Outcome Indicators of Quality Nursing Care (질적 간호의 결과적 지표)

  • This study was designed to obtain basic data for development of evaluation tool which would be needed to measure the outcome of general quality nursing care of individual patient. The purpose of this study was to analyze and classify the outcome indicators of quality nursing care. The 29 articles of quality nursing care and outcome measures were selected coveniently, and analyzed to classify the outcome indicators of quality nursing care using open coding method. The results of this study were as follows: 1. Quality nursing care was defined as level of excellence of nursing care to achieve good patient outcome. 2. The 6 domains of which were health status, satisfaction, self care, patient progress and prognosis, and compliance were identified in outcome indicators of quality nursing care 3. Seven indicators of health status domain which were perceived health status, quality of life, well-being, daily activities, physical-physiological status, psychoemotional status, and social role functioning were identified. 4. Two indicators of satifaction domain which were patient satisfaction and family satisfaction were identified. 5. Three indicators of self care domain which were skill, knowledge, and home management were identified. 6. Seven indicators of patient progress and prognosis domain which were change of clinical status, resolution of nursing diagnosis and problem, days of stay, dicahrge state, recovery state, survival were identified. 7. compliance with therapeutic direction compliance was identified as an indicator of compliance domain. 8. It was sugested that studies for development of evaluation tools for outcomes of quality nursing the results of this study could be executed

The Current Use of Human Patient Simulators in Nursing Schools in Korea (간호교육에서의 환자시뮬레이션 교육 현황)

  • Purpose: This study was conducted to explore the current usage of high integration mannequin human patient simulators (hereafter HPS) in nursing education. Methods: Thirty-two faculty members in 11 nursing schools participated in this survey. the questionnaire consisted of some multiple-choice questions and open questions to collect data on the experience of faculty members with using HPS as well as quantitative information. Results: The faculty members who used HPS in their classes recognized the need to use simulation in nursing education. They identified the advantages of simulation education as promoting adaptation to the clinical environment(46.9%), promoting self-confidence(28.1%), promoting critical thinking(28.15), and making direct nursing skill practice available(28.1%). However, they considered the obstacles to the use of simulation in nursing education to be insufficient manpower(62.5%), insufficient time(40.6%), and difficulty in preparing a teaching strategy(34.4%). Conclusion: HPS is useful in nursing education, but systematic plans and policies regarding on its operation are needed to confirm its effectiveness. The results of this study generally provide a basic information for use of HPS in nursing education.

An Analysis of Research on Nursing Practice Education in Korea (간호학 실습교육에 대한 국내 연구현황 분석)

  • Purpose: This study was done to propose the developmental direction of study related to nursing practice education by grasping the trend of study in fundamental and clinical practice. Method: A total of 48 research articles which were published on nursing practice education in Korea from 2002 to 2009 were analyzed with structured analysis forms. Result: Most research was related to clinical practice (n=40). A high percentage of non-experimental research design was related to fundamental practice (75%) and clinical practice (65%). Qualitative research was only used in clinical practice (n=8). Nursing students were predominantly selected as an object of research in fundamental practice (n=6) and clinical practice (n=32). In addition, many of the areas in clinical practice were a general clinical setting without any classification of the specific area. The concepts of research in fundamental practice were related to competency in basic nursing skill and most concepts of research in clinical practice were associated with satisfaction, stress, experience, critical thinking and problem solving ability of the nursing student. Conclusion: There's something to be desired in nursing research related to instructor methods, teaching-learning methods and nursing education programs. Therefore, more specific and continuous research focused on these topics to improve clinical nursing competence of the nursing student is needed.

  • The practical learning experience is the heart of nursing's professional program of study, have been the most widely disscussed and yet the least studied of all nursing education activities. The major goal of practical learning experience is to provide opportunities in realistic work settings that permit the nursing student to develop the knowledge, skill It attitudes of a beginning practitioner. The purpose of this study is to identify the experience of practical learning in the children's rehabilitation center by nursing students, to provide basic information for developing practical learning in pediatric nursing education. The study design was a descriptive study. Data were collected 74 cases of journaling which came from senior nursing students who cared for the handicapped children in P rehabilitation center from 24, August, 1997 to 29, Decmber, 29, 1998. The collected data were analyzed using the content analysis by Kim & Lee(1986), Kim(1987). The Present Status and Future Direction of Practicum of Fundamentals of Nursing in Baccalaureate Nursing Programs (4년제 간호대학(과) 기본간호학 실습교육의 현황과 발전방향)

  • Purpose: The purpose of this study was to analyze the present status of practicum of fundamentals of nursing in baccalaureate nursing programs. Method: This study used a descriptive design surveying status of practicum of fundamentals of nursing from 47 schools. The instruments developed by investigator based on Shin's(2003) findings. The data collected were analyzed by SAS and the content analyses. Result: 1) The aims of fundamental nursing practicum were 'the acquirement of knowledge on fundamental nursing skill and attitude, and their implementations'. 2) 45 schools had their own practicum labs and the number of a group was 5 or 6. The mean number of student was 37.8 and the mean size of the labs was 169m2(${\pm}\;93.6$). 3) The credit of lecture and practicum ranged from 3 to 4 and in 32% schools, one professor and one teaching assistant were responsible for the practicum of fundamental nursing. 4) The teaching methods of practicum were shown as demonstration, role play, case study presentation, special lecture, video, computer simulation, and so forth. 5) The practice evaluation was used practice examination and students' self evaluation. Conclusion: The studies require of the contents and the evaluation method corresponding to the course objectives.

A Study on Evaluation of an RN-BSN Program in a Nursing School (일 대학의 RN-BSN 교육과정 평가)

  • Purpose: The purpose of this study was to evaluate the overall RN-BSN program, current curriculum, and student outcomes in one nursing college. Method: A cross- sectional survey was used for current students (n=152) and graduates (n=54). Three different instruments were used in this study; K-Regis University RN-BSN Program Evaluation Tool, Curriculum Evaluation Tool, and Outcome Evaluation Tool. Result: Results showed that overall the program was satisfying to current students and graduates. However, curriculum evaluation showed that clinical practicum and nursing skill courses were generally unsatisfying, indicating further changes in these areas. Graduates showed higher leadership and professionalism than current students in the outcome evaluation. Conclusion: The current study indicates a need for further studies such as review of the RN-BSN program philosophy and education method, or program evaluation by periods. Additionally, there is a need for a strategic plan to develop the RN-BSN program in the future.

The Experience of Nursing Staff on the Dementia Patients′ Aggressive Behavior (간호제공자들의 치매노인 공격행동 경험에 대한 연구)

  • Providing care to the dementia elderly with behavioral problem is a major issue in nursing homes today. This study was aimed to explore the nursing staffs′ response to aggressive dementia patients, and the effect that the aggressive behavior had on Nsgstaff. The interviews used a semi-structured questionnaire are were carried out from May to July, 1999. The subjects were 23 nursing personnel working in the nursing homes for dementia elderly. The result are as follows; 1. The types of aggressive behavior cited by the subjects was "physical", "linguistic", and "sexual". 2. The factors that caused the behavior were "symptoms of disease", "under- conditioning", "context of nursing care", "unsatisfied need", "relationship to other patients", "change of outer environment", and "invasion of one′s own privacy". 3. The response of the subjects were "unhappiness", "stress", "anger", "exaggeration", "anxiety", and "fear". 4. The management strategies listed by nursing personnel used to alleviate aggressive behavior were "ignorance" "patience" "leaving the area" "soothing sounds" "verbal punishment" "restriction and isolation" and "various management skill appliance" "adaptation". 5. The effect that aggressive behavior and were "disturbance of relationship to elderly" "decline in the amount and quality of nursing care" "interruption of task performance" "job exhaustion" "desire to leave the job" "physical impact" "stress on the cognition of others" "anxiety about health and one own future" "interference to family life′. The findings of this study will be useful in understanding the difficulties of nursing personnel when confronting the aggressive behavior of dementia partients. It also is useful as basic data in preparing efficient intervention program for these difficulties.

The Effects of a Communication Training Program on Communication and Interpersonal Relationships of Nursing Students (의사소통 훈련 집단프로그램이 간호대학생의 대인 의사소통과 인간관계에 미치는 효과)

  • Purpose: The present study was to examine the effects of a communication training program on nursing students' interpersonal communication and relationships. Methods: This study was quasi-experimental research using a non-equivalent pre-post design. The subjects were 40 sophomore nursing students who were divided into an experimental group of 20 members and a control group of 20 members. The communication training program consisted of 8 sessions of small group training and each session lasted 120 minutes. A pretest, posttest and follow-up test (6 months after the program was terminated) were conducted to measure Inter-personal Communication Inventory and Relationships Scale. Data was analyzed by $X^2$-test, independent t-test, paired t-test and Friedman test with SPSS. Results: The communication training program was very effective in enhancing the nursing students' communication and interpersonal abilities. Nursing students' communication ability in the experimental group, however, decreases after 6 months while there is no change in the control group. Nursing students' interpersonal relationships in the experimental group was maintained at the follow-up test. Conclusions: This study supports that a communication training program has a positive effect on improving the nursing students' communication skill and interpersonal relationships.

Development of a Tool for Predicting Nursing Times in Occupational Accident Patients (산재환자 간병시간 예측 도구개발)

  • Purpose: This study aims to develop a tool to predict occupational health patients' nursing needs or nursing time. Method: A tool was composed based on the materials and opinion of experts, and to verify the reliability and validity of the tool, nursing time of total 311 inpatients in 2 occupational health hospitals were surveyed. Results: The developed tool was total 151 items and composed of physical function, cognitive function, communication, emotional and disturbance behavior, nursing skill, and rehabilitation need. The reliability of the tool was high, showing Cronbach's alpha coefficient of 0.95. But several items should be deleted for low corrected item-total correlation. According to the multiple regression analysis, physical function and rehabilitation need were shown to be the most powerful predictive variables for nursing time. Conclusion: The developed tool was appeared to have high reliability and validity. But further refinement of the tool is needed.

Exploratory Study on Scales of Nursing Education Outcome (간호교육 성과 측정 도구의 탐색)

  • Purpose: The purpose of this study was to identify and evaluate outcome measurement scales of nursing education such as communication, professionalism, leadership, and critical thinking. Methods: A methodological study design was used to evaluate the reliability and validity of the scales. Survey was done to the students (n=407) enrolled in baccalaureate nursing schools. Using convenience sampling method, we tried to include all grades of students. Internal consistency, convergent validity and group comparison were used to evaluate the reliability and validity of the scales. Results: All scales were reliable and valid. Only convergent validity of the scale to measure critical thinking was relatively low. It suggested that critical thinking skill and disposition might be measured separately. Also it was inefficient to measure the nursing education outcomes with separate scales. Conclusions: It is recommended to develop a new integrative scale to measure nursing education outcomes. Also it is necessary to set the norm of nursing students to evaluate nursing education outcomes for the quality control of nursing education.
