알코올성 지방간과 비알코올성 지방간 성인에 있어 혈청 철 표지자, 심장 표지자, 생화학적 표지자의 차이 (Difference in Serum Iron, Cardiac, and Biochemical Indices between Alcoholic and Non-alcoholic Fatty Liver)

  • 비록 음주가 간손상을 일으키는 지방간의 원인이 되긴 하나 다른 인자들 역시 지방간의 원인이 된다. 우리는 알코올성 지방간(알코올군) 및 비알코올성 지방간 남성(비알코올군)에 있어 철 표지자, 심장표지자, 그리고 생화학적 표지자의 변화와 차이를 연구하였다. 알코올군이 비알코올군 보다 신체지수, 수축기 및 확장기 혈압, 좌우 안압이 유의하게 더 높았다. 또한 혈색소 농도, 적혈구 용적률, 평균 적혈구 혈색소량, 평균 적혈구혈색소 농도, 단구 수 역시 알코올군이 비알코올군 보다 유의하게 더 높았다. 생화학적 지수인 alanine aminotransferase, ${\gamma}$-glutamyltranspeptidase, 총콜레스테롤, 중성지방, 저밀도콜레스테롤, 혈당, 크레아티닌, 요산 등도 알코올군이 비알코올군 보다 유의하게 높았고, 철 지수인 철, 총철결합능, 페리틴 농도 역시 알코올군이 의미있게 더 높았다. 이러한 결과들은 장기적인 알코올 기인성 지방간이 비음주 지방간에 비해 혈청 철농도, 심혈관 표지자, 그리고 생화학적 표지자들을 더 상승시키며, 이는 심혈관 질환 및 대사성 증후군과 같은 성인병에 노출될 가능성이 보다 높을 수 있음을 시사하고 있다.

  • Objectives This study aimed to investigate the efficacy of Ephedra sinica (E. sinica), Panax ginseng (P. ginseng), and Alisma orientale (A. orientale) Extract (MIT) on insulin resistance induced by Non-alcoholic fatty liver disease (NAFLD). Methods C57BL /6 male mice (8-week-old, 20 g) were divided into four groups: control group (Ctrl), high-fat diet group (HFDF), high fat diet with metformin administration group (METT), and high fat diet with MIT administration group (MITT). Each 10 mice were allocated to each group (a total of 40 mice). All mice were allowed to eat fat-rich diet freely throughout the experiment. To examine the effect of MIT, we observed Cannabinoid receptor type 1 (CB1), Cannabinoid receptor type 2 (CB2), G protein-coupled receptor 55 (GPR55), and Transforming growth factor-β (TGF-β). Results In the MITT group, positive reactions of the CB1, CB2, and GPR55 were significantly was significantly suppressed compared to the HFDF group. The positive reactions of the CD36 and TGF-β in the liver tissue were significantly suppressed in MITT. Conclusions MIT has the effect of improving NAFLD induced insulin resistance through the regulation of the lipid metabolism.

치자 추출물의 지방대사조절을 통한 비알콜성 지방간 유도 인슐린저항성 개선 효과 (Effect of Gardenia Jasminoides on Insulin Resistance Induced by Non-alcoholic Fatty Liver Disease)

  • Gardenia jasminoides is an herbal medicine that treats obesity and dampness-phlegm. This study aimed to investigate the efficacy of Gardenia jasminoides on insulin resistance induced by Non-alcoholic fatty liver disease (NAFLD). 8-week-old C57BL/6 male mice were divided into three groups: control group (Ctrl), high-fat diet group (HFF), and high fat diet with Gardenia jasminoides extract administration group (GJT). Each 10 mice was allocated to each group (a total of 30 mice). All mice were allowed to eat fat-rich diet freely throughout the experiment. To examine the effect of Gardenia jasminoides, we observed weight changes, lipid blot distributions, PPAR-${\gamma}$, p-IkB, p-JNK in liver tissue, total cholesterol, and glucose levels in serum. Comparing of body weight measurements between 3 groups, in the GJT group, weight gain was significantly suppressed compared to the HFF group. The distribution of lipid blots and positive reaction of PPAR-${\gamma}$ were significantly lower in GJT group. The expression levels of p-$I{\kappa}B$ and p-JNK that plays critical roles in the development of insulin resistance were significantly decreased by GJ treatment. Total cholesterol and glucose levels in serum were also significantly lower in GJT group. Gardenia jasminoides has the effect of improving non - alcoholic fatty liver induced insulin resistance through the regulation of lipid metabolism.

Association Between Protein Intake From Different Animal and Plant Origins and the Risk of Non-Alcoholic Fatty Liver Disease: A Case-Control Study

  • Previous studies have frequently reviewed how different macronutrients affect liver health. Still, no study centered around protein intake and the non-alcoholic fatty liver disease (NAFLD) risk relationship. This study aimed to examine the association between the consumption of total and different sources of protein and NAFLD risk. We allocated 243 eligible subjects to the case and control groups, including 121 incidence cases of NAFLD, and 122 healthy controls. Two groups were matched in age, body mass index, and sex. We evaluated the usual food intake of participants using FFQ. Binary logistic regression was conducted to estimate the risk of NAFLD in relation to different sources of protein intake. The age of participants was 42.7 years on average, and 53.1% were male. We found Higher intake of protein in total (odds ratio [OR], 0.24; 95% confidence interval [CI], 0.11-0.52) was significantly associated with a lower risk of NAFLD, despite adjusting for multiple confounders. in detail, higher tendency to the vegetables (OR, 0.28; 95% CI, 0.13-0.59), grains (OR, 0.24; 95% CI, 0.11-0.52), and nuts (OR, 0.25; 95% CI, 0.12-0.52) as the main sources of protein, were remarkably correlated with lower NAFLD risk. In contrary, increased intake of meat protein (OR, 3.15; 95% CI, 1.46-6.81) was positively associated with a higher risk. Totally, more calorie intake from proteins was inversely associated with lower NAFLD risk. This was more likely when the protein sources were selected less from meats and more from plants. Accordingly, increasing the consumption of proteins, particularly from plants, may be a good recommendation to manage and prevent NAFLD.

초음파 관찰을 통한 비알콜성 지방간 환자 치험 1례 (A Case Study on A Patient with Non-Alcoholic Fatty Liver Disease (NAFLD) by Using Ultrasound)

  • Objectives: The purpose of this study is to report the clinical effect of Shihosogan-tang extract on NAFLD through ultrasound. Methods: The patient was treated with Shihosogan-tang extract. We observed the changes in symptoms and ultrasound imaging on a patient with NAFLD. Results: After treatment, the symptoms of the patient with NAFLD were decreased and ultrasound imaging were improved from moderate to mild grade. Conclusion: The patient of NAFLD was improved in symptoms and ultrasound imaging.

Inhibitory Activity of Wild-Simulated Ginseng against Non-Alcoholic Fatty Liver Disease in HepG-2 Cells

  • In this study, we investigated in vitro inhibitory activity of wild-simulated ginseng (WSG) against non-alcoholic fatty liver disease using HepG-2 cells. T0901317 treatment increased the lipid accumulation in HepG-2 cells, but WSG treatment inhibited T0901317-mediated lipid accumulation. In addition, WSG downregulated T0901317-mediated expression of SREBP-1c, ACC, FAS and SCD-1 protein. In addition, WSG increased the phosphorylation level of LKB1 and AMPK. Compound C treatment blocked WSG-mediated downregulation of SREBP-1c protein. In conclusion, WSG is considered to inhibit the accumulation of lipids and triglycerides in HepG-2 cells by inducing the activation of LKB1 and AMPK successively, thereby reducing the expression of FAS, ACC, and SCD-1 through suppression of SREBP-1c expression.

알코올성 간 질환 환자의 Carbohydrate Deficient Transferrin의 임상적 유용성 연구 (A Study of Clinical Usefulness of Carbohydrate Deficient Transferrin in Patients with Alcoholic Liver Disease)

  • It is well known that intaking alcohol chronically and heavily causes many formsof physical systemic distress including serious chronic alcoholic liver disease such as alcoholic hepatitis, fatty liver, liver cirrhosis and hepatoma. Carbohydrate-deficient transferrin (CDT) is known as a specific marker in detecting and monitoring alcohol associated disease. Weanalyzed various alcohol-associated parameters to evaluate the clinical usefulness of CDT. The first patient group consisted of fifty patients, who had been diagnosed with alcoholic liver disease from January 2003 to June 2004 at Chungnam National University Hospital. The second group consisted of 12 patients with non-alcoholic liver disease. The third group consisted of 19 teetotalerswho had visited the hospital for the purpose of routine healthcare checks. Various hepatic parameters such as CDT, $\small{\Gamma}$-GT (gamma-glutamyl transferase), AST (aspartate aminotransferase) and ALT (alanine aminotransferase) were compared in the three groups. CDT and AST increased in direct proportion to the amount and duration of alcohol consumption with high significance, but $\small{\Gamma}$-GT and ALT showed arbitrary patterns with no statistical significance. The sensitivities of CDT, $\small{\Gamma}$-GT, AST and ALT were 74 %, 96 %, 68 % and 58 %, respectively and the specificities were 95 %, 74 %, 68 % and 79 % in order respectively. The correlation study of CDT with $\small{\Gamma}$-GT, AST and ALT showed a very low value of correlation coefficients, implying that CDT could be taken as an independent parameter in evaluating alcohol liver disease compared to $\small{\Gamma}$-GT, AST and ALT. This present study suggested that CDT coud be one of the most useful parameters in reflecting the amount and duration of alcohol consumption as well as being another independent parameter in assessing and following up patients with alcoholic liver disease. Moreover we recommend it is the best method of measuring both CDT and $\small{\Gamma}$-GT in patients with alcoholic liver disease.

  • Objectives : We try to compared the efficacy of six herbal medicines, Rhizoma Alismatis (RA), Fructus Crataegi (FC), Fructus Lycii (FL), Radix Curcumae (RC), Radix Salviae Miltiorrhizae (RSM), and Herba Artemisiae Scopariae (HAS), constituting KHchunggan-tang which was previously proven to be hepatoprotective on non-alcoholic fatty liver disease with combined properties of cellular steatosis, ROS production, and cytoprotection. Methods : HepG2 cells were pretreated with aqueous extracts of the six herb medicines at concentrations of 1, 10, 50 and 100 ${\mu}g/ml$ each, and treated with 0.5 mM palmitate consecutively. After 21 hrs, cell viability was assessed using MTT assay, and the percentage of cells with sub-G1 DNA content was measured using fluorescence-activated cell sorting after propidium iodide staining. Results : The first three extracts, RA, FC, and FL restored cell viability reduced by palmitate in MTT assay, and RA, FC, FL and RC inhibited palmitate-induced apoptosis in sub-G1 analysis. FL showed relatively weak potential only at tested maximal dose, and RA showed the greatest higher efficacy on this experimental cellular model of nonalcoholic fatty liver disease. Conclusions : According to this comparative experiment, Rhizoma Alismatis seems to have the most powerful potential among the six herbs constituting KHchunggan-tang, and consecutive further study seems to be required for more standardized and effective clinical application of KHchunggan-tang for treatment of non-alcoholic fatty liver disease.

비알코올 지방간질환에 대한 한의학 연구 동향 보고: 국내 논문을 중심으로 (A Review of Research Trends in Traditional Korean Medicine for Nonalcoholic Fatty Liver Disease)

  • Objectives: The aim of this study was to investigate the trends in research on non-alcoholic fatty liver disease (NAFLD) using Korean traditional medicine. Methods: This review included studies related to the treatment of NAFLD, irrespective of the year of publication. The search terms were "nonalcoholic fatty liver disease", "non-alcoholic fatty liver", "nonalcoholic hepatitis", "herb", "herbal medicine", "acupuncture", and "traditional medicine". The studies were analyzed according to the type of research. Results: After screening, 179 studies were selected from the 592 identified by the search. The types of studies were 151 in vivo or in vitro studies, 5 randomized controlled trials, 12 case reports, 2 unspecified clinical studies, 8 review articles, and 1 article that was difficult to classify. Conclusion: Analysis of the trends in Korean traditional medicine treatment by reviewing the studies on NAFLD revealed a focus on experimental studies rather than clinical studies. Therefore, multifaceted and well-designed studies are needed.

Mechanism of action of ferroptosis and its role in liver diseases

  • Ferroptosis is a type of regulated cell death recently discovered, characterized by the accumulation of iron-dependent lipid peroxides in the cell membrane, and it involves a complex network of signaling pathways, including iron metabolism, lipid peroxidation, and redox regulation. The dysregulation of these pathways can lead to the induction of ferroptosis and the development of liver diseases, such as alcoholic liver disease, non-alcoholic fatty liver disease, viral hepatitis, and liver cancer. Studies have demonstrated that targeting key molecules involved in iron metabolism, lipid peroxidation, and redox regulation can reduce liver injury and improve liver function in different liver diseases by inhibiting ferroptosis. Thus, modulation of ferroptosis presents a promising therapeutic target for treating liver diseases. However, further research is required to gain a more comprehensive understanding of the mechanisms underlying the role of ferroptosis in liver diseases and to develop more effective and targeted treatments.
