. Methods: The collected data were analyzed on the basis of the 2005 Korea National Health and Nutrition Examination Survey. Of 25,196 adults aged 19 or older, 7,802 respondents to health behavior survey and 5,526 respondents to physical examination survey. The analyzed data of the Korean Americans were the results of the California Health Interview Survey (CHIS) conducted by the UCLA Center for Health Policy Research in 2005. This study analyzed the responses of 645 known Korean respondents to the 2005 CHIS. Results: In regard to differences in overall health condition between the two groups, it was found that both the male and female Korean Americans thought their own overall health conditions were relatively poor compared to Koreans, especially in relation to diabetes and cardiovascular diseases. For smoking status considered as one of health behavior factors, smokers of the Koreans account for 46.1% which is higher than that of the Korean Americans and the proportion of the Koreans smoking daily is also at least two times higher than that of the Korean Americans. Similarly, for alcohol drinking, the Koreans showed higher drinking experience rate and no less than three times higher drinking frequency than that of Korean Americans. Conclusion: It is expected that this study will contribute greatly to solving health problems among foreign immigrants and overseas Koreans in future by clarifying any differences in health status and health behaviors resulting from sociocultural differences despite of similar genetic factors.

Achievement of Health Promotion Program in Korea (국민건강증진사업 10년의 성과)

  • There has been 10 years since the Health Promotion Act was legislated. The government began to establish a health promotion fund on the basis of Health Promotion Act in 1995, and to manage and operate the fund from 1998. It is evaluated that health promotion program have had various outcomes in many aspects. First, there has been growing awareness of the impotance of health promotion through the establishment of Health Plan 2010 and the effort to actualize the Plan. Second, the importance of securing health equity and identifying health determinants have been recognized during the planning process of Health Plan 2010. Third, the health promotion program have mainly focused on improving healthy life style of the population. As a result, desirable health behavior change of the population could be expected from the result of 2005 National Health and Nutrition Survey. Fourth, public health centers began to play a crucial role in implementing health promotion programs, and began to build infrastructure for health promotion programs. Fifth, management efficiency of private health related organizations have been improved. Finally, training for health promotion personnel and their participation in the program could be the foundation for the higher level of outcome achievement from the health promotion programs. Important challenges for future health promotion would be identification of the determinants and risk factors of health, formulating plan of regional health promotion programs, building infrastructure for health promotion, creation of specific action model by public health center, development of health promotion program for the elderly, conducting research for evidence concerning major factors reducing the need for health care through prevention disease activities, and establishment of evaluation and feed back system for health promotion programs.

Analysis of Adult Behaviors to Decrease Exposure to Endocrine Disruptors in Dietary Life (식생활에서의 내분비계 장애물질에 대한 성인들의 노출 저감화 행동 분석)

  • The purpose of this study was to examine the level of concern and the level of knowledge of endocrine disruptors, and the level of dietary behaviors to decrease exposure to endocrine disruptors, to determine the factors affecting the level of knowledge and behaviors, and to assess the causal relationship between them. The data were collected from 579 adults in Seoul, Incheon, Daejeon, Daegu, Busan and Gwangju provinces. Frequencies, t tests, analysis of variance, Cronbach's alpha, Pearson's correlation coefficient analysis, multiple regression analyses, and path analysis were carried out by SPSS for Win V.18.0. The results of this study were as follows: The level of concern about endocrine disruptors was higher than the medium level. The respondents were most concerned about a container of cup ramen among products related to foods. The levels of both knowledge and behaviors in order to decrease exposure to endocrine disruptors in dietary life were not high. The factors affecting the level of knowledge of endocrine disruptors were educational level, attendance in education for endocrine disruptors, and concern about endocrine disruptors. On the other hand, sex, age, concern about endocrine disruptors, and the level of knowledge of endocrine disruptors affected the level of behaviors to decrease exposure toward endocrine disruptors. The results of the path analysis showed that educational level and education of endocrine disruptors had an indirect influence on the level of behaviors in order to decrease exposure to endocrine disruptors in dietary life. The concern about endocrine disruptors had both a direct and an indirect effect on the level of behaviors to decrease exposure to endocrine disruptors in dietary life. In addition, sex, age and the level of knowledge of endocrine disruptors directly affected the level of behaviors to decrease exposure to endocrine disruptors in dietary life.

A Study on Hip Arthroplasty Patient Compliance of Medical Regimen (인공고관절 치환술 환자의 치료지시 이행에 관한 연구)

  • Purpose: The purpose of this study is to examine how well patients who had hip arthroplasty comply with medical regimens given to them after the operation. Method: The subjects of the study were patients who had arthroplasty at P Hospital between April 1, 2001 and August 30, 2002. 20 patients of the subjects experienced complications after the operation and the other 20 did not. Data from a survey using the qustionnaire were statistically analyzed in terms of real number, percentage point, mean and standard deviation by using $X^2$-test, t-test and ANOVA. Result: 1) the surveyed patients were significantly different in the compliance of medical regimen among them according to their education background as one of the subjects general characteristics. 2) It was found that the group of complication was higher in the compliance of medical regimen than that of non-complication. The two groups showed statistically significant difference with each other in the degree of compliance with therapeutic instructions than the experimental group in terms of the maintenance of abduction after the operation, training instructions on step-by-step basis, urination cotrol on bed, accurate use of crutch, compliance with medication, balance among medical treatment, training, leisure, rest and nutrition, instructions by physicians, nurses and physical therapists, use of low armchairs and toilet bowels and no bending of the body forward, and use of a non-operated leg in case of go upstairs or downstairs. Conclusion: It seems necessary to develop systematic and sessional education programs for improving the compliance of medical regimen, ultimately reducing complications following hip arthroplasty.

Effects of Fasting and High-fat Diet Feeding on Uncoupling Protein 3 mRNA Levels of Skeletal Muscle in Rats (절식과 고지방식 섭취가 골격근 UCP3 mRNA 발현에 미치는 영향)

  • PURPOSE. The purpose of this study was to investigate the effects of fasting and high-fat diet feeding on uncoupling protein 3 (UCP3) mRNA levels, uncoupling the respiratory chain and producing heat, of skeletal muscle in rat. METHODS. Fasting experiment: Forty Male Sprague-Dawley rats (5 wk) were divided into non-fasting groups (CON) and fasting groups (FG) for 0 day, 0.5 day (12 hr), 1 day, 2 day and 3 day. The rats of CON were sacrificed at 0 and 3 day. High-fat diet experiment: Forty Male Sprague-Dawley rats (5 wk) were divided into low-fat diet groups (LF) and high-fat diet group feeding for 0 day, 0.5 day (12 hr), 1 day, 2 day and 3 day. The rats of LF were sacrificed at 0 and 3 day. Analysis: Analysis of UCP3 mRNA expression was used by Real-time PCR. RESULTS. UCP3 mRNA levels of FG group were increased according to time course for 2 days- fasting but decreased at 3 day-fasting. UCP3 mRNA of HF were increased during HF diet feeding for 2 day, and peaked at 1 day-HF feeding, but decreased 2 day and 3 day-HF feeding CONCLUSION. Therefore, it may be rational that UCP3 is up-regulation when a large amount of fatty acids influx occurs in skeletal muscles as well as might have a role for fine adjustments of energy expenditure.

Recognition, Information Acquisition Behavior, Knowledge, Behaviors to Decrease Exposure and Education Need toward Endocrine Disruptors among Middle and High School Students (중.고등학생의 환경호르몬에 대한 인식도, 정보획득 행동, 지식, 노출저감화 행동 및 교육요구도 에 관한 연구)

  • The purpose of this study was to examine the level of recognition, information acquisition behavior, level of knowledge, level of behaviors to decrease exposure, and education need towards endocrine disruptors of middle and high school students. The data were collected from 180 middle and high school students in Seoul, Incheon, Daejeon, Daegu, Busan and Gwangju. Frequencies, Cronbach's alpha, t tests, analysis of variance, Duncan's multiple range tests, and chi-square tests were carried out by SPSS WINDOWS Version 14.0. The results of this study were as follows. The level of concerns about endocrine disruptors was somewhat low. Many respondents obtained information about endocrine disruptors from TV/radio and school, and wanted to get the information from school and TV/radio. Both the level of knowledge and the level of behaviors to decrease exposure were not particularly high. In addition, they showed high level of education need for endocrine disruptors.

Effect of Preexisting Musculoskeletal Diseases on the 1-Year Incidence of Fall-related Injuries

  • Objectives: People who have chronic diseases, as well as gait imbalance or psychiatric drug use, may be susceptible to injuries from falls and slips. The purpose of this study was to evaluate the effect of musculoskeletal diseases on incidental fall-related injuries among adults in Korea. Methods: We analyzed data from the 4th Korea National Health and Nutrition Examination Survey (2007-2009), which are national data obtained by a rolling survey sampling method. The 1-year incidence of fall-related injuries was defined by health service utilization within the last year due to injury occurring after a slip and fall, and musculoskeletal diseases included osteoarthritis, rheumatoid arthritis, osteoporosis, and back pain. To evaluate the effects of preexisting musculoskeletal diseases, adults diagnosed before the last year were considered the exposed group, and adults who had never been diagnosed were the unexposed group. Results: The weighted lifetime prevalence of musculoskeletal disease was 32 540 per 100 000 persons. Musculoskeletal diseases were associated with a higher risk of fall-related injury after adjustment for sex, age, residence, household income, education, occupation, visual disturbance, paralysis due to stroke, and medication for depression (odds ratio [OR], 1.41; 95% confidence interval [CI], 1.03 to 1.93). As the number of comorbid musculoskeletal diseases increased, the risk of fall-induced injuries increased (p-value for trend <0.001). In particular, patients who had any musculoskeletal condition were at much higher risk of recurrent fall-related injuries (OR, 6.20; 95% CI, 1.06 to 36.08). Conclusions: One must take into account the risk of fall-related injuries and provide prevention strategies among adults who have musculoskeletal diseases.

Relationships of Colorectal Cancer with Dietary Factors and Public Health Indicators: an Ecological Study

  • Background: Colorectal cancer (CRC) is the third most common cancer in Iranian women and fifth in men. The aims of this study were to investigate the relation of dietary factors and public health indicators to its development. Materials and Methods: The required information (2001-2006) about risk factors was obtained from the Non-Communicable Disease Surveillance Centre (NCDSC) of Iran. Risk factor data (RFD) from 89,404 individuals (15-64 years old) were gathered by questionnaire and laboratory examinations through a cross sectional study in all provinces by systematic clustering sampling method. CRC incidence segregated by age and gender was obtained from Cancer Registry Ministry of Health (CRMH) of Iran. First, correlation coefficients were used for data analysis and then multiple regression analysis was performed to control for confounding factors. Results: Colorectal cancer incidence showed a positive relationship with diabetes mellitus, hypertension, lacking or low physical activity, high education, high intake of dairy products, and non-consumption of vegetables and fruits. Conclusions: We concluded that many dietary factors and public health indicators have positive relationships with CRC and might therefore be targets of preliminary prevention. However, since this is an ecological study limited by potential ecological fallacy the results must be interpreted with caution.

A Study on How to Provide Support to Poor Single Families based on Case Studies (저소득 한부모가정, 사례에 비추어 본 지원방안 연구 : 건강가정지원센터 활용을 중심으로)

  • The purpose of this study is to identify the characteristics and the limits of, as well as, Propose an improvement of, the government based policies that support poor single families. For this purpose, this study has analyzed the government based supporting policies for poor single families into four different aspects; income support, dwelling support, medical support, and child-caring support. Also, in order to analyze the situation of the poor single family as well as the limits of the government based supporting policies, an in-depth interview has been conducted with 8 personnel (including 7 single parents and 1 social worker). In the final analysis, a total of 5 case studies have been used to identify the characteristics of the government based supporting policies for various poor single families. As a result, it turned out that the economic situation of the poor single families were extremely unfavorable, and the quality of life was extremely low in the aspects of dwelling, nutrition, health child nurturing and education. Therefore, we are proposing the following supporting policies for the improvement of these families' living conditions: increasing income levels, providing job opportunities, securing dwelling places, providing medical support, and implementing child care benefit policies. Furthermore, we are proposing an expansion of the human services provided by the healthy family support center to these poor single families.

Comparison of Salinity of Soups on the Use of Bluetooth Salinity Meter at Childcare Centers in Anyang Area (안양지역 어린이집에서의 블루투스 염도계 사용에 따른 국의 염도 비교)

  • We analyzed whether the use of the Bluetooth salinity meter could affect the proper level of salinity in childcare centers. This study was conducted on childcare centers that wish to participate in the sodium reduction project (Bluetooth salinity meter using group, BG) and those that did not participate (Bluetooth salinity meter not using group, NG) among childcare centers registered with Center for Children's Foodservice Management in Anyang from January to December 2019. As a result of comparing the salinity and sodium contents of the soup according to whether or not a Bluetooth salinity meter was used, the salinity and sodium contents of BG was significantly lower than that of NG (p < 0.0001). Salinity and sodium contents in soups tended to decrease as the number of use of the Bluetooth salinity meter increased (p = 0.020). In conclusion, Bluetooth salinity meter can be used as a means to induce voluntary salinity control by cookers regarding sodium reduction education at childcare centers.
