We propose a scheduling algorithm, Bandwidth Allocation Scheme (BAS), that guarantees bounded delay in a switching node. It is based on the notion of the GPS (Generalized Processor Sharing) mechanism, which has clarified the concept of fair queueing with a fluid-flow hypothesis of traffic modeling. The main objective of this paper is to determine the session-level weights that define the GPS sewer. The way of introducing and derivation of the so-called system equation' implies the approach we take. With multiple classes of traffic, we define a set of service curves:one for each class. Constrained to the required profiles of individual service curves for delay satisfaction, the sets of weights are determined as a function of both the delay requirements and the traffic parameters. The schedulability test conditions, which are necessary to implement the call admission control, are also derived to ensure the proposed bandwidth allocation scheme' be able to support delay guarantees for all accepted classes of traffic. It is noticeable that the values of weights are tunable rather than fixed in accordance with the varying system status. This feature of adaptability is beneficial towards the enhanced efficiency of bandwidth sharing.

This paper proposes a system called Wireless Link based on Global Communication Channel (WLGCC) to enhance the spectrum sharing between digital broadcasting (DB) services and the cognitive radio (CR) system in the licensed band of 470-790 MHz. The WLGCC aims to enhance the spectrum sensing and geolocation database (GLD) spectrum sharing methods in the CR system. Spectrum sensing can be enhanced by receiving the status of the used frequencies from the WLGCC, thereby eliminating the need for a low detection threshold (i.e., avoiding the hidden node problem). In addition, the GLD can be enhanced by providing a reliable communication link between the database and the CR device in the form of an unused TV white space that is reserved as the proposed Global Communication Channel (GCC). This paper analyzes the coexistence of the new WLGCC system and the DB service in terms of avoiding additional interference. Specifically, we mathematically determine the WLGCC parameters, such as the in-band and out-of-band power levels, and operation coverage, and verify them using Monte Carlo simulation. The results show that WLGCC does not degrade the existing DB service and reliably transmits information of the vacant (or used) frequency bands to the CR.
In this paper, we propose the Agricultural Environment Monitoring System based on Sensor Network which can collect information of crop cultivation environment and monitor it in real-time by using various environment sensors. Existing wireless sensor nodes, based on the sensor network, require extra conversion/control module depending on the characteristics. To solve this problem, we developed an integrated sensor module which can integrate various kinds of sensors used to obtain the necessary information for the area under crop cultivation. In addition, we developed sensor networks monitoring system which is suitable for an integrated sensor module. To verify the operating status of the proposed system, an integrated sensor node is installed in the test environment so that it can sense information of the environment and monitor it in real-time.

Some application of the sensor networks such as crop management must control the temperature within a proper range. The energy-efficient applications and protocols for them are needed. In this paper, we propose an energy efficient application protocol which can meet these needs. We use the concept of safety zone to manage the appropriate range of properties within our algorithm. We name our proposed algorithm "MSZ". To assess the performance of the proposed algorithm the actual temperature data was collected using the sensor node. The algorithm was implemented through programming and compared with the other algorithms. Experimental results show that the MSZ algorithm has a much better performance than that of APTEEN in terms of energy efficiency as well as has the ability of determining the status of sensor nodes with the periodical transmitting. Our MSZ algorithm can be useful in applications developments for the management of crops, fermented food and etc.
In mobile ad hoc networks, most of on demand routing protocols such as DSR and AODV do not deal with traffic load during the route discovery procedure. To solve the congestion and achieve load balancing, many protocols have been proposed. However, the existing load balancing schemes has only considered avoiding the congested route in the route discovery procedure or finding an alternative route path during a communication session. To mitigate this problem, we have proposed a new scheme which considers the packet bursting mechanism in congested nodes. The proposed packet bursting scheme, which is originally introduced in IEEE 802.11e QoS specification, is to transmit multiple packets right after channel acquisition. Thus, congested nodes can forward buffered packets promptly and minimize bottleneck situation. Each node begins to transmit packets in normal mode whenever its congested status is dissolved. We also propose two threshold values to define exact overloaded status adaptively; one is interface queue length and the other is buffer occupancy time. Through an experimental simulation study, we have compared and contrasted our protocol with normal on demand routing protocols and showed that the proposed scheme is more efficient and effective especially when network traffic is heavily loaded.
MicroRNAs (miRNAs) play an essential role in the development and progression of nasopharyngeal carcinomas (NPC). Despite advances in the field of cancer molecular biology and biomarker discovery, the development of clinically validated biomarkers for primary NPC has remained elusive. In this study, we investigated the expression and clinical significance of miRNAs as novel primary NPC diagnostic biomarkers. We used an array containing 2, 500 miRNAs to identify 22 significant miRNAs, and these candidate miRNAs were validated using 67 fresh NPC and 25 normal control tissues via quantitative real-time PCR (qRT-PCR). Expression and correlation analyses were performed with various statistical approaches, in addition to logistic regression and receiver operating characteristic curve analyses to evaluate diagnostic efficacy. qRT-PCR revealed five differentially expressed miRNAs (miR-93-5p, miR-135b-5p, miR-205-5p and miR-183-5p) in NPC tissue samples relative to control samples (p<0.05), with miR-135b-5p and miR-205-5p being of significant diagnostic value (p<0.01). Moreover, comparison of NPC patient clinicopathologic data revealed a negative correlation between miR-93-5p and miR-183-5p expression levels and lymph node status (p<0.05). These findings display an altered expression of many miRNAs in NPC tissues, thus providing information pertinent to pathophysiological and diagnostic research. Ultimately, miR-135b-5p and miR-205-5p may be implicated as novel NPC candidate biomarkers, while miR-93-5p, miR-650 and miR-183-5p may find application as relevant clinical pathology and diagnostic candidate biomarkers.

Purpose: This study was undertaken to evaluate the efficacy of postoperative radiotherapy, and to investigate the prognostic factors for FIGO stages IB-IIB cervical cancer patients who were treated with simple hysterectomy, or who had high-risk factors following radical hysterectomy and pelvic lymph node dissection. Materials and Methods: Between March 1986 and December 1998, 58 patients, with FIGO stages IB-IIB cervical cancer were included in this study. The indications for postoperative radiation therapy were based on the pathological findings, including lymph node metastasis, positive surgical margin, parametrial extension, lymphovascular invasion, invasion of more than half the cervical stroma, uterine extension and the incidental finding of cervix cancer fellowing simple hysterectomy. All patients received external pelvic radiotherapy, and 5 patients, received an additional intracavitary radiation therapy. The radiation dose from the external beam to the whole pelvis was $40\~50$ Gy. Vagina cuff Irradiation was peformed, after completion of the external beam irradiation, at a low-dose rate of Cs-137, with the total dose of $4488\~4932$ chy (median: 4500 chy) at 5 mm depth from the vagina surface. The median follow-up period was 44 months ($15\~108$ months). Results: The 5-yr actuarial local control rate, distant free survival and disease-free survival rate were $98\%,\;95\%\;and\;94\%$, respectively. A univariate analysis of the clinical and pathological parameters revealed that the clinical stage (p=0.0145), status of vaginal resection margin (p=0.0002) and parametrial extension (p=0.0001) affected the disease-free survival. From a multivariate analysis, only a parametrial extension independently influenced the disease-free survival. Five patients ($9\%$) experienced Grade 2 late treatment-related complications, such as radiation proctitis (1 patient), cystitis (3 patients) and lymphedema of the leg (1 patient). No patient had grade 3 or 4 complications. Conclusion: Our results indicate that postoperative radiation therapy can achieve good local control and survival rates for patients with stages IB-IIB cervical cancer, treated with a simple hysterectomy, as well as for those treated with a radical hysterectomy, and with unfavorable pathological findings. The prognostic factor for disease-free survival was invasion of the parametrium. The prognosic factor identified in this study for treatment failure can be used as a selection criterion for the combined treatment of radiation and che motherapy.

Purpose : To evaluate survival rate and prognostic factors affecting survival of patients with esophageal cancer treated with concurrent chemoradiation. Materials and Methods : Eligibility included biopsy proven invasive carcinoma of the cervical or thoracic esophagus, confined to esophagus and mediastinum with or without regional lymph node and supraclavicular lymph node, and ECOG Performance status $H_0-H_2$. Patients received radiation therapy with 5940cGy over 7 weeks and chemotherapy, consisted of 5-FU(1000 $mg/m^2/day$ in continuous infusion for 5 days, days 1 to 5 and days 29 to 33) and mitomycin C($8mg/m^2$ intravenous bolus at day 1). After concurrent chemoradiation, maintenance chemotherapy was followed with 5-FU(1000 $mg/m^2/day$ in continuous infusion for 5 days at 9th, 13th, and 17th weeks) and cisplatin($80mg/m^2$ intravenous bolus at the first day of each cycle). Results : From November 1989 to November 1995, 44 patients were entered in this study. After treatment, complete response rate and partial response rate were $59\%$ and $41\%$. Overall 1, 2, and 5-year survivals were $59\%$, $38\%$, and $9.6\%$(median 17 months), Prognostic factors affecting survival were response to treatment and T-stage. Among 26 complete responders, there were 6 local recurrences, 3 distant recurrences, 1 local and distant recurrence, and 2 unknown site recurrences Acute and chronic complication rates with grade 3 or more were $20\%$ and $13.0\%$ and there was no treatment-related mortality. Conclusion : Concurrent chemoradiation, compared with historical control groups that treated with radiation alone, improved median survival and did not significantly increase treatment-related complications. Complete responders had longer survival duration than partial responders. Predominant failure pattern was local failure. So, efforts to improve local control should be proposed.
Background: Sperm-associated antigen 9 (SPAG9) has been recently proposed as a novel biomarker for early diagnosis of several human tumors, including ovarian, cervical and breast cancers. Its clinical value remains to be clarified for endometrial cancer (EC). In this study, we investigated the utility of serum SPAG9 levels in diagnosis of EC and its association with important clinicopathological parameters. Materials and Methods: This cross-sectional study was performed at a tertiary women's referral center in Ankara, Turkey. Preoperative serum samples were collected from patients surgically treated for endometrial cancer between June 2012-April 2013. Similar aged women with a biopsy proven benign endometrium were used as controls. Serum SPAG9 levels were measured with an enzyme-linked immunosorbent assay (ELISA) method and assessed for links with clinicopathological factors. Receiver operating characteristic (ROC) curve analysis was performed to assess power of SPAG9 levels for EC prediction. P values less than 0.05 were considered statistically significant. Results: A total of 63 women with EC and 27 with benign endometrium were included in the study. Mean age in the EC group was $58.7{\pm}1.1$. Median SPAG9 levels in the EC and control groups were 18.3 (range, 12.7-53.8) and 14.1 (range, 4.3-65.3), respectively (p<0.001). A cut-off value of 17 ng/ml for SPAG9 predicted presence of malignant endometrium with 74% sensitivity and 83% specificity [Area under curve (AUC)=0.82, p<0.001]. SPAG9 levels did not demonstrate any significant association with histological type, FIGO stage, tumor grade, size, myometrial invasion, lymphovascular space invasion, cervical involvement, adnexal involvement, peritoneal cytology or lymph node status (all p>0.05). Conclusions: Testing for SPAG9 may be useful for early detection of EC in asymptomatic high-risk women. Its role in post-treatment follow-up and early detection of recurrence should be assessed in future trials.

Standard therapy for advanced head and neck cancer consists of a combination of surgery and radiation. However, survival of this patient population has not improved during the past 20 years. Many different multimodality treatment schedules have been proposed, and chemotherapy is often used with the intent of organ preservation. The present study was intended to establish the efficacy of concomitant chemoradiation with a single agent carboplatin in advanced head and neck cancers.The objectives were to investigate the feasibility of concomitant administration of carboplatin, monitor acute toxicity during radiotherapy, and determine subacute side effects, such as wound healing following surgery after chemoradiotherapy. A prospective study was conducted wherein a total of 40 patients with stage III and IV squamous cell carcinomas of oral cavity, oropharynx, hypopharynx and larynx were enrolled. All patients were treated with external beam radiotherapy and weekly carboplatin area under curve (AUC of 5). Radiotherapy was given in single daily fractions of 1.8-2 grays (Gy) to a total dose of 66-72 Gy. Salvage surgery was performed for any residual or recurrent locoregional disease. Neck dissection was recommended for all patients with neck disease showing less than a complete response after chemoradiation. A total of 40 patients were enrolled of whom 32 were males and 8 were females. Highest incidence of cancer was seen in the 5th-6th decades of life with a median age of 47.7 years. Oropharyngeal tumours constituted a maximum of 21 patients followed by hypopharynx in 10, larynx in 7 and oral cavity in 2. 80% of the patients had a neck node on presentation of which 40% had N2-N3 nodal status. TNM staging revealed that 58% of patients were in stage III and 43% in stage IV. Evaluation of acute toxicity revealed that 50% had grade II mucositis, 25% grade III mucositis, 2.5% grade IV mucositis. 50% of patients had grade I skin reactions, 65% of patients had grade I thrombocytopenia, and 24% of patients had grade I anaemia. After completion of treatment 65% of patients had complete response at the primary and regional sites, and 35% of patients had a partial response of whom 23% underwent neck dissection and 5% of them underwent salvage surgery at the primary site. At the end of one year there were six deaths and four recurrences and 70% were free of disease. Concurrent chemoradiation with carboplatin provided good locoregional control for locally advanced head and neck cancers. This regimen, although toxic, is tolerable with appropriate supportive intervention. Primary site conservation is possible in many patients. Chemoradiotherapy appears to have an emerging role in the primary management of head and neck cancers.
