  • The purpose of this study was to confirm the classification of nursing activity and to analyze the time of nursing workload in a new open hospital. The data were collected from 20 nurses working in 6 general nursing units by 4 trained observers. The tools used for this study were an observation recording sheet and a classification sheet of nursing activity. The classification sheet was constructed to be adaptable to each hospital system based on the instrument described in the literature. The results of the study are as follows : The direct nursing activities consisted of 6 sections, 33 subsections and the indirect nursing activities consisted of 14 sections, 53 subsections. The direct nursing activities included medication, measuring and observation, care of therapies, care of physical comfort, laboratory and treatment. The indirect nursing activities included preparation of medical utensils, collection of information and assessment, recording, phone communication, professional interaction related to patients, personal time, assigning work to staff, patient eaucation and training, interaction with lab, transfer of administration of utensils, checking physician's order, dietary service, management of pollution and contagion, guide direction. Nurses spent 127.6min for direct nursing activity during day duty. It was 24.5% of total nursing activity. Within that activity medication had the highest percentage of time(40.09%), followed by communication and education with patient(24.76%), measuring and observation (16.93%), laboratory and treatment (12.85%), care of therapies(3.21%) and care of physical comfort (2.16%). The time breakdown for indirect nursing activities is as follows ; the preparation of medical utensils 22.3%, collection of information and assessment 20.29%, recording 20.27%, phone communication 8.14%, professional interaction related to patients 7.33%, personal time 7.24%, with the remaining timeshared by staffing, patient education and training, interaction with lab, transfer of administration of utensils, checking physician's order, dietary service, management of pollution and contagion, guide direction. In the analysis of the relationships between the working time and the work allocation characters of the nurses(including nurse's experiences. nurse-patients ratio, nurse-rooms ratio, and character of nursing unit) ; There were no significant differences in direct-indirect nursing times between nurse's career years. There was significant difference in direct nursing time between assigned patient numbers. The nurses assigned larger number of patients spent significantly more time in direct nursing care than that of the smaller. On the other hand, there was no significant difference in indirect nursing workload between the assigned patient numbers. There were no significant differences in direct-indirect nursing time between an allocated patient's room numbers. There was significant difference in working time between working places. The nurse in the medical unit spent more time in direct nursing care than her counterpart in the surgical unit. However there was no difference in direct nursing time between two groups. The study results indicate that nurses spent less time in the direct nursing care than in the previous studies even though the hospital system has been modernized. On the other hand they spent much more time for the coordinating role within the interdisciplinary team and for the overlapping paperwork. Therefore it is recommended that patient oriented job description and more efficient usage of modernized utilities be made.

  • Purpose: This study aimed to assess the levels of and relationships among health literacy, knowledge about diabetes, and self-care activities in the Korean low-income elderly with diabetes and to identify factors influencing the self-care activities of this vulnerable population. Methods: This study surveyed a total of 151 low-income elderly patients with diabetes registered at 16 Visiting Health Care Services in B City, Korea. Health literacy was measured with the Newest Vital Sign. Diabetes knowledge was measured with the Diabetes Knowledge Test. The Summary of Diabetes Self-care Activities Questionnaire was used to assess diabetes self-care activities. A stepwise multiple regression analysis was conducted to identify significant factors influencing diabetes self-care activities in these patients. Results: In the regression model, diabetes knowledge (${\beta}=.322$, p<.001), exercise (${\beta}=.337$, p<.001), and experiences of diabetes education (${\beta}=.241$, p=.001) were significantly associated with increased diabetes self-care activities in low-income elderly patients with diabetes when gender, education, health literacy, and subjective health state were controlled. Conclusion: To improve diabetes self-care activities in the low-income elderly with diabetes, it is important to develop a customized program considering their knowledge, exercise, and diabetes education experience.

Methods: The research was conducted with 455 nurses from eight hospitals in B city. Data were collected according to the level of nurse personnel from second (nurse vs. patient ratio of 2.0-2.5) to fifth (ratio of 3.5-4.0) rank. The survey tools were, 'Questionnaire on Patient Safety, a Hospital Survey on Patient Safety Culture developed by AHRQ (2007), and 'Questionnaire on Patient Safety Nursing Act, in which the questions were selected from nursing-related items (Medication 6 & Safety Nursing Assurance Act 4) in the Safety Evaluation developed by Evaluation Institute of Medical Institution. Data were analyzed with SPSS PC 12.0 program using descriptive statistics, $x^2$ test, ANCOVA and $Scheff{\grave{e}}$. Results: The nurses' overall cognition level on patient safety and safe nursing activities showed that nurses who are in the second and third rank had higher scores than those in lower ranks. Conclusion: The results of this study indicate that hospitals need a higher ratio level for nurse personnel in order to assure patient safety and safe nursing activities.

The purpose of this study can be summarized as to find the effect of a voluntary program and to offer fundamental data for institutionalizing a voluntary program in college. The subjects were composed of two groups, one group attend the voluntary program and the other group doesn't. The subjects were 390 nursing students: 179 in the experimental group and 211 in the control group. The results of this study are as follows : 1. Before voluntary activities, the mean value of the control group was higher than that of the experimental group in social interest and altruistic motivation. The difference is statistically meaningful(t= -2.2.53, p=.25, t= -2.509, p= .013). 2. After voluntary activities, selfish motivation and altruistic motivation in the experimental group was higher than before. The difference is statistically meaningful(t= 2.404, p=.0l7, t=-2.751, p=.007). 3. The social interest, selfish motivation, altruistic motivation toward voluntary activities and selfish attitude, altruistic attitude toward voluntary activities will not have changed in the control group before or after voluntary program. However, the altruistic attitude lowered after voluntary activity. It is statistically meaningful(t=2.694, p=.008). 4. After the voluntary activities, there was a significant difference between the experimental and control groups in altruistic attitude. In the experimental group, the mean value of altruistic attitude increased significantly, but in the control group the mean value of altruistic attitude decreased ( t = 2.15, p = .032). The results of this study showed that voluntary activities not only increase social interest and altruistic attitude, but also the subject's understanding toward the community. 3. • This study was conducted to identify the levels of patient safety culture, critical thinking disposition, and patient safety nursing activities and determine the factors affected by patient safety nursing activities among nurses in comprehensive nursing care service wards. Subjects consisted of 150 nurses from general hospitals in B city, Korea, and data were analyzed using the SPSS Win 21.0 program. The mean scores of awareness of patient safety culture, critical thinking disposition, and patient safety nursing activities were $3.32{\pm}0.32$, $3.50{\pm}0.31$, and $4.28{\pm}0.46$, respectively. The awareness of patient safety culture was significantly different by clinical career in the present unit (F=4.79, p=0.001). The critical thinking disposition was significantly different by age (F=3.89, p=0.010) and position (F=6.40, p=0.002). There were differences in the scores of patient safety nursing activities according to position (F=3.19, p=0.044). Additionally, hospital environment (${\beta}=0.25$, p=0.014), supervisor attitude (${\beta}=0.20$, p=0.046), and position (${\beta}=0.14$, p=0.040) accounted for a 44.4% variance in patient safety nursing activities. To promote patient safety nursing activities in comprehensive nursing care service wards, positive awareness of the hospital's safety environment and support from the manager are required.
