신생아중환자실 간호사의 업무 스트레스, 이직의도 및 소진에 관한 연구 (Work Stress, Turnover Intention and Burnout among Nurses in Neonatal Intensive Care Units)

  • Purpose: This study was done to identify differences in work environment, work stress, turnover intention and burnout and investigate the relationship among these variables in nurses in Korean Neonatal Intensive Care Units. Method: Participants were 242 nurses working in 13 general hospitals. Burnout was measured by the Maslach Burnout Inventory (MBI), work stress with the instrument by Gu & Kim (1994), and turnover intention with the scale by Kim & Lee (2001). Size of the NICU, nurse to patient ratio, and communication satisfaction were included in work environment. Data were analyzed using Pearson correlation coefficients and multiple regression analysis with SPSS WIN program. Results: The mean score for work stress in NICU nurses was 3.43 points, for burnout, 2.72 points, and for turnover intention, 4.64 points. Burnout and turnover intention level of participants were moderate-high. Work stress, communication dissatisfaction with physician, and clinical career accounted for 33% of variance in burnout. Significant differences were found between size of NICU and staffing related to environmental characteristics in turnover intention and burnout. Conclusions: Results indicate that effective communication with coworkers and institutional support for appropriate staffing according to number of beds will help to prevent work stress, burnout, and ultimately, nurses' resignations.

중소병원 간호사의 간호전문직관, 학습민첩성, 간호근무환경이 간호업무성과에 미치는 영향 (The Influence of Nursing Professionalism, Learning Agility, and Nursing Practice Environment on Nurses' Performance in Small and Medium Hospitals)

  • Purpose: This study attempted to discern the factors that influence nursing professionalism, learning agility, and the nursing practice environment on the performance of nurses. Methods: Data were collected from 202 clinical nurses who both consented to participate and who have worked for more than one year in one of five small- and medium-sized hospitals located in three regions of Korea. The data were analyzed using the SPSS/WIN 26.0 statistical programs. Results: The nurses' performance showed a statistically significant correlation with nursing professionalism (r=.50, p<.001), learning agility (r=.54, p<.001), and nursing practice environment (r=.37, p<.001). Factors affecting the results of nurses' performance in relation to the subjects are those of learning agility (β=.33, p<.001), nursing professionalism (β=.25, p<.001), clinical career (β=.24, p=.001), education level (β=.16, p=.011), and nursing practice environment (β=.15, p=.016). Conclusion: To improve the performance of nurses in medium-sized hospitals, it is necessary to develop a nursing practice environment, programs, and strategies for enhancing nursing professionalism and learning agility.

종합병원 간호사의 재직의도에 미치는 영향요인 (Factors Associated with General Hospital Nurses' Intention to Remain)

  • Purpose: This study aimed to investigate the relationship between general hospital nurses' work environment, resilience, and intention to remain, as well as to identify the factors associated with nurses' intention to remain. Methods: Study participants were nurses at a general hospital in Jeollabuk-do, South Korea. Data collection was undertaken from July 6th to 20th, 2017, through questionnaire responses from 257 nurses. Descriptive statistics were employed for data analysis, including: independent t-test, one way ANOVA, Pearson's correlation coefficient, and multiple regression analysis using SPSS/WIN 22.0. Results: Nurses' work environment, resilience, and intention to remain were found to have a statistically significant correlation. Intention to remain showed statistically significant positive correlations with the nurse's work environment (r=.72, p<.001), resilience (r=.50, p<.001). Factors found to influence intention to remain were work environment, resilience, job satisfaction, and health status. Conclusion: The study findings support the development of a specialized program to strengthen nurses' intention to remain. During the program's formulation, it is necessary to improve nurses' work environment and find ways to bolster the resilience of individual nurses.

간호근무환경, 공감피로 및 공감만족이 간호사의 소진에 미치는 영향 (The Influence of Nursing Practice Environment, Compassion Fatigue and Compassion Satisfaction on Burnout among Clinical Nurses)

  • Purpose: The purpose of this study was to identify the influence of nursing practice environment, compassion fatigue and compassion satisfaction on burnout among clinical nurses in Korea. Methods: A descriptive crosssectional study was conducted. The sample consisted of 210 clinical nurses from three tertiary hospitals located in Seoul. Measurement instruments included the Korean version of the Practice Environment Scale of Nursing Work Index (K-PES-NWI) and Stamm's professional quality of life (ProQOL). Descriptive statistics, t-test, ANOVA, correlation analysis, and multiple regression were used to analyze the data. Results: Nursing practice environment had moderate negative influence on burnout (r=-.38, p<.001). Compassion fatigue had strong positive influence on burnout (r=.50, p<.001), while compassion satisfaction had strong negative influence on burnout (r=-.61, p<.001). The regression model explained 63% of variance of burnout and the compassion satisfaction was the most influencing factor for nurses' burnout. Conclusion: This study identified nursing practice environment, compassion fatigue and compassion satisfaction as influencing factors for nurses' burnout. Strategies to decrease compassion fatigue, enhance compassion satisfaction and create better nursing practice environment are recommended to decrease nurses' burnout.

병원규모에 따른 간호사의 간호업무환경, 전문직업성과 직무만족의 비교 (A Comparative Study on Nursing Practice Environment, Professionalism, and Job Satisfaction according to Hospital Size)

  • Purpose: This study was conducted to investigate and compare the characteristics and relations of nursing practice environment, professionalism and job satisfaction among nurses in general hospitals according to hospital size. Methods: The participants included 314 staff and charge nurses who were working in the general medical/surgical nursing units in one large hospital, three medium sized hospitals, and four small hospitals. Data collected through using self-report questionnaire were analyzed using the SPSS and SAS statistical programs. Results: Nursing practice environment and job satisfaction had significant differences according to hospital size. Both of these scales were highest for medium hospitals and lowest for small hospitals. For all hospital sizes there were positive correlations between each of the variables. Multiple regression analysis showed that both nursing practice environment and job satisfaction were affected by hospital size, but professionalism was not. Conclusion: The results of this study indicate that nursing practice environment and job satisfaction vary with the size of the hospital. Therefore, further study is necessary to identify the work environment variables of nurses for performance management and to implement appropriate policies.

교정간호사의 직무 스트레스 원인에 대한 내용분석 (Content Analysis of Correctional Nurses' Job Stressors)

  • Purpose: The purpose of this study was to identify correctional nurses' job stressors. Method: The data were collected from 51 correctional nurses using a self-completion questionnaire. To analyze data, content analysis was done using an analysis scheme developed by the investigators. Results: 160 significant statements were analyzed and categorized. The analysis scheme consisted of 8 categories and 23 subcategories. 8 categories analyzed by the investigators were as follow: relationship with other corrections staffs, overloaded work, career development, role conflict, work environment, relationship with patients, legal & ethical dilemma and administration environment. Conclusions: The correctional nurses' job need legal support from society and systematic support to improve nurses' job satisfaction.

병원 간호사의 업무환경, 감정노동이 이직의도에 미치는 영향 (The Effects of Work Environment, Emotional Labor on Turnover Intention by Hospital Nurses)

  • Purpose. The objective of this study was to investigate factors influencing turnover intention index of work environment, emotional labor by hospital nurses Methods. A survey was conducted with 290 nurses who were working hospital. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients and multiple regression with SPSS window 18.0 program. Result. Findings revealed that; 1) The mean scores were 2.81 [range : 1-5] for work environment, 3.24 [range : 1-5] for emotional labor, 3.43 [range : 1-5] for turnover intention. 2) Pearson's correlation revealed work environment was negatively correlated with turnover intention but emotional labor positively correlated with the turnover intention. 3) Hrarchical multiple regression analysis showed work environment, ages, assign ward, period in current hospital, emotional labor significantly influenced turnover intention in hospital nurses. Conclusion. These findings can be utilized to development of strategies to improving the work environment, managing the emotional labor for decreasing turnover intention of hospital nurses so that can be long time.

종합병원 간호사의 업무몰입 관련요인 (Work Engagement and Associated Factors among General Hospital Nurses)

  • 본 연구는 종합병원 간호사의 업무몰입 관련 요인을 파악하고자 실시한 서술적 조사연구이다. 연구의 대상자는 D광역시와 K지역에 위치한 3개의 종합병원에 재직하고 있는 114명의 간호사이다. 수집된 자료는 IBM SPSS Statistics 25 프로그램을 사용하여 descriptive statistics, t-test, one-way ANOVA, Pearson's correlations, stepwise multiple regression으로 분석하였다. 본 연구에서 업무몰입의 평균평점은 3.08점이었으며, 감정노동 하부요인 중 전문직 감정조절 노력은 3.82점, 대상자 중심 감정 억제는 3.28점, 규범에 의한 감정 가장은 3.22점이었으며 간호근무환경은 2.59점이었다. 업무몰입은 감정노동 하부요인 중 전문직 감정조절 노력(r=.30, p=.001), 대상자 중심 감정 억제(r=.20, p=.030)와 유의한 양의 상관관계를 보였고, 간호근무환경과도 유의한 양의 상관관계(r=.21, p=.024)가 있는 것으로 나타났다. 업무몰입은 연령, 직위, 전문직 감정조절 노력과 관계가 있었으며 이들 세 변수는 업무몰입을 20%(F=10.37, p<.001)설명하였다. 따라서 업무몰입 향상을 위한 전략 수립 시 연령과 직위를 고려해야 하며, 감정노동이 발생할 수 있는 상황에서 전문직 감정조절 노력을 강화할 수 있는 전략을 찾는 노력이 이어져야 할 것이다.

공감만족-공감피로(CS-CF) 모델에 근거한 임상간호사의 소진 구조모형 (Structural Equation Modeling on Burnout in Clinical Nurses based on CS-CF Model)

  • Purpose: The purpose of this study was to construct and test a structural equation modeling on burnout of clinical nurses based on CS-CF model. Methods: A survey using a structured questionnaire was conducted with 557 clinical nurses. Data were analyzed using structural equation modeling. Results: The modified hypothetical model yielded the following ${\chi}^2=289.70$, p<.001, RMSEA=.09, GFI=.93, TLI=.91, CFI=.94, PCFI=.65, AIC=363.21, SRMR=.05 or less and showed good fit indices. Nursing work environment, patient safety culture and resilience showed indirect effects on burnout while compassion fatigue and compassion satisfaction had direct effects. Conclusion: Results of this study suggest that compassion fatigue must be decreased and compassion satisfaction has to be increased, while burnout is lowered by enhancing the clinical nursing work environment, patient safety culture and resilience. In addition, more variables and longitudinal studies are necessary to validate the clear cause-and-effect relationship between the relevant variables.
