Factors Influencing Basic Nursing Skill Competency in Nursing Students (간호대학생의 기본간호수기 수행능력의 영향요인)

  • Purpose: This research was conducted to identify major factors that influence competency for nursing students in basic nursing skills. Method: Data were collected by questionnaires from 290 first year students in one college of nursing. The instrument tools included motivation for admission to nursing, concern about fundamentals of nursing, perceived importance of fundamentals of nursing, level of active participation in fundamentals of nursing, number of times to use open Lab, Lab class satisfaction, self-efficacy, self-evaluation and student attitude. Results: There was no significant correlation between competency in basic nursing skills and motivation for admission to nursing, concern about fundamentals of nursing, perceived importance of fundamentals of nursing, or number of times to use open Lab. Competency in basic nursing skill in these students showed a significantly positive correlation to Lab class satisfaction, self-efficacy, self-evaluation, student attitude and level of active participation in fundamentals of nursing. The major factors that influenced competency in basic nursing skills for nursing students were student attitude, self-efficacy and number of times to use open Lab. These factors explained 15.5% of the variance in competency. Conclusion: Basic nursing skill practice education programs should be developed to improve self-efficacy and active participation.

Measurement and Analysis of the Standard Nursing Practice Times of the ICU Nursing Activities (중환자실 간호행위에 대한 간호소요시간 측정 및 분석)

  • Purpose: This study was done to suggest the standard ICU nursing procedure and standard ICU nursing practice time carried out according to the standard ICU nursing procedure. Methods: Through the literature review, 35 standard ICU nursing activities were selected and classified under 6 categories. Standard nursing procedures were described by research team and 4 clinical practice experts. Content validity of descripted nursing procedure was performed by 6 clinical practice experts. After that standard ICU nursing procedures were identified, standard nursing practice times were checked through 1:1 observation. Results: 35 ICU nursing activities were identified and categorized into 6 domains of activities. Also, their standard nursing procedures were identified, and finally 33 standard nursing practice times were measured and analyzed. The result of the measurement, 1 ICU nursing activity was spent above 30 minutes, 3 activities done within 10-20 minutes, 4 activities done within 5-10 minutes, and 25 activities were spent below 5 minutes. Conclusion: Through this study, 35 standard ICU nursing procedures were described and 33 standard ICU nursing practice times which carried out according to standard ICU nursing procedures were checked. The result can be as fundamental data for the study of nursing activity time in the ICU.

Need for Gerontological Nursing Education Among Nursing Students (간호대학생의 노인간호 교육프로그램 개발을 위한 기초연구)

  • Purpose: This study is conducted to survey the nursing students' need for lectures on gerontological nursing. Methods: The subjects of this study were 257 nursing students, at a college in Chungbuk, who had just finished two weeks' of gerontological nursing practice. The subjects' need for gerontological nursing education was surveyed using a structured questionnaire, after gerontological nursing practice during the period from November 2, 2009 to April 25, 2011. Results: The mean need for gerontological nursing education was 4.07 out of 5. By domain, educational need was highest for the geriatric health problems (4.32), which were followed by gerontological nursing skills (4.05), geriatric nursing process (4.01), and gerontological nursing theories (3.84). The need for gerontological nursing education was different, according to the subjects' interest in the elderly, and their perceived necessity of gerontologcial nursing education. Conclusion: Nursing students' need for gerontological nursing education was high in general, and particularly, in the domains of geriatric health problems and gerontological nursing skills. In order to meet their educational needs, accordingly, we need to develop and apply reinforcement education programs related to that of the geriatric health problems and gerontological nursing skills, as well as to assess the students' educational needs continuously.

Identification of Nursing Diagnosis-Outcome-Intervention Linkages for Inpatients in Gynecology Department Nursing Units (부인과 간호단위 입원 환자에 적용되는 간호진단-간호결과-간호중재의 연계 확인)

  • Purpose: The aim of this study was to identify the nursing diagnosis-outcome-intervention (NANDA-NOC-NIC) linkages for gynecology inpatients shown in their electronic nursing records. Methods: This retrospective and descriptive research was conducted in two steps and based on the 287 electronic nursing records for 253 patients. First, nursing diagnoses, outcomes and interventions were collected. To identify major nursing diagnoses, a comparison was done with the top 10 nursing diagnoses from this research and with previous research selected using a content validity index developed by a team of professionals. Second, nursing outcomes and interventions that were associated with major nursing diagnoses were identified. Results: Nineteen nursing diagnoses, 12 nursing outcomes, and 40 nursing interventions were collected. The top 5 major nursing diagnoses were identified and 7 nursing outcomes and 18 nursing interventions associated with these diagnoses were checked. Conclusion: The identified NANDA-NOC-NIC linkages can contribute to improving nursing practice and will help in the establishment of standardized nursing care.

The status nursing care delivery system and the influencing factors on quality of nursing care (간호전달체계 현황 및 간호서비스의 질에 영향을 미치는 요인)

  • The aims of this study is to investigate the status of nursing care delivery systems and nurse staffing levels and to analyze differences in the quality of nursing care by the type of nursing care delivery system. This research was based on data from 723 nurses working in 55 medical and surgical units in 26 general hospitals. Descriptive statistics on nurse staffing levels and the nursing care delivery system, and multi-level logistic regression were used to estimate the determinants of quality of nursing care. The number of patients per nurse is 17.74 patients in functional nursing care and 15.56 patients in total nursing care. In comparison to hospitals adopting total nursing care, hospitals with functional nursing care had greater patients. The nurses rated units using total nursing care as significantly better quality of nursing care than the units with functional nursing care. Total nursing care or modified total nursing care, rather than functional nursing care, could lead to improvement in the quality of care(total nursing care OR=3.895, modified total nursing care OR=2.475). Patient-centered approaches under proper circumstances can be successfully implemented and the positive effects demonstrated.

  • Purpose: The purpose of this study was to identify the major trends of research in the Journal of Korean Nursing Administration from 2007 to 2009. Method: Research designs, participants, research domains, and key words were analyzed from the Journal of Korean Nursing Administration. Results: Job satisfaction, organizational commitment, job stress, turnover intention, nursing performance, self-efficiency, leadership, empowerment, nursing informatics, and quality control were the major key words commonly listed in the journal articles. Of the research in the Nursing Administration Journal, quantitative methods were used in 94.5% of the research studies and qualitative methods in only 5.5%. The major participants in the research were nurses, nurse managers, and patients. Statistical methods like ANOVA, correlation, t-test, regression, chi-square test, LISREL were the dominant method of analysis used in the research. The primary domains in the journal articles were directing, organizing, control, planning, and informatics. Conclusion: Through this study, the trend of research in nursing administration can be identified. We recommend that collaboration, nurse work environment, evidence-based practice, scheduling, coaching, patient falls and safety, and positive culture should be included as topics for the future research.

The Recognition of Achievement and Importance of Nursing Program Outcome among Nursing Students (간호대학생의 간호학 학습성과 성취도와 중요도 인식)

  • Purpose: The purpose of this study was to examine the recognition of achievement and importance of nursing program outcome among nursing students. Methods: The survey was performed with 439 nursing student and recruited from 3-year or 4-year nursing students, scheduled to graduate from university. The data was collected by questionnaires and was analyzed with SPSS statistics 19.0. Results: The achievement of nursing program outcome was 3.28. and the important of that was 4.38. The achievement of nursing program outcome on critical thinking and communication of 4-year nursing students were higher than 3-year nursing students. Achievement of program outcome were significantly different according to satisfaction of curriculum, club activity and voluntary activity. Conclusion: These findings indicate that outcome-based nursing curriculum is need to consider the importance and achievement level of program outcome for nursing students.

The Research for the Nursing Theory Development (간호이론 개발을 위한 일 연구)

  • Purpose: This study was to understand to the nursing theory frame contained in the traditional philosophy and western nursing theory for the theory development fit for Korean people and to identified the strategy for nursing theory development. Method: To perform this study various relevant texts-western, eastern, classical and contemporary were reviewed. Result: The metaparadigm of the nursing in western considers four major factors-Human being, Environment, Health and Nursing. In Korea, the metaparadigm of nursing considers three major factors-Universe, Human being (body, life, health, disease) and Nursing. The key strategy for establishing comprehensive nursing theory which combines both western and eastern is to study the nursing literature based on eastern and western philosophy. Theorists should study the life and culture of the specific nation in order to understand the essence of the thoughts and psychology of the patients and to investigate the phenomena, which will be the foundation of theory development. Conclusion: Western nursing theory must be utilized to provide practical framework for the development of nursing theory, and practical approach is necessary for combining western and eastern theory.

A Development of Nursing Outcome Indicators for Stroke Patients (뇌졸중 환자의 간호성과지표 개발)

  • Purpose: This study was conducted to develop nursing outcome indicators based on nursing activities done for stroke patients. Method: focus group meeting and delphi technique, which consisted of clinical nurse experts, neurologist, and nursing professors. CVI(Index of Content Validity) and user validity test was performed. Result: 12 nursing diagnoses, 29 nursing interventions and nursing outcomes were identified. The former were from NIC and the latter were developed according to nursing interventions. They were verified by experts in focus group. 199 nursing activities were identified, 133 nursing outcome indicators were developed. In user validity, usefulness and usability were tested. Conclusion: This systemic approach of measuring nursing outcomes verified nurses' positive effects in changing patients health status and nursing contributions in the health care system as a profession.

Theory and Knowledge Development for the East-West Nursing (동서간호를 위한 이론 및 지식개발)

  • Practice evidenced by reliable knowledge is essential for professionals and practical studies. Because nursing is a human science and practical science, theory and knowledge development is an indispensable task to improve nursing. Nursing is a very special paradigm not exactly correlated to Western medicine and Oriental medicine. However, nursing was influenced by on Western medicine at the beginning, and most nursing theories were established western philosophies. Caring is the essence of nursing. To provide qualitative care which satisfies clients, it is required to respect cultures of the clients. Western and Eastern approaches of thinking are coexisting in Korea no, so the needs for developing a nursing model, East-West Nursing, which blends two approaches are increasing. In this paper, concepts of the East-West Nursing, differences between Oriental and Western medicines, and comparison of nursing metaparadigm in prospects from and Western Philosophies were briefly covered to define the East-West Nursing. Strategies and directions to develop the East-West Nursing were also discussed.
