Nutritional Assessment of the Oldest-Old Population Aged 85 Over: Based on 2009 Korea National Health and Nutrition Examination Survey

  • As Korea is expected to become super high aged society in 2026, nutrition is important elements of health in the elderly and affects their life. In epidemiological surveys, anthropometric and biochemical measurements represent important components of nutritional assessment in elderly. In the present study, we investigated anthropometric index, biochemical markers, and nutrient intake for elderly aged over 85 by using data on 71 elderly (24 men, 47 women) over 85 years which was obtained from Korea National Health and Nutrition Examination Survey in 2009. According to BMI values, the prevalence of underweight and obesity was 9.7% and 34.5% in men, and 7.9% and 26.4% in women. The prevalence of sarcopenia was 70.7% in men and 25.0% in women. However, plasma biochemical data are normal range in the older population. In nutrients intake, daily Intake of energy, protein, dietary fiber, water, thiamin, riboflavin, niacin, calcium, and potassium was inadequate considering the Dietary Reference Intakes for Koreans(KDRIs). In conclusion, elderly need to be educated nutrition and their health dietary life to prevent malnutrition, and standard of nutrition intake should be rearranged in elderly aged 85 over considering their antrhopometric index.

일부(一部) 전남지역(全南地域) 여자중학생(女子中學生)들의 영양실태(營養寶態) 조사연구(調査硏究) - 전라남도(全羅南道) 장흥군(長輿郡)과 보성군(寶城郡)을 중심(中心)으로 - (A Study on Nutrition Intake of Middle School Girls in Chonnam Area)

  • The purpose of this study was to get a data which can provide a basis for future direction in nutritional education and to build up right eating habit and to find the way how to improve food life and the nutritional supply by evaluating the current nutritional intake of average middle school girls through the survey study of their daily food intakes and physical status. The survey of nutritional intakes and physical status in two girls middle schools, namely Jang'hung girls middle school at Jang'hung Up and Bo'song girls middle school at Bo'song Up, were carried by teachers majoring in Home Economics and nurse-teachers from 5 July to 7 July 1983. From a total of 1080 subjects of two girls middle schools, 887 subjects were surveyed. The results obtained from survey are summarized as follow: 1) Physical Status Average height, weight and chest girth of subjects in both middle schools were much lower than the Korean average standard (p<0.005${\sim}$p<0.001) and t-score of differences between two middle school girls were not significant. Rohrer index, nutrition rate and physique rate were also lower than Korean averag standard through all the ages in both girls middle schools. Menarche appeared mainly between the age of 13 to 15 (91.55 %), with a mean age of 14 (35. 01 %) and which was experienced by the subjects of 51.52 percentage among total surveyed Iniddlf school girls. 2) Nutritional Intakes The average daily intakes of all nutrient except iron, vitamin A, thiamin, ascorbic acid and niacin was lower than Korean Recommanded Dietary Allowances (RDA) . Especially average intake of calorie and calcium were much lower than Korean RDA(p<0.001) while average intake of thiamine and ascorbic acid were much higher than Korean RDA (p<0.001). T-score of differences of iron, thiamin, riboflavin between two middle school girls were very significant (p<0.001). Generally iron and riboflavin were the highest rate of intakes through three meals and breakfast took charge of considerable amount of all nutrient intake without omitting anything (about 30 % of daily nutrient intake). The nutrients of which ratio of nutrient intake by snack was more than 16.7 % were vitamin A, thiamin, riboflavin, ascorbic acid etc. and snack foods which were intake frequently in both middle school girls commonly were fruits, milk and milk products, bread and biscuits in sequence. 3) Correlations and t-score between physical status, menarche and nutrient intakes. Average intake of calorie at Jang'hung area was positively correlated with height, weight, nutrition rate and physique rate and this correlation was very significant (p<0.005). Average intake of calorie was also correlated with chest girth and Rohrer index (P<0.05). At Bo'song area, averse intake of riboflavin was negatively correlated with all the items of physical status (p<0.05-p<0.005), vitamin A was negatively correlated with chest girth and Rohrer index (p<0.05 respectively) and thiamin was also negatively correlated with chest girth, Rohrer index and nutrition rate (p<0.05 respectively). Especially niacin showed a negative cor-relation with all the items of physical status very significantly (p<0.005).The time of menarche showed a positive correlation with iron in both middle school girls commonly P<0.05 at Jang'hung, p<0.10 at Bo'song).

인천 지역 여대생의 체질량 지수와 체형 인식에 따른 식행동 및 영양소 섭취 상태 분석 (A Study on Food Behaviors and Nutrient Intakes according to Body Mass Index and Body Image Recognition in Female University Students from Incheon)

  • The purpose of this research was to study food behaviors and nutrient intakes according to body mass index(BMI) and body image recognition in 211 female university students in Incheon. The majority of the surveyed subjects were in the normal range for BMI. Most of these female university students preferred a thin body type and were not satisfied with their body shapes. Most also controlled their body weight for appearance by exercising and limiting food intake. The obese group often ate processed foods(p<0.01) and ate out(p<0.05). The underweight group and normal-weight group took vitamins and supplements more frequently(p<0.001). Those students who recognized their body image as a 'fat shape' had good appetites(p<0.05) and those who recognized themselves as 'normal shape' considered food formulations ate every meal (p<0.05) and consumed vitamins and other supplements frequently(p<0.01). The mean calorie intake of all students was about 80% of the recommend amount, so there was a risk of lack of various nutrients. The 'fat' students by body image recognition had lower calorie intakes in order to reduce body weight. In all the groups, there was a lack of vitamins $B_1$ and $B_2$, as well as the minerals Ca and Fe. Therefore, education on correct body image recognition and training on proper weight control are deemed necessary. In addition, education and counseling on nutrition that are tailored to individual status are needed.

Dietary Problems among Middle-Aged Japanese Men

  • Balanced intake of appropriate nutrients is the key to sustaining and promoting health as well as preventing and treating diseases. It is not always easy, however, to take balanced nutrition and various related factors must be taken into consideration. This report provides a summary of studies that have examined the nutrient intakes of Japanese middle-aged men and the relationship of this intake to drinking, job-related separation from families, and health practices. The alcohol consumption of Japanese middle-aged men has more than tripled in the last forty years. When nutrient intake was examined in relation to alcohol consumption, it was discovered that the maximum acceptable alcohol consumption was approximately 23 grams (about two drinks) of pure alcohol, provided the level of nutrient intake for drinkers was equal to that of non-drinkers. The alcohol energy ratio was approximately 5%. It was also discovered that middle-aged men's eating habits deteriorate when they relocate to new posts without their families and live by themselves. Compared to those living with their families, a higher proportion of those living alone have unfavorable eating habits including skipping breakfast or lunch, having a late lunch, and eating and drinking after dinner until bedtime. When Breslow's seven health practices, nutrient intake, and consumption weight by food group were examined, it was discovered that the group that had many beneficial eating and living habits consumed plenty of legume, pulses, fruit, green yellow vegetables and milk products. Their intake of vitamins and minerals was high and the results of a physical examination proved to be excellent. According to nutrition surveys conducted in Japan, China, the United Kingdom and the United States using a 24-hour recall method with common protocols and strict controls to ensure high levels of accuracy and cross-study validity, the Japanese had the highest cholesterol intake and the lowest dietary fiber intake among the four countries. Also, the alcohol energy ratio of the Japanese exceeded 8%, the highest among the four countries, while their intake of magnesium and iron was the lowest These results indicate that it is necessary to enhance nutritional education for middle-aged men and to reinforce the social environments in which they live and work in order to promote proper diet and nutrition in Japan.   • This study examined the effects of mulberry-leaf powder Tofu(MPT) on the anthropometric measurements, bone mineral density(BMD), biochemical markers of bone turnover, serum lipid levels of and macrominerals status in 30 female university students living in Choongnam with low dietary Ca intake was low(mean daily $intake=524.66{\pm}21.47$ mg/day). The subjects received the MPT supplemented(100 g/day) for 4 weeks. The nutrient content per 100 g MPT was 84.10 kcal(energy), 8.98 g(protein), 0.53 mg(fiber), 211.33 mg(Ca) and 1.59 g(fat). Anthropometric measurements, 24-hour dietary recall, carpus BMD using DEXA, select BMD biomarckers(serum alkaline phophatase activity, osteocalcin, urinary deoxypyridinoline), serum protein levels, albumin, and glucose levels, lipid profiles(cholesterol, triglyceride, HDL-cholesterol, LDL-cholesterol) and serum macrominerals(Ca, Ca/P ratio, Mg) were analyzed before and after MPT supplementation. Following MPT supplementation, there was no significant differences in anthropometric measurements, nutrient intakes, carpus BMD, serum alkaline phosphatase activity, osteocalcin, and urinary deoxypyridinoline, serum levels of protein, albumin, and glucose or lipid profiles with AI, HTR, LHR and CRF. However, there were significant increases in dietary calcium as well as decreases in serum triglycerides with MPT supplementation. Although further research is needed, these results that regular intake MPT could prevent bone and cardiovascular diseases.

위절제 환자들의 혈액의 생화학적 지표, 체중 및 영양소 섭취 변화에 관한 추적 연구 (Changes in Nutritional Status according to Biochemical Assay, Body Weight, and Nutrient Intake Levels in Gastrectomy Patients)

  • The purpose of this study was to investigate the changes in nutritional status of gastrectomy patients. The anthropometric and biochemical data were measured at pre-operation, at discharge, 1 month after discharge, and 3 months after discharge. Nutrient intake levels, nutrients adequacy ratio (NAR), mean adequacy ratio (MAR), and the proportion of patients with intake levels inferior to those of dietary reference intakes (DRIs) were analyzed at discharge, 1 month after discharge, and 3 months after discharge. Finally, the data on 23 patients (15 male and 8 female) were collected and used for statistical analysis. Fifteen patients underwent subtotal gastrectomy, and 8 patients underwent total gastrectomy. Compared to pre-operation, body weight and body mass index of subjects significantly decreased at discharge, 1 month after discharge, and 3 months after discharge (P<0.001). Serum albumin (P<0.001), total lympocyte count (P<0.001), total cholesterol (P<0.001), hemoglobin (P<0.001), hematocrit (P<0.001), and mean corpuscular hemoglobin concentration (P<0.05) were significantly different between pre-operation, at discharge, 1 month after discharge, and 3 months after discharge. The proportions of patients with lower nutrient intake levels than DRIs were substantial. MAR at discharge, 1 month after discharge, and 3 months after discharge were 0.70, 0.80 and 0.91, respectively. Especially, the NARs of folate, niacin, vitamin $B_2$, vitamin C, and zinc were all low. Considering the various nutritional problems of gastrectomy patients, systematic medical nutrition therapy is needed after gastrectomy.

어린이들의 식습관이 비만도와 혈청 지질 수준에 미치는 영향 (Eating Behavior, Obesity and Serum Lipid Levels in Children)

  • Interactive effects among eating behavior, obesity and serum lipid levels were studied in 117, 4~12 year old children residing suburban Seoul. Fasting blood samples are obtained and analyzed for serum triglycerides (TG), total cholesterol(TC), high density lipoprotein-cholesterol(HDL-C) and hematochrit. Obesity was determined by weight for length index(WLI)and the information on eating behavior including food habits and dietary intakes was obtained by questionaire using food record method for 2-consecutive days. Over 40% of children was classified overweight or obese by WIL and children's physical parameters were closely related to those of parents implying genetic influence on obesity. Although it did not reach the statistical significance, there was a tendency of higher TG, TC and low density lipoprotein-cholesterol(LDL-C)levels among girls compared to boys. Blood lipid levels of obese children were similar to those of other groups except TG, which was significantly higher(p<0.05) in obese group. Nutrient intakes seemed adequate in all subjects except iron, calcium and total calorie which were lower than RDAs. Lacking significant relationship between individual nutrient intake and obesity, there was significant correlation between food intake and blood lipid level especially in 10-12 year old group. Vegetable intake was negatively related to TG, LPH(LDL-C/HDL-C) and atherogenic index(AI), and positively to HDL-C. Skipping breakfast and frequent eating out appeared to cause imbalances in nutrient intake. These findings clearly revealed the influence of eating behavior on childhood obesity along with blood lipid profile. To ensure the proper growth and health of these children, devising method and developing media for nutrition education suited to our society should be accomplished first. With well-planned nutrition surveys and thorough intention, childhool obesity could be prevented from progress into adulthood obesity.

24시간 회상법을 사용한 한국 성인의 식이섭취조사에서 재회상 단계 추가의 영향 분석 (Analysis of Foods and Nutrients Intake Obtained at the Final Probing Step in 24-hour Recall Method)

  • This study was conducted to examine the usefulness of adding final probing step (step3) in dietary assessment by 24-hour recall method among Korean adults. One-hundred fifty five adults (37 males and 118 females) above 30 years of age who visited hospitals for health examination were recruited at three hospitals in Korea. One day dietary intake was obtained using 24-hour recall method from each subject. Dietary interview was conducted in 3 steps, (1) quick list of foods eaten during the previous day, (2) detailed information of all the foods eaten, (3) the final probing for any items forgotten. Items added at the step3 were identified and contributions of energy and nutrient intakes were calculated. The average duration of interview was 10.5 min, and time spent for each step was 4.12 minute for step 1, 5.62 minute for step 2, and 38 second for step 3. The average number of dishes reported by the subjects added at the step 3 was 2.2. (Males = 2.6, Females = 1.6) Frequently reported dishes in the step 3 were Beverage, Tea, alcohol (37.1%) and Fruits (31.8%). From mean total energy intake of 1,589 kcal (Men = 1,846 kcal, Women = 1,509 kcal), 179 kcal (11.3%) was added at the step 3. In the step 3, nutrient intakes increased significantly except retinol in total subjects and except retinol and cholesterol in males but all nutrients increased significantly in females. The final probing step can add significant information on intakes of foods and many nutrients with only about 38 seconds of interview time. Confirmation of the results with larger samples of different age groups is needed.

대구지역 임신부의 식품 및 영양섭취실태 (The Dietary Intake of Pregnant Women in Daegu Area)

  • This study was carried out to figure out the dietary intake of pregnant women in Daegu area. A total of 467 pregnant women was recruited at two hospitals in this area to collect the information on dietary intakes, general characteristics, and anthropometric measurements using an interview. Forty-seven women (10.1%) were in the first trimester, 101 women (21.6%) in the second trimester, and 319 women (68.3%) in the third trimester. The mean age, prepregnant weight and height of subjects were not significantly different among the trimester groups. The mean daily nutrient intakes of energy, fat, protein, carbohydrates, phosphate, sodium, vitamin B$_1$, vitamin B$_2$, and vitamin C were significantly different among the trimester groups, but the other nutrients were not different. For the calcium and iron, the percentage of subjects consumed less than 75% of the Korean RDA was over 80%, and for other nutrients such as energy, vitamin B$_1$, and vitamin B$_2$ over 50% of subjects consumed less than 75% of the Korean RDA in the first trimester. The correlations between nutrient intakes and weight gain during pregnancy showed different patterns among the trimester groups. The mean adequacy ratio and dietary variety scores were not significantly different among the trimester groups. The amount of food intake from cereals and cereal products were higher during the second and the third trimester compared to the first trimester, but those of the other food groups were not significantly different. In conclusion, nutrient intakes of the pregnant women for iron, calcium and some vitamins were inadequate and nutritional management programs for these nutrients are necessary. 농촌지역 노인들의 사상체질에 따른 영양상태 (A Study on the Nutrient Intake According to Sasang Constitutions for the Elderly in a Rural Community)

  • The purpose of this study was to investigate the food habits and nutrient intake, among the elder according to their Sasang constitutions. The investigation group was composed of 79 elder men and 117 elder women in Sungjugun Kyungsangbukdo. Data were collected from 1 July, 2002 to 31 July, 2002. In conclusion, diet habits and nutritional intakes were poor in the elderly older than 65 years of age so that they need regular food intake habit and balanced nutrition to obtain enough nutrients to maintain healthy lives. And the following suggestions are made. Firstly, multi-directional studies are needed to improve nutritional intakes, prevent diseases, and improve quality of life in the elderly. Secondly, in-depth studies are needed with various and more people related with Sasang constitutions. Thirdly, we need to screen for risk factors of poor nutrition in each constitution for systemic maintenance of nutritional status and health management in the ever increasing elderly population. For this purpose, an objective investigative tool befitting the situation in Korea should be developed. Fourthly, comparative studies on diet habit and nutritional status are needed by investigating the relationship between Sasang constitutions and diseases. Fifthly, food menu should be proposed according to Sasang constitutions to be utilized as basic data of longevity study.
