조기유방암에서 유방보존술 후 재발에 영향을 주는 인자 (Risk Factors for Recurrence after Conservative Treatment in Early Breast Cancer: Preliminary Report)

  • 목적 : 연세의대 연세암센터에서는 1991년도부터 조기유방암에 대하여 유방보존술을 실시해오고 있으며 본원에서 사용하는 치료방법과 치료에 따르는 부작용 등에 대해서는 이미 보고한 바 있다. 아직 추적 관찰 기간이 짧기 때문에 치료결과를 평가하기는 어려운 시점이지만 치료 후 수년 이내에 재발하는 양상을 알아보고 그에 관련되는 위험인자를 알아봄으로써 본원에서 시행 되고 있는 유방보존술을 중간 점검해 보고자 하였다. 대상 및 방법 : 1991년 1월부터 1994년 12월까지 AJCC 병기 1기, 2기 유방암으로 유방 보존적 수술을 받은 후 연세암센터 방사선종양학과에서 근치적 방사선치료를 받았던 216명의 환자들을 대상으로 하였다. 환자들의 연령 분포는 23세에서 80세로 중앙값이 44세였으며, 40대가 86명$(39.8\%)$, 30대가 73명$(33.8\%)$, 50대가 38명$(17.6\%)$였다. T 병기 별로는 T1이 117명, T2가 99명이었으며 이 중 Tlc와 3cm 이하의 T2가 전체의 $79\%$를 차지하였다. 액와림프절은 73명에서 양성이었다. 따라서 stage 1이 89명$(41.2\%)$, 113가 81명$(37.5\%)$, llb가 46명$(21.3\%)$이었다. 모든 환자들은 유방부분절제술과 액와림프절 곽청술을 시행받은 후 방사선치료를 받았으며 96명의 환자들이 방사선치료 후, 또는 방사선치료 전후에 항암화학요법을 받았다. 추적 기간은 3-60개월로 정중앙 추적기간이 30개월이었다. 결과 : 추적 기간 동안에 19명이 재발하였는데 3명은 국소 재발(true recurrence;1, elsewhere; 1, skin;1)만 있었고 13명은 원격전이만 있었으며, 3명은 국소재발과 원격 전이가 동시에 있었다. 이 외 반대편 유방암이 1예에서 발생하였고, 3예에서 2차 원발암이 발생하였다. 국소 재발을 보인 예들 중, 유방의 피부에서 재발하였던 예는 곧 이어 원격전이를 보였지만, 나머지 2예는 유방전적출술을 받은 후 무병 생존중이다. 원격전이를 보인 예들은 대부분(12/16) 2년 이내에 재발하여 빠르게 진행되어 11명이 사망하였고 원격전이 후 정중앙 생존기간이 8개월이었다. 병기별 5년 생존율은 1기 $96.7\%$, IIa기 $95.2\%$, IIb기 $69.9\%$이었고, 5년 무병생존율은 I기, IIa기, IIb기가 각각 $97.1\%,\;91.7\%,\;59.9\%$이었다. 원격전이에 영향을 주는 인자로는 연령, 1병기, N병기,AJCC stage가 의미 있었으며, 병리학적 유형, 화학요법 여부, 수술절제연 침범 유무는 원격전이에 영향을 주지 못하였다. 결론 : 조기유방암의 유방보존술 후 수년 이내에는 유방내 국소재발은 적으나, 원격전이는 주로 3년이내에 발생하였는데 40세 이하의 젊은 연령층에서, 종양이 클수록, 침범된 액와림프절의 숫자가 많을수록 원격전이율이 높았다.

암의 조기발견을 위한 CT촬영에서의 임상적 효능과 방사선위해에 대한 고찰 (Review of the Radiation Risk and Clinical Efficacy Associated with Computed Tomography Cancer Screening)

  • MDCT의 등장과 함께 병적 증상이 없는 개인이 조기검진을 위해 CT검사를 하는 예가 꾸준히 증가하고 있다. 이러한 검사의 임상적 효용성과 방사선에 의한 암 발생위험에 대한 여러 가지 논란을 살펴보았다. 1. 저선량흉부CT(LDCT)를 이용한 폐암의 조기 검진 효용성을 보기 위해 설계된 무작위배정대조검사(randomized controlled trial)인 NLST(National Lung Screening Trial)의 분석 결과, CT선별검사가 선별검사를 하지 않은 그룹의 사망률 1.67%를 1.33%로, 20% (p=0.004) 감소시키는 것으로 나타났다. CT선별검사군에서 양성결과를 보인 사람의 96.4%는 위양성(false-positive)이었다. 그리고 방사선에 의한 폐암의 ERR (excess radiation risk)은 다른 고형암과는 달리 나이와 함께 감소하지 않으며, 더욱이 흡연 위험과 방사선 위험이 상승작용을 하여 폐암 검진 대상인 고위험군에게 더욱 위험할 수 있는 것으로 평가되었다. 따라서 방사선에 의한 위험이 NLST에서 관측한 이득을 상쇄할 수도 있다. 아직은 최적의 전략이 확립되지 않은 상태이므로 임상에서 광범위하게 적용하기에는 한계가 있다. 2. 대장CT검사(CT colonography)는 10 mm보다 큰 용종의 검출에는 정확도가 뛰어나지만 이보다 작은 경우에는 민감도와 특이도가 급격히 감소하고, 용종이 발견되어 제거하고자 하는 경우 다시 기존의 대장내시경을 이용해야 하는 불편함이 있다. 현재 대장CT검사 1회 시행의 평균유효선량은 약 8-10 $mS{\nu}$로 보고되어, 만약 이 검사가 많은 사람들을 대상으로 일상적으로 행해지면 공중보건에 영향을 미칠 수 있다. 따라서 집단검진 방법으로 정착하기 위해서 는 용종 크기 6-10 mm 범위에서 높은 민감도와 특이도를 확립하고, 하제를 사용하지 않는 방법을 개발하여야 하며, 선량을 더 감소시킬 수 있는 최적화된 전략을 세워 표준화하여야만 한다. 3. 전신CT검사는 그 효용성이 검증된 선행연구가 없는 상태이다. 이 검사는 약 90%의 검사자들에게서 최소한 하나 이상의 이상소견을 확인하였지만 그 중 약 2%만이 임상적으로 중요한 질환으로 판명되었다. 전신CT검사 1회 시행으로 인한 평균유효선량은 약 12 $mS{\nu}$이며 45세부터 75세까지 매년 검사를 받는 경우 LAR (lifetime attributable risk)이 1.9%로 보고되었다. 현재 의학계의 어떤 단체도 이 검사를 권고하고 있지 않다. 현재까지 CT선별검사는 정당화에 대한 충분한 논의를 위해 심도 있는 다양한 연구가 필요하며 최적화된 프로토콜이 확립된 후 이용하는 것이 바람직하다.

Recent research activities on hybrid rocket in Japan

  • Hybrid rockets have lately attracted attention as a strong candidate of small, low cost, safe and reliable launch vehicles. A significant topic is that the first commercially sponsored space ship, SpaceShipOne vehicle chose a hybrid rocket. The main factors for the choice were safety of operation, system cost, quick turnaround, and thrust termination. In Japan, five universities including Hokkaido University and three private companies organized "Hybrid Rocket Research Group" from 1998 to 2002. Their main purpose was to downsize the cost and scale of rocket experiments. In 2002, UNISEC (University Space Engineering Consortium) and HASTIC (Hokkaido Aerospace Science and Technology Incubation Center) took over the educational and R&D rocket activities respectively and the research group dissolved. In 2008, JAXA/ISAS and eleven universities formed "Hybrid Rocket Research Working Group" as a subcommittee of the Steering Committee for Space Engineering in ISAS. Their goal is to demonstrate technical feasibility of lowcost and high frequency launches of nano/micro satellites into sun-synchronous orbits. Hybrid rockets use a combination of solid and liquid propellants. Usually the fuel is in a solid phase. A serious problem of hybrid rockets is the low regression rate of the solid fuel. In single port hybrids the low regression rate below 1 mm/s causes large L/D exceeding a hundred and small fuel loading ratio falling below 0.3. Multi-port hybrids are a typical solution to solve this problem. However, this solution is not the mainstream in Japan. Another approach is to use high regression rate fuels. For example, a fuel regression rate of 4 mm/s decreases L/D to around 10 and increases the loading ratio to around 0.75. Liquefying fuels such as paraffins are strong candidates for high regression fuels and subject of active research in Japan too. Nakagawa et al. in Tokai University employed EVA (Ethylene Vinyl Acetate) to modify viscosity of paraffin based fuels and investigated the effect of viscosity on regression rates. Wada et al. in Akita University employed LTP (Low melting ThermoPlastic) as another candidate of liquefying fuels and demonstrated high regression rates comparable to paraffin fuels. Hori et al. in JAXA/ISAS employed glycidylazide-poly(ethylene glycol) (GAP-PEG) copolymers as high regression rate fuels and modified the combustion characteristics by changing the PEG mixing ratio. Regression rate improvement by changing internal ballistics is another stream of research. The author proposed a new fuel configuration named "CAMUI" in 1998. CAMUI comes from an abbreviation of "cascaded multistage impinging-jet" meaning the distinctive flow field. A CAMUI type fuel grain consists of several cylindrical fuel blocks with two ports in axial direction. The port alignment shifts 90 degrees with each other to make jets out of ports impinge on the upstream end face of the downstream fuel block, resulting in intense heat transfer to the fuel. Yuasa et al. in Tokyo Metropolitan University employed swirling injection method and improved regression rates more than three times higher. However, regression rate distribution along the axis is not uniform due to the decay of the swirl strength. Aso et al. in Kyushu University employed multi-swirl injection to solve this problem. Combinations of swirling injection and paraffin based fuel have been tried and some results show very high regression rates exceeding ten times of conventional one. High fuel regression rates by new fuel, new internal ballistics, or combination of them require faster fuel-oxidizer mixing to maintain combustion efficiency. Nakagawa et al. succeeded to improve combustion efficiency of a paraffin-based fuel from 77% to 96% by a baffle plate. Another effective approach some researchers are trying is to use an aft-chamber to increase residence time. Better understanding of the new flow fields is necessary to reveal basic mechanisms of regression enhancement. Yuasa et al. visualized the combustion field in a swirling injection type motor. Nakagawa et al. observed boundary layer combustion of wax-based fuels. To understand detailed flow structures in swirling flow type hybrids, Sawada et al. (Tohoku Univ.), Teramoto et al. (Univ. of Tokyo), Shimada et al. (ISAS), and Tsuboi et al. (Kyushu Inst. Tech.) are trying to simulate the flow field numerically. Main challenges are turbulent reaction, stiffness due to low Mach number flow, fuel regression model, and other non-steady phenomena. Oshima et al. in Hokkaido University simulated CAMUI type flow fields and discussed correspondence relation between regression distribution of a burning surface and the vortex structure over the surface.

내독소처치 흰쥐에서 Tumor Necrosis Factor-$\alpha$치 상승에 따른 폐손상 악화 및 35 kDa 단백질 합성 (Lung Injury Indices Depending on Tumor Necrosis Factor-$\alpha$ Level and Novel 35 kDa Protein Synthesis in Lipopolysaccharide-Treated Rat)

  • 연구배경 : 급성폐손상의 병태생리학적 기전에는 염증세포들이 분비하는 다양한 염증성 매개물질들이 매우 중요한 역할을 한다. 이중 특히 tumor necrosis factor-$\alpha$ (TNF-$\alpha$) 는 다른 염증세포들의 화학주성 및 각종 염증성 매개물질 분비에 영향을 미치는 proin-flammatory cytokine으로 작용하는 한편, 직접적으로 세포손상을 야기시키는 세포독성 cytokine으로도 작용하는데, 급성폐손상에서 TNF-$\alpha$와 폐조직 손상과의 직접적인 관련성에 대해서는 아직 구체적으로 확인된 바가 많지 않다. 또한, 최근에 생체내 방어기전으로 스트레스 단백질에 대한 관심이 높아지면서, 단핵구에 내독소를 처치하거나, 동물에 내독소를 투여하기 전에 미리 스트레스 단백질을 합성시킨 경우, 내독소에 의한 손상을 감소시켜 준다는 연구가 보고되었지만, 내독소 자극 자체만으로 스트레스 단백질 합성이 유도되는지는 아직 분명하지 않다. 이에 저자들은 내독소 유도성 급성 폐손상에서 TNF-$\alpha$ 분비와 폐조직 손상을 포함한 일련의 염증반응과의 관계를 분석하고, 생체내 내독소 자극에 대하여 폐포대식세포에서 스트레스 단백질을 포함한 새로운 단백질 합성이 유도되는지 여부를 분석하고자 하였다. 연구방법 : 흰쥐의 기관내로 내독소를 투여한 후 시간별로 기관지폐포세척액내 TNF-$\alpha$농도, 염증세포 백분율 변화, 병리조직학적 소견을 관찰하고, 또한 각 시간대의 폐포대식세포에서 sodium dodesyl sulfate-polyacrylamide gel electrophoresis와 inducible heat stress protein72에 대한 면역화학염색을 시행하여 단백질 합성양상을 분석하는 한편, 폐포대식세포에 다양한 농도의 내독소 자극과 열처리를 가한 후, 배양상층액에서 tumor necrosis factor-a 농도를 측정하고, 폐포대식세포의 단백질 합성양상을 분석하였다. 연구결과 : 내독소 투여 후 tumor necrosis factor-$\alpha$는 첫 1시간째부터 현저하게 증가하여 (p< 0.0001) 3시간째 최고치에 이르렀고 6시간째는 감소하기 시작하여 12시간째는 정상 대조군 수준으로 감소하였다. 내독소 투여 후 염증세포 백분율의 변화는 2시간째부터 시작하여 6시간째 최고에 이르러 12시간째까지 지속하였으며, 장시간째에 정상 대조군 수준으로 회복하였다. 병리조직학적 소견상 폐손상 지표 점수는 내독소 투여후 6시간째 최고치에 이르러 24 시간째까지 지속하였다. 내독소 투여 후 분리한 폐포대식세포에서 첫 1시간째부터 장시간째까지 정상 대조군에서는 관찰할 수 없던 35kDa의 새로운 단백질 띠가 관찰되었으며, 면역화학염색상 inducible heat stress protein72는 관찰되지 않았다. 내독소 자극을 가하지 않은 정상 대조세포군에 비해 내독소 자극을 가한 세포군의 배양상층액에서 tumor necrosis factor-$\alpha$ 농도가 유의하게 높았으며 (p<0.001), 내독소 자극만 가한 세포군에 비해 열충격 전처치후 내독소 자극을 가한 세포군의 배양상층액에서 tumor necrosis factor-$\alpha$ 농도가 10 ${\mu}g/ml$ 내독소 자극군만 제외하고 모두 유의하게 감소하였다 (p<0.05). 내독소 자극만 가한 세포군은 10 ${\mu}g/ml$의 고농도에서만 35 kDa 의 단백질 띠가 합성되었고 inducible heat stress protein72는 관찰되지 않았다. 열충격 전처치후 내독소 자극을 가한 세포군은 모두 inducible heat stress protein72가 관찰되었다. 결 론 : 기관내 내독소 투여에 의한 급성 폐손상에서 tumor necrosis factor-$\alpha$는 폐손상 정도와 밀접한 관련이 있다. 또한 내독소 자극에 의해서는 폐포대식세포에서 inducible heat stress protein72 합성이 유도되지 않으며, 35 kDa의 새로운 단백질 합성이 유도되었는데, tumor necrosis factor-$\alpha$ 농도 및 병리조직소견과의 관계를 볼 때, 급성 폐손상에 있어 35 kDa 단백질이 방어적인 역할을 담당하지는 않을 것으로 보이며, 이에 대해서는 향후 더 연구가 필요할 것으로 생각된다.

해외농업투자에 따른 유통체계 개선방안에 관한 연구 (A study on the improvement of distribution system by overseas agricultural investment)

  • 세계경제 및 환경의 변화에 따라 농산물의 불안정적인 수급으로 인한 문제점이 노출되고 있으며, 우리나라의 경우 농산물의 안정적인 확보를 위하여 국가 전략적 차원에서의 해외농업투자의 필요성이 요구되고 있다. 하지만 정부차원의 지원 미진, 해외 농업에 대한 정보 및 기술 미비, 개발자금 확보의 어려움, 장기간의 투자금 회수기간, 사후관리 미흡 등의 이유로 성과를 이루지 못하고 있는 실정이며, 특히 해외 농산물의 국내 반입 시 관세의 장벽, 물류 유통비용 등으로 가격 경쟁력이 떨어지고 있어 국내에 반입되지 못하고 있는 실정이다. 이에 본 연구에서는 우리나라의 해외농업투자의 기본개념 및 실태를 살펴보고 해외농업투자의 필요성과 고려사항, 문제점 등을 도출하여 해외에서 재배된 농산물의 경쟁력을 위한 유통 측면에서의 개선방안을 정부의 간접적인 지원, 유통 현대화 및 유통정보기능 강화, 유통시설, 수송루트, 하역업무개선, 경쟁력 확보를 위한 정부의 정책적 지원, 교육 훈련을 통한 전문인력 양성 등 다섯 가지 측면에서 제시하였다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. On this base, we composed of the radial pulse diagnosis system in korean traditional medicine. The system composed of a radial pulse wave detection system and a radial pulse diagnosis system. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.
