Changes in Nodule-Specific Proteins during Nodule Development of Canavalia lineata (해녀콩(Canavalia lineata)의 뿌리혹 발달 단계에 따른 뿌리혹 특이 단백질의 변화 양상)

  • Total soluble proteins from three developmental stages of induced root nodules of Canavalia lineata were compared with those of non-nodulated roots by SDS-PAGE and two dimensional (2-D) gel electrophoresis. Thirteen nodule-specific protein (nodulin) bands were identified by the former and 30 nodule specific protein spots were detected by the latter method respectively. Some of the nodulins were detected differentially depending on the nodule's developmental stages. For example, only three leghemoglobin (Lb)-like protein spots appeared at stage I (d<2 mm), but two additional Lb-like protein spots appeared at stage II (d <4-5 mm). pI value and molecular weight of nomomers of Lb-like protein were narrower and greater than those of soybean, ranging from 4.4 to 5.0 and 15.7 kd respectively. Northern blot hybridization of total RNAs from roots and root nodules using soybean Lb cDNA as a probe made it clear that Lb gene was expressed tissue-specifically only in the root nodules.odules.

A Study of the Lesional Grade Discrimination Model for Vocal Fold Nodules and Polyps (성대 결절 및 폴립 병변 판별 예측모형에 대한 연구)

  • Background and Objectives : This study is purposed to investigate the statistically significant discrimination model for predicting vocal fold nodule and polyp's lesional grade, with patients' background data and objective voice evaluation parameters. Materials and Method : The retrospective research was carried out at the Ewha Womans University Hospital. 122 patients' voice examination data had been selected, and lesion screening (Grade I, II, and III) was conducted by 2 ENT specialists, with each patient's vocal fold pictures achieved during the laryngoscopy examination. Results : The Lesional Grade Discrimination Model with which the lesional grade of vocal fold nodules and polyps could be predicted was derived by the ordinal logistic regression analysis (using SPSS 10.0). With this model the lesional grades of 73 out of 122 patients(59.8%) were correctly predicted to their formerly screened ones. Conclusion : This model applied the multivariate approach, which statistically combined these currently used parameters, Jitter, Shimmer, MFR, MPT, and patient's background data such as gender and dysphonia period. It might explain the status of benign lesion of vocal folds, and furthermore expect the physiological function of vocal folds.

Retrospective Analysis of Cytopathology using Gray Level Co-occurrence Matrix Algorithm for Thyroid Malignant Nodules in the Ultrasound Imaging (갑상샘 악성결절의 초음파영상에서 GLCM 알고리즘을 이용한 세포병리 진단의 후향적 분석)

  • This study evaluated the applicability of computer-aided diagnosis by retrospective analysis of GLCM algorithm based on cytopathological diagnosis of normal and malignant nodules in thyroid ultrasound images. In the experiment, the recognition rate and ROC curve of thyroid malignant nodule were analyzed using 6 parameters of GLCM algorithm. Experimental results showed 97% energy, 93% contrast, 92% correlation, 92% homogeneity, 100% entropy and 100% variance. Statistical analysis showed that the area under the curve of each parameter was more than 0.947 (p = 0.001) in the ROC curve, which was significant in the recognition of thyroid malignant nodules. In the GLCM, the cut-off value of each parameter can be used to predict the disease through analysis of quantitative computer-aided diagnosis.

  • Several recent advances in our knowledge of thyroid physiology have broad application to the diagnosis and management of thyroid disorders. For in the thyroid, more than other endocrine organs, pathophysiology can be translated directly into the diagnosis and management of thyroid disease. Graves' disease is a syndrome including goiter with hyperthyroidism, exophthalmos and dermopathy. The pathogenesis of Graves' disease is not yet clearly identified, but various autoantibodies to the thyroid gland and immunopathologic studies indicate that autoimmune processes are involved in the pathogenesis of the disease. The diagnosis and management of Graves' disease are largely dependent on radionuclide techniques as radioimmunoassay, radioactive iodine therapy and so on. Several laboratory tests are also developed to determine the remission of this disase including TRH stimulation test, $T_3$ suppression test and detection of thyroid stimulating immunoglobulins. Autoimmune thyroiditis is almost certainly a primary immunologic disease and the incidence tends to increase recently, mainly due to the application of biopsy technique in thyroid diseases. Thyroid nodules have been a great challenge to physicians because of the possibility of malignancy. But recently, cytologic examination of thyroid aspirate provides a very simple and also reliable diagnostic method in patients with thyroid nodules. In 163 patients with thyroid nodules, only 19.3% was revealed to be malignant. Therefore cytologic examination of thyroid aspirate and thyroid biopsy should be included in the diagnosis of nodular patients prior to surgical intervention. In this paper, a comprehensive review is presented on the pathogenesis, clinical features, laboratory findings and therapeutic modalities of various thyroid diseases on the basis of over 80 researches performed during the past 20 years at radioisotope clinic, Seoul National University Hospital.

Pulmonary epithelioid hemangioendothelioma misconceived as pulmonary metastasis of other malignancies

  • Pulmonary epithelioid hemangioendothelioma (PEH) is a rare, low-to-intermediate malignant tumor of endothelial origin. Computed tomography (CT) findings of PEH demonstrate multiple small bilateral nodules; however, to the best of our knowledge, there were no reports on PEH coexisting with other malignancies. Here, we reported on a case involving PEH in a patient with colon cancer and breast cancer which was misconceived as pulmonary meta- stasis. A 63-year-old woman who suffered from constipation for 2 weeks visited our hospital. Colonoscopy showed a large mass with obstruction on hepatic flexure. The histological diagnosis was adenocarcinoma of the ascending colon. Multiple nodules in both lungs and breast were observed on a chest CT scan. A core biopsy of a breast nodule was performed and a diagnosis of invasive ductal carcinoma of the left breast was made. Pulmonary nodules observed on the chest CT scan was considered as pulmonary metastasis from colon or breast cancer. Laparoscopic right hemicolectomy was performed. At the same time, wedge resection of the lung was performed and pathological diagnosis was PEH. Radiologic features of PEH were difficult to distinguish from lung metastasis. Therefore the author reported a rare case involving PEH in a patient with primary malignancy of colon and breast.

Performance Improvement of Convolutional Neural Network for Pulmonary Nodule Detection (폐 결절 검출을 위한 합성곱 신경망의 성능 개선)

  • Early detection of the pulmonary nodule is important for diagnosis and treatment of lung cancer. Recently, CT has been used as a screening tool for lung nodule detection. And, it has been reported that computer aided detection(CAD) systems can improve the accuracy of the radiologist in detection nodules on CT scan. The previous study has been proposed a method using Convolutional Neural Network(CNN) in Lung CAD system. But the proposed model has a limitation in accuracy due to its sparse layer structure. Therefore, we propose a Deep Convolutional Neural Network to overcome this limitation. The model proposed in this work is consist of 14 layers including 8 convolutional layers and 4 fully connected layers. The CNN model is trained and tested with 61,404 regions-of-interest (ROIs) patches of lung image including 39,760 nodules and 21,644 non-nodules extracted from the Lung Image Database Consortium(LIDC) dataset. We could obtain the classification accuracy of 91.79% with the CNN model presented in this work. To prevent overfitting, we trained the model with Augmented Dataset and regularization term in the cost function. With L1, L2 regularization at Training process, we obtained 92.39%, 92.52% of accuracy respectively. And we obtained 93.52% with data augmentation. In conclusion, we could obtain the accuracy of 93.75% with L2 Regularization and Data Augmentation.

  • Mesenchymal cystic hamartoma of the lung is quite rare lesion which has an unknown prevalence, firstly described in 1986. It is characterized by multiple nodules and variable sized cysts in both lung field. The nodules of immature mesenchymal cells gradually enlarge and become cysts which are lined with normal or metaplastic respiratory epithelium and have the cambium layer of primitive mesenchymal cells. Main symptoms are hemoptysis, recurrent pneumothorax, and hemothorax. It is known to benign tumor, but has possibility of malignant degeneration. In gross findings in opertive field, multiple and variable-sized cystic lesions and nodules were observed. The cysts were lined with normal respiratory epithelium and had the cambium layer of primitive mesenchymal cell. Gross and microscopic findings were compatible with mesenchymal cystic hamartoma. We report a case of mesenchymal cystic hamartoma in a 27-year-old woman who had recurrent pneumothorax and hemoptysis.

  • Desmoid tumors are rare soft tissue tumors considered to have locally infiltrative features without distant metastasis until now. Although they are most commonly intraabdominal, very few cases have extra-abdominal locations. The origin of intrathoracic desmoid tumors is predominantly the chest wall with occasional involvement of pleura. True intrathoracic primary desmoid tumors with no involvement of the chest wall or pleura are extremely rare. We recently experienced a case of true intrathoracic desmoid tumor presenting as multiple lung nodules at 13 years after resection of a previous intraabdominal desmoid tumor.
