신생아중환자 분류도구 개발을 위한 간호활동 규명 및 표준간호시간 조사연구 (A Study on Identifying Nursing Activities and Standard Nursing Practice Time for Developing a Neonatal Patient Classification System in Neonatal Intensive Care Unit)

  • Purpose: It was necessary for developing a neonatal classification system based on nursing needs and direct care time. This study was, thus, aimed at identifying nursing activities and measuring the standard nursing practice time for developing a neonatal patient classification system in Neonatal Intensive Care Unit (NICU). Methods: The study was taken place in 8 general hospitals located in Seoul and Kyungi province, South Korea from Dec, 2009 to Jan, 2010. By using 'the modified Workload Management System for critical care Nurses' (WMSN), nursing categories, activities, standard time, and task frequencies were measured with direct observation. The data were analyzed by using descriptive statistics. Results: Neonatal nursing activities were categorized into 8 areas: vital signs (manual), monitoring, activity of daily living (ADL), feeding, medication, treatment and procedure, respiratory therapy, and education-emotional support. The most frequent and time-consuming area was an ADL, unlike that of adult patients. Conclusion: The findings of the study provide a foundation for developing a neonatal patient classification system in NICU. Further research is warranted to verify the reliability and validity of the instrument.

신생아 집중치료실 기반 사별 간호의 문헌고찰 (Bereavement Care for Parents in the Neonatal Intensive Care Unit: A Literature Review)

  • Purpose: The purpose of this study was to review studies investigating the effect of bereavement care provided for parents in the neonatal intensive care unit. Methods: We conducted a literature review of databases (MEDLINE, Embase, Cochrane Library, and CINAHL) for studies published in English and four databases (RISS, KISS, NANET, and KoreaMed) for Korean studies. The selection criteria included original articles that evaluated the parents' perceptions or responses to bereavement care provided in the neonatal intensive care unit. Of 889 articles from the initial screening, 66 articles underwent full-text review and five articles were finally selected for analysis. Results: None of the studies was conducted in Korea. Only one study used a randomized control trial design. The forms of bereavement care reviewed included a provision of a memory package, grief information, and emotional and/or social support from peers or health care providers. The effectiveness of bereavement care was measured by including grief, social support, and depression. Conclusion: Bereavement care can be considered to be beneficial for relieving grief reactions and enhancing the personal growth of bereaved parents. Future research should assess the needs of bereaved parents in Korea. Nurses may play a role in developing the bereavement care for parents.

간호사의 신생아 통증관리 수행에 영향을 미치는 요인 (Factors Influencing Neonate Pain Management Performed by Nurses)

  • Purpose: The purpose of this study was to identify factors influencing neonatal pain management by nurses in order to provide evidence-based data for the development of more efficient neonatal pain care programs. Methods: This study used a descriptive research design to survey 204 registered nurses working in neonatal intensive care units and nurseries in Busan. Data collection was done from July to September 2010. General knowledge of pain and pain scale, pain intervention, neonatal pain management, and barriers to pain management were measured. Data were analyzed using stepwise multiple regression with SPSS 17.0. Results: it was found that among the factors affecting neonatal pain management by nurses, the unit where the nurse worked was the most important influential factor, followed by knowledge of pain management, education level, and barriers from medical personnel in that order. The explanatory power of these four variables accounted for 42.0% of neonatal pain management. Conclusion: Study results indicate that it is necessary to develop education programs to increase knowledge of neonatal pain management for nurses caring for neonates. Also, plans for reduction of obstacle factors in medical personnel should be prepared.

Regionalization of neonatal intensive care in Korea

  • In the current era of low-birth rate in Korea, it is important to improve our neonatal intensive care and to establish an integrative system including a regional care network adequate for both high-risk pregnancies and high-risk newborn infants. Therefore, official discussion for nation-wide augmentation, proper leveling, networking, and regionalization of neonatal and perinatal care is urgently needed. In this report, I describe the status of neonatal intensive care in Korea, as well as nationwide flow of transfer of high-risk newborn infants and pregnant women, and present a short review of the regionalization of neonatal and perinatal care in the Unites States and Japan. It is necessary not only to increase the number of neonatal intensive care unit (NICU) beds, medical resources and manpower, but also to create a strong network system with appropriate leveling of NICUs and regionalization. A systematic approach toward perinatal care, that includes both high-risk pregnancies and newborns with continuous support from the government, is also needed, which can be spearheaded through the establishment of an integrative advisory board to propel systematic care forward.

Leukocyte Adhesion Deficiency Associated with Neonatal Septic Hip in a Late Preterm Infant

  • Leukocyte adhesion deficiency is a rare primary immunodeficiency and autosomal recessive disorder caused by a mutation in the gene encoding CD18, which is a constituent of leukocyte integrins. Clinical features usually begin with a delay in the separation of the umbilical cord in the neonatal period, and are characterized by marked leukocytosis with infection, delayed wound healing, and repeated bacterial and fungal infections. We experienced a case of leukocyte adhesion deficiency diagnosed in the neonatal period, in which a late preterm infant admitted to neonatal intensive care unit presented with a septic hip. Flow cytometry analysis of whole blood showed a decrease in the expression of CD11b/CD18. This is the first case of leukocyte adhesion deficiency with neonatal septic hip diagnosed in Korea.

극소저출생체중아에서 신생아 패혈증의 원인균 및 항생제 감수성의 변화 (Changes in Pathogens and Antibiotic Sensitivities in very Low Birth Weight Infants with Neonatal Sepsis)

  • 목 적 : 신생아 패혈증의 원인균 및 항생제 감수성의 변화를 조사하여 극소 저출생 체중아 및 미숙아의 이환율 및 사망률 증가와 관련된 패혈증에 대하여 가능한 신속하고 적절한 치료 방침을 세우는데 도움을 주고자 하였다. 방 법 : 2000년 1월부터 2006년 6월까지 을지대학병원 신생아집중치료실에 입원한 출생체중 1,500 g 미만의 극소 저출생 체중아를 대상으로 의무기록을 후향적으로 분석하여 패혈증의 원인균과 항생제 감수성에 대해 조사하여 시기별 분포를 비교하였다. 결 과 : 대상 환아 164명 중 균이 증명된 환아는 19명(11.6%)이었다. 균이 증명된 19명의 환아에서 총 26례의 패혈증 발생을 보였으며, 후기 지발형 패혈증의 발생빈도가 제일 높았다. K. pneumoniae가 가장 흔한 원인균이였으며, 그 뒤로 Streptococcus spp., CNS, Enterobacter spp.의 순으로 배양되었고, 대부분이 경험적 항균제에 내성을 나타내었다. 결 론 : 과거 연구나 타 의료기관에서 증명된 흔한 원인균과 항생제 감수성 결과에 의존하여 광범위 경험적 항생제를 사용하기보다는 주기적으로 원인균 및 항균제 감수성의 변화를 파악하고 적절한 항생제를 사용하는 것이 신생아 패혈증의 치료의 효과뿐만 아니라, 다제내성균의 출현을 막을 수 있을 것으로 사료된다.

21세기 신생아 전문 간호사의 역할과 전망 (The role of the Neonatal Nurse Specialist in 21st Century)

  • The role of the neonatal nurse specialist has been well established over the past decade and now reform in 21st century. Neonatal nurse specialists responsibilities in caring for critically and long-term chronically ill infants and their families are very important. Neonatal nurse specialists have a two fold responsibility in caring for these infants. First, through acquiring advanced practice education in complex neonatal care and diagnostic skills, neonatal nurse specialists meet the physiologic needs of the infant. Second, neonatal nurse specialists provide a more holistic approach to their care through evaluating the family in treatment plans and involving the family in discharge planning for the infant. In some institutions, neonatal nurse specialists are directly involved in institutional and/or home follow-up care and case management also. It is the neonatal nurse specialists responsibility to function collaboratively with the multidisciplinary team in managing critically or chronically ill infants from admission to discharge. The role of the neonatal nurse specialist case manager can be described as one that focuses on individualized care of the infant, while providing continuity of care to both the infant and family. The neonatal nurse specialist's role will vary depending on the neonatal intensive care unit(NICU). Therefore, the multidisciplinary collaborative approach to long-term management of infants in the NICU is extremely important to provide successful transition to home or to long-term rehabilitative care facilities because care for the chronically ill infant is complex and multifaceted. I suggest the role of neonatal nurse specialist in 21st century are as follows. 1. Diagnostic/patient assessment 2. Management of patient health/illness 3. Administering/monitoring therapeutic interventions and regimens 4. Monitoring/ensuring quality of health care practices 5. Organization and work role 6. Helping role 7. Teaching/coaching role 8. Management of rapidly changing situations 9. Consulting role The advanced practice nursing model of care delivered by neonatal nurse specialist's in the NICU incorporates medical and nursing role functions and emphasizes holism, caring, and a health perspective for critically and chronically ill neonates and their families.

신생아 중환자실의 사망에 관한 연구 (Death in the Neonatal Intensive Care Unit)

  • 목 적 : 최근 신생아집중치료술의 발전으로 신생아 중환자실의 생존률은 매우 향상되었으나 아직도 신생아 중환자실 입원환아들의 사망은 지속적으로 발생하고 있다. 본 연구는 지난 10년간 서울아산병원 신생아 중환자실에 입원한 환아들 중 사망한 환아들을 분석하여 사망률, 사망원인 및 사망의 특징에 대하여 알아보고자 하였다. 방 법 : 1998년 1월부터 2007년 12월까지 10년간 서울아산병원 신생아중환자실에 입원했던 환아들을 대상으로 의무기록을 후향적으로 조사하여 사망률, 사망원인질환 및 출생 후 사망까지의 시간을 분석하였다. 결 과 : 조사기간 동안 총 입원환아는 6,289명이었고 총 사망환아는 264명으로 사망률은 4.2%였다. 총 사망환아 중 극소 저출생 체중아는 103명, 초극소 저출생 체중아는 80명이 사망하여 사망률은 각각 10.6%, 21.4%였다. 10년간 연도별 사망률의 유의한 변화추세는 보이지 않았다. 사망관련 주요질환으로 미숙아 관련 사망이 102명(38.7%)으로 가장 많았고, 선청성 기형, 패혈증, 원발성 폐동맥 고혈압, 신생아가사 순으로 나타났다. 미숙아 관련 합병증 중에서는 패혈증이 가장 많은 수를 차지하였고, 선천성 기형 중에서는 선천성 심기형으로 인한 사망이 가장 많았다. 생후 첫 1주 이내에 66명(37.1%)이 사망하여 가장 높은 사망률을 보였고, 178명(67.4%)이 생후 28일 이내의 신생아기에 사망하였다. 결 론 : 신생아 중환자 치료의 방전에도 불구하고 지난 10년간 사망률은 크게 변화하지 않았다. 미숙아관련 합병증과 패혈증이 사망원인 중 많은 부분을 차지하므로 이를 줄이는 노력이 필요할 것으로 생각된다. 또한 생후 첫 1주 이내에 사망하는 경우가 많으므로 이 시기에 신생아 중환자 치료에 있어 세심한 주의 필요할 것으로 생각된다.

신생아의 신생아집중치료실 입원에 대한 어머니 경험 (A Mother's Experience of Hospitalization of Her Newborn in the Neonatal Intensive Care Unit)

  • Purpose: The purpose of this study was to explore and describe the meaning and essence of a mother's experience of hospitalization of her newborn in the neonatal intensive care unit (NICU). Methods: This study employed a qualitative research design. An interview was conducted with a mother whose newborn was hospitalized in the NICU, and the data were analyzed using Giorgi's phenomenological method. Results: Five main themes and 19 formulated meanings were indentified. The 5 themes were 'drowning in pain', 'just look outside the glass door', 'being a pillar', 'a deepening attachment', and 'prepare for nurturing with hope'. Conclusion: The results of this study provided an in-depth understanding of the experience of a mother with a newborn in the NICU. These results can be used in the development of a nursing intervention program that provides psychological and emotional support to the mother and family.

신생아집중치료실 간호사의 미숙아 체위지지 간호에 대한 지식 및 수행 (Knowledge and Performance of Developmentally Supportive Positioning for Premature Infants among Neonatal Intensive Care Unit Nurses)

  • Purpose: This study was conducted to assess the knowledge and performance of developmentally supportive positioning for premature infants (DSPP) among neonatal intensive care unit (NICU) nurses. Methods: The participants in this study were 131 nurses working in the NICU at five university hospitals in Daejeon and the city of Cheong-ju. The collected data were analyzed with descriptive statistics, the t-test, analysis of variance, and Pearson correlation coefficients using SPSS for Windows version 22.0. Results: NICU nurses' average DSPP knowledge score was 24.7 out of 33.0, and their average DSPP performance score was 3.2 out of 4.0. A statistically significant positive correlation was observed between knowledge and performance of DSPP (r=.32, p<.001). Conclusion: To improve the level of NICU nurses' knowledge and performance of DSPP, educational programs should be developed and their effectiveness should be verified.
